노인요양시설의 시설·인력 기준 비교 연구 (Comparing standards and guidelines of long-term care facilities based on physical environment and manpower in Korea, Japan, USA, and Australia)

  • The purpose of this study is to compare the standards and guidelines of long-term care facilities based on the physical environments and human resources in Korea, Japan, USA, and Australia. Ultimately, this study suggests the directions for amendments of long-term care service or running of the facilities in Korea. For achieving this purpose, we reviewed the homepage of national health departments, reports and articles of long-term care service, and acts related with long-term care in each country. This comparisons were carried in terms of physical environments, human resources by long-term care related acts and legal sanctions as means of quality control. This study implies that long-term care service guidelines or standards should be revised for developing the quality of our long-term care services.

국내 가정간호 사업의 성과 측정 연구의 고찰 (Literature review on the domestic studies of the Outcomes of Home Care Service for 1981-1996)

  • The purpose of this study is to identify the domestic research results related to the outcomes of home care services for 1981-1996. 12 studies were analized by the characteristics of the subjects, home care services provided, and outcome variables and tools during the period of Apirl-June, 1997. The results were as follows: the subjects of 8 studies conducted before 1993 were discharged patients comparing to early discharged patients of 2 studies conducted after 1994. The main home care service delivered to the clients and their families were education before 1993. The outcome variables were used the level of sat. isfaction of home care services, quality of care, cost-effectveness, health status, quality of life, and self -care ability. A variable, health status, were mainly measured by the global outcome measures such as quality of life, health hehelief, health perception, activity of daily living, health management pattern but also foused outcome measures that could be specified by the medical diagnoses.

아동병동 간호사와 입원아동 어머니가 인식한 간호의 질 비교 (Comparison of the Quality of Nursing Care as Perceived by Pediatric Nurses and Mothers of Hospitalized Children)

  • Purpose: This study aimed to compare the quality of nursing care as perceived by registered nurses and mothers of hospitalized children in South Korea. Methods: This was a descriptive study that recruited 70 mothers of hospitalized children and 70 nurses in pediatric units in university hospitals as participants. The quality of pediatric nursing care was measured using importance and performance scores for 19 items describing various elements of nursing care. The collected data were analyzed using descriptive statistics and the independent t-test. Results: The hospitalized children mothers'mean importance scores were significantly higher than those of the nurses (t=2.94, p=.004). However, there were no significant differences in the mean performance scores of nurses and mothers (t=0.91, p=.363) or between nurses'and mothers'perceptions of quality of nursing care, with the exception of a significant difference for the quality of explanations (t=2.78, p=.006). The quality of explanations was assessed more positively by nurses than by mothers. Conclusion: This study suggests that when developing strategies to improve the quality of nursing care in pediatric wards, ensuring that pediatric nurses provide detailed explanations should be considered as a way to improve the quality of nursing care in pediatric units.

  • Quality improvement activities are especially important in middle to small-sized hospitals as well as in large hospitals. Hospital accreditation would play a crucial role in the re-establishment of the healthcare delivery system, which is now nearly collapsed in Korea. To achieve all these goals, it is also important to educate prospective medical personnel at college on the concepts of quality improvement and patient safety.

SERVQUAL을 이용한 간병서비스 질에 대한 탐색적 연구 - 대전지역 요양병원 입원환자를 대상으로 - (An Exploratory Study on Quality of Care-Giving Service with Use of SERVQUAL -Targeting Inpatients of Long-Term Care Hospitals in Daejeon Region-)

  • The purpose of this study is ultimately to contribute to enhancing quality of patients' life, by grasping quality level of care-giving service that is recognized by inpatients of long-term care hospital, and by pursuing a plan for improving quality of care-giving service based on this. For this, targeting 500 patients who entered 40 long-term care hospitals in Daejeon Metropolitan City, the quality of service was multi-dimensionally grasped by using SERVQUAL scale. As a result of research, the quality of care-giving service, which is recognized by patients as survey subjects, tended to be relatively high with about 4 points as a whole out of 5-point perfection. By item, reliability was indicated to be the highest with 4.56 points. The next was surveyed to be in order of tangibility, empathy, responsiveness, and assurance.

Factors influencing the quality of nursing care as perceived by mothers of hospitalized children in South Korea

  • Purpose: This study aimed to identify the factors affecting the quality of nursing care as perceived by mothers of hospitalized children and provide basic data for the development of nursing care quality improvement programs. Methods: The participants consisted of 167 mothers, each of whom had a child hospitalized at a specific children's hospital. Data were collected from June 22 to August 8, 2019. Results: The factors that affected how mothers perceived the quality of nursing care were the communication ability of nurses (β=.44, p<.001) and the mother-nurse partnership (β=.33, p=.001). The total explanatory power of these factors was 54.1%. Conclusion: To improve the quality of pediatric nursing care, it is necessary to improve the communication abilities of nurses and to promote partnership between nurses and the mothers of hospitalized children.

요양병원 입원급여 적정성 평가 결과를 활용한 요양병원 입원환자의 장기입원 관련 요인 탐색 연구 (An exploratory study of factors related to long-term hospitalization of inpatients using the quality assessment data for long-term care hospitals)

  • Purpose: The purpose of this study was to analyze the factors associated with long-term hospitalized patients in long-term care hospitals using the quality assessment data for long-term care hospitals by the Health Insurance Review. Methods: Among 1,376 long-term care hospitals, frequency analysis and descriptive statistics were used to analyze the characteristics of these hospitals. Multiple linear regression was conducted to examine the associations between infrastructure characteristics, medical personnel characteristics, health outcomes and the proportion of long-term hospitalized patients. Results: The research findings indicate that the number of patients per doctor, the number of patients per nurse, and the number of patients per nursing staff were positively associated with the proportion of long-term hospitalized patients. Among health outcomes, a higher proportion of patients with more than a 5% weight loss compared to the previous month and the proportion of patients showing improvement in ADL, were more likely to have a lower proportion of long-term hospitalized patients. However the proportion of diabetic patients with HbA1c test results within the appropriate range was positively associated with the proportion of long-term hospitalized patients. Conclusion: The present study results provide fundamental data for the establishment of policies for long-term care hospitals. Based on this study, it is important to suggest screening methods for unnecessary long-term hospitalizations, such as sufficient medical personnel to improve the quality of care in long-term care hospitals. It is also necessary to clearly separate the roles of medical institutions and long-term care facilities and implement policies to support patients' social reintegration.
