GMFM과 ICF의 하위영역 기능이 ICF의 하위영역 활동과 참여문제에 미치는 영향 비교 (A Comparative Study on the Effects of GMFM and ICF Sub-item Function on the Sub-item Activity and Participation Restriction of the ICF)

  • Background: The purpose of this study is to explore the effect of the functions of GMFM and ICF-CY on the activities and participation of ICF-CY sub-items. Design: Cross-sectional study. Method: This study compared and analyzed 95 children with cerebral palsy [type of CP: spasticity 86 (90.5%), hypotonia 4 (4.2%), mixed 5 (5.3%); type of palsy: quadriplegia 13 (13.7%), diplegia 71 (74.7%), hemiplegia 11 (11.6%)] using sub-items of functions, activities and participation from GMFM and ICF-CY. Result: The results show that the activities and participation of ICF-CY (9 sub-items) have significant effect on the functions of GMFM and ICF-CY (8 sub-items) (p<0.05). Conclusion: It is intended to provide data to establish practical therapeutic goals and interventions for functions, activities and participation, which are sub-categories of ICF-CY in cerebral palsy.

경추증성 척수증의 증상발현과 경미한 외상 사이의 관련성에 대한 의료감정 1례 (A Case of Medical Examination for the Relationship between Symptom onset of cervical spondylotic myelopathy and minor trauma)

  • The Cervical spondylotic myelopathy (CSM) is degenerative compressive myelopathy which initiation of symptoms seems to be induced by minor cervical trauma or spontaneous event. There was a case of Claim medical examination which was requested to discriminate the cause of ambulatory quadriparesis. Patient asserted that the onset of his myelopathy was followed by minor trauma. The author considered the medical recordings, MRI scan, Claim reports by claim manager. The space available for cord was the smallest at C3-4 level. But on MRI findings, the spinal cord at C3-4 level seemed to be already damaged. There were no recent injury evidences such as hemorrhages, spinal cord contusions, edema, soft tissue hemorrhages. If the space available for cord was small enough to compress the spinal cord, the serious neurologic deficits, non-ambulatory quadriplegia, etc were commonly induced by cervical extension trauma. Patient's asserts did not correspond to his clinical course after cervical trauma. The author reports a case of medical examination for the relationship between symptom onset of cervical spondylotic myelopathy and minor trauma within author's experience.

  • Guillain-Barre syndrome (GBS) is a group of autoimmune syndromes consisting of demyelinating and acute axonal degenerating forms of the disease. Typically, Gullain-Barre syndrome can be diagnosed from the patient's symptoms and physical examination such as the rapid onset of weakness, paralysis and loss of reflexes. In most patients, resolution is complete or near complete. Treatment consists of supportive care, ventilatory management (in about one third of patients), and specific therapy with intravenous immunoglobulin or plasmapheresis. This clinical report is about suspected acute severe Guillain-Barre syndrome patient, 61-year-old man had quadriplegia, facial palsy, dysphasia, respiratory failure. After 5 weeks of East-West integrative medicine therapy - Conventional Conservative therapy(plasmaphresis and intravenous immunoglobulin) and Korean traditional medicine(Sasang medicine and acupuncture treatment) - most symptoms improved.

경추부 척수손상 사지마비 환자의 한방치료와 기능적 전기 자극요법을 통한 상지 재활 치험 2례 (A Case Report of Upper Extremities Rehablitation of 2 Cervical Spinal Cord Injured Patients by Functional Electrical Stimulation with Korean Medical Interventions)

  • Two patients, diagnosed as cervical spinal cord injury, were hospitalized and treated by Korean medical interventions with Functional Electrical Stimulation (FES). The range of motion of the shoulder joint, manual muscle test were adopted to measure the resulting motor recovery after treatment. And the Korean version of Barthel index (K-MBI) was adopted as a method of measuring the improvement of quality of life after treatment. The range of motions and muscle strengths of the shoulder joints were improved. The activities of daily living were slightly improved in case 2, but showed no improvement in case 1. FES was effective for upper extremities rehabilitation of spinal cord injured patients. Further studies are needed to set up an Korean medical protocol for spinal cord injury rehabilitation.

A rare case of Sjogren-Larsson syndrome with recurrent pneumonia and asthma

  • Sjogren-Larsson syndrome (SLS) is a rare autosomal recessive neurocutaneous disorder with worldwide incidence of 0.4 per 100,000 people. It is characterized by the triad of congenital ichthyosis, spastic diplegia or quadriplegia, and mental retardation. Herein we report a 2-year-old male child with SLS, asthma, and recurrent pneumonia. SLS was confirmed by a molecular genetics study that revealed a deletion mutation in the ALDH3A2 gene. An ALDH3A2 gene mutation results in dysfunction of the microsomal enzyme fatty aldehyde dehydrogenase and impaired metabolism and accumulation of leukotriene B4, which is a key molecule and a pro-inflammatory mediator in developing allergic diseases, especially asthma. An increased level of leukotriene B4 has been reported in SLS patients. As far as we are aware, this is the first report of SLS associated with asthma and recurrent pneumonia. In conclusion, pediatricians should be aware of and evaluate patients with SLS for possible associated asthma and allergic disorders.

  • 저자 등은 1987넌에 인공 관막 치환수술을 받고 심방 내 혈전 형성을 예방하기 위하여 항응호제를 계속적으로 복용하여 오던 71세 남자가 우측 안면 및 상, 하지 운동 마비와 실어증을 주소로 1994년(9원 28일) 성남 인하병원에 입원하게 되었고 입원 11일째부터 사망하기까지 특수 유통식 공금을 위하여 위장 내로 투입한 비강 영양 튜우브(nasogastrictube)를 통하여 수 마리의 파리 유충(maggot)이 역류되어 나왔고, 구강 내에서도 총 45마리의 구더기가 발견된 증례를 경험하였다 이 파리 유충은 병균 11.5 mm 길이로서 걸정파리과(Calliphoridae) 금파리속(Lucllia)의 1종이었으며 위. 장관계에 병소를 갖고 있었음이 추정되었다. 본 내부 승저증(internal myiasis) 증례는 우리 나라에서 처음 보고되는 것이다.

  • 본 증례의 환아들의 경우, PEG-tube를 통한 식이로 구강 위생관리가 용이하며, 사지마비 및 의식 저하로 스스로 장치의 탈착이 어렵다는 점에서 가철성 장치를 장착한 결과 양호한 치유 양상이 관찰되었다. 구강조직에 자해로 인한 손상이 발생한 경우, 환자의 병력, 현재 상태, 치료 기간 등을 고려하여, 환자에게 가장 바람직한 치료 계획을 세워야 한다. 가철성 장치를 이용하는 경우, 구강 위생 관리 및 병소의 관찰을 위하여 주기적인 치과 검진이 필요하다.

  • Finding solutions for the disabled is a major challenge for our society. In the case of a disability due to a malfunction of the nervous system, the origin may be accidental, genetic, or induced by environmental factors. This type of loss can cause loss or movement disorders (paraplegia, hemiplegia, quadriplegia, epilepsy, Parkinson's disease, multiple sclerosis, etc.) or malfunction of certain sensory functions (blindness, deafness, chronic pain, etc.). Many alternatives, more technology, have been imported to create interfaces between the human body and an artificial prosthesis in order to restore some functions of the human body. A wireless system, battery neurons probe was developed for one hand reading neural signals in the brain, and on the other hand also able to excite the neuron in the brain using a surface acoustic wave one ports (SAW) delay line reflection.

자기공명영상에서 전형적인 이상 소견을 보인 소아마비양 증후군 환자 1예 (A Case of Poliomyelitis-like Syndrome with Typical Abnormalities in MRI)

  • A 28-year-old man presented with headache, fever, and myalgia. Subsequently, rapidly progressive quadriplegia with areflexia developed. CSF examination revealed moderate pleocytosis and protein elevation. MRI of brain and spinal cord showed hyperintense lesions on T2-weighted image at midbrain and ventral horns along the whole spinal cord. Serial serologic examinations of CSF for Epstein-Barr virus and cytomegalovirus were negative. Culture and neutralization tests of stool and CSF for enterovirus were negative. Although the etiologic pathogen was not identified, we diagnosed him as poliomyelitis-like syndrome by clinical features and findings of MRI.

Phrenic Nerve Stimulation for Diaphragm Pacing in a Quadriplegic Patient

  • Chronic hypoventilation due to injury to the brain stem respiratory center or high cervical cord (above the C3 level) can result in dependence to prolonged mechanical ventilation with tracheostomy, frequent nosocomial pneumonia, and prolonged hospitalization. Diaphragm pacing through electrical stimulation of the phrenic nerve is an established treatment for central hypoventilation syndrome. We performed chronic phrenic nerve stimulation for diaphragm pacing with the spinal cord stimulator for pain control in a quadriplegic patient with central apnea due to complete spinal cord injury at the level of C2 from cervical epidural hematoma. After diaphragmatic pacing, the patient who was completely dependent on the mechanical ventilator could ambulate up to three hours every day without aid of mechanical ventilation during the 12 months of follow-up. Diaphragm pacing through unilateral phrenic nerve stimulation with spinal cord stimulator was feasible in an apneic patient with complete quadriplegia who was completely dependent on mechanical ventilation. Diaphragm pacing with the spinal cord stimulator is feasible and effective for the treatment of the central hypoventilation syndrome.
