  • Recently, higher speed optical disk drives including computer CD-ROM drives tend to be increasingly demanded to read or write the enormous volume of digital data. To this end, both structure and controller designs of the optical pick-ups should be improved concurrently. In this paper, the pick-up during auto-focusing motion is mathematically modelled retaining all its peculiar features. The model turns out a linear time invariant system suitable for a control design namedH${\infty}$ which ensures robust stability in the presence of system uncertainties. Numerical simulations are performed to demonstrate the robustness with appropriate performance specifications being satisfied. In addition, as the implementation issue of it, procedures of temporal discretization as well as model reduction of the controller are also addressed.

The Effect of a Pulsed Electromagnetic Field with Time on Pain in Muscle Crushed Rat Model

  • Acute injuries to skeletal muscles can lead to significant pain and disability. Muscle pain results in muscle weakness and range of motion (ROM) decreases. Pulsed electromagnetic fields (PEMF) promote tissue repair, healing rates and reduce musculoskeletal pain. The results of many previous studies suggest that PEMF can contribute to chronic pain reduction, particularly in musculoskeletal injurys. However, we do not have enough information of its effects compared to a placebo. The principal objective of this study was to investigate differences in acute pain induced by the direct destruction of muscle tissue (extensor digitorum) with varying times of the application of PEMF, measured through the expression of c-fos on the spinal cord. Significant reduction of pain was found in groups exposed to PEMF and the group exposed to PEMF immediately after muscle injury showed the most significant differences. In conclusion, PEMF may be a useful strategy in reducing acute pain in muscle injury.

Effect of Korean Medicine Treatments in Patients with Proximal Tibia Fracture: A Retrospective Observational Study (경골 근위부 골절 환자에 대한 한의학적 치료 효과: 후향적 관찰 연구)

  • Objectives The purpose of this study is to find the effect of Korean medicine treatments on pain reduction and range of motion recovery in patients with proximal tibia fractures. Methods We studied 15 patients who had been diagnosed as proximal tibia fracture. This study was conducted as retrospective observational study which analyzed patient's medical records with IBM SPSS statistics 25 program. We used numeric rating scale to evaluate pain reduction and range of motion to observe the patient's recovery. Results The average of numeric rating scale reduced statistically significantly from 5.26±1.38 to 2.73±1.17 (p<0.001). The average of knee flexion range of motion increased statistically significantly from 81.11±34.34 to 117.66±19.01 (p<0.01). Conclusions We found that Korean medicine treatments have a positive effect on pain reduction and knee rom improvement in patients with proximal tibia fractures.

Effects of Korean Medicine on Post-rotator Cuff Surgery Patients : A Retrospective Observational Study (회전근개 파열 수술 후 환자에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of this study was to investigate the effect of Korean medicine treatments on pain reduction and functional improvement of patients after rotator cuff surgery. We studied 10 patients who had admitted to Dunsan Korean Medical hospital after have being diagnosed as rotator cuff tear and got a surgery at other hospital from April 30, 2014 to April 30, 2019. This study was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 24 program. We used Numeric Rating Scale to evaluate pain reduction, Range of Motion to evaluate shoulder functional improvement and Five-point likert scale to evaluate the patient's satisfaction. The result showed that Korean medicine treatments statistically significantly reduced pain on the patients after rotator cuff surgery. Although there were improvement of Range of Motion, it wasn't statistically significant. In conclusion, we found that the Korean medicine treatments showed a positive effect on pain reduction of patients after rotator cuff surgery and got a high satisfaction.

Real-time Implementation of Variable Transmission Bit Rate Vocoder Integrating G.729A Vocoder and Reduction of the Computational Amount SOLA-B Algorithm Using the TMS320C5416 (TMS320C5416을 이용한 G.729A 보코더와 계산량 감소된 SOLA-B 알고리즘을 통합한 가변 전송율 보코더의 실시간 구현)

  • In this paper, we real-time implemented to the TMS320C5416 the vocoder of variable bit rate applied the SOLA-B algorithm by Henja to the ITU-T G.729A vocoder of 8kbps transmission rate. This proposed method using the SOLA-B algorithm is that it is reduced the duration of the speech in encoding and is played at the speed of normal by extending the duration of the speech in decoding. At this time, we bandied that the interval of cross correlation function if skipped every 3 sample for decreasing the computational amount of SOLA-B algorithm. The real-time implemented vocoder of C.729A and SOLA-B algorithm is represented the complexity of maximum that is 10.2MIPS in encoder and 2.8MIPS in decoder of 8kbps transmission rate. Also, it is represented the complexity of maximum that is 18.5MIPS in encoder and 13.1MIPS in decoder of 6kbps, it is 18.5MIPS in encoder and 13.1MIPS in decoder of 4kbps. The used memory is about program ROM 9.7kwords, table ROM 4.5kwords, RAM 5.1 kwords. The waveform of output is showed by the result of C simulator and Bit Exact. Also, for evaluation of speech quality of the vocoder of real-time implemented variable bit rate, it is estimated the MOS score of 3.69 in 4kbps.

Treatment of Tibial Condyle Fracture (경골과 골절의 치료)

  • Background: Clinical and radiological results based on fracture types and associated injuries after the treatment of tibial plateau fracture were evaluated for analyzing prognostic factors. Materials and Methods: From June 1997 to June 2002, 50 cases were followed for at least 1 year. Mean age was 47.4 years, and mean follow period was 30.0 months. Fracture classification was performed by the Schatzker method. Clinical and radiological evaluation were performed by the Porter and Rasmussen method. Evaluation was based on degree of reduction and associated injuries, etc. Results: The most common cause of injury was traffic accident (37 cases, 74%), The common fracture types by Schatzker classification were type II (14 cases) and VI (12 cases). Methods of treatment were screw fixation (15 cases), plate and screw (21 cases), external fixator (5 cases), and conservative treatment (9 cases). The most common associated injuries were ipsilateral fibular fracture (18 cases) and MCL (medial collateral ligament) injury (8 cases). Conclusion: Acceptable results after treatment of tibial plateau fracture were obtained from the anatomical reduction group, non-associated injury group, the young age group, and the early ROM (range of motion) beginning group.

  • We describe in this report the accurate reduction of a transverse displaced glenoid fracture through arthroscopic control . We used the lateral and superior (Neviaser portal) portal of the arthroscopic surgery in the shoulder joint for the U 4.0-mm titanium cannulated cancellous screw fixation. The advantages of this treatment are excellent intra-articular visualiBation, decreased soft tissue dissection ,less blood loss, shortened postoperative recovery and early ROM exercise.   • In this research, we investigated synthetic method of 2,6-DANT that can be used as a chain extender of urethane/urea and epoxy materials in a demilitarization method via chemical transformations considering environmental, economical aspects and stability of process. We was able to identify through GC monitoring that 2,4-DANT and 2,6-DANT were produced when we inject 'hydrazine monohydrate 3.3 eq. by TNT 1 eq.' with a fine metering pump for 30 minutes and then, reflux for an additional 2 hours. We was able to isolate only 2,6-DANT(99.3% purity and 45.0% yield) from mixture of 2,4-DANT and 2,6-DANT through the separating and refining methods using 2,6-DANT solubility in methanol and crystallinity of 2,6-DANT.

Treatment of Distal Tibial Metaphyseal Fracture Using MIPPO Technique (MIPPO 수기를 이용한 원위 경골 골간단 골절의 치료)

  • Purpose: To evaluate the results of MIPPO (minimal invasive percutaneous plate osteosynthesis) technique for distal tibial metaphyseal fractures. Materials and Methods: It is a retrospective study of 13 patients who were treated by MIPPO technique for distal tibial metaphyseal fractures from Jan. 2001 to Jan. 2003. The average age was 46.7 years and mean follow-up period was 13.3 months. According to AO classification, there were 8 cases of A1, 3 cases of A2, 1 case of B1 and 1 case of C2. One case of A1 was a Gustilo-Anderson type I open fracture and fibular fractures were combined in 12 cases. We applied anatomical reduction and internal fixation for the fibular fractures and internal fixation on the medial side of the tibia by MIPPO technique for distal tibial metaphyseal fractures. Clinical results were evaluated using radiographic results, Neer score, the starting time of postoperative exercise and clinical complications. Results: According to the Neer score, all cases showed satisfactory results. Active ankle ROM was started at average 2.4 weeks ($2{\sim}4$ weeks) and full weight bearing ambulation at average 5.2 weeks ($4{\sim}8$ weeks) postoperatively. Union of fractures was obtained by average 14.4 weeks ($8{\sim}18$ weeks) postoperatively. Two cases showed $5^{\circ}$ limitation of motion without functional deficits and other cases showed satisfactory ROM results. One case had $6^{\circ}$ valgus deformity without functional deficits. There were not any other complications like soft tissue problems and delayed-or non-union. Conclusion: MIPPO technique for the treatment of distal tibial metaphyseal fractures is a feasible technique with a good clinical outcomes.

All-arthroscopic, Guideless Single Suture-button Fixation of Acute Acromioclavicular Joint Dislocation: A Description of the Technique and Early Treatment Results

  • Background: The purpose of this study was to examine the clinical and radiological results of the all-arthroscopic, suture-button fixation technique to treat acute acromioclavicular (AC) joint separations. Methods: All patients with acute AC joint separations received all-arthroscopic, single suture-button (TightRope) procedure without a special guide. Postoperative Constant score (CS), pain level according to visual analogue scale, and range of motion (ROM) were evaluated. For radiological evaluation, coracoclavicular distances were measured bilaterally. Results: Between December 2010 and June 2012, 18 consecutive patients (4 women and 14 men; mean age, 29.3 years) with acute AC joint separations underwent surgical treatment after 6.4 days (range, 2-20 days) following the initial trauma. The average postoperative follow-up was 16.9 months. The mean CS was 92.4 (range, 84-96). The mean external rotation, forward flexion, and abduction were $75.8^{\circ}$ (range, $50^{\circ}-90^{\circ}$), $170^{\circ}$ (range, $150^{\circ}-180^{\circ}$), and $163.8^{\circ}$ (range, $140^{\circ}-180^{\circ}$), respectively. Five patients exhibited coracoclavicular ossifications. In two patients, superficial wound infections were successfully treated with antibiotic therapy. In one patient, a coracoid fracture was observed. No significant differences were found regarding pain, ROM, or strength parameters between both sides. The coracoclavicular distance was discovered to be approximately 2.8 mm greater on the affected side; however, this minimal reduction loss did not affect the functional results. Conclusions: The findings of this study suggests that all-arthroscopic treatment of AC joint separations using the single suture-button technique without a drill guide is safe, yielding good to excellent clinical results.
