A Study on the Visual System of the Digital Periapical Images (디지털 치근단방사선영상에 관한 시각 특성 연구)

  • Objectives: The purpose of this study was to evaluate the optimal distance and angle of observers by modulation transfer functions(MTFs) and receiver operating characteristics(ROCs), Material and Methods: Digital periapical radiograms were taken from 43 patients who have dental diseases(19 patients : dental caries. 12 patients : periapical lesions, 12 patients : periodontal diseases). Segmental images(4×4cm) were evaluated by 4 MTFs and ROC analysis. Results: The optimal distance(magnification) using MTF by Mannos & Sakrison was 12.97. and those by Nill. Ngan and Rao were 8.39, 4.78. 5.84 respectively. The optimal distance obtained from 4 radiologists by ROC analysis was 32cm(Az value: 0.89). and it was 40cm(Az value: 0.78) from 4 non-radiologists. There were significant differences of Az values between 4 radiologists and 4 non-radiologists at 24. 32 and 40cm (P<0.05). No significant differences of optimal distances were observed using 4 MTFs among +20, +10, 0, -10, -20 degrees(P>0.05). The optimal angle obtained from 4 radiologists by ROC analysis was +20 degree(Az value: 0.91). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). The optimal angle obtained from 4 non-radiologists by ROC analysis was 0 degree(Az value : 0.81). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). And there was significant difference of Az value between 4 radiologists and 4 non-radiologists at +20 degree(P<0.05). but no significant differences of Az values were observed among +10, 0, -10 and -20 degrees(P>0.05).

  • In this paper, we conducted survival analyses by fitting the Cox proportional hazards model to stage III proximal colon cancer data obtained from the Surveillance, Epidemiology, and End Results program of the National Cancer Institute. We investigated the effect of covariates on the hazard function for death from proximal colon cancer in stage III with surgery performed and estimated the survival probability for a patient with specific covariates. We showed that the proportional hazards assumption is satisfied for covariates that were used to analyses, using a test based on the Schoenfeld residuals and plots of the Schoenfeld residuals and $log[-log\{{\hat{S}}(t)\}]$. We evaluated the model calibration and discriminatory accuracy by calibration plot and time-dependent area under the ROC curve, which were calculated using 10-fold cross validation.

  • Cinnamate-4-hydroxylase (C4H) is a key enzyme of phenylpropanoid pathway, which leads a variety of secondary metabolites to participate in differentiation and protection of plant against environmental stresses. In this study, we isolated a full-length cDNA of the C4H gene from a black raspberry (Rubus occidentalis L.), using a reverse transcriptase-PCR and rapid amplification of the cDNA ends (RACE)-PCR. The full-length cDNA of the RocC4H gene contained a 1,515 bp open reading frame (ORF) encoding a 504 amino acid protein with a calculated molecular weight of about 57.9 kDa and an isoelectric point (pI) value of 9.1. The genomic DNA analysis revealed that RocC4H gene had three exons and two introns. By multiple sequence alignment, RocC4H protein was highly homologous with other plant C4Hs, and the cytochrome P450-featured motifs, such as the heme-binding domain, the T-containing binding pocket motif (AAIETT), the ERR triad, and the tetrapeptide (PPGP) hinge motif, were highly conserved. Southern blot analysis revealed that RocC4H is a single copy gene in R. occidentalis.

Odds curve for two classification distributions (두 분류 분포를 위한 오즈 곡선)

  • The ROC, TOC, and TROC curves, which are visually descriptive methods of exploring the performance of the binary classification model, are implemented with TP, TN, FP, FN which consist of the confusion matrix, as well as their ratios TPR, TNR, FPR, FNR. In this study, we consider two types odds and then propose an odds curve representing these odds. And show the relationship between the odds curve and ROC curve. Based on the odds curve, we propose not only two statistics that measure the discriminant power of the odds curve but also the criteria for validation ratings of the odds curve. According to the shape of the odds curves, two classification distributions can be estimated and a criterion for validation ratings can be determined. The odds curve can be meaningfully used like other visual methods, and two kinds of measures for the discriminant power can be also applied together as an alternative criterion.

High-impact chronic pain: evaluation of risk factors and predictors

  • Background: The concept of high-impact chronic pain (HICP) has been proposed for patients with chronic pain who have significant limitations in work, social life, and personal care. Recognition of HICP and being able to distinguish patients with HICP from other chronic pain patients who do not have life interference allows the necessary measures to be taken in order to restore the physical and emotional functioning of the affected persons. The aim was to reveal the risk factors and predictors associated with HICP. Methods: Patients with chronic pain without life interference (grade 1 and 2) and patients with HICP were compared. Significant data were evaluated with regression analysis to reveal the associated risk factors. Receiving operating characteristic (ROC) analysis was used to evaluate predictors and present cutoff scores. Results: One thousand and six patients completed the study. From pain related cognitive processes, fear of pain (odds ratio [OR], 0.92; 95% confidence interval [CI], 0.87-0.98; P = 0.007) and helplessness (OR, 1.06; 95% CI, 1.01-1.12; P = 0.018) were found to be risk factors associated with HICP. Predictors of HICP were evaluated by ROC analysis. The highest discrimination value was found for pain intensity (cut-off score > 6.5; 83.8% sensitive; 68.7% specific; area under the curve = 0.823; P < 0.001). Conclusions: This is the first study in our geography to evaluate HICP with measurement tools that evaluate all dimensions of pain. Moreover, it is the first study in the literature to evaluate predictors and cut-off scores using ROC analysis for HICP.

  • 가뭄은 과거부터 현재까지 국내뿐만 아니라 국외에서도 지속적으로 발생하고 있으며 가뭄피해를 줄이기 위하여 많은 연구가 과거에 수행되었고 현재까지도 진행되고 있다.(강동호 외 2명, 2019명) 가뭄은 일반적으로 강수량의 부족이 원인이 되어 나타나며 홍수와 달리 장기간에 걸쳐 광범위한 지역에 피해가 나타나는 특징이 있다. 그렇기에 가뭄은 감시 및 예측에 대한 어려움이 존재하며 우리나라에서도 가뭄으로 인해 수자원 공급 긴장상태를 경험하는 빈도가 점차 증가하고 있는 추세이다.(유지영 외 3명, 2016) 적조현상은 특정 조류의 폭발적인 증식으로 해수가 붉은 빛을 띄며 용존산소량을 부족하게 만들어 해양 생태계를 파괴하는 현상이다. 적조현상은 1990년대 이후 대규모 적조가 발생하여 1995년에는 적조로 인해 약 764억의 손실이 발생한 사례가 있다.(연합뉴스, 2019.09) 이후에도 적조현상은 점점 광역화, 장기화, 고밀도화 되면서 연평균 110억 이상의 피해를 지속적으로 주고 있다. 본 연구에서는 대상 지역을 대한민국 완도로 설정 하였다. 섬이라는 지역의 특성 상 가뭄이 발생하면 식수원의 공급이 어렵고 평시에도 물을 아끼기 위한 노력이 끊이지 않는 곳이다. 실제로 완도는 우리나라에서 가장 많은 피해이력이 있는 지역이다. 또 2016년 완도에서는 적조로 인해 전복이 집단 폐사하며 358억의 피해를 입은 사례가 있다.(중앙일보, 2016.09) 그렇기에 완도지역을 대상 지역으로 하여 비강우의 장기화 등의 이유로 나타나는 가뭄을 기상학적 가뭄지수 SPI를 활용하여 분석하고 ROC 분석을 통해 수온의 상승 시 나타나는 적조현상과의 관계를 분석함으로서 적조현상이 발생하는 한 척도로 가뭄의 발생 및 장기화를 생각 할 수 있는지 판단하고자 한다.

  • 가뭄은 다른 자연재해에 비해 진행 속도가 느리고 발생 시작 시기가 명확하지 않다. 또한 피해지역이 광범위하다는 점에서 사회, 경제적 피해와 농업 생산 시스템 및 수확량 등 농업 전반에 걸쳐 직접적인 영향을 미치고 있다. 전지구적 기후변화로 인해 국내의 가뭄 발생빈도는 2000년 이후 증가하고 있으며, 가뭄의 정량적 분석은 선제적 가뭄 대응을 위해 필요하다. 현재 국내에서는 여러 유관기관에서 지상 관측 데이터를 활용하여 가뭄을 모니터링하고, 가뭄 공간 분포 지도를 제공하고 있다. 하지만 지상 관측 데이터를 통한 가뭄 분포 지도는 미계측 지역에 대한 데이터 취득이 어렵고, 지형학적 특성을 고려하지 못하는 한계점이 있다. 이러한 한계점을 보완하기 위해 수자원 및 재해 분야에서 위성영상이 활용되고 있다. 위성영상을 활용한 가뭄 판단 및 예측에는 정규식생지수 (Normalized Difference Vegetation Index, NDVI)가 사용되고 있으며, 식생지수는 가뭄 발생, 진행 등에 있어 즉각적인 반응이 어렵다는 단점이 있다. 본 연구에서는 잠재 증발산과 실제 증발산의 비를 이용해 산정된 위성영상 기반 가뭄 지수인 Evaporative Stress Index (ESI)를 활용하였다. NASA (National Aeronautics and Space Administration)에서 제공하는 ESI는 전지구를 대상으로 5km 해상도로 제공하고 있다. 하지만 국내 가뭄 판단을 위해서는 높은 해상도의 영상이 필요하며, 본 연구에서는 MODIS (Moderate Resolution Imaging Spectroradiometer) 영상을 활용한 ESI의 산정을 통해 해상도의 문제를 개선하고자 한다. 산정한 500m 해상도의 ESI는 기존 5km 해상도의 ESI와 비교 검증하였으며, SPI 및 과거 가뭄 발생 현황 자료를 근거로 ROC (Receiver Operating Characteristics) 분석을 통해 시군 단위 농업가뭄평가의 적용성을 확인하고 한다.

A comparison of film and 3 digital imaging systems for natural dental caries detection: CCD, CMOS, PSP and film (치아 우식증 진단시 필름 방사선사진상과 디지털 방사선영상의 비교:CCD, CMOS, PSP와 film)

  • Purpose: To evaluate the diagnostic accuracy of occlusal and proximal caries detection using CCD, CMOS, PSP and film system. Materials and Methods : 32 occlusal and 30 proximal tooth surfaces were radiographed under standardized conditions using 3 digital systems; CCD (CDX-2000HQ, Biomedysis Co., Seoul, Korea), CMOS (Schick, Schick Inc., Long Island, USA), PSP (Digora/sup (R)/FMX, Orion Co./Soredex, Helsinki, Finland) and I film system (Kodak Insight, Eastman Kodak, Rochester, USA). 5 observers examined the radiographs for occlusal and proximal caries using a 5-point confidence scale. The presence of caries was validated histologically and radiographically. Diagnostic accuracy was evaluated using ROC curve areas (Az). Results: Analysis using ROC curves revealed the area under each curve which indicated a diagnostic accuracy. For occlusal caries, Kodak Insight film had an Az of 0.765, CCD one of 0.730, CMOS one of 0.742 and PSP one of 0.735. For proximal caries, Kodak Insight film had an Az of 0.833, CCD one of 0.832, CMOS one of 0.828 and PSP one of 0.868. No statistically significant difference was noted between any of the imaging modalities. Conclusion: CCD, CMOS, PSP and film performed equally well in the detection of occlusal and proximal dental caries. CCD, CMOS and PSP-based digital images provided a level of diagnostic performance comparable to Kodak Insight film.

Evaluating the Validity of the Pediatric Index of Mortality Ⅱ in the Intensive Care Units (소아중환자를 대상으로 한 PIM Ⅱ의 타당도 평가)

  • Purpose: This study was to evaluate the validity of the Pediatric Index of Mortality Ⅱ(PIM Ⅱ). Method: The first values on PIM Ⅱ variables following ICU admission were collected from the patient's charts of 548 admissions retrospectively in three ICUs(medical, surgical, and neurosurgical) at P University Hospital and a cardiac ICU at D University Hospital in Busan from January 1, 2002 to December 31, 2003. Data was analyzed with the SPSSWIN 10.0 program for the descriptive statistics, correlation coefficient, standardized mortality ratio(SMR), validity index(sensitivity, specificity, positive predictive value, negative predictive value), and AUC of ROC curve. Result: The mortality rate was 10.9% (60 cases) and the predicted death rate was 9.5%. The correlation coefficient(r) between observed and expected death rates was .929(p<.01) and SMR was 1.15. Se, Sp, pPv, nPv, and the correct classification rate were .80, .96, .70, .98, and 94.0% respectively. In addition, areas under the curve (AUC) of the receiver operating characteristic(ROC) was 0.954 (95% CI=0.919~0.989). According to demographic characteristics, mortality was underestimated in the medical group and overestimated in the surgical group. In addition, the AUCs of ROC curve were generally high in all subgroups. Conclusion: The PIM Ⅱ showed a good, so it can be utilized for the subject hospital. better.

Psychometric Properties of Korean Minimal Insomnia Screening Scale (불면증 최소스크리닝척도의 심리측정적 특성과 적합성 검증)

  • Purpose: The purposes of this study were to develop a Minimal Insomnia Screening Scale for Korean adults (KMISS) and to evaluate psychometric properties and discriminant ability of the developed scale. Methods: Data from a cross-sectional survey of 959 Korean adults were analyzed to develop the summated insomnia scale, which was evaluated in terms of reliability, validity, and discriminant ability by receiver operating characteristics (ROC) curve analysis. Results: Item-total correlations ranged between .71-.79 and Cronbach's ${\alpha}$ was .87. Adequate validity was also evident. ROC-curve analysis showed area under ROC was .87 (95% CI: .84-.90) and identified the optimal cut-off score as ${\leq}20$ (sensitivity, .83; specificity, .75; positive/negative predictive values, .40/.95). Using this cut-off score, the prevalence of insomnia in the study sample was 26.3% and most frequent among women and the oldest group. Conclusion: Data supports the psychometric properties of KMISS as a possible insomnia screening instrument. KMISS also shows promise as a convenient ultra-short screening measure of insomnia for adults and epidemiological studies in community health care settings.
