Prognostic value of FDG PET/CT during radiotherapy in head and neck cancer patients

  • Purpose: To evaluate the prognostic value of $^{18}F$-fluorodeoxyglucose positron-emission tomography (FDG PET) with computed tomography (CT) before and during radiotherapy (RT) in patients with head and neck cancer. Methods: Twenty patients with primary head and neck squamous cell carcinoma were enrolled in this study, of whom 6 had oropharyngeal cancer, 10 had hypopharyngeal cancer, and 4 had laryngeal cancer. Fifteen patients received concurrent cisplatin and 2 received concurrent cetuximab chemotherapy. FDG PET/CT was performed before RT and in the 4th week of RT. The parameters of maximum standardized uptake value, metabolic tumor volume, and total lesion glycolysis (TLG) of the primary tumor were measured, and the prognostic significance of each was analyzed with the Cox proportional hazards model. Results: Higher TLG (>19.0) on FDG PET/CT during RT was a poor prognostic factor for overall survival (OS) (p = 0.001) and progression-free survival (PFS) (p = 0.007). In the multivariate analysis, TLG during RT as a continuous variable was significantly associated with OS and PFS rate (p = 0.023 and p = 0.016, respectively). Tumor response worse than partial remission at 1 month after RT was another independent prognostic factor for PFS (p = 0.024). Conclusions: Higher TLG of the primary tumor on FDG PET/CT during RT was a poor prognostic factor for OS and PFS in patients with head and neck cancer.

치과 임프란트 치료 계획을 위한 나선형 일반 단층촬영과 전산화 단층촬영시 흡수선량 및 유효선량 평가 (Absorbed and effective dose from spiral and computed tomography for the dental implant planning)

  • Objectives : To evaluate the absorbed and effective doses of spiral and computed tomography for the dental implant planning. Materials and Methods: For radiographic projection, TLD chips were placed in 22 sites of humanoid phantom to record the exposure to skin and the mean absorbed dose to bone marrow, thyroid, pituitary, parotid and submandibular glands and nesophagus. Effective dose was calculated, using the method suggested by Frederiksen et al.. Patient situations of a single tooth gap in upper and lower midline region, edentulous maxilla and mandible were simulated for spiral tomography. 35 axial slices (maxilla) and 40 axial slices (mandible) with low and standard dose setting were used for computed tomography. All the radiographic procedures were repeated three times. Results: The mean effective dose in case of maxilla was 0.865 mSv, 0.452 mSv, 0.136 mSv and 0.025 mSv, in spiral tomography of complete edentulous maxilla, computed tomography with standard mAs, computed tomography with low mAs and spiral tomography of a single tooth gap (p<0.05). That in case of mandible was 0.614 mSv, 0.448 mSv, 0.137 mSv and 0.036 mSv, in spiral tomography of complete edentulous mandible, computed tomography with standard mAs, computed tomography with low mAs and spiral tomography of a single tooth gap (p<0.05). Conclusions: Based on these results, it can be concluded that low mAs computed tomography is recommended instead of spiral tomography for the complete edentulous maxilla and mandible dental implant treatment planning.

Economic Scale of Radiation Application in Japan

  • The economic scale of nuclear application is a good indicator to show how the radiation technology is useful and contribute to improve public welfare and living standard. Recent research in Japan shows that the economic scale of nuclear field was 4,112 B¥ for radiation application(46%) and 4,741 B¥ for nuclear energy (54%) playing a role of "two wheels of one cart" in nuclear field and the total 8,853 B¥ constitutes 1.8% of gross domestic products (GDP). The radiation application consisted of 2,295 B¥ (56%) in industry (semiconductor, sterilization, nondestructive testing, radiation processing of tires, etc.), 1,538 B¥ (37%) in medicine (therapy and diagnosis such as X-ray, nuclear medicine, computed tomography, etc.) and 279 B¥ (7%) in agriculture (mutation breeding, food irradiation, sterile insect technique, etc.). Radiation application by ${\gamma}$-ray, electron beam and ion beam is steadily increasing in Japan.

폐암 환자의 정위체부방사선치료 시 기준선 변화에 따른 3D-CBCT(Cone Beam Computed-Tomography)와 Gated-CBCT의 영상 품질 및 유용성 평가 (Image quality and usefulness evaluaton of 3D-CBCT and Gated-CBCT according to baseline changes for SBRT of Lung Cancer)

  • 목 적: 본 연구는 폐암 환자의 정위체부방사선치료 시(Stereotactic Body Radiation Therapy, SBRT) 기준선(Baseline) 변화에 따른 3D-CBCT(Cone Beam Computed-Tomography)와 Gated-CBCT의 영상 품질(Image quality)을 비교 분석하여 호흡에 따른 움직임의 보정에 유용한 CBCT 촬영 방법을 찾고자 한다. 대상 및 방법: QUASARTM(Modus Medical Devices Inc, Canada) 팬텀에 지름이 3 cm인 고체 종양 물질(Solid Tumor)을 삽입하여 팬텀의 속도를 주기 3 sec, 최대 진폭 20 mm 로 4DCT(4-Dimentional Computed-Tomography)를 촬영하였다. 전산화 치료계획시스템 EclipseTM을 이용하여 고체 종양 물질에 육안적 종양용적(Gross Target Volume, GTV)을 윤곽 묘사하였다. Truebeam STxTM을 이용해 4DCT 촬영 시와 동일하게 설정한 후, 기준선 변화가 1 mm, 3 mm, 5 mm인 호흡 패턴을 팬텀에 입력하여 3D-CBCT(Spotlight, Full)와 Gated-CBCT(Spotlight, Full) 영상을 5회 반복하여 획득하였다. 획득된 영상을 4DCT 영상을 기준으로 신호대잡음비(Signal-to-Noise Ratio, SNR), 대조대잡음비(Contrast-to-Noise Ratio, CNR), 종양 체적 길이(Tumor Volume Length) 및 모션 블러링 비(Motion Blurring Ratio, MBR)를 측정하여 비교하였다. 결 과: Spotlight Gated-CBCT 영상이 기준선 변화에 따른 신호대잡음비가 평균 13.30±0.10%, 대조대잡음비가 평균 7.78±0.16%, 종양 체적 길이가 평균 3.55 ± 0.17%, 모션 블러링 비가 평균 1.18 ± 0.06%로 Spotlight 3D-CBCT보다 우수한 값을 보였다. 또한, Full Gated-CBCT 영상이 기준선 변화에 따른 신호대잡음비가 평균 12.80 ± 0.11%, 대조대잡음비가 평균 7.60 ± 0.11%, 종양 체적 길이가 평균 3.54 ± 0.16%, 모션 블러링 비가 평균 1.18 ± 0.05%로 Full 3D-CBCT보다 우수한 값을 보였다. 결 론: 3D-CBCT 촬영 영상과 비교하여 Gated-CBCT 촬영 영상이 기준선 변화에 따른 영상 품질이 우수한 값을 보였으며, 호흡으로 인한 모션 블러링 인공물(Motion Blurring Artifacts)의 영향이 적은 것을 확인했다. 따라서 단기간에 고선량을 전달하는 폐암의 정위체부방사선치료 시 불규칙한 호흡으로 인한 기준선 변화가 발생하는 경우 Gated-CBCT를 이용해 영상유도(Image Guide)를 하는 것이 유용하다고 판단된다.

Observational approach on regional lymph node in cutaneous melanomas of extremities

  • Purpose: We evaluated failure pattern and treatment outcomes of observational approach on regional lymph node (LN) in cutaneous melanoma of extremities and sought to find clinico-pathologic factors related to LN metastases. Material and Methods: We retrospectively reviewed 73 patients with cutaneous melanoma of extremities between 2005 and 2016. If preoperative 18-F-fluorodeoxyglucose (FDG)-positron emission tomography/computed tomography (PET/CT) findings were non-specific for regional LNs, surgical resection of primary tumors with adequate margins was performed without sentinel lymph node biopsy (SLNB) and/or complete lymph node dissection (CLND), irrespective of tumor thickness or size. In patients with suspicious or positive findings on PET/CT or CT, SLNB followed by CLND or CLND was performed at the discretion of the surgeon. We defined LN dissection (LND) as SLNB and/or CLND. Results: With a median follow-up of 38 months (range, 6 to 138 months), the dominant pattern of failure was regional failure (17 of total 23 events, 74%) in the observation group (n = 56). Pathologic LN metastases were significant factor for poor regional failure-free survival (hazard ration [HR] = 3.21; 95% confidence interval [CI], 1.03-10.33; p = 0.044) and overall survival (HR = 3.62; 95% CI, 1.02-12.94; p = 0.047) in multivariate analysis. In subgroup analysis for cN0 patients according to the preoperative PET/CT findings, LND group showed the better trend of LRFFS (log rank test, p = 0.192) and RFFS (p = 0.310), although which is not statistically significant. Conclusion: Observational approach on regional LNs on the basis of the PET/CT in patients with cutaneous melanoma of extremities showed the dominant regional failure pattern compared to upfront LND approach. To reveal regional lymph node status, SLND for cN0 patients may of importance in managing cutaneous melanoma patients.

복부 CT 프로토콜에서 필터 보정 역투영법과 반복적 재구성기법에 따른 화질 및 선량에 관한 연구 (Evaluation of Image Quality and Radiation Dose for Filtered Back-Projection and Iterative Reconstruction Algorithm in Abdominal Computed Tomography Protocol)

  • 최근 복부 CT 검사 건수가 증가하고 있으며 이에 피폭선량을 감소시키기 위해 많은 노력이 요구되고 있다. 최근 도입된 반복적 재구성기법(Iterative Reconstruction, IR)을 복부 CT검사에 적용하여 기존 필터 보정 역투영법(Filtered Back Projection, FBP)과 화질 및 선량을 비교평가하여 유용성을 알아보고자 하였다. 반복적 재구성기법은 SIEMENS사의 ADMIRE, GE사의 ASIR-V를 이용하였고 화질평가를 위해 ACR phantom 영상을 이용하여 Noise, % Contrast, High contrast resolution를 측정하였다. 또한 선량평가는 CT장치에서 표시되는 CTDIvol, DLP를 이용하였다. 필터 보정 역투영법과 반복적 재구성기법을 비교 평가한 결과 반복적 재구성기법 ADMIRE 2~5단계, ASIR-V 30, 50, 70, 90%를 적용한 경우, Noise가 ADMIRE에서 0.46~2.38, ASIR-V에서 0.51~2.5 감소하였다. % Contrast, High contrast resolution 유의한 차이가 없었다. 선량의 경우 반복적 재구성기법을 사용할 경우 ADMIRE에서 25.39%, ASIR-V에서 16.61% 감소시킬 수 있음을 알 수 있었다. 결론적으로 복부 CT검사 시 반복적 재구성기법을 적용한다면 화질을 유지함과 동시에 선량을 감소시킬 수 있을 것이라 사료된다.

IMAGING IN RADIATION THERAPY

  • Radiation therapy is an important part of cancer treatment in which cancer patients are treated using high-energy radiation such as x-rays, gamma rays, electrons, protons, and neutrons. Currently, about half of all cancer patients receive radiation treatment during their whole cancer care process. The goal of radiation therapy is to deliver the necessary radiation dose to cancer cells while minimizing dose to surrounding normal tissues. Success of radiation therapy highly relies on how accurately 1) identifies the target and 2) aim radiation beam to the target. Both tasks are strongly dependent of imaging technology and many imaging modalities have been applied for radiation therapy such as CT (Computed Tomography), MRI (Magnetic Resonant Image), and PET (Positron Emission Tomogaphy). Recently, many researchers have given significant amount of effort to develop and improve imaging techniques for radiation therapy to enhance the overall quality of patient care. For example, advances in medical imaging technology have initiated the development of the state of the art radiation therapy techniques such as intensity modulated radiation therapy (IMRT), gated radiation therapy, tomotherapy, and image guided radiation therapy (IGRT). Capability of determining the local tumor volume and location of the tumor has been significantly improved by applying single or multi-modality imaging fur static or dynamic target. The use of multi-modality imaging provides a more reliable tumor volume, eventually leading to a better definitive local control. Image registration technique is essential to fuse two different image modalities and has been In significant improvement. Imaging equipments and their common applications that are in active use and/or under development in radiation therapy are reviewed.

방사와 투과를 이용한 층류확산화염내 매연입자의 온도 및 농도 측정 (Soot Temperature and Concentration Measurement Using Emission/Transmission Tomography in Laminar Diffusion Flame)

  • The measurements of monochromatic line-of-sight flame emission and light transmission in the same path having small spatial resolution were performed in an axisymmetric laminar propane $C_{3}H_{8}$ diffusion flame. The light wavelengthes of 632 nm, 800nm, 900nm were used. From these measurements, local point soot radiances (by Kirchhoff's law) and absorption coefficients were reconstructed by tomography. Thus local point soot temperatures and concentrations were obtained. The reconstructed soot temperatures and concentrations of local points have no differences between the case of visible range (632 nm) and the case of infrared range (800 nm and 900 nm). In these ranges, the scattering coefficient is much lower than the absorption coefficient. Soot mean temperature over the path also matches well with local soot temperature in outer region of the flame. Temperature measurement by thermocouple with different bead diameters $(222{\mu}m and 308{\mu}m)$ was carried in the same flame. Rapid insertion technique was used and radiation effect was considered. Radiation correction in the sooting region was carried out and the corrected result was in good agreement with the local soot temperature.

형광 X선 CT에서 촬상 시간의 단축화 알고리즘 (Algorithm to Shorten Imaging Time in Fluorescent X-ray Computed Tomogrpahy)

  • 방사광을 이용한 형강 X선 CT 이미징의 고속화를 위한 촬상 시스템과 처리 알고리즘을 검토하였다. 본 논문에서 사용된 촬상시스템은 고계수율 영역에서 안정된 동작을 한 전자 시스템으로 불감시간이 약 6%로 감소되고 계측시간의 경우도 1 점 당 3초로 단축되었다. 또한 재구성 알고리즘의 효율화를 증명하였고, 메모리와 계산량을 약 1/100로 감소시켰다. 물리적 phantom으로 그 정량성을 확인하였고, 시험관내의 갑상선의 화상으로부터 요드 분포를 추정하였다. 이러한 결과는 생체내에서 형광 X선 CT 계측의 실현 가능성을 보여준 것이다.

EFFECTIVE DOSE MEASUREMENT FOR CONE BEAM COMPUTED TOMOGRAPHY USING GLASS DOSIMETER

  • During image-guided radiation therapy, the patient is exposed to unwanted radiation from imaging devices built into the medical LINAC. In the present study, the effective dose delivered to a patient from a cone beam computed tomography (CBCT) machine was measured. Absorbed doses in specific organs listed in ICRP Publication 103 were measured with glass dosimeters calibrated with kilovolt (kV) X-rays using a whole body physical phantom for typical radiotherapy sites, including the head and neck, chest, and pelvis. The effective dose per scan for the head and neck, chest, and pelvis were $3.37{\pm}0.29$, $7.36{\pm}0.33$, and $4.09{\pm}0.29$ mSv, respectively. The results highlight the importance of the compensation of treatment dose by managing imaging dose.
