목 자세에 따른 선 자세에서의 신체균형능력 평가 (Effects of Abnormal Neck Posture on Postural Stability)

  • Postural instability can increase the likelihood of hazardous slip and fall accidents in workplaces. The present study intended to extend understanding of the effect of abnormal neck posture on postural control during quiet standing. The effect of body fatigue on the postural control was also of primary concern. Twelve healthy undergraduate students volunteered to participate in the experiment. Standing on a force platform with the neck neutral, flexed, extended, or rotated, subjects' center of pressures (COP) were measured under the two levels of body fatigue. For the fatigue condition, Subjects exercised in a treadmill to meet the predetermined level of body fatigue. Analyzing the position coordinates of COPs, the length of postural sway path was assessed in both medio-lateral (ML) axis and anterior-posterior (AP) axis. Results showed that, in AP direction, neck extension or rotation significantly increased the sway length as compared with neck neutral. Neck extension led to greater sway length compared to neck rotation. Neck flexion did not differ from neck neutral. The sway length in the AP direction also became significantly larger as the body fatigue accumulated after treadmill exercise. In ML direction, as compared to neutral posture, the neck extension, flexion, or rotation did not significantly affect the length of postural sway path. However, the sway length seemed to increase marginally with the neck extended during the fatigued condition. This study demonstrates that abnormal neck posture may interfere with postural control during standing. The ability to maintain postural stability decreases significantly with the neck extended or rotated. Body fatigue leads to postural instability further.

A Study on EMG Activation Changes of Spinal Stability Muscles during Forced Respiratory Maneuvers

  • Purpose: The purpose of this study was to investigate whether changes in electromyography (EMG) activations of spinal stability muscles with respiratory demand change were due to changes in respiratory demand or in postural demand. Methods: Forty healthy subjects (19male, 21female, $20.8{\pm}1.9$years old) performed quiet breathing and four different forced respiratory maneuvers (FRM) (Pulsed Lip Breathing, Diaphragmatic Breathing, Combination breathing, and respiratory muscle endurance training breathing) while in sitting and standing positions. EMG data for four muscles (TrA/IO, EO, RA, and ES) were collected and filtered using a band pass filter (20~200Hz) and a notch filter (60, 120, 180Hz). Results: There were no significant differences on percentage of change on %MVIC between QB and FRM (PLB, CB, DB, and RMET) between positions (all p>0.05).

ENVIRONMENTAL NOISE POLICIES AND NOISE CONTROL PRACTICE IN CHINA

  • In China, environmental noise policies are composed of correlated laws adopted by the Standing Committee of the National People's Congress and promulgated by the President of the country, regulations promulgated by the State Council and/or local government, standards issued by the Standardization Administration Committee (SAC) under the State Council. The laws mainly include the 'Environmental Protection Law' and the 'Law on Prevention and Control of Pollution From Environmental Noise' Regulations are often applied to a special noise pollution phenomenon of wide influence. They are generally only effective in a given area and/or a specific period. Tens of correlated standards specify the noise level limits of different functional zones of land use and of different equipment, machines, devices, appliances etc., and the measurement methods. In this presentation, a brief introduction to these policies and their operations is given and discussed. The conclusion is that the policies supply an effective legislative basis for environmental noise prevention and control in China, but still a lot of work should be conducted and completed to strive for a quiet society.

부분적 체중부하를 통한 트레드밀 훈련이 만성요통환자의 균형능력과 기능장애, 통증에 미치는 영향 (Effect of Partial Weight Supported Treadmill Training on Balance, Dysfunction and Pain in Patients With Chronic Low Back Pain)

  • Background: Patients with chronic low back pain (CLBP) functionally adapt to decreased postural control due to impaired processing of sensory information. Standing postural control has been the focus of recent research in CLBP. Change in postural control may be a risk factor for CLBP, although available studies are not conclusive. Objects: This study aimed to identify the role of partial weight supported treadmill training (PWSTT) in improving balance, dysfunction, and pain in patients with chronic low back pain. Methods: The study included 22 patients with CLBP. Patients in the control group ($n_1=8$) performed three 20 min stabilization exercise sessions per week, for 4 weeks. Patients in the full weight treadmill training group ($n_2=7$) performed treadmill training for 30 min after stabilization exercise. Patients in the PWSTT group ($n_3=7$) performed PWSTT with 20% of their body weight unloaded after stabilization exercises. By using the Biodex balance system, the dynamic balance abilities of the patients in the three groups were assessed in the quiet standing position under combined conditions of visual feedback (eyes open and closed) and platform stability (level 8). The Korean version of the Oswestry Disability Index and visual analogue scale score were used as the main measure. Results: The results of this study showed that dysfunction and pain were significantly improved in all groups. Although dynamic postural stability with eyes closed was significantly improved only in the PWSTT group (p<.05), no significant difference was found in the other groups. Conclusion: The results of this study indicate that PWSTT improved balance, dysfunction and pain in the patients with CLBP. Thus, this intervention is necessary for patients with CLBP with decreased postural control.

기립자세동안 전정기관에 인가된 정현파 전류자극에 대한 자세균형 응답 (A Response to Postural Response to Sine Curve Vestibular Electric Stimulation during Standing)

  • This study is vestibular electric stimulation applied between the mastoids during quiet standing elicits postural sway. The aim of this study was to characterize the postural sway response to continuous sinusoidal vestibular electric stimulation across various stimulus frequencies and amplitudes. Binaural bipolar sinusoidal vestibular electric stimulation was applied to the skin overlying the mastoid processes of 10 subjects while they stood on a force plate. The position of the center of pressure(COP) and signals at the feet are obtained on an force plate, while the head and whole body center of mass(COM) was measured with motion analysis system. The stimulus conditions included eight frequencies (1/64, 1/32, 1/16, 1/8, 1/4, 1/2, 1, and 2Hz) and six peak amplitudes (0.1, 0.25, 0.5, 0.7, 1 and 2mA). Each subject experienced one trial at each amplitude-frequency pair. The stimuli elicited sway in lateral plane in all subjects, as evidenced by changes in the stimulus frequency. Our results demonstrate that the vestibular system is sensitive to vestibular electric stimulation intensity changes and responds by altering the magnitude of the response accordingly.

The effect of lateral wedge on postural sway in Parkinson's disease

  • Background: Although postural instability is one of the major symptoms of Parkinson's disease (PD), dopaminergic treatment is ineffective for treating postural instability. Recent reports have shown that somatosensory deficit is associated with postural instability, and that somatosensory input improved postural instability. The purpose of this study is to evaluate the effects of lateral wedges for quiet standing postural control in people with PD. Methods: Twenty-two patients who were diagnosed with PD were enrolled in this study. The participants stood on a force plate under two conditions (wedge and no wedge) with or without having their eyes open or closed. The center of pressure (COP) range and velocity were analyzed using a two-way repeated-measures analysis of variance. Results: The range and velocity of COP in the anterioposterior and mediolateral (ML) directions were significantly improved after the patients stood on the lateral wedge with their eyes closed (p < 0.05). The range in ML direction and velocity in both directions of COP were significantly decreased when their eyes were open (p < 0.05). Conclusions: Regardless of vision, standing on lateral wedges improved postural sway in people with PD.

편마비 환자의 균형 기능과 감각조직화 (The Relationship Between Sensory Organization and Balance in Patients With Hemiplegia)

  • The ability to maintain an upright position during quiet standing is a useful motor skill. The Sensory Organization Test (SOT) is a timed balance test that evaluates somatosensory, visual, and vestibular function for maintenance of upright posture. The Fugl-Meyer Sensorimotor Assessment (FMSA) balance subscale is the functional status assessment that indicates amount of assistance needed during various balance tasks. Functional Independence Measure (FIM) is the functional status assessment tool and FIM can be used clinically as an outcome measure. The purpose of this study was to see if the SOT can be used as a evaluation tool to measure hemiplegic patients' balance ability. Thirty-six patients with hemiplegia participated in this study. SOT scores were significantly correlated with FMSA balance scores and FIM scores(p<0.05). However correlation coefficients were not so high (r=0.60, and r=0.51, respectively). Therefore, further study is needed to verify the SOT's usefulness when physical therapists are evaluating for hemiplegic patients' balance ability.

뇌졸중 환자의 라이프케어 증진을 위한 발바닥 감각역치와 균형의 상관관계 (Relationship between Plantar Tactile Sensory Thresholds and Balance for Life-Care Increase in Patients with Stroke)

  • 감각장애는 뇌졸중에서 흔히 나타나며, 발에서 나오는 촉각의 감각정보는 균형을 위해 중추신경계에 중요한 정보를 제공한다. 본 연구의 목적은 뇌졸중 환자의 발바닥 감각역치와 균형의 상관관계를 연구하는 것이다. Semmes-Weinstein Monofilaments를 사용하여 발바닥의(엄지발가락과 뒤꿈치) 감각역치를 평가하였다. 균형은 버그균형척도와 MTD system을 이용하여 정적 선 자세와 앉은 자세에서 일어서기 동안의 마비쪽과 비마비쪽의 체중 분포 정도를 측정하였다. 정적 선 자세와 앉은 자세에서 일어서기 동안 체중 분포 정도는 비마비쪽보다 마비쪽에서 더 낮았다(P<0.05). 버그균형척도는 36.00±1.53이었다. 마비쪽의 엄지발가락 감각역치는 버그균형척도(r=-0.444, P<0.05), 정적 선 자세(r=-0.332, P<0.05), 앉은 자세에서 일어서기(r=-0.390, P<0.05)와 상관관계가 있었다. 마비쪽의 뒤꿈치 감각역치는 버그균형척도(r=-0.467, P<0.05), 정적 선 자세(r=-0.532, P<0.05), 앉은 자세에서 일어서기(r=-0.516, P<0.05)와 상관관계가 있었다. 이러한 결과는 발바닥의 높은 감각역치가 균형 능력 저하에 영향을 미칠 수 있음을 시사한다. 앞으로 뇌졸중 환자의 감각 역치에 대한 연구는 뇌졸중 환자의 재활과 감각 평가에 도움이 될 것이다.

전신 피로와 시야 제한이 자세균형 능력에 미치는 영향 (Effects of Whole Body Fatigue and Limited Visual Field on Postural Stability)

  • Loss of postural stability can possibly lead to slip and fall accidents in the number of workplaces and everyday life. This study was aimed to examine the effects of whole body fatigue and partially limited visual field on the ability of maintaining postural balance during quiet standing. A group of twelve healthy male subjects participated in the experiment. Before and after experiencing the whole body fatigue induced by bicycling exercises, the position coordinates of subject's center of pressure (COP) were obtained under the two levels of visual field condition (i.e., open visual field and limited visual field). Four levels of the whole body fatigue examined were rest, 300watt, 600watt, and 900watt. Position coordinates of COPs measured on a force plate were then converted into the total length of postural sway path in both the medio-lateral (ML) direction and the anterior-posterior (AP) direction. Two-way ANOVA result showed that the length of sway path in the AP direction became significantly larger as the whole body fatigue accumulated. Post-hoc test revealed statistically significant differences between rest and 900watt and between 300watt and 900watt. The significant increase of the sway length was also found when the visual field was partially obstructed by the boxes. In the ML direction, however, there was no statistically significant difference of the postural sway in both the AP and ML directions. The results imply that the ability of maintaining postural stability can be reduced significantly due to such tasks along with whole body fatigue. The postural balance can also be impaired by the limited visual field.

납 앞치마 착용시 허리벨트가 자세근 활성도에 미치는 영향 (Effect of a Waist Belt on Postural Muscle Activity While Wearing a Lead Apron)

  • The purpose of this study was to compare the postural muscle activity during wearing a lead apron with and without applying waist belt at working posture. Ten healthy male subjects were recruited for this study. Electromyography using a surface EMG recorded the activity of the splenius capitis, trapezius, and erector spinae. EMG activity was recorded at quiet standing, 45 degrees of neck flexion, 45 degrees of neck flexion with 15 degrees of trunk flexion. The testing order was selected randomly. The subjects were asked to maintain the each posture for 3 minutes. The mean root mean square (RMS) of EMG activity was calculated. EMG activity was normalized using the maximum voluntary isometric contraction (MVIC) elicited using a manual muscle testing technique. Two-factor repeated measures analysis of variance (ANOVA) was used to compare the average RMS value of EMG activity for each condition. The EMG activity of trapezius muscle was significantly decreased with applying waist belt (p<.05). The muscle activity of splenius capitis and erector spinae showed significant difference according to postures (p<.05). These results suggest that applying waist belt during wearing a lead apron will be useful to prevent shoulder pain.
