Pulmonary function test is a group of tests which are composed of measurement for lung function. Thy are spirometry, blood-gas analysis, lung volumes, exercise test, diffusion capacity, and bronchial challenge test. In this article, I will review the pulmonary function test and it's application in terms of clinical aspect and insurance medicine. The standard spirometric indicies are forced vital capacity(FVC), forced expiratory volume at 1 second(FEV1), and the ratio of FEV1 over FVC(FEV1/FVC). If the value of FEV1/FVC less than 70%, the examinee has obstructive ventilatory dysfunction.
Surgical resection of lung cancer or other disease is recently required in patients with severely impaired lung function resulting from chronic obstructive pulmonary disease or disease extension. So prediction of pulmonary function after lung resection is very important in thoracic surgeon. We studied the accuracy of the prediction of postoperative pulmonary function using perfusion lung scan with 99m technetium macroaggregated albumin in 22 patients who received the pneumonectomy. The linear regression line derived from correlation between predicting[X and postoperative measured[Y values of FEV1 and FVC in patients are as follows: 1 Y[ml =0.713X + 381 in FEV1 [r=0.719 ,[P<0.01 2 Y[ml =0.645X + 556 in FVC [r=0.675 ,[P<0.01 In conclusion,the perfusion lung scan is noninvasive and very accurate for predicting postpneumonectomy pulmonary function.
Objective: The interest of clinicians is increasing due to the newly established medical insurance for pulmonary rehabilitation. Improvement of respiratory muscle strength and pulmonary function is an important factor in pulmonary rehabilitation, and this study aims to investigate the correlation between changes in respiratory muscle contraction thickness that can affect respiratory muscle strength and pulmonary function. Design: Cross-sectional observational study. Methods: Thirty-one subjects (male=13, female=18) participated in this study. The respiratory muscle strength was measured by dividing it into inspiratory/forced expiratory muscles, and the pulmonary function was measured by forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and FEV1/FVC. To evaluate the respiratory muscle length increase, in resting and concentric contraction thickness of diaphragm, external/internal oblique, transverse abdominis, and rectus abdominis were measured by using ultrasonography. Results: Inspiratory muscle strength showed a significant correlation with the length increase of the inspiratory muscle (r=0.368~0.521, p<0.05), and forced expiratory muscle strength showed a significant correlation with length increase of forced expiratory muscle (r=0.356~0.455, p<0.05). However, pulmonary function was not correlated with the length increase of the respiratory muscle. Conclusions: In this study, a correlation between respiratory muscle strength and respiratory muscle length increase was confirmed, but no correlation with the pulmonary function was found. It is considered that the respiratory muscle strength can be improved by increasing the respiratory muscle thickness through appropriate respiratory muscle training.

A new auscultation system for the detection of breath sound form trachea was developed in house. Small size microphone(panasonic pin microphone) was encapsuled in a housing for resonant effect, and hardware for the sound detection was fabricated. Pulmonary function test results were compared with the parameters extracted from frequency spectrum of breath sound obtained from the developed system. Results showed that the peak frequency and relative ratio of integral values between low(80∼400Hz) and high(400∼800Hz) frequency ranges revealed the significant differences. Developed system could be used for distinguishing normal subject and the patients who have pulmonary disease.
Purpose: This study aims to investigate the changes in pulmonary function, gait ability, and quality of life when NMES is applied along with CBE and to provide basic clinical data to be used in pulmonary rehabilitation guidelines for patients with COPD to treat patients with severe COPD. Methods: For this study, CBE and NMES of quadriceps femoris on both sides were applied to the experimental group (n=10), and only CBE was applied to the control group (n=10). For a pre-test, a 6-minute walk test was performed, and pulmonary function and health-related quality of life were measured. Moreover, an exercise program was applied to each group for 30 minutes per session, 5 times a week, for 6 weeks. After that, a post-test was conducted the same way as the pre-test. Results: In the within-group comparison, there were significant differences in forced expiratory volume in one second, 6-minute walk test and health-related quality of life between the experimental group and the control group (p<0.01)(p<0.05). In the between-group comparison, the experimental group showed an increase in forced expiratory volume in one second and 6-minute walk test (p<0.05) and showed a decline in health-related quality of life (p<0.05). Conclusion: The 6-weeks NMES program improved health-related quality in patients with severe COPD by increasing expiratory volume by reinforcing the function of quadriceps femoris. This finding implies that NMES could be an alternative mode for improving physical functions of patients with severe COPD, who cannot participate in a breathing exercise program or are reluctant to participate.

The purpose of this study was an determine whether elastic band on treadmill training might effect the chest expansion and pulmonary function of the 20's men. 40 subjects with experimental group(male: 20) and control group(male: 20) was participated in experiment. During four weeks, each group participated thirty minutes for three times per week. Subjects were assessed using pre-value and post-value measurement chest length(chest length for resting, chest expansion) and pulmonary function(forced vital capacity, forced expiratory volume at one second, FEV1/FVC, peak expiratory flow, vital capacity, tidal volume, expiratory reserve volume, inspiratory reserve volume) by the CardioTouch 3000S(BIONET, USA). These findings suggest that experimental group can be used to improve chest expansion, pulmonary function than control group. In comparison of both groups, post-test was more improved in experimental group. In conclusion, experimental group helped improving function of pulmonary volume and respiratory muscle, and thus it indicates that the functions will be more improved through the continued respiratory exercise program.
Determination of preoperatibe pulmonary function is crucial in avoiding complications from pulmonary resection, especially pneumonectomy. Postoperative morbidity and mortality were correlated with the preoperative results of five widely used tests of pulmonary function in 40 patients who underwent pneumonectomy for bronchiectasis, pulmonary tuberculosis, and carcinoma of the lung. Factors analyzed following operation included 30-day mortality, the incidence of arrhythmia, the frepuency of respiratory complications, and the number of individuals requiring prolonged mechanical ventilation. There were statistically significant differences[p<0.001]in mean values among FVC, FEV1, FEV1/FVC and MVV. But the difference of the FEF25-75% was not statistically significant.
Background: Abdominal bracing exercise (ABE) and abdominal hollowing exercise (AHE) improve the lumbar flexibility and pulmonary function in various patients, yet the efficacy of ABE or AHE have not yet been evaluated. Objects: The purpose of this study was to compare the lumbar flexibility and pulmonary function during both ABE and AHE in healthy adults. Methods: The study included 40 healthy adults, who were randomly divided into the experimental group and control group, each with 20 subjects. All subjects performed ABE (experimental group) and AHE (control group). The lumbar flexibility such as trunk flexion test (sitting and standing position) and schober test and pulmonary function such as the spirometer including forced vital capacity (FVC) and force expiratory volume in one second ($FEV_1$) and chest circumference measurement (middle and lower chest) were measured, respectively. Two-way repeated analysis of variance was used to compare the lumbar flexibility and pulmonary function, respectively. Results: No significant effects of lumbar flexibility were observed on trunk flexion test from the sitting position (P=.478) and standing position (P=.096) in the ABE than in the AHE. However, the length of ABE was longer significantly than it of AHE (P=.024). No significant effects of lung function were observed on the FVC (P=.410) and $FEV_1$ (P=.072) in the ABE group than in the AHE group. And also, no significant effects of chest circumference measurement were observed on the inspiration (P=.468) and expiration (P=.563) in middle chest circumference and inspiration (P=.104) and expiration (P=.346) in lower chest circumference. Conclusion: This study indicated that the ABE is only more effective in lumbar flexibility by lumbar length difference than AHE in healthy adults.
Pulmonary function test has been know to be greatly affected by body indices, such as sex, age, height, body weight, body surface area (BSA) and body mass index (BMI), so hat this study was focused to see the relationship between body index and flow-volume curves. Subjects were 156 (male 90, female 66) and they were examined for pulmonary function test in terms of body index and correlation/multiple regression analysis of flow-volume curves at Presbyterian Medical Center from March to August, 2009. The followings results after analyzing the correlation between body index and flow-volume curves. Although flow-volume curve FEF25-75% showed close correlation with age, body weight, and body surface area, but not with body mass index. In addition, multiple regression analysis was performed to see how each body index affects flow-volume curve FEF25-75%, and FEF25-75% dispersion was explained as 74.5% with age only, 94.2% with age and height, and 96% with age, height, and sex. Therefore, sex, age and height that are mainly used for predictive formular of pulmonary function test and nomogram were important factors for pulmonary function test itself, and further study must be done for other body index.

The purpose of this study was to analyze the correlation between pulmonary function, basic physical fitness (PF), and physical activity (PA), and to compare the differences by gender in Korean college students. Measurements of body composition, basic PF, PA (questionnaire), and pulmonary function tests of forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) using a Quark pulmonary function test were carried out on 312 healthy participants (150 males [mean age: 19.29±1.72 years] and 162 females [mean age: 19.05±1.17 years]). The pulmonary function of male students was related to right-handedness, left-handedness, and back strength, and the pulmonary function of female students was related to all basic PF. The pulmonary function of male students was related to all PA variables, whereas the pulmonary function of female students was related to middle-intensity PA. The findings of this study suggest that male students need to increase PA, and female students need to improve basic PF to sustain a healthy pulmonary function. Understand gender differences for pulmonary function-related factors and the gender-specific educational efforts are needed to improve and maintain pulmonary capacity in college students.
