This study was designed to examine the effects of complex breathing exercise and neuromuscular electrical stimulation of Quadriceps Femoris muscle on pulmonary function and cerebral cortex activity in patients with severe chronic obstructive pulmonary disease. After collecting samples from 20 patients with severe chronic obstructive pulmonary disease aged 60 to 80, 10 patients each were randomly placed in an experimental group and a control group. The experimental group conducted complex breathing exercise and neuromuscular electrical stimulation of Quadriceps Femoris muscle, and the control group only conducted complex breathing exercise. As a pretest, pulmonary function and cerebral cortex activity were measured. The intervention program was applied to each group for 30 minutes, once a day, for 4 days a week, for 6 weeks, and the posttest was carried out the same way as the pretest. As a result, both groups showed significant differences in FEV1.0(Forced Expiratory Volume in One Second)(p<.001)(p<.05), and there were significant differences between the groups as well(p<.05). When comparing alpha waves in each domain of cerebral cortex, both of the experimental and control groups showed significant differences in Fp1, Fp2, F3 and F4 domains (p<.01)(p<.05). During the 6-week experiment, complex breathing exercise and neuromuscular electrical stimulation of Quadriceps Femoris muscle improved pulmonary function of patients with severe chronic obstructive pulmonary disease, and in relation to cerebral cortex activity, a positive breathing change was found due to the increase of alpha waves in the forehead domain. Therefore, it is considered that applying neuromuscular electrical stimulation of Quadriceps Femoris muscle to patients with severe chronic obstructive pulmonary disease additionally along with complex breathing exercise will bring a better therapeutic effect.
Purpose: This study identified the co-activation of quadriceps and hamstring muscles during hamstring strengthening exercises in healthy adults. Methods: Twenty-one participants were required for the present study design to achieve 80% power, 0.8 effect size (η2), and an alpha level of 0.05. Thus, this study recruited 21 healthy adults. All participants performed Nordic exercises, bridge exercises, and one-leg deadlifts randomly. The activity of the rectus femoris, vastus medialis (VM), vastus lateralis (VL), biceps femoris (BF), and semitendinosus (SM) were measured. In addition, the ratios of VM/VL and hamstring/quadriceps (HQ) were measured during the three hamstring strengthening exercises using electromyography. One-way ANOVA was used to compare the co-activation of quadriceps and hamstring muscles in the three exercises. Results: The activity of VM and VL during the performance of one-leg deadlifts was significantly higher than the other two exercises. The BF had significantly higher activity during the Nordic exercises compared to the other two exercises. In addition, the SM activation was significantly greater during Nordic exercises than one-leg deadlifts. Additionally, there was significant difference in HQ ratio among hamstring strengthening exercises. In specific, the one-leg deadlifts yielded a significantly lower HQ ratio. Conclusion: This study revealed that one-leg deadlifts are effective in rehabilitation for anterior cruciate ligament injury. In addition, Nordic exercises can be recommended to facilitate hamstring muscle activation.
Purpose: This study aims to investigate the changes in pulmonary function, gait ability, and quality of life when NMES is applied along with CBE and to provide basic clinical data to be used in pulmonary rehabilitation guidelines for patients with COPD to treat patients with severe COPD. Methods: For this study, CBE and NMES of quadriceps femoris on both sides were applied to the experimental group (n=10), and only CBE was applied to the control group (n=10). For a pre-test, a 6-minute walk test was performed, and pulmonary function and health-related quality of life were measured. Moreover, an exercise program was applied to each group for 30 minutes per session, 5 times a week, for 6 weeks. After that, a post-test was conducted the same way as the pre-test. Results: In the within-group comparison, there were significant differences in forced expiratory volume in one second, 6-minute walk test and health-related quality of life between the experimental group and the control group (p<0.01)(p<0.05). In the between-group comparison, the experimental group showed an increase in forced expiratory volume in one second and 6-minute walk test (p<0.05) and showed a decline in health-related quality of life (p<0.05). Conclusion: The 6-weeks NMES program improved health-related quality in patients with severe COPD by increasing expiratory volume by reinforcing the function of quadriceps femoris. This finding implies that NMES could be an alternative mode for improving physical functions of patients with severe COPD, who cannot participate in a breathing exercise program or are reluctant to participate.
The purpose of this study was to investigate the effect of isometric hip adduction and abduction on the muscle activities of vastus medialis oblique(VMO) and vastus lateralis(VL) during leg squat exercises. This study consisted of 21 healthy subjects who had no medical history of anterior knee pain or lower extremity disorders. The ball and belt were used to isometrically adduct and abduct the hip joint during the leg squat exercise, respectively. The surface electromyograms of VMO and VL were analyzed, and the findings were used to calculate the VMO:VL ratio during 3 different quadriceps-strengthening exercises(leg squat, LS leg squat with isometric hip adduction, LSHD leg squat with isometric hip abduction, LSHB). The muscle activities of VMO and VL and the VMO:VL ratios were compared using the paired t-test with Bonferroni adjustment. The results showed that the muscle activities of VMO and VL during LSHD were greater than those during LSHB. The VMO:VL ratio was the highest during LSHD. This finding suggests that LSHD using a ball is more effective than LS and LSHB in selectively increasing the muscle activities of VMO. Therefore, we suggest that leg squat exercise with isometric hip adduction using a ball would be useful for maintaining correct patella tracking and for selectively strengthening VMO.
Objective: The purpose of this study was to investigate the effects of resistance applied in various directions on lower extremity muscle activity and balance during squat exercise performance. Design: Cross-sectional study. Methods: Forty-one adults (19 males and 22 females) agreed to the study purpose and procedures. All subjects randomly performed squat exercises with an intensity of overcoming 10% of body weight resistance pulled forward, backward and general squats with $60^{\circ}$ of knee joint flexion. Electromyography was used to measure muscle activity of the vastus medialis oblique (VMO), rectus femoris (RF), vastus lateralis oblique (VLO), biceps femoris (BF), and semitendinosus (ST) muscles. The Wii Balance Board was used to assess balance during the three-method squat operation. Each operation was measured three times for 10 seconds. Results: There were significant differences in muscle activities of the VMO, RF, VLO, ST and balance ability with the application of three directions of resistance (p<0.05). Post hoc comparisons revealed that squats performed with resistance pulled in the backward direction resulted in higher VMO, RF and VLO activity than with the resistance placed in a pulled forward direction (p<0.05). In the ST, resistance applied in the pulled forward direction showed greater muscle activity compared to the pulled backward direction (p<0.05). With balance, squats performed with resistance pulled in the forward direction showed greater muscle activity than with resistance applied in the pulled backward direction and during general squat performance (p<0.05). Conclusions: In this study, squat exercises performed with resistance applied in the direction of pulling backwards was found to be the most effective in improving quadriceps muscle strength and balance. It is effective to provide resistance that is placed in the forward when it is difficult to perform a general squat due to weakness of the quadriceps.
Purpose: This study was to examine the effects of electric stimulation therapy on chronic knee pain, exercise self-efficacy, and quality of life in Korean elderly women. Methods: The design was an unequivalent control pretest-posttest study. Samples were total of 60 (experimental: 30, control: 30) elderly women with healthy cognitive and communication abilities aged 65 years old and above. The experimental treatment involved was the electric stimulation of both thigh quadriceps muscles for 15 minutes per treatment, 3times per a week, for a total of 12 weeks. Measurements taken were S-F MPQ and AIMS for chronic knee pain, exercise self-efficacy measurement for exercise self-efficacy, and S-F 36 scale for quality of life. Data were analyzed using SPSS PC+ 12. Results: Chronic knee pain according to the Short-Form McGill Pain Questionnaire (t=43.563, p=.000) and Arthritis Impact Measurement Scale (t=31.364, p=.000) were significantly decreased in the experimental group by the application of electrical stimulation therapy. Exercise self-efficacy (t=107.116, p=.000) and quality of life (t=76.429, p=.000) were significantly increased in the experimental group by the application of electrical stimulation therapy. Conclusion: Electrical stimulation therapy could be a more effective primary nursing intervention in decreasing chronic knee pain, and on increasing exercise self-efficacy and quality of life for Korean elderly women.

Purpose : This study was to comparison of EMG of an stable exercise group and unstable exercise group on patients who have had anterior cruciate ligament reconstruction(ACL reconstruction). Methods : The subjects of the study were patients who had no less than 4 weeks after ACL reconstruction and could stand on one leg, and they divided into a control group with 9 patients doing closed kinetic chain exercises on the stable floor and an experimental group with 10 patients doing closed kinetic chain exercises on the unstable floor and in order to compare degrees of the muscle activity of the thigh extensor and flexor was tested each before the exercise, in 3 weeks and 6 weeks after doing exercises by using surface electromyography (Surface EMG). The patients made 3 sets of exercises (10 times per set), each of which consisted of exercises using elastic bands and the squat. Results : There was statistic significance about the vastus medialis muscle. Conclusion : It is thought that the closed-chain exercise could be an exercise program through which patients could enhance the muscle activity of the vastus medialis muscle optionally among the quadriceps muscle and the hamstring muscles which should weaken after ACL reconstruction.

Purpose: This study examined the effects of pre-eccentric exercise to the quadriceps for the prevention of delayed onset muscle soreness (DOMS) and recovery of muscular function, depending on the training intensities. Methods: Subjects were divided into one of three groups that control group, a low intensity eccentric exercise group (LIEE group) and a high intensity eccentric exercise group (HIEE group). Subjects who underwent pre-eccentric exercise undertook exercise at an intensity of 25% and 75% of maximal voluntary contractions, respectively. After undertaking pre-eccentric exercise for eight weeks, eccentric exercise was applied again to induce DOMS. Measurements were conducted to examine pain and muscular function changes before, immediately after, after the induction of DOMS, and at the first, third, fifth and seventh days after the induction of DOMS. Results: Subjects who underwent pre-eccentric exercise showed a significant difference from the control group for the changes in the visual analogue scale (VAS) pain threshold, pressure pain threshold and muscle thickness by isometric contraction from measuring DOMS, and in particular, pain threshold by isometric contraction had remarkable effect in the LIEE group of subjects. For the change of the root mean square values using mechanomyography (MMG) as the measurement of muscular function recovery, subjects who had undertaken pre-eccentric exercise group showed a significant difference as compared to the control group. Conclusion: Pre-eccentric exercise was very effective in preventing and recovering delayed onset muscle soreness and was helpful to prevent and recover from decreased muscular function. The difference based on the intensity of exercise was not great, but it was more effective in the low intensity eccentric exercise group of subjects. Therefore, it can be considered that pre-eccentric exercise has a high application value as a physical therapy intervention for prevention and rehabilitation of sports injuries.

The purpose of study was to compare the effectiveness of sling exercise on the muscle strength and range of motion in female patients who received a total knee replacement. The participants were allocated randomly into 2 groups: sling exercise group (n=15) and control group (n=15). The subjects were evaluated using the Biodex system for the muscle strength test and a goniometer for the range of motion test. The data was analyzed using a paired t-test and independent t-test to determine the statistical significance. As a result, the sling exercise group before and after intervention showed a statistical significance difference in the flexion angle, quadriceps femoris, and hamstring muscle strength. The control group before and after the intervention revealed a statistically significant increase in the flexion & extension angle, quadriceps femoris, and hamstring muscle strength. Muscle strength test and flexion range of motion test in the sling exercise group showed statistical significance differences compared to the control group (p<.05). Therefore, the sling exercise group has a positive influence on the muscle strength and ROM in patients with a total knee replacement.
The purpose of this study was to determine if there were any beneficial effects of massage or microwave diathermy regarding delayed onset muscle soreness (DOMS) and indices of muscle damage. Twenty-one adult women, randomly divided in two treatment groups and a control group, performed eccentric stepping exercise with the quadriceps until exhaustion. The treatment groups additionally performed massage or microwave diathermy after the stepping exercise. Pressure pain threshold measure for DOMS and muscle enzymes in the blood were obtained before, and 0, 24, 48 and 72 hours after exercise. The results were as follows; 1. Eccentric exercise caused DOMS and elevations of muscle enzymes in the blood, with peak values exercise levels by 24 hours after exercise and GOT and CRP by 72 hours after exercise. DOMS and CK activity remained elevated 72 hours after exercise. 2. DOMS and blood muscle enzymes response to eccentric exercise were reduces by massage or micro diathermy therapy. DOMS was significantly decreased at 72 hours after exercise by massage and microwave diathermy. CK activity was significantly decreased at 72 hours after exercise by microwave diathermy. There was the significant reduction in LDH at 48 hours, GOT at 24, 48, 72 hours. and CRP at 24, 48 hours after exercise by massage and microwave diathermy. These results indicate that massage or microwave diathermy is had effect on recovery from exercise-induced muscle damage. In our's suggestion. microwave diathermy is particularly more appropriate therapeutic modality because it is more simple and economic than massage.
