The purposed of this study was to identify compliance with sick role behavoir and support by families of patients with pulmonary tuberculosis and to identify factors affecting these variables md factors that increase compiance with sick role behavoior This study was a survey design done in K. general hospital from 1992. 7 1 to 1992. 7. 20. The data were collected through personal interviews with 70 subjects who had pulmonary tuberculosis. A questionnaire was used to collect the data. 1. Demographic characteristics of the subjects The factor causing the tuberculosis for 28.6% of the subjects was that they lived with another family member who had pulmonary tuberculosis, 11.4% of the subjects had a family member who died of tuberculosis, 24.3% of the subjects had experienced an adverse reaction to medication. Gastroenteric disorders(28.6%) had the highest rate for this kind of adverse reaction to medication. 2. Support by families for patients with pulmonary tuberculosis The items which showed high support for compliance were taking medicine regularly(80.0% ) eat-ing a balanced diet(80.0%), loving them(84.3%), taking care of them(82.8%), getting the support from people for being sick(81.1%) The items which showed low support for compliance were meeting relatives or clergymen(20%), talking with the people suffering from pulmenary tuberculosis to relieve frustrated because they were not recovering. 3. General characteristics and family support The degree of family support showed that 75 was highest score and 30, the lowest score with 55.5(74. 1%)the average score. The degree of family support appeared similar in cases where the person was married and where the person did not have a family member with pulmonary tuberculosis(P〈.01) 4. Purpose: The purpose of this study was to examine the effects of breathing exercises performed using panflutes in elderly patients undergoing spinal surgery. Methods: The study design was a nonequivalent control group non-synchronized pre-post test. The study included 24 patients in both the experimental group and the control group. The experimental group completed a daily breathing exercise regimen using panflutes for 30minutes after meals, whereas the control group was provided standard preoperative education, including breathing exercises using incentive spirometers. After the exercise regimen, breathing exercise compliance, pulmonary infections, and life satisfaction were measured in both groups, and the data were analyzed using the SPSS/WIN program. Results: The compliance rate of breathing exercises was significantly higher in the experimental group. The experimental group presented no pulmonary infections in the later period, whereas the control group presented higher pulmonary infection rates in the same period. In addition, the life satisfaction score in the experimental group significantly increased. Conclusion: The breathing exercise program using panflutes for elderly patients undergoing spinal surgery enhanced their breathing exercise compliance and their daily life satisfaction in addition to reducing their pulmonary infection rates.
Purpose: The purpose of this study is to get basic user guidelines of safe and efficient bag-valve-mask application on patients having abnormal pathophysiological pulmonary conditions. Methods: This study was performed by pre-qualified 35 EMS junior and senior students. Participants were instructed to compress ambulatory bag randomly about half, one-third, one-fourth within different airway resistance and pulmonary compliance. Resultant tidal volume and pulmonary wedge pressures on RespiTranier monitor were analysed in relation to pulmonary physiologic index. Results: At least over half compression of bag guaranteed minimal tidal volume regardless of pulmonary conditions. There was no increase of pulmonary wedge pressure above the level of barotrauma on half compression at any pulmonary conditions. Conclusion: Assisted ventilation with ambulatory bag on patients with pathological pulmonary conditions should be over half compressed regardless of respiratory disease entity.
Background: Positive end, expiratory pressure (PEEP) has become one of the standard therapies for adult respiratory distress syndrome (ARDS). Total static compliance has been proposed as a guide to determine the size of PEEP ('best PEEP') which is of unproven clinical benefit and remains controversial. Besides increasing functional residual capacity and thus improving oxygenation, PEEP stimulate prostacyclin secretion and was proposed for the treatment of acute pulmonary embolism. But little is known about the effect of PEEP on hemodynamic and gas exchange disturbances in acute pulmonary embolism. Methods: To study the validity of total static compliance as a predictor of 'best PEEP' in ARDS and acute pulmonary embolism, experimental ARDS was induced in mongrel dog with oleic acid and acute pulmonary embolism with autologous blood clot. Then hemodynamic and gas exchange parameters were measured with serial increment of PEEP. Results:In ARDS group, total static compliance and oxygen transport were maximal at 5 cm$H_2O$, and decreased thereafter (p<0.05). With increment of PEEP, arterial oxygen tension ($PaO_2$) and arterial carbon dioxide tension ($PaCO_2$) increased and cardiac output and physiological shunt decreased. In pulmonary embolism group, total static compliance, oxygen transport, physiological shunt and cardiac output decreased and $PaO_2$ and $PaCO_2$ increased with increment of PEEP (p<0.05). Comparing the change induced by increment of PEEP by 1 cm$H_2O$ in ARDS group with that in pulmonary embolism group, there was no significant difference between two groups except cardiac output which decreased more in pulmonary embolism group (p<0.05). In ARDS group, oxygen transport and total static compliance increased after PEEP application, and total static compliance was maximal at the PEEP level where oxygen transport was maximal. However in pulmonary embolism group, oxygen transport and total static compliance decreased after application of PEEP. There was significant correlation between change of total static compliance and change of oxygen transport in both groups. Conclusion: In both ARDS and acute pulmonary embolism, it can be concluded that total static compliance is useful as a predictor of 'best PEEP'.
The effect of reticuloendothelial hyperfunction on hypothermic preservation of lung was studied in dogs. In order to evaluate the viability after hemodynamic_ load in preserved isolated lung, observations were made on the rate of increase in weight, degree of edema,compliance and surface activity of lung. The results obtained as follows: l. In the group of activating of the reticuloendothelial system by injection of sodium thiosulfate intravenously before pneumonectomy and infusion of naphthionine through the pulmonary artery before hypothermic preservation of isolated lung the limit of preservation was eight hours whereas four hours in non-treated control group. 2.Therefore the method of activating of the reticuloendothelial system before and after pulmonary resection seems effective in preserving for prolonging the period of preservation of lung by means of inhibition of pulmonary edema. 3. Pulmonary surface activity is expected to be valuable as a method in evaluation of the viability of preserved lung along with compliance and rate of increase in weight of lung.
Purpose: This study was conducted to examine the effects of the nurse-led one-on-one discharge education on the levels of symptom experience and self-care compliance among patients with chronic obstructive pulmonary disease (COPD) who had not previously participated in an education or respiratory rehabilitation program. Methods: This study used a quasi-experimental pre-and post-test design with nonequivalent control group. Sixty seven hospitalized patients (34 for experimental and 33 for control groups) were recruited from in-patient units of a general hospital from August to November, 2010. The one-on-one education session of symptom management and daily self-care was administered to experimental group for 50 minutes along with a follow-up phone call. Self-administered questionnaire was administered at the pretest and at 1 month after discharge. Results: The subjects were 67.43(${\pm}10.24$)years old in average. The mean differences of symptom experience (t=3.39, p=.001) and self-care compliance (t=-38.13, p<.001) in the experimental group was significantly higher than those of the control group. Conclusion: Nurse-led one-on-one discharge education was effective in reducing the level of symptom experience and enhancing self-care compliance at home within one month after discharge. Therefore, this form of one-on-one education provided by nurses might be applicable effectively for COPD patients who do not have access to respiratory rehabilitation programs.
Background: Preoperative risk analysis for Fontan candidates is still less than optimal in that patients with apparently low risks may have poor surgical outcome; prolonged pleural drainage, protein losing enteropathy, pulmonary thromboembolism and death. We hypothesized that low pulmonary vascular compliance (PVC) is a risk factor for prolonged pleural effusion drainage after the Fontan operation. Material and Method: A retrospective review of 96 consecutive patients who underwent the Extracardiac Fontan procedures (median age: 3.9 years) was performed. Fontan risk score (FRS) was calculated from 12 categorized preoperative anatomic and physiologic variables. PVC $(mm^2/m^2{\cdot}mmHg)$ was defined as pulmonary artery index $(mm^2/m^2)$ divided by total pulmonary resistance $(W.U{\cdot}/m^2)$ and pulmonary blood flow $(L/min/m^2)$ based on the electrical circuit analogue of the pulmonary circulation. Chest tube indwelling time was log-transformed (log indwelling time, LIT) to fit normal distribution, and the relationship between preoperative predictors and LIT was analyzed by multiple linear regression. Result: Preoperative PVC, chest tube indwelling time and LIT ranged from 6 to 94.8 $mm^2/mmHg/m^2$ (median: 24.8), 3 to 268 days (median: 20 days), and 1.1 to 5.6 (mean: 2.9, standard deviation: 0.8), respectively. FRS, PVC, cardiopulmonary bypass time (CPB) and central venous pressure at postoperative 12 hours were correlated with LIT by univariable analyses. By multiple linear regression, PVC (p=0.0018) and CPB (p=0.0024) independently predicted LIT, explaining 21.7% of the variation. The regression equation was LIT=2.74-0.0158 PVC+0.00658 CPB. Conclusion: Low pulmonary vascular compliance is an important risk factor for prolonged pleural effusion drainage after the extracardiac Fontan procedure.
Purpose: The aim of this study was to assess knowledge, attitude, correct metered dose inhaler (MDI) use and compliance with self management among patients with chronic obstructive pulmonary disease (COPD). Methods: The participants of this study consisted of 109 COPD patients who were outpatients in C and K hospital located in G city from March 1 to September 30th, 2010. Data were measured using self-administered questionnaires and observational checklist. The data were analyzed using SPSS/WIN 18.0 program that included mean, standard deviation, ANOVA, Sheffe test, and Pearson's correlation. Results: There were significant relationships between knowledge and attitude (r=.33, $p$ <.001), between knowledge and correct MDI use (r=.37, $p$ <.001), and between knowledge and self-management compliance (r=.28, $p$=.003). There was significant relationship between attitude and self-management compliance (r=.33. $p$ <.001). In contrast, attitude was not significantly related to correct MDI use. Conclusion: The study showed that COPD knowledge and attitude of patients were related compliance in managing their disease. Therefore, strategies need to be utilized in education programs which would improve knowledge and subsequently improve attitude and compliance.
Purpose. This study was designed to examine the effect of asthma management education program applied to allergic asthma patients receiving immunotherapy due to house dust mite on their stress and compliance with health care regimens. Methods. A quasi experimental design with non-equivalent control group and non-synchronized design was used. The subjects of this study were 61 patients who were receiving immunotherapy at intervals of a week after their symptoms were diagnosed as house dust mite allergic asthma at the pulmonary department of a university hospital in Seoul. They were divided into an experimental group of 29 patients who received asthma management education and a control group of 32 patients. The asthma management education pro-gram was composed of group education (once) and reinforcement education (three times) with environmental therapy and immunotherapy to house dust mite. Results. Stress significantly decreased in the experimental group compared to that in the control group. Compliance with health care regimens significantly increased in the experimental group compared to that in the control group. Conclusions. The results suggested that the asthma management education program is effective for the management of stress and the improvement of compliance in patients with allergic asthma to house dust mite.
Sepsis is a life-threatening condition caused by infection and represents a substantial global health burden. Recent epidemiological studies showed that sepsis mortality rates have decreased, but that the incidence has continued to increase. Although a mortality benefit from early-goal directed therapy (EGDT) in patients with severe sepsis or septic shock was reported in 2001, three subsequent multicenter randomized studies showed no benefits of EGDT versus usual care. Nonetheless, the early administration of antibiotics and intravenous fluids is considered crucial for the treatment of sepsis. In 2016, new sepsis definitions (Sepsis-3) were issued, in which organ failure was emphasized and use of the terms "systemic inflammatory response syndrome" and "severe sepsis" was discouraged. However, early detection of sepsis with timely, appropriate interventions increases the likelihood of survival for patients with sepsis. Also, performance improvement programs have been associated with a significant increase in compliance with the sepsis bundles and a reduction in mortality. To improve sepsis management and reduce its burden, in 2017, the World Health Assembly and World Health Organization adopted a resolution that urged governments and healthcare workers to implement appropriate measures to address sepsis. Sepsis should be considered a medical emergency, and increasing the level of awareness of sepsis is essential.
