Comparison of breast volume change between oncoplastic breast-conserving surgery with radiation therapy and a simultaneous contralateral balancing procedure through the inverted-T scar technique

  • Background Reduction mammoplasty or mastopexy is performed as an additional balancing procedure in patients with large or ptotic breasts who undergo breast-conserving surgery (BCS). Radiation therapy on breasts that have undergone surgery may result in changes in the volume. This study presents a comparative analysis of patients who received post-BCS balancing procedures to determine whether volume changes were larger in breasts that received radiation therapy than on the contralateral side. Methods Thirty-six participants were selected among patients who received BCS using the inverted-T scar technique between September 2012 and July 2017, were followed up for 2 or more years, and had pre-radiation therapy computed tomography images and post-radiation therapy images taken between 12 and 18 months after completion. The average age of the participants was 53.5 years, their average body mass index was 26.62 kg/㎡. Results The pre- and post-radiation therapy volumes of the breasts receiving BCS were 666.08±147.48 mL and 649.33±130.35 mL, respectively. In the contralateral breasts, the volume before radiation therapy was 637.69±145.72 mL, which decreased to 628.14±166.41 mL after therapy. The volume ratio of the affected to the contralateral breasts was 1.05±0.10 before radiation therapy and 1.06±0.12 after radiation therapy. Conclusions The ratio of the volume between the two breasts immediately after surgery and at roughly 18 months postoperatively was not significantly different (P=0.98). For these reasons, we recommend a simultaneous single-stage balancing procedure as a reasonable option for patients who require radiation therapy after BCS without concerns regarding volume change.

  • Nasopharyngeal carcinoma is an endemic disease within specific regions in the world. Radiotherapy is the main treatment. In recent decades, intensity-modulated radiation therapy has undergone a rapid evolution. Compared with two-dimensional radiotherapy and/or three-dimensional conformal radiotherapy, evidence has shown it may improve quality of life and prognosis for patients with nasopharyngeal carcinoma. In addition, helical tomotherapy is an emerging technology of intensity-modulated radiation therapy. Its superiority in dosimetric and clinical outcomes has been demonstrated when compared to traditional intensity-modulated radiation therapy. However, many challenges need to be overcome for intensity-modulated radiation therapy of nasopharyngeal carcinoma in the future. Issues such as the status of concurrent chemotherapy, updating of target delineation, the role of replanning during IMRT, the causes of the main local failure pattern require settlement. The present study reviews traditional intensity-modulated radiation therapy, helical tomotherapy, and new challenges in the management of nasopharyngeal carcinoma.

  • The treatment setup of patients during irradiation is an important aspect in relation to the success of radiotherapy. Imaging with the treatment beam is a widely used method or verification of the radiation field position relative to the target area, prior to or during irradiation. In this paper, Real time digital radiography system was implemented or verification of local error between simulation plan and radiation therapy machine. Portal image can be acquired by CCD camera, image board and pentium PC after therapy Radiation was converted into light by a metal/fluorescent Screen. The resulting image quality is comparable to film, so the imaging system represents a promising alternative to film as a method of verifying patient positioning in radiotherapy. Edge detection and field size measurement were also implemented and detected automatically for verification of treatment position. Field edge was added to the original image or checking the anatomical treatment verification by therapy technicians. By means of therapy efficiency improvement and decrease of Radiation side effects with these techniques, Exact Radiation treatments are expected.

유방암 환자의 수술후 방사선치료의 부작용의 변화 : 유방절제술과 유방보존술의 비교 (Survey for the Side effects of Radiation Therapy Following Breast Cancer Surgery : Comparision of Modified Raidcal Mastectomy and Breast Conservative Surgery)

  • This study was performed to survey the specific information about the time of onset, frequency, duration, and severity of the side effect of radiation therapy following breast cancer surgery, and identify the difference of these data according to the type of breast cancer surgery : modified radical mastectomy(MRM) vs. breast consevative operation(BCO). 38 breast cancer patients were interviewed with side effect profile about radiation therapy. Interview was done weekley from the start of radiation therapy through 6 weeks and 3 month follow-up interview was done at 3 month after completion of the treatment. The results are as follow : 1. Total score of side effect experienced by the breast cancer patients was rapidly increased at 2-3 week after intiating treatement and continousely raised maintaing high score until completion of the treatement. Some problems like cough, dyspnea and pain were more experienced after treatment. 2. Patients with modified radical mastectomy showed more total score of side effects than patients with breast conservative operation. And both patients with MRM and BCO experienced similar pattern of side effect to radiation therapy. Through these data we concluded that side effect to radiation therapy was not ended at completion of treatement. Patents will continously experiend various problems and suffer from not only acute side effects like skin problem, sore throat and swollowing difficulty but also late effect of the radiation therapy. Clinically these data can be used for oncologic nurse to provide informational interventions to prepare breast cancer patients for the radiation therapy.

발광·무광 적외선등 조사가 실험적 통증역치에 미치는 효과 비교 (Comparison of the Effects of Luminous Lamp, and Nonluminous Lamp Radiation on Experimental Pain Threshold Sensitivity)

  • The purpose of this study was to compare the experimental pain threshold when used in luminous lamp radiation and nonluminous lamp radiation with healthy person. Thirty normal subjects were randomly assigned two groups: a luminous lamp radiation group, and a nonluminous lamp radiation group. The infrared lamps were applied on L3 for thirty minutes. Each group was measured for experimental pain threshold and local temperature before, 15 and 30 minute radiation. For statistical differences in change of the experimental pain threshold and local temperature due to differences in lamp ray was compared using the independent t-test. And, General linear model for profile plots test was used. The results were as: 1. Local temperature was significantly increased in the nonluminous lamp group (p<.01). 2. Experimental pain thershold was significantly increased in the luminous lamp group (p<.05),(p<.01). This study indicate that luminous lamp radiation was more effects of increase experimental pain thershold than nonluminous lamp radiation. Further study is needed to compare the effects of after period radiation.
