Clinical and radiographical evaluation of mineral trioxide aggregate, biodentine and propolis as pulpotomy medicaments in primary teeth

  • Objectives: The purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine and Propolis as pulpotomy medicaments in primary dentition, both clinically and radiographically. Materials and Methods: A total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system. Results: The clinical success rates were found to be similar among the three groups at 3 and 6 mon where as a significant decrease in success rate was observed in Group III (84%) compared to both Group I (100%) and Group II (100%) at 9 mon. Radiographic success rates over a period of 9 mon in Groups I, II, and III were 92, 80, and 72%, respectively. Conclusions: Teeth treated with MTA and Biodentine showed more favorable clinical and radiographic success as compared to Propolis at 9 mon follow-up.

치수 절단술 약제의 치수 섬유모세포에 대한 세포독성 연구 (THE CYTOTOXICITY OF PULPOTOMY MEDICAMENTS ON HUMAN PULP FIBROBLAST CELLS)

  • 유치의 치수 절단술에 이용되는 약제들(formaldehyde, formocresol, paraformaldehyde, ferric sulfate)이 농도 변화에 따라 치수 섬유모세포의 활성, 단백질 기질의 합성 및 무기질 침착에 미치는 영향을 규명하고자, 각각 0.01, 0.05, 0.10, 0.25, 0.50, $1.00{\mu}l/ml$의 농도로 사람의 치수 섬유모세포에 24시간 적용시킨 후 세포활성검사와 함께, 조직의 재생과정 중 유기기질이 되는 단백질의 합성능과 무기질 침착에 관여하는 알칼리성 인산효소의 활성에 미치는 영향을 완충액만을 투여한 대조군과 비교 평가한 결과 다음과 같은 결론을 얻었다. 치수 절단술 약제를 첨가한 후 24시간 배양하였을 때, formaldehyde, formocresol, paraformaldehyde등의 경우 약제 농도가 증가함에 따라 대조군에 비하여 치수 섬유모세포의 세포활성이 크게 저하되었으며, ferric sulfate를 이용한 경우에는 세포활성이 유지되거나 약간 촉진되었다. formaldehyde, formocresol, paraformaldehyde사이의 세포활성간에는 유의성 있는 차이가 보여지지 않았다. 치수 절단술 약제 첨가 후 배양액내의 총단백질양에는 큰 변화가 없었으나, paraformaldehyde에 의해 총단백질양이 약간 감소하였다. 약제 첨가 후 세포내의 단백질양을 측정하였을 때, ferric sulfate의 경우, 약제의 농도가 증가하였을 때도 지속적으로 단백질 합성이 저해됨을 알 수 있었다. formaldehyde, formocresol, paraformaldehyde, ferric sulfate는 치수 섬유모세포의 알칼리성 인산효소 활성을 약간 저해하였다.

유치의 치수치료 : 치료법 선택의 근거 (Considerations for Pulp therapy in primary teeth)

  • According to the results of the questionnaire survey and the HIRA data released in KAPD conference in 2019, the pulpotomy in primary teeth is gradually decreasing while the one-visit pulpectomy is rapidly increasing. However, these changes seem to be for a more definite prognosis rather than based on academic grounds, and a more accurate diagnosis and conservative approach are needed. In addition, many studies have suggested that MTA pulpotomy was superior to any other materials and which are currently preferred as medicaments for pulpotomy. In the long term period, MTA pulpotomy is expected to be cost effective option.

Ferric Sulfate를 이용한 유치의 치수절단술 (PRIMANY TOOTH PULPOTOMY USING FERRIC SULFATE)

  • Pulpotomy is a frequently used treatment modality in primary teeth. It is method by which infected coronal pulp is removed while retaining vital radicular pulp. Since its introduction in 1930 by Sweet formocresol remains the most popular medicament for this treatment. However, despite its outstanding bactericidal properties, formocresol is known to cause adverse tissue reactions. Theoretically, formocresol disinfects and fixes radicular pulp and thus prevents infection and internal resorption. In reality, however, it leads to chronic inflammation and is sometimes responsible for failures through abscess formation and internal root resorption. Also, Myers et al., in 1978, reported on the systemic distribution of FC and other studies have followed with reports of its immunological, mutagenic and carcinogenic effects. Much effort has, therefore, focused on the development of alternative medicaments and techniques. Since its introduction in 19C, ferric sulfate proven itself as an effective hemostatic agent and is used as an astringent in dentistry. In 1988, Landau and Johnsen suggested ferric sulfate be used as a medicament in pulpotomy and many studies have focused on it to overcome the toxic effects of FC. Ferric sulfate acts through its ferric ion and iron ion, which react with blood protein leading to aggregation. The aggregated protein acts to plug the blood vessels, causing mechanical hemostasis. As blood clot formation is minimal, there is reduced inflammation of radicular pulp and enhanced healing. There are no reports regarding its systemic distribution. This is a report of cases treated by the author using pulpotomy with ferric sulfate.

Glutaraldehyde 및 Formocresol이 유견유치 치수조직에 미치는 영향에 관한 병리조직학적 연구 (A Histopathological Study of Pulp Tissue Reactions to Glutaraldehyde and Formocresol in Puppy's Primary Teeth)

  • This study was undertaken to evaluate the pulpal responses to the pulp-capping materials such as glutaraldehyde and formocresol in pulpotomy technique, especially in the primary dentition. Mandibular primary canines and molars of 5 dogs (aged about 8-9 weeks)were selected for this study. The intervals of observation for histologic study of pulpotomized primary teeth with 2% glutaraldehyde, formocresol and calcium hydroxide in the usual manner ranged from 2 hours, 1 week, 2 weeks, 3 weeks and 5 weeks after experiments respectively. Each specimens were fixed with 10% formalin and decalcified in 5% nitric acid. All slides were stained with Hematorylin-Eosin and examined histopathologically. The results were as follows; 1. In calcium hydroxide groups, formation of dentin bridge was initiated in 1 week after experiments and completed in 5 weeks after experiments. 2. Formation of dentin bridge was not seen, whereas necrosis of pulp tissue was noted, in formocresol and glutaraldehyde groups. 3. Duration of tissue reactions and tissue changes were similar, in formocresol and glutaraldehyde groups. 4. In formocresol and glutaraldehyde groups, amputation surfaces of the pulp were covered with blood clots, beneath which coagulation necrois was noted, but inflammatory cells were not prominent, in 2 hours and 1 week after experiments. But coagulation necrosis was proceeded to the apical portion, accompanied by infiltration of inflammatory cells, since 2 weeks after experiments. And suppuration or gangrene of the pulp tissue were noted in 3 weeks and 5 weeks groups. 5. Suppuration or gangrene of pulp seemed to provoke the resorption of dentin wall, and inflammatory changes and resorption of roots were noted in the periodontal membrane near the periapical region. 6. As compared with calcium hydroxide groups, resorption of the root was pronounced in form or cresol and glutaraldehyde groups. Effects of medicaments to the succedaneous tooth germ were not seen.

접착용 레진을 이용한 유치의 직접 치수복조술에 관한 연구 (DIRECT PULP CAPPING WITH BONDING RESIN)

  • 오랜 기간동안 다양한 약제를 통한 직접 치수복조술이 시도되어 왔다. 그 중 가장 보편적으로 사용된 약제는 수산화칼슘이다. 하지만 유치에서 수산화칼슘을 이용한 직접 치수복조술의 성공률은 생활치수절단술에 비해 낮다고 보고되어 왔다. 수산화칼슘의 단점으로 인해 다른 치수복조제를 찾기 위한 많은 연구가 있었으며 최근 여러 연구가들은 치수복조제로 접착용 레진의 이용을 제안하였다. 그들은 노출된 치수에 접착용 레진이 적용되었을 때 임상적 증상이 없이 염증이나 치수괴사가 없다고 주장하였다. 이 연구의 목적은 유치에서 기계적으로 노출된 생활치수에 치수복조제로써 수산화칼슘과 접착용 레진을 적용하여 단 기간동안의 예후를 비교해 보고, 또한 각각의 직접 치수복조술을 시행한 유치에 있어 생리적인 치근흡수 유무에 따른 예후의 차이를 비교해보기 위함이다. 와동형성 중 기계적으로 치수가 노출되어 생활 치수인 유치 41개를 대상으로 하였다. 그중 접착용 레진으로 치수복조제로 사용한 치아 21개를 1군으로 하였고, 수산화칼슘을 이용한 치아 20개를 2군으로 하였다. 그리고 제 1군과 2군의 치아들은 치근단방사선 소견을 통해 생리적인 치근흡수의 유무여부에 따라 다시 세분되었다. 모든 치아는 복합 레진으로 수복하였으며, 시술 3개월 후 타진반응, 냉 검사, 전기치수검사 등의 치수 생활력검사와 치근단방사선 소견으로 성공여부를 판단하였다. 결과는 다음과 같다. 1. 1군과 2군 사이의 성공률에는 유의한 차이가 없었다. 2. 각 군에서 생리적인 치근흡수가 없는 유치보다 있는 유치에서 성공률이 현저히 높았다. 3. 전치와 구치사이의 성공률에는 유의한 차이가 없었다.
