Overview of RCT for Non-Alcoholic Fatty Liver Disease and Non-Alcoholic Steatohepatitis

  • Objective: This study aimed to get information on the current status of therapies to date for non-alcoholic fatty liver disease (NAFLD), including non-alcoholic steatohepatitis (NASH). Methods: All randomized clinical controlled trial (RCT)-derived papers for NAFLD or NASH were reviewed via PubMed Database. Results: 39 RCTs met the review criteria, of which 15 and 24 papers were for NAFLD and NASH, respectively. 83% of the papers were released since 2006, and 30 studies were conducted for western medicines, antioxidants and lifestyle intervention whereas nine trials were done using herbal medicine or acupuncture which showed positive outcome. Conclusions: NAFLD and NASH are new epidemic disorders which can be a target of traditional Oriental medicine. This study will be helpful for the Oriental medicine-based strategies or therapeutic development for them.

Systemic Review of RCTs focusing on Chronic Fatigue

  • Objective: This study aimed to build an overview of randomized clinical controlled trials (RCTs) for chronic fatigue-related symptoms to extract the useful data for management of patients and development of therapeutics using Korean traditional medicine in the future. Methods: All RCT-derived papers for chronic fatigue-related symptoms were collected via PubMed Database. We surveyed elementary information of RCTs such as clinical question, study design, and its quality and results. Results: A total of fifty-three RCTs met these review criteria. Most of the RCTs were performed in Western countries, particularly the UK and USA. The major portion of RCTs focused on chronic fatigue syndrome using immune modulators, psychotherapeutic and anti-depressants. Five RCTs using complementary and alternative medicine, including herbal remedies, showed positive results. Conclusions: Fatigue-related symptoms are a main target of Oriental medicine. This study provides helpful information for planning clinical study of chronic fatigue-related symptoms using traditional Korean medicine.

기능성 자궁출혈에 대한 한약치료 : 체계적 문헌 고찰 (Traditional Herbal Medicine for Dysfunctional Uterine Bleeding versus Western Medicine: A Systematic Review of Randomized Controlled Trials)

  • Objectives: To evaluate the effectiveness and safety of traditional herbal medicine (THM) in the treatment of dysfunctional uterine bleeding (DUB) versus conventional western medicine. Methods: Randomized Controlled Trials (RCTs) comparing THM vs. conventional western medicine for DUB, were obtained from PubMed, Cochrane Library, Embase, CNKI, RISS, NDSL, KISS and OASIS. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 16 RCTs with 1,659 patients were identified and reviewed. 10 RCTs reported THM was statistically effective than control group in effective rate. Also recurrent rate was estimated in 6 RCTs and was lower than control group. 7 studies observed adverse events (AEs) and severe AEs were not reported. Conclusions: Despite several limitations, this review suggested that THM was safe and effective in the treatment of DUB. THM may also decrease the recurrence rate. However, this could not be proven conclusively. To ensure evidence-based clinical practice, more sternly designed trials are warranted.

위축성 질염에 대한 한약 투약의 효과 : 체계적 문헌 고찰 (Application of Traditional East Asian Herbal Medicine for Atrophic Vaginitis: A Systematic Review of Randomized Controlled Trials)

  • Objectives: To evaluate the effectiveness and safety of application of Traditional East Asian Herbal Medicine (TEAM) in the treatment of Atrophic Vaginitis (AV). Methods: Randomized Controlled Trials (RCTs) were obtained from PubMed, Cochrane Library, Embase, CNKI, RISS, NDSL, and KISS. The risk of bias was assessed by using Cochrane's risk of bias tool, and RevMan 5.3 software was used. Results: 26 RCTs with 3,162 patients were identified and reviewed. Among them, 21 RCTs observe the effect of integrated traditional Chinese and Western medicine. 23 RCTs reported treatment groups was statistically effective than control groups in the study. Also, the recurrence rate was estimated in 10 RCTs and was lower than control groups. 12 studies observed adverse events (AEs) and severe AEs were not reported. Conclusions: This review suggested that TEAM was safe and effective in the treatment of AV. TEAM may also decrease the recurrence rate. However, this could not be proven conclusively. To ensure evidence-based clinical practice, well-designed trials with larger sample sizes are needed.

Acupuncture Treatment for Restless Legs Syndrome: A Review of Randomized Controlled Trials

  • To determine the effectiveness of acupuncture in treating restless legs syndrome (RLS), we conducted a literature review of randomized controlled trials (RCTs) that utilized acupuncture as an intervention for patients diagnosed with RLS. Relevant clinical studies (n = 158) from seven databases (the Cochrane Library, PubMed, Embase, CNKI, KISS, RISS, and OASIS) were included based on the inclusion and exclusion criteria and analyzed. Moreover, 6 RCTs were selected for review. In all six studies, it was indicated people who underwent acupuncture treatment showed significant improvements in their overall health. An increase in the treatment efficacy rate, sleep quality, and quality of life indicators after the acupuncture treatment was confirmed. The severity of pain as assessed using the visual analog scale (VAS) scores and International RLS Study Group Rating Scale (IRLSRS) scores and the severity of RLS symptoms were significantly reduced. Any significant side effects were not reported. Acupuncture is suggested as an effective and safe treatment method for RLS. However, further large-scale RCT studies are needed to confirm our findings.

  • Objectives The purpose of this study is to review the effectiveness of acupuncture treatment in irritable bowel syndrome (IBS). Methods We reviewed 8 randomized controlled trials (RCT) of acupuncture treatment in IBS which were published from 2008 to 2018 in PubMed. The acupoints, methods of assessment, treatment, results and adverse events were analyzed. Results There were 8 RCT of acupuncture treatment in IBS. The most commonly used acupoints were 天樞 (ST25), and 上巨虛 (ST37). Frequently used assessments in IBS were Bristol scale, visual analogue scale, IBS-symptom severity score, quality of life, and symptom scale. 3 out of 8 studies, the acupuncture treatment group showed significant effectiveness than that of the control group. Conclusions All of the studies have not shown that the acupuncture treatment was effective in relieving IBS symptoms. More clinical studies are needed to prove the effectiveness of the acupuncture treatment in IBS.

  • Objectives The purpose of this study is to review the effectivenss of Neurofeedback Therapy in children with Attention Deficit/Hyperactivity Disorders Methods 16 randomized controlled trials (RCT) of Neurofeedback Treatment in ADHD from 2009 to 2019 in PubMed were reviewed and analyzed their methods of assessment, treatment, results and adverse effect. Results In 16 RCT, the most frequently used diagnostic tool in ADHD was DSM-IV. Most of the studies reported Neurofeedback Treatment isn't superior than placebo or medication group but still effective in ADHD. In 6 out of the 16 studies, there were no adverse effect reported. Conclusion Most of the studies have shown neurofeedback treatment was effective in ADHD symptoms. More clinical studies are needed to prove the effectiveness of neurofeedback in ADHD.

안면마비의 테이핑 요법에 대한 임상 연구 동향 (Clinical Research Trend for Taping Therapy on Facial Palsy)

  • Objectives : The purpose of this research is to review the efficacy and safety of taping therapy for facial palsy. Methods : Two foreign databases(Pubmed, CNKI) and one domestic database(OASIS) were used to search, and papers retrieved from the starting point of literature provision in each database until June 19, 2023 were included in the study. All studies in which other treatments were performed as a treatment intervention along with taping therapy were included, but cases where taping therapy was a comparative intervention were excluded. Results : 18 studies were selected. 14 studies were two-arm randomized controlled trials(RCT), and 4 studies were case reports. There was improvement in facial palsy in all 4 case reports, and in all 14 RCTs, the treatment effect in the observation group was significantly higher than in the control group. Conclusions : Taping therapy can be an effective and safe treatment method for facial palsy. However, the risk of bias in the selected studies was high, and there were many cases in which multiple interventions other than taping therapy were applied simultaneously. Therefore, well-designed studies with high quality are needed in the future.

경피적외선요법의 경혈(經穴) 조사(照射) 치료법에 대한 고찰 (An Effect of Infrared Therapy on Acupoints ; Randomized Controlled Trials Review)

  • Objectives : The aim of this study is to establish evidence-based guideline on infrared therapy on acupoints. Methods : We investigated all the articles in websites including Pubmed, China national knowledge Infrastructure(CNKI) and medical journals authorized by national research foundation of Korea. The key words are acupoints and infrared. For the first time, we withdrew some papers no relationship to key words by reading titles and abstracts. In the second, we only selected randomized clinical trials(RCT) published from 2000 to 2010. Results : Out of 62 papers, only 2 RCT were analyzed. One (RCT 1) was about depression patients with insomnia(jadad score: 1). The other(RCT 2) was about urimic pruritus patients(jadad score: 5). In RCT 1, experimental group was significantly better than control group. In RCT 2, there was not statistical signification between two groups. Conclusions : In reality, there were not many papers on infrared therapy applications. Further study just like RCTs may be needed to establish evidenced-based guideline on infrared.

제왕절개술 후 발생한 위장관 기능장애에 대한 최신 RCT 연구 동향 분석 - 경혈 자극을 중심으로 - (Analysis for Randomized Controlled Clinical Trials of Acupuncture-type Treatment on Gastrointestinal Dysfunction after Ceasarean Section)

  • Objectives: The purpose of this study was to show effectiveness of Acupuncture treatment on gastrointestinal dysfunctions after Cesarean section by analyzing randomized controlled clinical trials. Methods: We searched randomized controlled clinical trials related with acupuncture treatment on gastrointestinal dysfunctions after Cesarean section through national and overseas database and analyzed them in detail. Results: 6 articles were included according to our selection criteria and 1,084 women were involved. 3 studies used TEAS (Transcutaneous Electrical Acupoint Stimulation), 2 studies used Elastic band and 1 study used Acupressure by hand. Their results were statistically more effective than control groups. The most frequently used acupoints were Neiguan (PC6), Zusanli (ST36) followed by Sanyinjiao (SP6), Hegu (IL4). Conclusions: There was significant difference in the effectiveness of the intervention including Acupuncture treatment. Based on analysis, it could be an effective way for the treatment of gastrointestinal dysfunctions after Cesarean section in clinical practice.
