Results of Radiotherapy for the Uterine Cervical Cancer (자궁경부암의 방사선치료성적)

  • One hundred fifty-four patients with the carcinoma of the uterine cervix were studied retrospectively to assess the result and impact of treatment at Department of Radiation Oncology, Korea University, Hae-Wha Hospital from Feb 1981 through Dec. 1986. Prior to radiotherapy, the patients were evaluated and staged by recommendation of FIGO including physical examination, pelvic examination, cystoscopy, rectosigmoidoscopy, chest X-ray, IVP. Ba enema. Also, an additional pelvic CT scan was obtained for some of the patients. The patients were treated by radiotherapy alone or adjuvant postoperative irradiation; in case of radiation therapy only, whole pelvic irradiation was given with Co-60 teletherapy unit via AP and PA parallel opposing fields or 4-oblique fields, 180 cGy per day, 5 days per week and intracavitary insertion was performed. In satges Ia, Ib, and IIa with small primary lesion, external irradiation was initially given to pelvis up to $2,000\~3,000\;cGy/2frac{1}{2}\;-3frac{1}{2}$ weeks and then intracavitary insertion was performed using Fletcher-Mini-Declos Applicator with cesium-137 cources and followed by external irradiation of $1,000\~2,000\;cGy/1frac{1}{2}\;-2frac{1}{2}$weeks via AP and PA parallel opposing fields with midline shield to spare of bladder and rectum. However, if the primary lesion is large, external irradiation was given without midline shield. More than stages IIb, the patients were treated by external beam irradiation up to 5,400cGy/30f for 6 weeks via 4-oblique portals and at the dose of 5,040cGy/28f the field was cut 5cm from the top margin for spare of small bowel, and followed by intracavitary irradiation, If there was residual tumor an additional dose of $900\~l,200cGy/5\~7f$ was given to parametrium and/or residual tumor area. Total dose of radiation to A and B-point were as follows; A-point; In early stages, Ia, Ib, IIa; $8,000\~9,000$ B-point $5,000\~6,000 cGy$ A-point; In advanced stages IIb, IIIa, IIIb; $9,000\~10,000$ B-point $60,000\~7,000cGy$ The results were obtained and as fellows; 1 The patients distribution according to FIGO staging system were stage Ia 6, Ib 27, IIa 28, IIb 54, IIIa 12, IIIb 18, and stage IVa 9. 2. Value of CT scan were demonstration of cervix tumor mass, parametrial and pelvic side wall tumor spread, pelvic and inguinal lymph nodes metastases, and hydronephrosis. Three dimensional quantitative demonstration of tumor volume is also important in planning radiation therapy. Another advantage of CT scan was detection of recurrent tumor after radiation or surgery. 3. Local control rate of tumor according to the size was $91.3\%$ for less than 5cm in size and $44.6\%$ in tumor over 5cm (p<0.0068). 4. Thirty out of 50 recurrent sites has locoregional failures and 17 cases has distant metastases. And the para-aortic lymph nodes were the most common site for distant metastases. 5. The most common complication was temporal rectal bleeding which was controlled most by conservative management. However, 4 patients required for endoscopic cauterization. 6. The 5-year survival rates showed; stage la and Ib $95\%,\;stage\;IIa\;81\%\;stage\;lIb\;67\%,\;stage\;IIIa\;37.7\%,\;stage\;IIIb\;23\%$ and 3-year survival rate of stage IVa showed $11.6%$, retrospectively.

Dosimetric Characteristics of a Thermal Neutron Beam Facility for Neutron Capture Therapy at HANARO Reactor (하나로 원자로 BNCT 열중성자 조사장치에 대한 선량특성연구)

  • A thermal neutron beam facility utilizing a typical tangential beam port for Neutron Capture Therapy was installed at the HANARO, 30 MW multi-purpose research reactor. Mixed beams with different physical characteristics and relative biological effectiveness would be emitted from the BNCT irradiation facility, so a quantitative analysis of each component of the mixed beams should be performed to determine the accurate delivered dose. Thus, various techniques were applied including the use of activation foils, TLDs and ionization chambers. All the dose measurements were perform ed with the water phantom filled with distilled water. The results of the measurement were compared with MCNP4B calculation. The thermal neutron fluxes were $1.02E9n/cm^2{\cdot}s\;and\;6.07E8n/cm^2{\cdot}s$ at 10 and 20 mm depth respectively, and the fast neutron dose rate was insignificant as 0.11 Gy/hr at 10 mm depth in water The gamma-ray dose rate was 5.10 Gy/hr at 20 mm depth in water Good agreement within 5%, has been obtained between the measured dose and the calculated dose using MCNP for neutron and gamma component and discrepancy with 14% for fast neutron flux Considering the difficulty of neutron detection, the current study support the reliability of these results and confirmed the suitability of the thermal neutron beam as a dosimetric data for BNCT clinical trials.

  • Purpose: SLE (systemic lupus erythematosus) is an inflammatory autoimmune disease, characterized by various autoantibody. The detection of Anti double-stranded DNA (Anti-ds DNA) is important in the diagnostics of SLE, and include the American College of Rheumatology (ACR) diagnostic criteria for SLE. Also SLE disease activity and correlativity with the level Anti-ds DNA antibody have been reported and Anti-ds DNA antibody quantitative test is very useful for tracing before and after SLE treatment. When These Anti-ds DNA antibody test (Farr assay: $^{125}I$ labeled ds-DNA and bound Anti-ds DNA antibodies complex in serum is precipitated by ammonium sulfate and used to centrifugation, measured it) inhaled supernatant after centrifugation, a lipemic specimen does not facilitate the formation of precipitate and also occurs situation was inhaled with precipitate. To solve these problems, The Influence of the degree of lipemic specimen was evaluated. Materials and Methods: September 2012 to February 2013, We selected lipemic samples (n=81) of specimen commissioned by Anti-ds DNA antibody test. Lipemic samples were done pre-treatment (high-speed centrifugation: 14,000 rpm 5 mins) used a micro-centrifuge (Eppendorf Model 5415D). At the same time lipemic specimen and pre-treatment samples were performed Anti-ds DNA antibody test (Anti-ds DNA kit, Trinity Biotech, Ireland). Statistical analysis were analyzed Pearson's correlation coefficients and regression and paired t-test, and Difference (%). Results: Experimental group 1 (Lipemic Specimen Anti-ds DNA Ab concentration ${\leq}7IU/mL$) at y=0.368X+4.732, $R^2=0.023$, Pearson's correlation coefficient was 0.154, paired t-test (P=0.003), Difference (%) mean 65.7 and showed a statistically significant difference. Experimental group 2 (Lipemic Specimen Anti-ds DNA Ab concentration ${\geq}8IU/mL$) at y=0.983X+0.298, $R^2=0.994$, Pearson's correlation coefficient showed 0.997, paired t-test (P=0.181), Difference (%) mean -5.53 made no statistically significant difference. Conclusion: Lipemic sample of low Anti-ds DNA Ab concentration (2.5-7 IU/mL) and the result is obtained pre-treatment (high-speed centrifugation: 14,000 rpm 5 mins) were made a significant difference statistically. Anti-ds DNA is one of the primary auto-antibodies present in patients with SLE, and remain an important diagnostic test for SLE. Therefore, we recommend preprocessing (high-speed centrifugation: 14,000 rpm 5 mins) in order to exclude the influence of lipemic specimen.

Monitoring of Benzoic Acid, Sorbic Acid, and Propionic Acid in Spices (향신료에서 유래되는 안식향산, 소브산, 프로피온산의 함유량 조사)

  • In this study, we investigated the levels of natural preservatives of benzoic acid, sorbic acid, and propionic acid in spices. The quantitative analysis was performed using high performance liquid chromatography-tandem mass spectrometry (LC-MS/MS) for benzoic acid and sorbic acid and gas chromatography-mass spectrometry (GC-MS) for propionic acid. The sample was extracted with ethanol using sonication, then centrifuged and evaporated to dryness and redissolved to 1 mL with ethanol to use for the instrumental analysis. The analytical method was validated based on linearity, recovery, limit of detection (LOD), and limit of quantification (LOQ). This method was suitable to determine low amounts of naturally occurring preservatives (benzoic acid, sorbic acid, and propionic acid) in various spices. Benzoic acid, sorbic acid, and propionic acid were found in 165 samples, 88 samples, and 398 samples, respectively from the total of 493 samples. The concentration of benzoic acid, sorbic acid, and propionic acid were ranged at ND-391.99 mg/L, ND-57.70 mg/L, and ND-188.21 mg/L in spices, respectively. The highest mean levels of benzoic acid, sorbic acid, and propionic acid were found in cinnamon (167.15 mg/L), basil leaves (22.79 mg/L), and white pepper (51.48 mg/L), respectively. The results in this study provide ranges of concentration regarding naturally occurring benzoic acid, sorbic acid, and propionic acid in spices. Moreover, the results may use to the case of consumer complaint or trade friction due to the inspection services of standard criteria for the preservatives of spices.

Urban Climate Impact Assessment Reflecting Urban Planning Scenarios - Connecting Green Network Across the North and South in Seoul - (서울 도시계획 정책을 적용한 기후영향평가 - 남북녹지축 조성사업을 대상으로 -)

  • When making urban planning, it is important to understand climate effect caused by urban structural changes. Seoul city applies UPIS(Urban Plan Information System) which provides information on urban planning scenario. Technology for analyzing climate effect resulted from urban planning needs to developed by linking urban planning scenario provided by UPIS and climate analysis model, CAS(Climate Analysis Seoul). CAS develops for analyzing urban climate conditions to provide realistic information considering local air temperature and wind flows. Quantitative analyses conducted by CAS for the production, transportation, and stagnation of cold air, wind flow and thermal conditions by incorporating GIS analysis on land cover and elevation and meteorological analysis from MetPhoMod(Meteorology and atmospheric Photochemistry Meso-scale model). In order to reflect land cover and elevation of the latest information, CAS used to highly accurate raster data (1m) sourced from LiDAR survey and KOMPSAT-2(KOrea Multi-Purpose SATellite) satellite image(4m). For more realistic representation of land surface characteristic, DSM(Digital Surface Model) and DTM(Digital Terrain Model) data used as an input data for CFD(Computational Fluid Dynamics) model. Eight inflow directions considered to investigate the change of flow pattern, wind speed according to reconstruction and change of thermal environment by connecting green area formation. Also, MetPhoMod in CAS data used to consider realistic weather condition. The result show that wind corridors change due to reconstruction. As a whole surface temperature around target area decreases due to connecting green area formation. CFD model coupled with CAS is possible to evaluate the wind corridor and heat environment before/after reconstruction and connecting green area formation. In This study, analysis of climate impact before and after created the green area, which is part of 'Connecting green network across the north and south in Seoul' plan, one of the '2020 Seoul master plan'.

Genetic Analysis of Natural Microflora in the Stored Joraengyi Rice Cake and Their Capability of Propionic Acid Production (조랭이 떡에 존재하는 자연균총 유전자 군집분석 및 천연유래 프로피온산 생성능 분석)

  • This study was conducted to analyze the microbial community and propionic acid production ability of natural microflora in the rice cakes. Genetic analysis of natural microflora in Jorangyi rice cake was performed to select propionic acid - producing bacteria. Selected propionic acid-producing bacteria were cultivated in TSB (tryptic soy broth) supplemented with glucose, and growth characteristics were analyzed by temperature and production of propionic acid was analyzed by gas chromatography (GC-FID). Linearity, detection limit, quantitative limit, and recovery rate were measured to verify propionic acid assay. A total of 98 microbial strains were detected from microflora of Joraengyi rice cake that grew after expiration of shelf life. Lactobacillus casei group accounted for 50.48% and Lactobacillus buchneri was 29.60%. Propionic acid - producing bacteria were Propionibacterium thoenii, P. cyclohexanicum, Propionibacterium_uc, P. jensenii, and P. freudenreichii. Natural bacteria and Lactobacillus spp. did not produce propionic acid during 14 days but P. cyclohexanicum, P. freudenreichii subsp. Shermanii, P. thoenii and P. jesenii produced $263.47{\mu}g/mL$, $338.90{\mu}g/mL$, $325.43{\mu}g/mL$ and $222.17{\mu}g/mL$ during 4 days and 2,462.02 and 2,904.78, 2,220.64, $3,519.17{\mu}g/mL$ during 14 days. As a result of this study, it was affirmed that the natural microflora of Joraengyi rice cake during storage can produce propionic acid from natural sources even if a high concentration of propionic acid is not intentionally added. Because of characteristics of rice cake composed of starch and glucose. This study will be used as a recognition criterion to detect natural preservatives such as propionic acid in starchy foods such as rice cakes and as reference standard safety management data.

DETECTION OF EARLY PROXIMAL CARIES WITH LASER FLUORESCENCE (레이저 형광법을 이용한 인접면 우식증의 진단)

  • Artificial carious lesions in various depths were observed with visual examination using light transillumination, bite-wing radiography, laser fluorescence, and dye-enhanced laser fluorescence to determine the reproducibility, correlation of each diagnostic method, diagnostic sensitivity and diagnostic specificity. And optical densities according to demineralized times were measured whether laser fluorescence could be used as a quantitative diagnostic method. The following results were obtained whether laser fluorescence could be used for diagnosis of initial proximal caries. 1. Tau-c values of visual examination was 0.08 which showed lowest reproducibility and those of bite-wing radiography, laser fluorescence, dye-enhanced laser fluorescence were 0.60, 0.48, and 0.64, respectively which showed relatively high reproducibility. 2. The correlation between demineralization time and each examination was the highest in dye-enhanced laser fluorescence$({\gamma}=0.51)$ followed by laser fluorescence$({\gamma}=0.43)$, bite-wing radiograph$({\gamma}=0.35)$, and visual examination$({\gamma}=0.33)$. Dye-enhanced laser fluorescence and laser fluorescence showed significant correlation with demineralization time. 3. The sensitivity of laser fluorescence and dye-enhanced laser fluorescence for diagnosing approximal caries based on bite-wing radiography were 67%, 100% and those of specificity were 57%, 11% which showed diagnostic specificity was relatively lower than sensitivity. 4. The difference in optical density(DFR) between sound teeth and carious lesions according to lesion depth was high with dye-enhanced laser fluorescence compared with laser fluorescence. DFR measured with laser fluorescence according to changes in lesion depth was statistically significant but was not statistically significant with dye-enhanced laser fluorescence. Based on these results, laser fluorescence and dye-enhanced laser fluorescence have comparable diagnostic power as bite-wing radiography in early diagnosis of proximal caries.

Homologous and Heterologous Antibody Response of the Patients with Aspergillosis Against Young Mycelia of Aspergilli by Fluorescence Antibody Reaction (형광항체반응을 이용한 Aspergillus 증 환자의 균사표면항원에 대한 항체반응 양상에 관한 연구)

  • Detection of antibody against pathogenic fungi in serum specimens of the patients with pulmonary tuberculosis or other lung diseases has been carried out(male) using the indirect fluorescence antibody technique and immunodiffusion tests. Immunodiffusion tests revealed that 104(36.5%) out of 285 patients examined showed a positive precipitin reaction against one or more of fungal antigens. The majority of ID positive patients 64(61.5%) reacted with Aspergillus fumigatus antigen and 49(47.1%) patients reacted with Candida albicans antigen ID positive reaction to A. fumigatus was found little more frequently among male patients, while Candida albicans reactors were found more frequently among female patients. Age distribution of ID positive reactors was high(49.1-43.3%) in age group of 40-59 years, but least or none in age group of less than 30 years. Age of fungal mycelium used as antigen did not effect sensitivity of the indirect flubrescence (IF) technique in detecting antibody to A. fumigatus. Antibody class against A. fumigatus that showed highest titer was IgG and thus FITC labeled anti-IgG immunoglobulin shoul be preferable. As relatively large amount of cell wall components of Aspergilli shared antigenically, a considerable cross-reaction was observed among A. fumigatus, A. flavus and A. niger, but not much with C. albicans. While (IF) has much better sensitivity when compared with ID, relative specificity of the latter procedure cannot to be overried, so that they could be batter used together in order to obtain quantitative measurement of antibody with relative specificity.

The Effect of Antioxidant Vitamins on Aflatoxin $B_{1}-DNA$ Adduct the Formation in Aflatoxin $B_1$ Administered Mice Liver (항산화비타민이 Aflatoxin $B_1$ 투여 마우스의 간세포에서 Aflatoxin $B_{1}-DNA$ Adduct 형성에 미치는 영향)

  • The objective of this study was to examine the effects of antioxidant vitamins on the formation of $AFB_{1}-DNA$ adduct and $AFB_{1}-inducing$ cellular oxidative damage. Intraperitoneal(i.p.)injections of 10 mg/kg vitamin C(VC) and 63.8 mg/kg vitamin E(VE) were repeatedly administrated 4 times with 2 days interval to 6 week old male ICR mice. After one hour of vitamin treatments, 0.4 mg/kg $AFB_1$ was injected in $AFB_1$ plus vitamin treated groups by same way. On the other hands, $AFB_1$ treated group was only injected with $AFB_1$ by the same method described above without vitamins. According to quantitative analysis of the $AFB_1$ in mice serum by indirect competitive ELISA, 12.28 and 18.78 ng/mL were detected in $AFB_1-treated$ groups, but 7.60 and 4.85 ng/mL in $AFB_1$ plus VC and VE treated groups, respectively. 23.78, 25.48 ng/mL of $AFB_1-DNA$ adduct were detected in mice liver of $AFB_1$treated groups, while 5.26, 7.81 ng/mL in $AFB_1$ plus VC and VE treated groups, respectively. Consequently, the differences in the concentrations of $AFB_1$ related materials between vitamin treated and non-treated groups were significant. Immunohistochemistry revealed brownish infiltration of $AFB_1$ around central vein and sinusoid in $AFB_1-treated$ group. This manifestation was distinctly reduced in $AFB_1$ plus VC and VE treated groups.

Comparison of Population Monitoring Methods for Breeding Forest Birds in Korean Temperate Mixed Forests (국내 온대 혼효림에 서식하는 산림성 조류의 번식기 개체군 모니터링 방법에 대한 비교)

  • Birds are effective ecological indicators but there is no national protocol in place to monitor population dynamics of forest birds in Korea. To support the establishment of future monitoring protocols, we compared the results of two generally used monitoring methods for forest bird surveys in two temperate mixed forests in central Korea. There was no statistical difference in the number of species and individuals detected per unit survey effort when comparing line transects and point counts. The number of species and individuals were higher in a five-minute count than in a three-minute point count, but the total accumulated number of expected observed species showed no difference between the two count durations. The number of observed species and individuals increased in both methods as plot radius or transect width increased, suggesting that multi-layer or multi-band surveys may be useful for quantitative and qualitative objectives. The decreasing number of observed species and individuals after sunrise suggested that bird monitoring should be conducted earlier in the morning, within four hours after sunrise. To detect 70% of the total number of species, 7.0 to 7.6 survey hours, equivalent to 42 three-minute counts (95% confidence interval [CI]: 26 to 61) or 33 five-minute counts (95% CI: 19 to 53) were needed for unlimited radius point counts. On the other hand, 4.8 survey hours, equivalent to 26 line transect counts (95% CI: 15 to 45) using 200-m transects with unlimited width, were required to achieve the same level of species detection. Therefore, the line transect method may be more effective than the point count method, at least in terms of local species richness assessment. For national forest bird monitoring, our data indicated that one or both survey methods can be selected as a basic protocol, based on the goals and scales of monitoring, forest types, and the conditions of the target areas.
