수술 후 보행지지 프로그램이 노인 상복부 수술환자의 폐 환기능 회복에 미치는 효과 (The Effects of Walking-Support Program on the Pulmonary Ventilatory Functions of Elders Following Upper-abdominal Surgery)

  • Purpose: The purpose of this study was to evaluate the effects of a walking-support program on the pulmonary ventilatory functions of elderly people who had undergone upper-abdominal surgery. Method: The study was a quasi-experimental research design. There were 26 partcipants who were admitted for upper-abdominal surgery to P University Hospital in B city. Walking exercise education was provided individually to the experimental group the day before their operation and 20 minutes a day for five days after the surgery using the 'Walking Exercise Guide Document'. Pulmonary ventilatory function was with FVC (Forced Vital Capacity), PEF (Peak Expiratory Flow), FEVI (Forced Expiratory Volume in 1 Second), FER (Forced Expiratory Ratio), Oxygen Saturation, and VAS (Visual Analog Scale). Results: The objective indexes of pulmonary ventilatory function were not significantly different between the two groups, but the subjective index was significantly different. Conclusion: With the above results, the walking support program could be an effective nursing intervention for improving pulmonary ventilatory function of surgical patients.

Do the Types of Seat Surface influence the pulmonary Functions during Prolonged Sitting?

  • Purpose: The purpose of this study was to identify the effects of the types of seat surface (static or dynamic seat surface) on the pulmonary functions during prolonged sitting. Methods: Thirty-four participants (20 males and 14 females) were recruited, and distributed randomly into dynamic prolonged sitting (DPS, n=17) and static prolonged sitting (SPS, n=17) groups. The DPS group was seated on a chair with a dynamic air cushion, and the SPS group was seated on a chair without a dynamic air cushion. The pulmonary function was assessed before sitting, and after participants had been seated for one hour. The pulmonary function [forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and Peak expiratory flow (PEF)] was measured using a spirometer. Results: Statistical analyses revealed significant differences in the time x group interactions of FVC, FEV1, PEF, and FEV1/FVC. The DPS group were significantly different in FVC, FEV1, PEF, and FEV1/FVC after prolonged sitting for one hour, compared to the SPS group (p<0.05). Conclusion: These findings suggest that dynamic sitting can prevent a decrease in the physiological function, such as pulmonary functions, rather than static sitting during prolonged sitting.

결핵성 폐질환의 폐절제술후 폐기능 및 동맥혈가스 분석에 대한 임상적 고찰 (Clinical Evaluation of Pulmonary Resection With Arterial Blood Gas Analysis and Pulmonary Function Test in the Pulmonary Tuberculosis)

  • We performed serial pulmonary function test and arterial blood gas analysis at preoperative period and postoperative 1st week in 337 patients who underwent pulmonary resection from May 1988 to April 1992 at Dept. of Thoracic and Cardiovascular Surgery, Seoul adventist hospital. Follow-up study for PFT and ABGA were possible in 30 % of the patients at postoperative 3rd or 4th month. In patient who underwent pneumonectomy, VC was decreased from 57.7% to 46.1%, FVC was decreased from 53.5 % to 41.2 % and MBC also decreased from 68.1% to 49.6 % at postoperative 1st week. But ABGA revealed that POa-, was increased from 87.2 mmHg to 92.7 mmHg, and PCO2 was decreased from 43.2 mmHg to 35.9 mmHg at postoperative 1st week. In patients who underwent lobectomy, VC was decreased from 78.1% to 68.30 %, FVC was decreased from 72.5% to 55.3% and MBC was decreased from 73.5% to 68% at postoperative 1st week.But, ABGA revealed that PO2 was increased from 95.2 mmHg to 97.9 mmHg and PCO2 was decreased from 42.3 mmHg to 39.0 mmHg at postoperative 1st week. The pulmonary function recovered at postoperative 3rd or 4th month and its ratio to preoperative value was 90% in lobectomy cases, but in pneumonectomy cases VC and MBC were recovered 20% and 15 % above the preoperative values. We concluded that resection of atelectasis, destructed lung, open negative and open positive cavity in the pulmonary tuberculosis were beni~t to improve ventilation-perfusion ratio,and pulmonary function was recovered nearly to preoperative level at postoperative 3rd or 4th month.

경피적 전기신경자극이 폐암 환자의 폐엽절제술 후 통증과 폐기능에 미치는 효과 (The Effect of Transcutaneous Electrical Nerve Stimulation on Pain and Pulmonary Function with Post-lobectomy Patient with Lung Cancer)

  • Purpose: This study was designed to evaluate the effect of TENS on pain and pulmonary function of post-lobectomy patients with lung cancer. Methods: The study data collection was done from February 4, 2008 to February 7, 2009. The subjects were assigned at random to the experimental group and control group with 20 subjects in each group. The experimental group was measured for pain and pulmonary function after surgery and then again after applying TENS 100 Hz frequency and 40 mA output for 20 minutes. The control group was measured the same as the experimental group except applying sham TENS. Results: The pain score of the experimental group which had TENS applied revealed that there were more significant reductions than the control group which had sham TENS applied. There was no significant difference with the number of times of receiving analgesics between the experimental and control group. The effect of TENS on pulmonary function was significantly different between the experimental group and the control group on VC 2 hours after surgery. There was no significant difference between FVC and FEV1. Conclusion: The findings of the study indicate that the TENS is effective in easing the pain of patients after a lobectomy.

심호흡 방법에 따른 흉관 삽입 기흉 환자의 폐환기능 (The Effects of Deep Breathing Methods on Pulmonary Ventilatory Function of Pneumothorax Patients undergoing a Thoracotomy)

  • Purpose: This study was to examine the effects of deep breathing exercises with Incentive Spirometer on the pulmonary ventilatory function of pnemothorax patients undergoing a thoracotomy. Mothod: This experiment used anonequivalent control group non-synchronized design which compared pre-experimental measures with post-experimental ones. The subjects of this study were 34 inpatients who were scheduled for a thoracotomy and classified into the experimental group (17 patients) or control group (17 patients) by using an Incentive Spirometer or not. The collected data was analyzed by a SPSS Win I PC (percentage, mean, standard deviation, chi-square test, t-test, repeated measured two-way ANOVA). Result: The Pulmonary Ventilatory Function of the experimental and control group were significantly increased on the first day, third day, and fifth day after the thoracotomy, but the group interaction period was not significant. Conclusion: This study showed that the deep breathing exercises with an Incentive Spirometer and deep breathing exercise without an Incentive Spirometer were both effective for recovering the pulmonary ventilatory function after a thoracotomy.

흉막 박피술후 폐기능회복에 관한 연구 (Pulmonary function improvement after decortication)

  • To study the recovery pattern of pulmonary function after decortication, the author performed serial pulmonary function tests using spirometry before and at lst., 3rd., 4th. week, lst., 3rd., 6th. month and 1st. year in 36 patients who underwent decortication from January 1989 to September 1991 at the Department of Thoracic and Cardiovascular Surgery, Pusan National University Hospital, Pusan, Korea. Patients were divided into 3 groups by the degree of compression of lung parenchyme. Group I was classified below 20%, Group II between 21 to 40%, Group III above 41%. Their serial changes of pulmonary function test were compared. The obtained results were as follows; 1. Maximal voluntary ventilation was recovered in 1st post perative week and even greater improvement was noted in group III in which ratio to 44 % of the preoperative value. 2. Vital capacity reached nearly to preoperative values in 3rd postoperative week and had increased much further to 26 % above the preoperative figure in group II. 3. Forced expiratory volume in 1 second returned rather slowly in 3rd-4th postoperative week and the mean VC was improved more higher in group II than the other groups following decorti cation. 4. There was an greatest improvement over all tests[MW, VC, FEV1] in 2nd decade which ratios to preoperative value were 34, 25 and 22 % respectively.

The Effects of Circuit Training and Circuit Training with Whole Body Vibration on Pulmonary Function in Adolescent

  • The purpose of this study was to parallel circuit training and circuit training with sonic systemic mechanism was to compare the differences in pulmonary function and chest expansion in adult men. This study was performed on 20 subjects. 20 subjects were divided into two groups; Circuit training group(n=10), Circuit training with sonic systemic mechanism(n=10). Both of the group performed the exercise 3 times a week for 5 weeks. The data was analyzed by the Repeated t-test for comparing before, during and after changes of factors in each group and the Independent t-test for comparing the between groups. The result are as follows. Circuit training group was statistically significant difference FVC, FEV1/FVC(p<.05), Circuit training with sonic systemic mechanism group was statistically significant difference PEF, VC in pulmonary function(p<.05). Circuit training group was statistically significant difference FEV1/FVC of between the two group in pulmonary function(p<.05). Circuit training group and circuit training with sonic systemic mechanism group was statistically significant difference in chest expansion(p<0.05) and there was no statistically significant difference of between the two group in chest expansion(p>.05).

최대부하운동 후 20대 남성 흡연자와 비흡연자의 폐기능과 심박수 회복 반응 (The Pulmonary Function and Heart Rate Recovery Response of Smoker and Nonsmoker in Males Aged 20s after Graded Maximal Exercise)

  • Purpose : The purpose of this study was to investigate the response of pulmonary function and heart rate recovery of smoker and nonsmoker in males aged 20s after graded maximal exercise. Method : The subjects were composed of smoker group (n=12) and nonsmoker group (n=12) in males aged 20s. Each groups completed an graded maximal exercise with Bruce protocol and were assessed on the pulmonary function(forced vital capacity : FVC, forced expiratory volume-one second : FEV1, FEV1/FVC) and heart rate. Result : The results were as follows: First, heart rate in the measurement point was a statistically significant difference for smoker and non-smoker group after maximal exercise, but FVC, FEV1, FEV1/FVC was no difference. Second, FEV1/FVC between smoker and nonsmoker group was a statistically significant difference after maximal exercise, but FVC, FEV1, heart rate was no difference. Conclusion : The results of this study is that smoking is negative effects on FEV1/FVC of pulmonary function in males aged 20s after maximal exercise.

젊은 여성의 머리 전방전위 자세의 정도와 폐기능의 관계 (The Relationship Between Forward Head Posture and Pulmonary Function in Young Women)

  • Purpose: This study aimed to investigate the relationship between forward head posture and pulmonary function in young women. Methods: Thirty-three young women participated in this study. The participants were grouped into non-forward head posture, mild forward head posture, and moderate-severe forward head posture groups. The craniovertebral angle (CVA) and pulmonary function were measured using ImageJ and a spirometer, respectively. Results: Statistically significant differences in the participants' forced expiratory volume for one second (FEV1) and predicted forced expiratory volume for one second (predicted FEV1) were found among the three groups. No statistically significant differences in the participants' forced vital capacity (FVC), predicted forced vital capacity (predicted FVC), FEV1/FVC, and peak expiratory flow (PEF) were found among the three groups. Furthermore, a significant positive correlation was found between CVA and FVC and among the predicted FVC, FEV1, and predicted FEV1. Conclusions: The results of this study demonstrate that severe forward head posture has a negative effect on pulmonary function in young women.

  • A new auscultation system for the detection of breath sound form trachea was developed in house. Small size microphone(panasonic pin microphone) was encapsuled in a housing for resonant effect, and hardware for the sound detection was fabricated. Pulmonary function test results were compared with the parameters extracted from frequency spectrum of breath sound obtained from the developed system. Results showed that the peak frequency and relative ratio of integral values between low(80∼400Hz) and high(400∼800Hz) frequency ranges revealed the significant differences. Developed system could be used for distinguishing normal subject and the patients who have pulmonary disease.
