보건소 모성과 어린이 건강관리사업의 효율적 운영을 위한 조직구축 연구 (Development of Organization System for Health Management Program of Maternal-child in Public Health Centers)

  • Purpose: The purposes of this study are: to evaluate the effectiveness of organizations engaged in MCH programs; and to suggest the executive organizational system of maternal and child health (MCH) services linked to HP services at public health centers. Method: This study analyzes data on the present conditions and evaluates the organizational effectiveness of MCH workers at public health centers nationwide. The organizational effectiveness of MCH organization is assessed by MCH workers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Result: The results are as follows. Of public health centers, 61.9% have a MCH organization for their MCH programs. For the other 38.2%, MCH services are undertaken by HP organization. Executive organizational systems for MCH services at leading MCH health centers are classified into three types: Type I (provides MCH and HP services through MCH organizations); Type II (provides MCH and HP Services through HP organizations) ; and Type III (provides MCH services through MCH organizations and HP services through HP organizations). The evaluation of the organizational effectiveness of MCH organizations shows that Type II is the most effective in terms of teamwork, autonomy, service quality and resource utilization. Conclusion: This study suggests that executive organizations for MCH and HP services for women and children should be established in accordance with lifetime health programs in order for public health centers to utilize the limited MCH and HP resources efficiently.

도시보건소 공무원의 조직몰입도 인과요인에 관한 연구 - 한 가설적 인과모형분석을 통해 - (A Study on Causal Factors of Organizational Commitment of Public Servants in Urban Health Centers: Testing a Hypothetical Canusal Model)

  • To find causal factors and improvement plans of organizational commitment of public servants in urban health centers, a hypothetical causal model, which included 2 endogenous variables(organizational commitment & organizational satisfaction) and 15 exogenous variables, was constructed. Exogenous variables consisted of individual factors (sex, age, education, job-grade, and annual salary), psychological variables(pride for organization, extrinsic motivation, intrinsic motivation and support of supervisor) ad structural variables(formalization, centralization, communication, job-conflict, job-decision, and workload). In the hypothetical causal model, organizational commitment was supposed to be effect variable, and organizational satisfaction was presumed to be intervening variable to mediate between organizational commitment and exogenous variables. For data collection, cross-sectional self-administered questionnaire survey was conducted to 1,295 public servants from 32 urban health centers nationwide. The survey responses were from 934, 72.1% of subjects. But 756 responses(58.4%) were analyzed because of excluding ones with missing values. The hypothetical causal model was fitted by covariance structural analysis with maximum likelihood method. Main results were as follows: (1) The fitted causal model accounted for 33 and 55 percent of total variance of organizational commitment and organizational satisfaction of public servants, respectively. (2) In order of effect size, pride for organization, supervisor support, communication, extrinsic motivation and centralization had an indirect effect effect on organizational commitment through organizational satisfaction. However, the effect of centralization was negative. (3) Pride for organiztion, intrinsic motivation, organizational satisfaction, job-conflict, supervisor support, communication, age, centralization, annual salar and extrinsic motivation had indirect or direct effects on organizational commitment in order of effect size. Among them, effects of job-conflict and centraldization were negative. In conclusion, these results suggested that organizational commitment of public servants in urban health centers could be enhanced by pride for organization, intrinsic and extrinsic motivations, prevention of job-conflict and excess centralization, supervisor support and active communication. Especially, pride for organization and intrinsic motivation were expected to play the most important role.

Postpartum Depression in Young Mothers in Urban and Rural Indonesia

  • Objectives: Young mothers are vulnerable to postpartum depression due to role transition-related stress. Understanding the causes underlying these stressors is essential for developing effective interventions. Methods: This study analyzed the 2018 Indonesian Basic Health Research data. The Mini International Neuropsychiatric Interview was used to assess postpartum depression symptoms in mothers aged 15-24 years with infants aged 0-6 months. In 1285 subjects, the risk factors for postpartum depression were evaluated using multivariate logistic regression. Results: The overall prevalence of depression in the 6 months postpartum was 4.0%, with a higher prevalence in urban areas (5.7%) than in rural areas (2.9%). Urban and rural young mothers showed distinct postpartum depression risk factors. In urban areas, living without a husband (odds ratio [OR], 3.82; 95% confidence interval [CI], 1.24 to 11.76), experiencing preterm birth (OR, 4.67; 95% CI, 1.50 to 14.50), having pregnancy complications (OR, 3.03; 95% CI, 1.20 to 7.66), and having postpartum complications (OR, 5.23; 95% CI, 1.98 to 13.80) were associated with a higher risk of postpartum depression. In rural areas, postpartum depression was significantly associated with a smaller household size (OR, 3.22; 95% CI, 1.00 to 10.38), unwanted pregnancy (OR, 4.40; 95% CI, 1.15 to 16.86), and pregnancy complications (OR, 3.41; 95% CI, 1.31 to 8.88). Conclusions: In both urban and rural contexts, postpartum depression relates to the availability of others to accompany young mothers throughout the postpartum period and offer support with reproductive issues. Support from the family and the healthcare system is essential to young mothers' mental health. The healthcare system needs to involve families to support young mothers' mental health from pregnancy until the postpartum period.

  • 2005년 8윌 현재 우리나라 공립 초 ${\cdot}$${\cdot}$ 고교 교육시설은 9,007개가 운용되고 있다. 그러나 향후에는 인구감소 등으로 인하여 시설물의 신축보다는 기존시설물의 합리적인 이용에 더 큰 관심과 예산이 투입될 것으로 예상된다. 현재 교육시설물 유지관리는 예방보전이 아닌 사후보전에 의존하고 있고, 유지관리 조직 및 체계가 구축되지 못하여 시설물 노후화 및 예산낭비를 초래하고 있다. 이에 본 연구는 기존 공립학교 시설물의 유지관리를 검토하고, 기존 유지 관리 조직의 효율성을 극대화할 수 있는 개선방안 제시를 목적으로 한다.

공공도서관 상급자의 윤리적 리더십이 조직시민행동에 미치는 영향 (The Effects of Leader's Ethical Leadership on Organization Citizenship Behaviors in the Public Libraries)

  • 이 연구는 공공도서관에서 근무하는 상급자의 윤리적 리더십이 구성원의 조직시민행동에 미치는 영향을 실증적으로 규명하고자 하였다. 타 분야에서의 윤리적 리더십에 관한 연구와 조직시민행동과의 연관성에 대한 연구는 여러차례 진행되어 왔으나, 도서관 분야의 윤리적 리더십과 조직시민행동에 대한 연구는 매우 미미하다. 따라서 이 연구는 윤리적 리더십과 조직시민행동간의 관계를 가설로 설정하고 이를 검증하였다. 실증분석 결과 윤리적 리더십은 조직시민행동의 구성요소 중 성실성, 정중함, 신사적 행동에 유의한 정(+)의 영향을 미치는 것으로 나타났다. 이는 윤리적 리더십의 중요성을 입증하는 것으로, 다양한 변인의 투입을 통해 윤리적 리더십이 구성원의 조직몰입 및 직무만족에 미치는 연구로 확장이 필요함을 시사한다.

Healthcare Workers' Knowledge and Attitudes Regarding the World Health Organization's "My 5 Moments for Hand Hygiene": Evidence From a Vietnamese Central General Hospital

  • Objectives: Although the World Health Organization (WHO) initiative "My 5 Moments for Hand Hygiene" has been lauded as effective in preventing hospital-associated infections, little is known about healthcare workers (HCWs)' hand hygiene behavior. In this study, we sought to assess knowledge and attitudes towards the concepts in this initiative, as well as associated factors, among Vietnamese HCWs at a general hospital. Methods: A structured questionnaire was administered to HCWs at a central Vietnamese general hospital in 2015. Multiple logistic regression analysis was used to identify factors associated with HCWs' knowledge and attitudes towards hand hygiene. Results: Of 120 respondents, 65.8% and 67.5% demonstrated appropriate knowledge and a positive attitude, respectively, regarding all 5 hand hygiene moments. Logistic regression indicated better knowledge of hand hygiene in workers who were over 30 years old, who were direct HCWs (rather than managers), who had frequent access to clinical information, and who received their clinical information from training. Those who worked in infectious and tropical disease wards, who had frequent access to clinical information, and who received information from training were more likely to have a positive attitude towards hand hygiene than their counterparts. Conclusions: Although many Vietnamese HCWs displayed moderate knowledge and positive attitudes towards the WHO hand hygiene guidelines, a key gap remained. Regular education and training programs are needed to increase knowledge and to improve attitudes and practices towards hand hygiene. Furthermore, a combination of multimodal strategies and locally-adapted interventions is needed for sustainable hand hygiene adherence.

언론 프레이밍과 이미지 회복 전략이 공중의 위기 인식에 미치는 영향 (The Effects of Media Framing and Image Restoration Strategies on the Public's Crisis Perceptions)

  • 이 연구의 목적은 위기 상황에서 조직이 내세우는 이미지 회복 전략이 대부분 언론을 통해 전달된다는 점에 착안하여, 언론보도 프레임이 공중의 이미지 회복 전략에 대한 판단에 영향을 주는가를 살펴보는 것이다. 이러한 연구 목적을 달성하기 위해서 신문을 중심으로 한 내용분석과 일반인을 대상으로 한 실험연구를 진행했다. 특히, 이 연구에서는 구체적인 위기 상황을 중심으로 조직의 이미지 회복 전략과 언론보도 프레임의 영향력을 알아보고자 하였으며, 위기 사건 중에서도 산업 전반 및 공중의 삶에 큰 영향을 주는 대표적인 사회 '위기'인 식품관련 사건을 선정하였다. 신문 내용분석 결과, 조직의 이미지 회복 전략은 대부분 일화중심 기사를 통해 공중에게 전달되는 경향을 보이고 있었다. 또한, 조직들이 구사하는 대부분의 이미지 회복 전략은 방어적인 것으로 이는 위기의 유형에 따른 이미지 회복 전략의 차별성을 보여주지 못하는 결과이다. 실험 결과, 조직들의 이미지 회복 전략은 공중이 선호하는 전략과 큰 차이를 보였으며 이러한 공중들의 인식은 프레임 유형에 따라 변화하는 양상을 보였다.

조직성과에 대한 인식연구: 공공기관을 중심으로 (Study on Organizational Performance: Focused on Public Institutions)

  • 본 연구에서는 조직성과를 재무적 관점이 아니라 정책관점, 고객관점, 조직내부관점 등으로 구분하여 공공기관 직원들을 대상으로 분석하고자 한다. 본 연구의 사례로 선정된 공공기관은 환경부 산하의 공공기관으로서 환경산업육성과 환경기술개발사업 등의 관리, 개발된 환경기술의 보급 및 실용화 촉진을 목적으로 설치된 기관이다. 연구대상이 된 공공기관은 조직성과를 어떻게 정의하며 어떻게 하면 조직성과를 제고할 수 있는지에 대한 고민을 가지고 있었다. 아울러 재무적 성과보다는 고객이나 정책의 관점에서 성과를 측정해보자 하는 데에 관심을 가지고 있었다. 본 연구 결과 연구대상이 된 공공기관의 정책적인 측면, 고객적인 측면, 조직내부적 측면에서의 성과에 대해서는 그리 높지 않은 것으로 나타났다. 조직의 정책성과, 고객성과 및 조직내부 성과를 제고하기 위해서는 조직구성원들간의 원활한 의사소통, 수평적 조직구조, 고객의 정책과정에의 참여독려, 업무 프로세스를 조직의 고객입장에서 재설계하고자 하는 노력이 필요함을 알 수 있었다.

  • Large hospital designs try to accommodate functional needs and spatial organization. The purpose of this study is to propose an effective compositional method by horizontal interconnection of public circulation spaces with 'hospital street'. 'Hospital street' and network of public circulation spaces are major elements of consideration for the design of large hospitals, which not only connect functional components but also provide satisfying environment, amenity and patients' life.

공립학교 시설물 유지관리 체계 개선방안 (A Study on the Maintenance System Improvement for Public Educational Buildings)

  • As of August 2005, there are 9,007 facilities for public primary, middle and high schools in Korea. But because of depopulation, hereafter it is expected to allocate much more budget and interest to utilizing the existing facilities reasonably rather than new buildings construction. At present the maintenance of educational facilities is depend on not a preventive but a post preservation. And there is no established maintenance organization and system, which causes both facility deterioration and squandering of the nation's resources. Thus the object of this paper is to examine a maintenance system of established public school facilities and to improve the system which is enable to maximize the efficiency for the existing maintenance organization.
