Attitudes about rapid Oral HIV screening test in dental clinics (치과 진료실에서의 신록 구강 점막 도말 HIV 항체 검사에 한 태도 및 지식에 관한 연구)

  • Purpose : The aim of this studγ was to evaluate the patient's and dentist's perspective and knowledge on rapid human immunodeficiency virus screening test in dental setting. Material and Methods: In March 2009, 100 patients and 100 dentists at College of Dentistry, Yonsei University were provided an attitude assessment survey. Results : Results were analyzed for acceptance of testing and potential barriers. 94% of patients agreed to take a rapid HIV screening test and 77% of dentists were willing to provide the test to their patients. Also the current knowledge state of patients and dentists on HIV infection was not accurate as expected. Conclusion : Overall, dental clinic patients widely accepted the offer of rapid oral HIV screening. Rapid HIV screening test in the dental setting ran be an important option to increase the number of individuals who know their HIV status.

Development of Ultra-rapid Multiplex Real-time PCR for the Detection of Genes from Avian Influenza Virus subtype H5N1 (조류인플루엔자 H5N1 바이러스 유전자의 신속 검출을 위한 초고속 다중 실시간 PCR법의 개발)

  • Cause of high lethality and dissemination to human being, new development of rapid method for the detection of highly pathogenic Avian Influenza Virus (AIV) is still necessary. For the detection of AIV subtype H5N1, typical pathogenic AIV, new method to confirm sub-typing of this virus is also needed. For the purpose of ultra-rapid detection and sub-typing of hemagglutinin and neuraminidase of AIV, this study was planned. As the results we could demonstrate an ultra-rapid multiplex real-time PCR (URMRT PCR) for the detection of AIV In this study, the URMRT PCR were optimized with synthesized AIV H5- and AIV Nl-specific DNA templates and GenSpector TMC, which is a semiconductor process technology based real-time PCR system with high frequencies of temperature monitoring. Under eight minutes, the amplifications of two AIV subtype-specific PCR products were successfully and independently detected by 30 cycled ultra-rapid PCR, including melting point analysis, from $1{\times}10^3$ copies of mixed template DNA. The URMRT PCR for the detection of AIV H5N 1 developed in this study could be expected to apply not only detections of different AIVs, but also various pathogens. It was also discussed that this kind of the fastest PCR based detection method could be improved by advance of related technology in near future.

A Study on Rapid Residual Analysis of Benzo(a)pyrene in Agricultural Products and Soils (농산물 및 경작지 토양 시료 중 Benzo(a)pyrene 신속잔류분석법 개선 연구)

  • BACKGROUND: Benzo(a)pyrene is a highly toxic substance which has been listed as a Group I carcinogen by the International Agency for Research on Cancer. There have been numerous studies by researchers worldwide on benzo(a)pyrene. Soxhlet, ultrasound-assisted, and liquid-liquid extractions have been widely used for the analysis of benzo(a)pyrene. However these extraction methods have significant drawbacks, such as long extraction time and large amount of solvent usage. To overcome these disadvantages, we aimed to establish a rapid residual analysis of benzo(a)pyrene content in agricultural products and soil samples. METHODS AND RESULTS: A Quick, Easy, Cheap, Effective, Rugged, and Safe (QuEChERS) method was used as the pretreatment procedure. For rapid residual analysis of benzo(a)pyrene, a modified QuEChERS method were used, and the best codition was demonstrated after various performing instrument analysis. The extraction efficiency of this method was also compared with Soxhlet extraction, the current benzo(a)pyrene extracting method. Although both methods showed high recovery rates, the rapid residual analysis method markedly reduced both the measurement time and solvent usage by approximately 97% and 96%, respectively. CONCLUSION: Based on these results, we suggest the rapid residual analysis method established through this study, faster and more efficient analysis of residual benzo(a)pyrene in major agricultural products such as rice, green and red chili peppers and also soil samples.

  • The rapid prototyping process and development tool which enable the control law evaluation efficiently are needed to minimize the development cycle, cost and risk of aircraft flight control system. This paper describes a development process that integrates the designed control law into HETLAS to evaluate simulation effectively using nonlinear mathematical models. The desktop engineering simulator was developed using HETLAS for the piloted simulation evaluation of a various control modes and the procedure was developed, which quickly integrates the HETLAS into HQS(Handling Quality Simulator) and HILS(Hardware In the Loop Simulation) environments. This paper presents a rapid prototyping process using HETLAS that significantly shortens the integration process of the control law into the nonlinear math model, HETLAS, and allows the control law designs to be quickly tested in the piloted simulation and HILS environments.

  • The seismic vulnerability of Turkey is relatively high due to its active fault systems with potential to create destructive earthquakes. Thus, reducing the loss of life and property, the number of the earthquake-prone buildings and their retrofit requirements are considerably significant key issues under the scenario earthquakes. The street survey based rapid assessment (SSRA) method can be considered as a powerful tool to determine the seismic vulnerability of building stock of an earthquake-prone city/state. In this study, the seismic vulnerability of the building stock of the Kirikkale province in Turkey is aimed to be estimated adopting the street survey based rapid assessment method (SSRA). For this purpose, central 2074 existing reinforced concrete (R/C) buildings were structurally surveyed with rapid visual site screening and disadvantages such as, the existence of short-column, soft-story, heavy overhangs, pounding effect and local soil conditions were determined for obtaining the structural performance score of each. The results obtained from the study demonstrate that 11-25% of the surveyed buildings in the study region needs to be investigated through more advanced assessment methods. Besides, higher correlation between increasing story number and unsafe/safe building ratio is obtained for the buildings with soft-story parameter than that for those with heavy overhangs and short-column parameters. The conformity of the results of the current study with the previous documented cases of rapid assessment efforts in the recent earthquakes in Turkey shows that the SSRA method for the Kirikkale province performed well, and thus this methodology can be reliably used for similar settlement areas.

Development and Clinical Evaluation of a Rapid Serodiagnostic Test for Toxoplasmosis of Cats Using Recombinant SAG1 Antigen

  • Rapid serodiagnostic methods for Toxoplasma gondii infection in cats are urgently needed for effective control of transmission routes toward human infections. In this work, 4 recombinant T. gondii antigens (SAG1, SAG2, GRA3, and GRA6) were produced and tested for the development of rapid diagnostic test (RDT). The proteins were expressed in Escherichia coli, affinity-purified, and applied onto the nitrocellulose membrane of the test strip. The recombinant SAG1 (rSAG1) showed the strongest antigenic activity and highest specificity among them. We also performed clinical evaluation of the rSAG1-loaded RDT in 182 cat sera (55 household and 127 stray cats). The kit showed 0.88 of kappa value comparing with a commercialized ELISA kit, which indicated a significant correlation between rSAG1-loaded RDT and the ELISA kit. The overall sensitivity and specificity of the RDT were 100% (23/23) and 99.4% (158/159), respectively. The rSAG1-loaded RDT is rapid, easy to use, and highly accurate. Thus, it would be a suitable diagnostic tool for rapid detection of antibodies in T. gondii-infected cats under field conditions.

  • For the recycling of rapid-cooled steel slag, various specimens were prepared with the various replacement ratios of the rapid-cooled steel slag and the addition ratios of polymer binders. The physical properties of these specimens were then investigated by absorption test, compressive strength test, flexural strength test and hot water resistance test, and the pore and the micro-structure analysis was performed using scanning electron microscope. Results showed that the flexural strength increased with the increase of rapid-cooled steel slag and polymer binder, but the compressive strength showed a maximum strength at a certain proportion. By the hot water resistance test, compressive strength and flexural strength decreased remarkably and the total pore volume increased but the pore diameter decreased. SEM observation of the structure before the hot water resistance test revealed a very compact infusion of structure but the decomposition or thermal degradation appeared in polymer binders when observed after the hot water resistance test.

A Comparative Evaluation of Three Rapid Tests of Syphilis and ARCHITECT Syphilis TP

  • The infection rate of syphilis is still increasing in the world especially in developing countries and the infection is often seen in large amounts of clinical specimens. For the diagnosis of this disease, Rapid Plasma Reagin (RPR)/Venereal Disease Research Laboratory (VDRL) has still been used as one of major primary methods to diagnose syphilis even though the test readings are somewhat subjective with high false positive rates. Recently, the automatic ARCHITECT Syphilis TP, which is based on the detection of the TP-specific antibodies, has been introduced in many laboratories. Therefore, the clinical assessment of the method is needed to provide primary diagnosis of syphilis at the moment. We evaluated 3 different manual rapid kits and ARCHITECT Syphilis TP comparing with RPR/FTA-ABS and analysed their diagnostic properties. From February 2006 to April 2008, 203 positive and 250 negative specimens, obtained from Chungbuk National University Hospital were used for the evaluation. In the evaluation between manual rapid kits, their specificities were as high as 99.2 ~ 99.6% while their sensitivities were observed with little differences; 98.0% (199/203) for Kit A, 96.6% (196/203) for Kit B, and 97.4% (197/203) for Kit S. In the case of ARCHITECT Syphilis TP test, it showed 100% specificity (250/250) and 98.5% sensitivity (249/250). Kappa values comparing with RPR/FTA-ABS were 0.978 for Kit A, 0.964 for Kit B and Kit S, and 0.987 for ARCHITECT Syphilis TP. From our evaluation, we found out that manual rapid tests and ARCHITECT Syphilis TP have very good clinical accuracies and high kappa agreements with RPR/FTA-ABS. Due to its automation and quick simultaneous diagnosis with another serological markers, we suggest that the ARCHITECT Syphilis TP is one of best suitable method for the primary diagnosis of syphilis and that it might be able to replace RPR method in the laboratories.

Rapidly quantitative detection of Nosema ceranae in honeybees using ultra-rapid real-time quantitative PCR

  • Background: The microsporidian parasite Nosema ceranae is a global problem in honeybee populations and is known to cause winter mortality. A sensitive and rapid tool for stable quantitative detection is necessary to establish further research related to the diagnosis, prevention, and treatment of this pathogen. Objectives: The present study aimed to develop a quantitative method that incorporates ultra-rapid real-time quantitative polymerase chain reaction (UR-qPCR) for the rapid enumeration of N. ceranae in infected bees. Methods: A procedure for UR-qPCR detection of N. ceranae was developed, and the advantages of molecular detection were evaluated in comparison with microscopic enumeration. Results: UR-qPCR was more sensitive than microscopic enumeration for detecting two copies of N. ceranae DNA and 24 spores per bee. Meanwhile, the limit of detection by microscopy was 2.40 × 104 spores/bee, and the stable detection level was ≥ 2.40 × 105 spores/bee. The results of N. ceranae calculations from the infected honeybees and purified spores by UR-qPCR showed that the DNA copy number was approximately 8-fold higher than the spore count. Additionally, honeybees infected with N. ceranae with 2.74 × 104 copies of N. ceranae DNA were incapable of detection by microscopy. The results of quantitative analysis using UR-qPCR were accomplished within 20 min. Conclusions: UR-qPCR is expected to be the most rapid molecular method for Nosema detection and has been developed for diagnosing nosemosis at low levels of infection.

Comparison of Fentanyl-Based Rapid Onset Opioids for the Relief of Breakthrough Cancer Pain: Drug Price Based on Effect Size

  • Background and Objective: With the advancement of cancer treatments and increased life expectancy, managing breakthrough cancer pain (BTcP) is essential to improve the quality of life for cancer patients. This study aimed to compare the major rapid onset opioids in Korea based on their characteristics and costs to determine the best option for each patient. Methods: Based on sales information from IQVIA-MIDAS, sublingual fentanyl tablet (SLF), fentanyl buccal tablet (FBT), and oral transmucosal fentanyl citrate (OTFC) were selected as the top three drugs for the treatment of BTcP in Korea, considering them the most comparable drugs. The cost and cost-pain relief ratio of the drugs for short-term (1 month) and long-term (1 year) treatment were compared and the ease of administration based on various factors, including pharmacokinetics, onset of action, and administration procedures were evaluated. Results: SLF was evaluated as the best overall in terms of rapid onset of action, ease of administration, and drug cost and also had the highest market share. SLF had the lowest cost pain relief ratio for both the initial and supplemental treatment for the 1-month pain intensity difference 15 (PID15) ratio. However, for the 1-month PID30 ratio, SLF was not superior to OTFC or FBT. The longer the breakthrough cancer pain duration, the more cost-effective the other rapid onset opioids. Conclusion: The rapid onset opioids that fit the patient's breakthrough cancer pain pattern have the best cost-effectiveness.
