Comparison of computer-based treatment prediction with true results in bimaxillary protrusion cases (치조성 양악 전돌 환자에서 컴퓨터를 이용한 치료 결과 예측의 정확성)

  • This study was performed to assess the accuracy of computer-based treatment prediction for soft tissue profile using Quick Ceph Image $Pro^{TM}\;&\;Quick\;Ceph\;2000^{TM}$ in bimaxillary protrusion cases. The Ore- and post-treatment lateral cephalograms of 21 female adults treated by low first premolar extraction were imaged and 9 landmarks and 27 specific soft tissue mesurements were digitized for comparing actual treatment results with computer simulations. The results of this study showed that Quick Ceph Image $Pro^{TM}\;&\;Quick\;Ceph\;2000^{TM}$ tends to overestimeate horizontal changes and underestimate vertical changes. In the computer simulation, upper lip showed rolling tendency. The upper lip measurements were disposed to be regular direction hut lower lip measurements were varied case by case even if it was statistically insignificant.

  • The purpose of this study was to test and compare the accuracy and reliability of soft tissue profile predictions generated from two computer software programs (Quick Ceph Image $Pro^{TM}$ (ver 3.0) and $V-Ceph^{TM}$(ver 3.5)) for mandibular set-back surgery. The presurgical and postsurgical lateral cephalograms of 40 patients (20 males and 20 females) were traced on the same acetate paper with the reference taken as the cranial base outline. The presurgical skeletal outlines were digitized onto each computer program and the mandible was moved to mimic the expected surgical procedure with reverence to the mandibular anterior border and lower incisor position of the actual postsurgical skeletal outline. The soft tissue profile was generated and the amount and direction of skeletal movement was calculated with each software. The predicted soft tissue profile was compared to the actual postsurgical soft tissue profile. There were differences between the actual and the predicted surgical soft tissue profile charges in the magnitude and direction, especially the upper lip. lower lip and the soft tissue chin (P<0.05). Quick Ceph had more horizontal measurement errors and thickness errors for the upper lip and lower lip, but V-Ceph had more vertical measurement errors of the lower lip (P<0.05). There was a positive correlation between the prediction errors and the amount of mandibular movements in the vertical position of Sn, the horizontal position of Ls and the upper lip thickness for V-Ceph, and there was a negative correlation in the horizontal position and the thickness of the lower lip for Quick Ceph (P<0.05). However all of the Prediction errors of both imaging softwares were ranged within 3mm, and this was considered to be allowable clinically.

  • The purpose of this study was to evaluate the accuracy of the soft tissue profile predicted by Quick Ceph Image Pro in mandibular setback surgery. Preoperative and postoperative lateral cephalograms of 24 patients(9 males and 15 females) who had completed treatment that involved orthodontics and mandibular setback by BSSRO were used. Computerized cephalometric lines and video image predictions were generated and compared with the actual postoperative results. In horizontal measurement, predicted values were smaller than actual measurements. And lips were thicker than actual values. Most of the different between values and actual measurement were than 2mm, and it was clinically acceptable.

Reproducibility of Lateral Cephalometric Landmarks According to Radiographic Image Enhancement (방사선상 enhancement 정도에 따른 측모두부방사선규격사진 계측점 설정의 재현도)

  • The purpose of this study was to evaluate the reproducibility of lateral cephalometric landmarks according to radiographic image enhancement, and to contribute to the identification of cephalometric landmarks. Lateral cephalograms of ten individuals were taken and stored into computer. The images were then enhanced up to four grades by Quick Ceph Image Pro$^{TM}$ on condition that the gray-scale equalization number was 50 and the detail enhancement number was 50. After thirty two landmarks were identified on monitor images by five observers, the deviations from the mean, the distances estimated between identified points and the mean point of five identified points, were evaluated for each landmark at each enhancement grade. Through the statistical analysis, following results were obtained. 1. In case of unenhanced radiographic images, the inter-observer reproducibility of the landmarks showed a large variation. 2. The comparison of deviation from the mean according to the degree of radiographic image enhancement for each landmark showed that the inter-observer reproducibility was significantly different at 5 landmarks. 3. The landmark of pterygomaxillary fissure showed higher reproducibility at enhancement grade 1 and 2 images than at unenhanced images. So did the landmark of posterior nasal spine at enhancement grade 1 images, and the landmark of menton at enhancement grade 2, 3 and 4 images respectively. The above results suggest that the reproducibility of some landmarks can be increased by radiographic image enhancement during the identification of the lateral cephalometric landmarks on the monitor.

The correlation between dental compensation and craniofacial morphology in skeletal Class III malocclusion (골격성 III급 부정교합자의 치성보상과 두개안면골격의 상관관계에 관한 연구)

  • This investigation was designed to analyze the degree of dental compensation according to horizontal components of craniofacial skeleton and to investigate correlation between dental compensation and craniofacial pattern in skeletal class III malocclusion. The material selected for this study consisted of standard lateral cephalogram of 59 subjects in normal occlusion group, 91 subjects in mild skeletal class III malocclusion group and 58 subjects in severe skeletal class III malocclusion group. The mild skeletal class III malocclusion group was divided into two groups, one was class III malocclusion without anterior crossbite group and the other was class III malocclusion with anterior crossbite group. The data were analyzed by Quick-ceph image program. The results were as follows. 1. Mild skeletal class III malocclusion without anterior crossbite group showed the most labial inclination of upper incisors, followed by severe skeletal class III malocclusion group and mild skeletal class III malocclusion with anterior crossbite group, the Latter showing the least. The amount of lingual inclination of lower incisors was the largest in severe skeletal class III malocclusion group, and there was no statistically significant difference between mild skeletal claw III malocclusion without anterior crossbite group and mild skeletal class III malocclusion with anterior crossbite group. 2. There were little differences in vertical skeletal structure between mild skeletal class III malocclusion without anterior crossbite group and mild skeletal class III malocclusion with anterior cwssbite group, they showed statistically significant differences in the upper incisors measurements. 3. The measurements of lower incisors in mild skeletal class III malocclusion without anterior crossbite group and upper incisors in mild skeletal class III malocclusion with anterior crossbite group represented a high correlation with skeletal structure. Especially, ∠IMPA and ∠FMIA of lower incisor measurements, and ∠U1-FH ∠U1-SN of upper incisor measurements showed high correlation with skeletal structure in each group. 4. ∠IMPA and ∠FMIA of lower incisor measurements showed high correlation with skeletal structure in all groups. ∠U1-FH, ∠U1-SN and U1-facial plane(mm) of upper incisor measurements represented higher correlation with skeletal structure than any other upper incisor measurements.

THE STDUY OF THE RELAPSE OF HARD AND SOFT TISSUE AFTER MAXILLARY PROTRACTION (상악골 전방견인 후 경조직과 연조직의 재발에 관한 연구)

  • The purpose of this stdudy was to evaluate the effect of maxillary protraction and the relapse of hard and soft tissue after maxillary protraction. For this study 29 patients who were treated with maxillary protractor and labiolingual archwire were selected. Their mean age was 9 years 4 months and mean treatment period was 8.5 months. Lateral cephalograms were taken at pretreatment, immediately after treatment and one to three months after removal of the maxillary protractor. They were traced on skeletodental and soft tissue structures based on Burstone's analysis and analyzed by Quick-Ceph Image Digitizing System(ORTHODONTIC PROCESSING). The mean and standard deviation between pretreatment and posttreatment and between posttreatment and retention period for each cephalometric variable were calculated. Student t-test was used to determine the statistical significance of the changes in each variable. Correlation coefficients between hard tissue and soft tissue were used to determine interrelationship. The results were as follows. 1. After maxillayy protraction, the maxilla and maxillary dentition moved antero-inferiorly, the mandibld and mandibular dentition moved postero-interiorly and palatal plane rotated antero-superiorly by $0.59^{\circ}$. 2. After maxillary protraction, the soft tissue of upper lip moved antero-interiorly with the movement of hard tissue but the antero-posterior position of lower lip was stable in spite of the change of hard tissue. The thickness of upper lip was decreased and that of lower lip was increased after maxillary Protraction. 3. During the retention period, the position of jaws was relatively stable but upper and lower anterior teeth and antero-superiorly rotated palatal plane relapsed to original position. 4. During the retention period, the soft tissue of lips was stable antero-posteriorly and moved mote inferiorly than posttreatment. 5. The correlation coefficients between the postion of upper and lower incisal edge and that position of lips were high, especially in horizontal change.
