Although 3.0T magnetic resonance imaging (MRI) has the advantages of a higher signal to noise ratio (SNR) and contrast than 1.5T MRI, there are limitations on the contrast between white and grey matter because of the long T1 recovery time when T1 images are obtained using the Spin Echo Technique. To overcome this, T1 weighted images are obtained occasionally using the inversion recovery (IR) technique, which employs a relatively long TR. The aim of this study was to determine the optimal TI in a brain examination when a T1 weighted image is obtained using the IR technique. Eight participants (male: 7, female: 1, average age: $34{\pm}14.11$) with a normal diagnosis were targeted from February 18, 2012 to February 27, 2012, and the contrast between white and grey matter as well as the contrast to noise ratio (CNRs) in each participant were measured. The CNRs of white matter and grey matter were highest at TI = 600, 650, 750, 900, 1050 and 1100 ms when the TR was 1100, 1400, 1700, 2000, 2300 and 2600 ms, respectively. Therefore, as the TIs were $44.425{\pm}0.877%$ of the TRs in the TR range of 1400-2300 ms, the optimal T1 weighted images that describe the contrast between white and grey matter can be obtained if the TIs are compensated for with $44.425{\pm}0.877%$ of the TRs in the time of setting TIs.
상완신경총에서 STIR(short TI inversion recovery) 지방소거 강조영상과 SPAIR(spectral adiabatic inversion recovery) T2, T1 지방소거 강조영상기법을 비교 평가하여 최적의 지방소거 기법을 알아보고자 하였다. 총 30 명 상완신경총에 질환이 없는 정상인 지원자를 대상으로 3.0 T MRI 기기를 이용하여 검사를 하였고, 분석방법으로 세 기법에 대하여 SNR, CNR 및 4단계 점수로 평가를 하였다. 정량적 분석 결과로 SPAIR T1 기법에 대한 SNR, CNR 값은 상완신경총기시부($03.07{\pm}0.98$, $-2.25{\pm}0.54$), 상완신경총몸체($06.70{\pm}1.81$, $36.31{\pm}2.17$)에서 높은 값을 제공하였다 (p<0.05). 정성적 분석결과 상완신경총 묘출도와 지방소거 정도, 영상의 인공물은 SPAIR T1($3.2{\pm}0.70$, $3.6{\pm}0.51$, $3.4{\pm}0.10$)기법이 의미 있게 높게 나타났다(p<0.05).
Objective : The aim of this study was to evaluate the morphometric changes in neuroforamen in grade I isthmic spondylolisthesis by anterior lumbar interbody fusion [ALIF]. Methods : Fourteen patients with grade I isthmic spondylolisthesis who underwent single level ALIF with percutaneous pedicle screw fixation were enrolled. All patients underwent standing lateral radiography and magnetic resonance imaging [MRI] before surgery and at 1 week after surgery. For quantitative analysis, the foraminal height, width, epidural foraminal height, epidural foraminal width, and epidural foraminal area were evaluated at the mid-portion of 28 foramens using T2-weighted sagittal MRI. For qualitative analysis, degree of neural compression in mid-portion of 28 foramens was classified into 4 grades using T2-weighted sagittal MRI. Clinical outcomes were assessed using Visual Analogue Sale [VAS] scores for leg pain and Oswestry disability index before surgery and at 1 year after surgery. Results : The affected levels were L4-5 in 10 cases and L5-S1 in 4. The mean foraminal height was increased [p<0.001], and the mean foraminal width was decreased [p=0.014] significantly after surgery. The mean epidural foraminal height [p<0.001], epidural foraminal width [p<0.001], and epidural foraminal area [p<0.001] showed a significant increase after surgery. The mean grade for neural compression was decreased significantly after surgery [p<0.001]. VAS scores for leg pain [p=0.001] and Oswestry disability index [p=0.001] was decreased significantly at one year after surgery. Conclusion : Foraminal stenosis in grade I isthmic spondylolisthesis may effectively decompressed by ALIF with percutaneous pedicle screw fixation.
A PET-MR system is particularly useful in diagnosing brain diseases. We have developed a prototype positron emission tomography (PET) system which can be inserted into the bore of a whole-body magnetic resonance imaging (MRI) system that enables us to obtain PET and MRI images simultaneously with a reduced cost. Silicon photomultipliers (SiPM) are appropriated as a PET detector at PET/MR system because detectors have a high gain and are insensitive to magnetic fields. Despite of its improved performance compared to that of PMT-based detectors, there is a problem of the photo-peak channel shift which is due to the increase of the temperature inside the ring detector. This problem will occur decreasing sensitivity of the PET and image distortion. In this paper, I quantitative analyze parameters of the KAIST SiPM depending on temperature by experiments. And I designed cooling methods in consideration of the degradation of sensors for correction of the temperature in the PET gantry. According to this research, we expect that distortive images and degradation of the sensitivity will not be occurred with using the above idea to reduce heat even if the PET system operates for a long time.
Background: This study aimed to compare the subscapularis muscle volume between the intact groups (group I) and supraspinatus tendon tear groups (group T) based on the sex and three different age groups. Methods: Subjects with a group I and subjects with group T without any other lesions were retrospectively evaluated from among patients who received a magnetic resonance imaging (MRI) scan between January 2011 and December 2013. The MRI scans were studied by a consultant radiologist. The subscapularis muscle volume was compared according to the age and sex; the age groups were categorized as patients in their 40s, 50s, and 60s. The volume of subscapularis muscle was measured by three-dimensional reconstructed images acquired through the axial section of 1.5T MRI. Results: No statistically significant differences were observed between subscapularis muscle volume of the group I and group T, except for male patients in their 50s (group I: $100,650mm^3$ vs. group T: $106,488mm^3$) and 60s (group I: $76,347mm^3$ vs. group T: $99,549mm^3$) (p<0.05). Males had a larger mean volume of subscapularis muscle than females, and the subscapularis muscle volume decreased in a linear manner with increasing age. Conclusions: Decrease in subscapularis muscle volume was observed with increasing age, and the impact of supraspinatus tear on subscapularis muscle volume is age and sex dependent.
삼각 섬유성 연골(TFCC) 손상 환자의 평가를 위하여 fat suppressed 3D fast spoiled gradient recalled T1 기법과 fat suppressed Isotropic 3D fast spin echo T1 기법을 이용하여 영상을 획득하였다. 정량적 평가를 위해 각각의 영상에서 signal to noise ratio 및 contrast to noise ratio 값을 측정하고, Mann-Whitney U 검정으로 두 기법 간의 통계적 유의성을 검증하였다. 또한 정성적 평가를 위해 영상의학의 2명이 각각의 영상을 관찰하여, TFCC의 형태, 영상 내 인공음영, 병변의 묘사 정도의 3가지 항목을 선정한 후 이를 4점 척도(0: 진단 불가, 1: 부족함, 2: 충분함, 3: 좋음)로 평가하였고, Kappa-value 검정을 이용하여 두 관찰자 간의 일치도 검증을 하였다. 영상획득에는 3.0 Tesla MR 장비와 8-channel RF coil을 사용하였다. 정량적 평가 결과, 모든 영상 단면에서 signal to noise ratio 및 contrast to noise ratio 값이 Isotropic 3D fast spin echo T1 기법이 높게 나타났으며, Mann-Whitney U 검정을 이용한 두 영상기법 간의 검증도 통계적으로도 유의하였다(p < 0.05). 정성적 평가 결과, 관찰자 1, 2 모두에서 Isotropic 3D fast spin echo T1 기법의 평가 결과가 더 높게 나타났으며, Kappa-value 검증을 이용한 두 관찰자 간 평가 결과의 일치도 검증도 통계적으로 유의하였다(p < 0.05). 결론적으로 TFCC 손상 환자의 자기공명영상 검사 시 fat suppressed Isotropic 3D fast spin echo T1 기법의 적용은 TFCC 병변의 감별에 보다 유용한 진단적 정보를 제공 할 수 있을 것이라 생각된다.
본 연구의 목적은 악성 및 양성 여성골반 병변에서 자기공명 확산강조영상의 정량적, 정성적평가을 통하여 최적의 확산경사계수(b-values) 값을 구하고, 임상적 유용성을 알아보고자 하였다. 확산강조영상의 b-value($s/mm^2$)값을 600, 800, 1000, 1200, 1400까지 변화시키면서 얻었다. 정량적 분석결과 b-values 값 중 800에서 자궁근종의 평균 신호대 잡음비 $84.6{\pm}4.57$(p=0.024) 와 대조도대 잡음비 $50.13{\pm}5.47$(p=0.028)로 가장 높았고, 자궁경부암의 평균 신호대 잡음비 $12.0{\pm}2.04$(p=0.047)와 대조도대 잡음비 $10.6{\pm}1.24$(p=0.001)로 가장 높았으며, 이때 현성확산계수는 자궁근종 $1.19{\times}10^{-3}$(p=0.008), 자궁경부암 $0.96{\times}10^{-3}$(p=0.027) 이었다. 정성적 분석결과로서 병변의 묘출도와 명확성은 $4.02{\pm}0.18$(p=0.028), $4.39{\pm}0.25$(p=0.015)로 800에서 가장 높았다. 결론적으로 확산강조 자기공명영상은 여성골반 병변 검출에 있어서 중요한 방법이고, 최적의 b-value($s/mm^2$)값은 $800s/mm^2$ 이었으며, 정량적, 정성적 평가는 여성골반의 악성 및 양성 감별진단에 도움을 주었다.

Objective: Cyclin-dependent kinase inhibitor (CDKN)2A/B homozygous deletion is a key molecular marker of isocitrate dehydrogenase (IDH)-mutant astrocytomas in the 2021 World Health Organization. We aimed to investigate whether qualitative and quantitative MRI parameters can predict CDKN2A/B homozygous deletion status in IDH-mutant astrocytomas. Materials and Methods: Preoperative MRI data of 88 patients (mean age ± standard deviation, 42.0 ± 11.9 years; 40 females and 48 males) with IDH-mutant astrocytomas (76 without and 12 with CDKN2A/B homozygous deletion) from two institutions were included. A qualitative imaging assessment was performed. Mean apparent diffusion coefficient (ADC), 5th percentile of ADC, mean normalized cerebral blood volume (nCBV), and 95th percentile of nCBV were assessed via automatic tumor segmentation. Logistic regression was performed to determine the factors associated with CDKN2A/B homozygous deletion in all 88 patients and a subgroup of 47 patients with histological grades 3 and 4. The discrimination performance of the logistic regression models was evaluated using the area under the receiver operating characteristic curve (AUC). Results: In multivariable analysis of all patients, infiltrative pattern (odds ratio [OR] = 4.25, p = 0.034), maximal diameter (OR = 1.07, p = 0.013), and 95th percentile of nCBV (OR = 1.34, p = 0.049) were independent predictors of CDKN2A/B homozygous deletion. The AUC, accuracy, sensitivity, and specificity of the corresponding model were 0.83 (95% confidence interval [CI], 0.72-0.91), 90.4%, 83.3%, and 75.0%, respectively. On multivariable analysis of the subgroup with histological grades 3 and 4, infiltrative pattern (OR = 10.39, p = 0.012) and 95th percentile of nCBV (OR = 1.24, p = 0.047) were independent predictors of CDKN2A/B homozygous deletion, with an AUC accuracy, sensitivity, and specificity of the corresponding model of 0.76 (95% CI, 0.60-0.88), 87.8%, 80.0%, and 58.1%, respectively. Conclusion: The presence of an infiltrative pattern, larger maximal diameter, and higher 95th percentile of the nCBV may be useful MRI biomarkers for CDKN2A/B homozygous deletion in IDH-mutant astrocytomas.
Chemical Exchange Saturation Transfer (CEST) imaging is a method to detect solutes based on the chemical exchange of mobile protons with water. The solute protons exchange with three different patterns, which are fast, slow, and intermediate rates. The CEST contrast can be obtained from the exchangeable protons, which are hydroxyl protons, amine protons, and amide protons. The CEST MR imaging is useful to evaluate tumors, strokes, and other diseases. The purpose of this study is to review the mathematical model for CEST imaging and for measurement of the chemical exchange rate, and to measure the chemical exchange rate using a 3T MRI system on several amino acids. We reviewed the mathematical models for the proton exchange. Several physical models are proposed to demonstrate a two-pool, three-pool, and four-pool models. The CEST signals are also evaluated by taking account of the exchange rate, pH and the saturation efficiency. Although researchers have used most commonly in the calculation of CEST asymmetry, a quantitative analysis is also developed by using Lorentzian fitting. The chemical exchange rate was measured in the phantoms made of asparagine (Asn), glutamate (Glu), ${\gamma}-aminobutyric$ acid (GABA), glycine (Gly), and myoinositol (MI). The experiment was performed at a 3T human MRI system with three different acidity conditions (pH 5.6, 6.2, and 7.4) at a concentration of 50 mM. To identify the chemical exchange rate, the "lsqcurvefit" built-in function in MATLAB was used to fit the pseudo-first exchange rate model. The pseudo-first exchange rate of Asn and Gly was increased with decreasing acidity. In the case of GABA, the largest result was observed at pH 6.2. For Glu, the results at pH 5.6 and 6.2 did not show a significant difference, and the results at pH 7.4 were almost zero. For MI, there was no significant difference at pH 5.6 or 7.4, however, the results at pH 6.2 were smaller than at the other pH values. For the experiment at 3T, we were only able to apply 1 s as the maximum saturation duration due to the limitations of the MRI system. The measurement of the chemical exchange rate was limited in a clinical 3T MRI system because of a hardware limitation.
Objective: To assess the feasibility of quantitatively assessing pancreatic steatosis using magnetic resonance imaging (MRI) and its correlation with obesity and metabolic risk factors in pediatric patients. Materials and Methods: Pediatric patients (≤ 18 years) who underwent liver fat quantification MRI between January 2016 and June 2019 were retrospectively included and divided into the obesity and control groups. Pancreatic proton density fat fraction (P-PDFF) was measured as the average value for three circular regions of interest (ROIs) drawn in the pancreatic head, body, and tail. Age, weight, laboratory results, and mean liver MRI values including liver PDFF (L-PDFF), stiffness on MR elastography, and T2* values were assessed for their correlation with P-PDFF using linear regression analysis. The associations between P-PDFF and metabolic risk factors, including obesity, hypertension, diabetes mellitus (DM), and dyslipidemia, were assessed using logistic regression analysis. Results: A total of 172 patients (male:female = 125:47; mean ± standard deviation [SD], 13.2 ± 3.1 years) were included. The mean P-PDFF was significantly higher in the obesity group than in the control group (mean ± SD, 4.2 ± 2.5% vs. 3.4 ± 2.4%; p = 0.037). L-PDFF and liver stiffness values showed no significant correlation with P-PDFF (p = 0.235 and p = 0.567, respectively). P-PDFF was significantly associated with obesity (odds ratio 1.146, 95% confidence interval 1.006-1.307, p = 0.041), but there was no significant association with hypertension, DM, and dyslipidemia. Conclusion: MRI can be used to quantitatively measure pancreatic steatosis in children. P-PDFF is significantly associated with obesity in pediatric patients.
