A Survey on Patient's Experience and Perception on Health Care Utilization for Developing of a Korean Medicine Clinical Practice Guideline for Puerperal Wind Disorder (산후풍 한의표준임상진료지침을 위한 환자의 의료 이용 경험 및 인식도 조사)

  • Objectives: The purpose of this study is to reflect the patient's perspective in the process of developing Korean medicine clinical practice guideline (CPG) of puerperal wind disorder by survey. Methods: Five hundred fifty patients were surveyed from November 3rd, 2021 to November 8th, 2021 by internet. This study is an exploratory cross-sectional survey study, and descriptive statistics and frequency analysis were conducted on respondents' general characteristics, postpartum symptoms, the history of using treatment institution for puerperal wind disorder, satisfaction of medical institutions and perception of postpartum care. Results: Survey results showed that 92.0% of respondents experienced symptoms after childbirth, and 56.2% of the symptoms were arthralgia, followed by obesity with 41.8%. Among puerperal wind disorder patients, 34.2% had treatment history, and 54.3% received Korean medical treatment. Treatment satisfaction was confirmed to be higher in Korean medical treatment. The necessity of postpartum care was recognized at 95.7% of respondents, and the performance rate of traditional Korean postpartum care was also high. Conclusions: Based on a realistic patient-centered basis, it is a study that can lay the foundation for standardizing Korean medicine treatment and strengthening coverage in the future.

A Delphi Survey for the Revision of the Diagnostic Criteria for Sanhupung (Puerperal Wind Disorder, U32.7) (산후풍 진단 기준 개정 위한 델파이조사 결과)

  • Objectives: The definition of Sanhupung (Puerperal wind disorder) has been varied and there has been a prior study to establish the definition, but no clear conclusion has been reached on diagnostic criteria. Therefore, the aim of this study was to clearly redefine the definition of Sanhupung using the Delphi method. Methods: This study used the Delphi technique. A panel consisting of 13 experts of Korean medicine, particularly in Obstetrics & Gynecology, participated in the Delphi survey that included answering the 3rd round survey. The Delphi survey was conducted by evaluating and correcting the questionnaire using e-mail. Results: Through the Delphi survey, we have reached an agreement regarding the diagnostic criteria of Sanhupung. They are as follows: 1) Sanhupung can be diagnosed based on basic symptoms. If one or more symptoms are expressed in the basic symptom group, it can be diagnosed as Sanhupung. It is diagnosed in detail as a pain type or a sensory impairment type according to the category of basic symptoms. 2) Incidental symptoms are not essential for diagnosis, and are referred to for checking general weakness and autonomic nervous system conditions. 3) In order to meet the diagnostic criteria, the symptoms should occur within 6 months after childbirth or miscarriage, and the cause of the symptoms should not be classified as other diseases. Conclusions: The diagnostic criteria of Sanhupung were suggested based on the Delphi survey among experts in the field. Further research is necessary to improve the reliability and validity of the criteria.

  • The literatual study on the Bogi haehun-tang(補氣解暈湯), the results were obtained as follows : 1. Bogihaehun-tang(補氣解暈湯) is reported Fuqingzhu's Gynecology, Qing dynasty at the first and is used when puerperal faintness, a disorder due to deficiency of vital energy and blood and up ward attack of the asthenic yang, or due to lochistisis and blood stasis attacking the heart. 2. Bogihaehun-tang(補氣解暈湯) is composed of Radix Ginseng(人蔘), Radix Astragali, Radix Angelicae sinensis(當歸) Rhizoma Zingiberis praeparatae(乾臺), Spica Schizonepetae(荊芥). 3. Bogihaehun-tang(補氣解暈湯) have an effect on homeostasis by in vigorating the vital energy, a treatment for prolonged bleeding due to deficiency of vital energy(補氣攝血) applicable to the patient suffening from chronic hemorrhagic disease accompanied with the anifestations of the deficiency of vital energy.
