The Effects of CI Technique in PNF on the Muscle Activity, Fatigue, and Balance in Hemiplegic Patients (PNF의 CI기법이 편마비 환자의 근활성도, 근피로도 및 균형에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of a combination of isotonic (CI) technique on the balance, muscle fatigue and muscle activities of the quadriceps muscle in the paretic leg of hemiplegic patients. Method: All the subjects received a combined isotonic technique of proprioceptive neuromuscular facilitation (PNF) for about 30 minutes 3 times per week for 6 weeks. The root mean square (RMS) and mid frequency (MF) were recorded by surface electromyography (EMG)(EMG/EP system, OXFORD Medelec, UK) during maximal voluntary isometric contractions (MVIC). The balance ability was measured by a balance system (BIODEX, USA). Result: The RMS of the quadriceps muscle at post-treatment was significantly increased compared to that at pretreatment (p<0.05). Yet there was no significant difference among the three muscles. There were no significant difference in the MF of the vastus lateralis and rectus femoris between pre- and post-treatment, but the MF of the vastus medialis was significantly decreased (p<0.05). There was a significant difference in the vastus medialis and lateralis between pre- and post-treatment (p<0.05). Moreover, the overall stability index at post-treatment was significantly decreased compared to that at pre-treatment (p<0.05). But there was no significant difference between the anterior/posterior and medial/lateral stability indexes. Conclusion: The results of this study indicated that a CI technique of PNF had an effect to improve the balance and muscle activity in hemiplegic patients.

The Effect of Graded Exercise-Induced Fatigue on Position Sense of the Knee (근피로를 유발하는 운동강도 변화가 슬관절의 위치감각 인지에 미치는 영향)

  • It was recently reported that exercise-induced fatigue is related to joint position sense although some controversy remains. The purposes of this study were to examine the effect on the accuracy of reproducing the knee angles after a fatiguing isokinetic quadriceps exercise at four different levels (10%, 30%, 50%, and 70% of maximal force) and to find the optimal exercise level without causing knee joint proprioception impairment. Forty healthy women, ages 19 to 27, were randomly assigned to four experimental groups. Before and after the exercise, accuracy of positioning with respect to auditory feedback for specific angles was estimated by calculating the mean errors between specific angles and reproduction angles. Fatigue was measured by EMG signals displayed by a frequency spectrum analysis during the quadriceps exercise. Results showed that there was no significant difference in accuracy of the knee joint positioning sense following the exercises in group 1, group 2, and group 3 (10%, 30%, and 50% of maximal force, respectively); the exception being group 4 (70%). Fatigue level was significantly increased in group 4 but there were no significant increases of fatigue level in group 1, group 2, or group 3. The results concluded that the optimal exercise level to acquire the therapeutic exercise effectiveness without position sense impairment was at 50% of maximal force. Further studies using large sample size and patient groups with poor knee joint proprioception would be needed to confirm this conclusion and to clarify the possibility of clinical applications.

The Relationship Between the Range of Hip Rotation and the Quadriceps Angle in Subjects With and Without Flat Foot

  • Background: Alignment of the lower limb is an important factor, influencing balance and gait in kinematics and kinetics, in patients with and without a flat arched foot. Flat arched foot are associated with the range of motion (ROM) of the hip and alignments of the knee joints, is strongly influenced. Objects: The purpose of this research was to investigate the relationship between hip joint ROM and quadriceps angle (Q-angle), by dividing them into two groups according to the presence or absence of flat feet, using a navicular drop test (NDT) and resting calcaneal stance position (RCSP). Methods: Forty elderly patients were allocated to the experimental group (flat foot group, n1=20) or the control group (non-flat group, n2=20). Universal and digital goniometer, tractograph and tape measure were used to determine the related changes in the hip ROM, Q-angle, NDT and RCSP. Results: Data were analyzed using the Pearson correlation coefficients. Active internal ROM of the hip joint (right, r=.803, p<.001), (left, r=.951, p<.001) were highly correlated with NDT, and also, was moderately correlated with Q-angle (right, r=.562, p=.019), (left, r=.757, p<.001). Passive internal ROM of the hip joint (right, r=.742, p=.001), (left, r=.922, p<.001) were highly correlated with NDT, and also, was moderately correlated with RCSP (right, r=-.530, p=.029) and with Q-angle (right, r=.710, p=.001), (left, r=.698, p=.002) in the flat foot group. However, no strong correlation among the hip ROM, NDT, RCSP and Q-angle were found in the non-flat foot group. Conclusion: This research may provide evidence of the correlations between hip internal ROM and flat foot.

Effects of Strengthening and Stretching Exercise for Individuals with Intrinsic Patellofemoral Pain Syndrome (잠재적인 슬개대퇴동통증후군에 대한 근력 및 유연성운동의 효과)

  • Purpose: This study examined the effect of the strengthening exercise and stretching exercise to decrease symptom patellofemoral pain syndrome (PFPS). Methods: The Anterior Knee Pain Scale (AKPS) and Clark's test performed for diagnosis of intrinsic PFPS among young adults. Selected thirty young adults subjects who aged 20~26 years participated in the study. Participants were randomly assigned to strengthening, stretching, or control groups. Strengthening group consisted of quadriceps, hamstring and iliotibia band training used elastic band. Stretching group consisted of quadriceps, hamstring and iliotibia band trainings used stretching exercises program. Participants received 50-minute individualized exercise sessions, 3 times a week for 6 weeks. Results: The results were as follow: there were significantly difference stretching exercise group by all muscles on muscular strength test (p<.05). there were significantly difference both strengthening and stretching exercise group by all muscles on flexibility test (p<.05). There were significantly difference stretching exercise group by all muscles on step-down test (p<.05). There were significantly difference both strengthening and stretching exercise group by all muscles on visual analog scale (p<.05). Conclusion: Results suggest important implications for exercise programs of PFPS that stretching exercise is more improved knee pain, functional performance, patella mobility than strengthening exercise.

The Immediate Effects of Kinesio Taping on Quadriceps Induced Fatigue by Short-term Squat (단시간의 스쿼트 운동으로 피로 유발된 대퇴사두근에 대한 키네시오 테이핑의 즉각적 효과)

  • Background: The purpose is to investigate the immediate effects of kinesio taping on quadriceps induced fatigue by short-term squat. Methods: This research is cross-over designed study and conducted as a single-blind. Eleven students (Age: $18.91{\pm}0.49yrs$, height: $167.09{\pm}8.46cm$, $62.55{\pm}11.32kg$) were participated in this study. All the participants were applied short-term squat for inducing fatigue, and then intervented all three conditions (kinesio taping applied condition, placebo taping applied condition, non-taping applied condition). The interval of each intervention was at least a seven days to prevent carry-over effects. The participants were tested peak torque (isometric contracture) and active joint position sense (active JPS) pre-post intervention. A paired t-test was used to find the significance of pre-post intervention results and one-way ANOVA was used to find the significance between interventions with significant level as .05. Results: Peak torque was significantly increased on KT condition (p<.05). But there were no significant effects on active JPS. Because kinesio taping facilitates Ia afferent by tactile stimulation with stretched skin, reduced muscle power by short term squat which induce muscle fatigue was increased. But kinesio taping does not affect mechanoreceptor in muscles, induced active JPS by short-term squat had no changes.

Electromyographic Study of Lumbosacral Radiculopathy by Lumbar Disc (요추 추간원판에 의한 요천추 선경근 병변의 근전도 연구)

  • The aim of this study is to examine the sensitivity and characteristics of electromyography abnormalities detected by using various paramenters in patients with lumbosacral radiculopathies. EMG is widely used for disgnosing and localizing the level of radiculopathy. The results of the study were as follow : 1. In electromyography, L5 radiculopathy usa 95 cases(51.690). S1 radiculopathy was $45m(24.5\%)$ L4 radiculopathy was 18cases $(9.8\%)$, and L2, 3 radiculopathy was 8cases$(4.3\%)$. Remains 18cases$(9.8\%)$ had no definite radiculopathy. 2. Peroneal and tibial motor nerve conduction velocity studies were not significant as compared to the side to side. 3. Latency of H-reflex in L5 radiculopathy was $30.55\pm2.47$ in affected side, $29.47\pm2.29$ in unaffected side, in S1 radiculopathy was $33.00\pm2.03$ in affected side, R30.18\pm2.21$ in unaffected side. It was statistically significant(p<0.01). H-reflex mean difference of S1 radiculopathy group was significantly prolonged as compared to the L5 and S1 radiculopathies(p<0.001). 4. In L2, 3 radiculopathy, abnormal spontaneous activities and motor unit action potentials were showed high sensitivity in upper lumber paraspinal, hip adductors, quadriceps and iliopsoas muscles. 5. In L4 radiculopathy, lower lumbar paraspinal, tibialis anterior, quadriceps muscles were showed high sensitivity. 6. In L5 radiculopathy, lower lumbar paraspinal, extensor hallucis longus, extensor digitorum longus, peroneus longus, extensor digitorum brevis, gluteus maximus, tensor fasciae latae muscles were showed high sensitivity. 7. In S1 radiculopathy, lower lumbar paraspinal, gluteus maximus, peroneus longus, soleus, abductor hallucis, hamstrings, extensor digitorum brevis, extensor hallucis lognus, gastrocnemius muscles were showed high sensitivity.

Optimal Design of a Novel Knee Orthosis using a Genetic Algorism (유전자 알고리즘을 이용한 새로운 무릎 보장구의 최적 설계)

  • The objective of this paper is to optimize the design parameters of a novel mechanism for a robotic knee orthosis. The feature of the proposed knee othosis is to drive a knee joint with independent actuation during swing and stance phases, which can allow an actuator with fast rotation to control swing motions and an actuator with high torque to control stance motions, respectively. The quadriceps device operates in five-bar links with 2-DOF motions during swing phase and is changed to six-bar links during stance phase by the contact motion to the patella device. The hamstring device operates in a slider-crank mechanism for entire gait cycle. The suggested kinematic model will allow a robotic knee orthosis to use compact and light actuators with full support during walking. However, the proposed orthosis must use additional linkages than a simple four-bar mechanism. To maximize the benefit of reducing the actuators power by using the developed kinematic design, it is necessary to minimize total weight of the device, while keeping necessary actuator performances of torques and angular velocities for support. In this paper, we use a SGA (Simple Genetic Algorithm) to minimize sum of total link lengths and motor power by reducing the weight of the novel knee orthosis. To find feasible parameters, kinematic constraints of the hamstring and quadriceps mechanisms have been applied to the algorithm. The proposed optimization scheme could reduce sum of total link lengths to half of the initial value. The proposed optimization scheme can be applied to reduce total weight of general multi-linkages while keeping necessary actuator specifications.

The Myoelectrical Activities of Trunk Muscle and Quardriceps Femoris According to Treadmill Gait Different Inclination and Speeds (트레드밀 보행시 경사도와 속도에 따른 체간근육과 대퇴사두근의 근활동성 분석)

  • Purpose: The objective of this study is to analyze the activities of muscles importantly functioning when walking with different inclinations and speeds of a treadmill, in order to provide basic data on walking exercise using a treadmill. Method: The selected subjects of this study were 16 men and women who had lower extremity injury. A treadmill was used to provide the activation of muscle, and the electromyography was used to analyze the muscle activity variables. The Biodex was used to measure the value of maximum isometric contraction. The inclinations of the treadmill were 0%, 5% and 10%, respectively, and its speeds were 2Km/h. 3Km/h, 4Km/h, 5Km/h, and 6Km/h, respectively. Result: For quadriceps femoris muscle and trunk muscle, there were significant differences in muscle activity when different speeds were applied at 0%, 5% and 10% inclinations.(p<0.05) The activity of vastus medialis muscle was 9.78% at 0% inclination and 2km/h speed, whereas it was 9.32% at 0% inclination and 3km/h, which was slightly lower. The activity of erector spinae muscle was 24.93% at 0% inclination and 2km/h speed, whereas it was 24.84% at 0% inclination and 3km/h, whereas it was 23.99% at 0% inclination and 4km/h, which was slightly lower. The activity of vastus medialis muscle was 11.89% at 10% inclination and 2km/h speed, whereas it was 10.65% at 10% inclination and 3km/h, which was slightly lower. The activity of rectus femoris muscle was 10.26% at 10% inclination and 2km/h speed, whereas it was 9.77% at 10% inclination and 3km/h, which was slightly lower. Conclusion: It was found that the activities of trunk muscle and quadriceps femoris muscle increase as the inclination and the speed of a treadmill increase during treadmill walking.

Changes of Biophysical and Physical Fitness Function on Combined Exercise Program in 20' Female University Students with Obesity according to Heights of Insole (인솔 높이에 따른 복합운동프로그램 적용 시 20대 비만 여대생의 신체생리 및 체력적 기능 변화)

  • This study conducted the following experiment to examine change of biophysical and physical fitness function on combined exercise program in 20' female university students with obesity according to heights of insole. Obesity related biophysical function was measured by basal metabolism and percent body fat(% Fat) and health related physical fitness was measured by isokinetic strength of quadriceps femoris muscle, muscular endurance, flexibility and power on comparative analysis of pre, post exercise and each groups(0cm, 5cm, 9cm) in 36 female university students with obesity subject according to heights of insole. These result lead us to the conclusion that each group were statistically improved at isokinetic muscular strength of quadriceps femoris muscle and basal metabolism, but 5cm group compared with 0cm and 9cm group was more significant improvement. Consequently, combination exercise applying appropriate heights of insole would be lead to positive increment of muscular strength and basal metabolism.

Convergence of Comparison of Muscle Activities of Healthy Adults' trunk and Lower Extremities according to wearing/not-wearing and forms of backpack (백팩 착용 유무 및 형태에 따른 건강한 성인의 몸통과 하지의 근 활성도 비교분석의 융합연구)

  • This study aims to compare muscle activities of trunk and lower extremities according to wearing/not-wearing and forms of backpack. Twenti-six healthy adults were participated in the study. In the backpack with 10% body weight, trunk and lower extremities were measured during carrying the no backpack state(NBs), general backpack state(GBs), a decrease moment arm backpack state(DMBs). The muscle activity in a fore muscles(Obilique, Quadriceps, Tibialis anterior, Gastrocnemius) were analyzed with using wireless electromyogram measuring system. DMBs showed significant improvemet in obilique, quadriceps and gastrocnemius muscle RVC.
