  • This study is to optimize the condition of pulse parameters using the response surface method in micro pulse TIG welding of pleated type metallic filter. The input parameters used were pulse current, base current, pulse duty, frequency and welding speed and the hydraulic pressure was used as the output parameter. The central composite design was designed using second order regression model, As the results, the optimal welding condition to manufacture the pleated type metallic filter was obtained.

An Example of Test on Differences of Pulse Waveform Characteristics at Cun, Guan and Chi (촌, 관, 척 위치의 맥파 특징 차이 검정에 대한 1례)

  • Although the pulse diagnosis position, Guan is apart from Cun or Chi by only $10{\sim}20$ mm at most, traditional medical doctors applies different indent pressures and even they states different pulse images are felt at Cun, Guan and Chi, To support their clinical behaviors, in this study, we tested statistically whether there are differences in pulse waveform measured at these three positions with SphygmoCor system used world widely, A 30 years old female subject without any evidence of cardiovascular diseases was involved in this experiment. Radial pulse waves were recorded at three different positions on left lower arm 10 times at three positions-Cun, Guan and Chi. With ANOVA, we tested whether, among three different positions. there are any differences in 12 parameters of radial pulse waveform and in estimated AIx(Augmentation Index) as an arterial stiffness index extracted from radial pulse waveform. As results, differences in optimal indent pressure h0 were observed at different measuring positions(P<0.001) but not significantly different. And pulse pressure his were found to be different(Chi$22.60{\pm}3.06%,\;18.60{\pm}3.37%\;and\;26.4{\pm}5.02%$ respectively. Consequently. AIx at Gwan seems to be lowest and that at Chi seems to be highest. So. we assert the AIx at Chi is likely to be overestimated. In further studies. we want to examine what make differences in these parameters between measuring positions. And it also seems to be worthy to investigate the relationship between the depth of radial artery and AIx. And, ultimately, we need to determine the best measuring process including measuring position, hold-down pressure, signal quality validation and so on. so to achieve the optimal waveform which represents subject's health condition for both western medicine and traditional medicine.

Effect of changing position from supine to standing up-right on the circulation in young men and women (앙와위에서 직립자세로의 체위변화가 정상인의 순환에 미치는 영향)

  • This study investigated the effect of changing position from supine to standing upright on the circulation of young men and women. Healthy men and women ranging from age of 18 to 24 were examined. Two minutes before standing, the baseline heart rate and blood pressure were measured. Heart rate and blood pressure were recorded immediately and at every minute for 10 minutes after standing upright. Differences in heart rate and blood pressure between supine and standing upright position were evaluated. The results were summarized as follows : 1. Heart rate increased significantly immediately and at every minute for 10 minutes after standing upright. 2. Systolic blood pressure increased significantly immediately and at every minute for 4 minutes after standing upright. 3. Diastolic and mean blood pressure increased significantly immediately and at every minute for 10 minutes after standing upright. 4. Pulse pressure immediately and at every minute for 10 minutes after standing upright was significantly narrower than that of supine position. 5. There was no significant difference of heart rate between men and women after standing upright. 6. Systolic and diastolic blood pressure of men after standing upright was significantly greater than those of women. From these results, it may be concluded that heart rate, systolic, diastolic and mean blood pressure and pulse pressure increase after standing upright, and systolic and diastolic blood pressure in men is greater than those of women after standing upright.

Numerical Study of Effect of counter-pulsation on Hemodynamic Response in the ECLS (체외생명구조장치에서 역박동 방법이 혈류역학 응답에 미치는 영향에 대한 수치적 연구)

  • Extra-corporeal Life Support System (ECLS) is the device used in emergency cases to substitute a extracorporeal circulation in open heart surgery, cardiac arrest or in acute cardiopulmonary failure. To obtain the effect of counter-pulsation on hemodynamic response in the ECLS quantitatively, we developed cardiovascular model which consists of 12 compartment model of heldt et al. and 3 compartment model of Schreiner et al. based on windkessel approximation. We compared coronary perfusion, arterial pulse pressure, cardiac output, and left ventricular pressure-volume diagram according to flow configuration such as counter-pulsation, copulsation, and continous flow. When counter-pulsation was applied, 5% higher coronary perfusion, 26% lower pulse pressure, and 2% higher cardiac output than copulsation condition were calculated. We conclude that counter-pulsation configuration in the ECLS is hemodynamically more stable than copulsation and influences the positive effect to recover ventricles.

  • Purpose: This study was intended to determine how the choice of music affects the physiological and psychological status of women college student during music therapy. Methods: A nonequivalent experimental group pretest-posttest design was used. 19 out of 54 subjects were assigned to listen to their favorite music and 17 to their unfavorite music for 20 minutes using MP3 players and headphones. Anxiety, blood pressure, pulse and blood glucose levels of the subjects were measured before treatment. The data was analyzed by Mann-Whitney and Wilcoxon signed rank test using the SPSS/WIN 10.0 program. Results: The result showed that systolic blood pressure, blood glucose and anxiety level decreased significantly in the favorite music group. however systolic blood pressure, pulse rate and anxiety level increased significantly in the unfavorite music group. The favorite music group showed a significantly higher level of satisfaction than the unfavorite music group. Conclusion: Choice of music affects the physiological and psychological status of an individual. Favorite music listening would enhance the effect of music therapy. Therefore, selection of music must be considered in light of the subject's preference and characteristics.

The Effect of Aroma Inhalation Method on the Preoperative Anxiety of Abdominal Surgical Patients (향기흡입법이 복부수술 환자의 수술 전 불안 감소에 미치는 효과)

  • Purpose: The study was done to analyze the effects of aroma inhalation method on preoperative anxiety of abdominal surgical patients. Methods: The research design was a nonequivalent control group non-synchronized design. The data were collected from July 25 to October 21, 2005 at C Medical Center in Seoul. The patients were divided into two group of 24 subjects each. In order for measuring the all patient's anxiety before operation and aroma inhalation, Spidlberger(1975) trait anxiety, VAS(visual analogue scale) state anxiety, blood pressure, pulse rate were taken. After experimental group was taken aroma inhalation, VAS state anxiety, blood pressure, pulse rate were measured for two group. Results: After aroma inhalation, VAS state anxiety level, systolic blood pressure, diastolic blood pressure, pulse rate of the experimental group were decreased significantly than those of control group(p = .000, p = .000, p = .030. p = .000). Conclusion: The aroma inhalation method can be considered an effective nursing intervention that relieves the preoperative anxiety of abdominal surgical patients and stabilizes vital signs.

A New Method for Unconstrained Pulse Arrival Time (PAT) Measurement on a Chair

  • A new method of measuring pulse arrival time (PAT), which is usually used for the estimation of systolic blood pressure, in an unconstrained manner using a chair, is proposed. The capacitive-coupled ECG (CC-ECG) measurement system and the air cushion with balancing tubes system were used for unconstrained PAT measurement. Firstly, the correlation between the standard PAT (S-PAT) from the photoplethysmography (PPG) and the PAT measured in an unconstrained manner (U-PAT) was evaluated. It was observed that U-PAT, which is the time delay from the R-peak of ECG to the steepest decent point of air cushion pressure wave, is significantly correlated with the S-PAT. Secondly, systolic blood pressure (SBP) measured by the radial tonometer is compared to the U-PAT. The ten-beat averaged U-PAT removed respiration effects and demonstrated a high intra-subject correlation with SBP in all participants. Finally, the tonometry SBP was estimated from these U-PAT values for one participant intermittently during half a day.

Novel Detection Algorithm of The Upstroke of Pulse Waveform for Continuously Varying Contact Pressure Method (연속 가압방식의 맥파 측정방법을 위한 시작점 검출 알고리즘 개발)

  • We propose a continuously varying contact pressure(CVCP)-adaptive feature extraction algorithm for pulse diagnostic analysis. The CVCP method measures the pulse waveform with continuously increasing contact pressure(CP). This method offer a high resolution signal of the pulse waveform amplitude(PWA) as a function of the contact pressure. Therefore it enables us to overcome the limitation of commercially available pulse-taking devices whose analysis rely on a few number of PWA-CP pairs. We show that an efficient feature extraction algorithm which covers the features of the CVCP-method can be developed by sequentially applying Fast Fourier Transform, peak detection by center-to-edges method, baseline drift removal, detection of the percussion wave upstroke by intersecting tangent method and detection of the analysis region. Finally, by a clinical study with 30 subjects, we show that our CVCP-adaptive feature extraction algorithm detected the upstroke with accuracy of 99.46% and sensitivity of 99.51%, which were about 4.82% and 2.46% increases respectively, compared to a conventional feature extraction method. The proposed CVCP method and the CVCP-adaptive feature extraction algorithm are expected to improve the accuracy in the pulse diagnostic algorithms such as floating/sunken pulse qualities and deficient/excess pulse qualities.

Precise Measurement Method of Radial Artery Pulse Waveform using Robotic Applanation Tonometry Sensor (로보틱 토노메트리 센서를 이용한 요골 동맥 파형 정밀 측정 방법)

  • In this paper, a novel measurement method of radial artery pulse waveform using robotic applanation tonometry (RAT) was present to reduce the errors by the pressing direction of the vessel. The RAT consisted of an array of pressure sensors and 2-axis tilt sensor, which was attached to the universal joint with a linear spring and five-DOF robotic manipulator with a one-axis force sensor. Using the RAT mechanism, the pulse sensor could be manipulated to perpendicularly pressurize the radial artery. A pilot experimental result showed that the proposed mechanism could find the optimal pressurization angles of the pulse sensor within ${\pm}3^{\circ}$standard deviations. Coefficient values of variation of maximum pulse peaks extracted from the pulse waveforms were 4.692, 6.994, and 11.039 % for three channels with the highest magnitudes. It is expected that the proposed method can be helpful to develop more precise tonometry system measuring the pulse waveform on the radial artery.

  • In this paper, we proposed noninvasive and continous measurement of blood pressure using pulse transit time(PTT) and physical characteristic parameters and verified its clinical effectiveness. PTT can be obtained by electrocardiogram and phtoplethysmogram. There are many researches for estimating blood pressure using PTT which can be used for individual. However, it is not enough for extracting general regression equation which can estimate blood pressure for unspecified people. In this study, we suggested the regression equatin using PTT and physical characteristic parameters related to blood pressure and did measure blood pressure of many people. we compared the performance between two methods. As the results, we knew that the regressin model using PTT combined with physical characteristic parameters can estimate blood pressure more acurately and is closer to American National Standards Institute of the Association of the Advancement of Medical Instrument(ANSI/AAMI).
