대전 동구 보육원생의 구강건강 및 구강건강관련 삶의 질 (Oral Health and Quality of Life of the Orphans in Dong-gu, Daejeon)

  • 본 연구는 대전 동구 보육원생 대상 계속구강건강관리사업의 효과를 평가하고자 대전 동구 보육원생 109명을 대상으로 하여 2012년 7~8월에 구강검사와 COHIP를 비롯한 설문조사를 실시하였다. 2010년 국민구강건강실태조사 대전시 표본과 대전 동구에 거주하고 보건소 계속구강건강 관리사업을 받지 않은 아동의 자료를 검정값으로 이용하여 일표본 검정을 하였으며, 연구 결과는 다음과 같다. 1. 계속구강건강관리사업을 받은 보육원 9~12세 아동과 13~18세 청소년은 우식경험영구치수에 있어서 대전시 12세, 15세 표본과 각각 비교하여 차이가 없으나, 13~18세 청소년은 대전시 15세 표본과 비교하여 우식치아수와 우식치면수가 유의하게 많았다(p<0.001). 2. 계속구강건강관리사업을 받은 보육원 9~12세 아동은 보건소 계속구강건강관리사업을 받지 않은 아동과 비교하여 치면세균막지수는 유의하게 적어서(p<0.001), 잇솔질 방법을 올바르게 수행하고 있는 것으로 판단되었다. 3. 계속구강건강관리사업을 받은 보육원 13~18세 청소년은 대전시 15세 표본과 비교하여 유의하게 적은 잇솔질 횟수, 많은 치은출혈삼분악수를 보여(p<0.001), 연령이 증가하면서 자가구강건강관리가 부족한 것으로 판단되었다. 4. 계속구강건강관리사업을 받은 보육원 9~12세 아동은 보건소 계속구강건강관리사업을 받지 않은 아동과 비교하여 아동 구강건강관련 삶의 질(COHIP) 총점과 자기이미지와 학교환경의 항목에서 부정적이었다(p<0.05). 따라서, 보육원생의 구강건강증진을 위해서는 현재 발생되어 있는 우식치면수의 감소가 필요하므로 구강병예방사업과 더불어 구강병치료사업이 추가되어야 한다. 연령이 증가할수록 구강건강이 악화되는 양상을 나타내므로 청소년은 아동과 분리하여 청소년에 맞는 구강보건교육, 보육원내의 구강보건행동에 대한 지속적인 모니터링과 심리적 지지가 필요하다.

초등학교 고학년 학생의 구강보건에 관한 인식 및 행동 (A Study on the Oral Health Awareness and Behavior in the Higher Grades of Elementary School)

  • The purpose of this study was to collect basic data for the development of effective oral health education program in elementary school. The subjects in this study Were 604 fourth-year, fifth-year and sixth-year students of an elementary school in Seoul, Oral health awareness and behaviors were surveyed and analyzed. The findings of this study were as follows: 1. The most common time for toothbrushing was after dinner(62.0%) and before bedtime(62.3%). By school year, the largest number who brushed their teeth after dinner were fourth and fifth graders while the greatest number who did so after breakfast were sixth graders. Seventy-four percent of respondents brushed the teeth twice or more a day. By school year, the fifth-year students brushed their teeth the least while girls outperformed the boys in daily toothbrushing frequency(p<0.05). Regarding the intake of cariogenic food, 62.1% took that kind of food once or more a day. By school year, the fifth graders took the most (p<0.05), and girls took more cariogenic food than the boys(p<0.05). 2. Ninety percent of the respondents had visited a dental clinic. By grade, the sixth-year students had visited a dental clinic more frequently and girls more frequently than boys. Eighty percent of the respondents visited a dental clinic to receive treatment but just 12.3% visited a dental clinic to prevent oral diseases. Concerning the fear of dental treatment, only 14.4% expressed fear. School year made no difference, yet the percentage of girls who experienced fear was higher than that of boys(p<0.05). Regarding preventive treatment, 39.7% experienced pit and fissure sealing and 24.2% experienced the application of fluorides. All the students hadn't much experience in preventive treatment regardless of school year -- gender didn't make a difference. 3. Fourty-six percent of respondents had a preference for the fluoride mouth rinsing program; 38.4% wanted that program to continue. As to the reason for preference, 38.2% preferred it because of the prevention of dental caries, 43.0%, the largest percentage, didn't favor it because they found it too much trouble to do(some of the respondents gave two different answers). 4. Despite an increase in the frequency of toothbrushing at the right time, they still took cariogenic food frequently. Their visit to a dental clinics also aimed at treatment more than at prevention, and they had very narrow experience of preventive treatment Furthermore, the fluoride mouth rinsing program failed to draw their interest Therefore, it is recommended that a successful oral health education program should be developed.

일부 노인장기요양환자의 구강실태 및 구강기능 (Dental Status and Oral Function in Some Long-term Care Elderly Patients)

  • Objective: This study aims to investigate the relationship between dental status and oral function by analyzing those in some long-term care elderly patients. Methods: It performed oral examination and 4 oromotor function examinations such as repeated swallowing function, correct pronunciation function, saliva secretion rate and maximum mouth opening to 91 elderly patents aged 65 and older in the municipal geriatric hospital located in Cheonan. Results: Dental status of patients such as the number of dental caries, treated teeth, retained teeth and function teeth were better in mobile elderly patients than in immobile elderly patients. Attachment rate of dental plaque and the number of teeth being extracted were more in mobile patients than in immobile patients. More retained teeth, the number of function teeth and dental caries and the score in pronunciation status test were significantly higher. With more function teeth, the score in pronunciation status test was significantly higher. As saliva secretion rate is higher, repeated swallowing function was significantly better. repeated swallowing function rate is higher pronunciation status was significantly better. Conclusions: With the results of this study, it was found that among long-term care elderly patients, oral function was worse in immobile patients than in mobile patients. Therefore, it may be necessary to plan and perform an oral function improvement program preferentially for elderly patients requiring long-term care.

미국 연방 장애법과 동법이 장애인의 의료서비스에 미친 영향 (FEDERAL DISABILITY LAW AND ITS IMPACT ON HEALTH CARE FOR PERSONS WITH DISABILITIES IN THE UNITED STATES)

  • Federal disability law has evolved from several laws geared to protect people with disabilities since the late 1960s and early 1970s. When U.S. Congress passed the Americans with Disabilities Act (ADA) in 1990, no federal statute prohibited the majority of employers, program administrators, owners and managers of places of public accommodation and others from discriminating against people with disabilities. Toward the ends to assure equality of opportunity, full participation, independent living, and economic self-sufficiency for individuals with the disabilities, the ADA pursues three major strategies: Title I addresses inequality in employment, Title II, inequality in public services, and Title III, inequality in services and accommodations offered by private entities. The purposes of the study were to analyze the impact of the ADA on health care for persons with disabilities and to review the ongoing health policy reforms at the federal and state governments. Essential remedies that the ADA contemplates are based on two principles, simple discrimination and reasonable accommodation, which significantly improved access to quality care, especially long-term care, by persons with disabilities. However, the ongoing Medicaid policy reforms to control rising health care costs in the U.S. could threaten the access to care by persons with disabilities in optional groups and to optional care services by persons with disabilities in mandatory groups.

시각장애인의 일반적 특성과 구강보건행태가 주관적인 구강건강상태에 미치는 영향 (Effect of the general Characteristics and Oral Public Health Behaviors of the Visually Impaired on the Oral Health Condition)

  • 특수감각의 하나인 시각에 장애가 생겨 보는 것에 대한 많은 제약을 받는 시각장애인은 집안에서의 아주작은 행동에서부터 사회생활을 위한 외부 활동에 이르기까지 적지 않은 어려움을 가진다. 최근 많은 관심을 가지고 있는 건강을 지키기 위한 여러 행동들에서도 시각 부분의 장애는 많은 문제를 초래한다. 이에 수집된 자료는 연구대상자의 일반적인 특성과 구강보건행태를 빈도분석 하였으며, 구강보건 형태와 구강 건강상태를 t-test와 F-test 분석을 통해 평균비교를 하였고, 검정결과 집단 간 유의한 차이에 대해 Duncan으로 사후검정을 하였다. 일반적인 특성과 구강보건행태의 상관성을 알아보기 위해 Spearman의 상관분석을 하였으며, 주관적인 구강건강상태에 미치는 영향을 파악하기 위해 단계적 다중 회귀분석 (multiple regression analysis)을 하였다. 통계프로그램은 SPSS(SPSS20.0 for Windows, SPSS Inc, USA)를 이용하였으며, 통계적 유의성 판정을 위한 유의수준은 0.05로 고려하였다. 누구보다도 건강을 지키기 위한 행동들이 정확하게 인지되어야 하는 그룹이므로 그들의 구강에 대한 실태와 행태를 알아보고 그러한 행위에 의한 구강상태를 분석하여 시각장애인에게 알맞은 정보전달 매체 등을 만드는 것에 기초자료로 활용하고자 한다.

방문보건대상 노인의 구강건강 수준에 영향을 미치는 관련요인 (Clinical predictors related to oral health in elderly for visiting healthcare)

  • Objectives : This study was to investigate the level of oral health, xerostomia, and oral health related activities of daily living(ADL) and the clinical factors influencing oral health related quality of life(OHIP-14) in elderly for visiting healthcare. Methods : Data were obtained from a cross-sectional survey of 106 elderly people(mean age=$77.5{\pm}4.98$). Participants in the study were recruited when they attended the public health center in Cheonan-si. Data were collected by means of a personal interview from July 19 through to August 20, 2010. Oral health related ADL, OHIP-14 and dry mouth was measured by the total 24-items. The data was analysed with t-test, one-way ANOVA, and multiple regression using the SPSS program. Results : Participants had a mean of 13.6 remaining natural teeth, which was significantly with age and perceived oral health. Xerostomia score achieved a mean of 6.39 out of a maximum 12 points. Powerful predictors of OHIP-14 were natural teeth, masticatory difficulty, oral pain, PHP index, perceived oral health, age and living together for the elderly Conclusions : Based on the findings, OHIP-14 has a significantly impact on oral health related ADL. These results suggest that elder people oral health promotion should be developed assessment index which was measured predictors related oral health in the elderly for visiting healthcare.

40세 이상 성인의 구강건강상태와 폐 환기장애의 관련성 (Association between oral health status and pulmonary ventilatory defects in adults 40 years or older)

  • Objectives: The purpose of this study is to identify the association between oral health status and pulmonary ventilatory defects. Methods: The 6th (2013-2015) National Health and Nutrition Examination Survey data was used. The study subjects were those aged 40-79 who had pulmonary function examination. Complex samples general linear model analysis, Complex samples cross-tabulation analysis, and Complex samples logistic regression analysis were conducted. Results: In terms of restrictive ventilatory defects, the study subjects (8.3%) who recognized that their oral health status was bad outnumbered those who recognized that their oral health status was good (6.1%) (p<0.05). Many of the study subjects who experienced difficulty in biting, chewing and/or speech, and who had an unhealthy periodontal had restrictive ventilatory defects and obstructive ventilatory defects (p<0.05). The association between oral health status and pulmonary ventilatory defects was analyzed. The findings showed that those who had unhealthy periodontal had a 1.33 times higher probability of pulmonary ventilatory defects than those who had a healthy periodontal (p<0.05). After taking into account general characteristics (age, sex, incomes, education, and smoking) of the subjects, the association between oral health status and pulmonary ventilatory defects was analyzed. The result found that only in cases where one experienced dental caries was an association with pulmonary ventilatory defects found. In other words, those who had dental caries showed a 0.73 times higher probability of pulmonary ventilatory defects than those who had no dental caries (p<0.05). Conclusions: Based on the findings of this research, oral health status was found to be associated with pulmonary ventilatory defects. To improve oral health, it is necessary to provide life-cycle stages based oral health education. Therefore, it is required to develop an oral health education program and develop a national oral health policy.

다문화가족 청소년의 스트레스 수준과 구강건강증상에 관한 연구 (A study on stress levels and oral health symptoms of adolescents in multicultural families)

  • Objectives: The purpose of this study was to investigate the relationship between stress and oral health symptoms in adolescents in multicultural families. Methods: The data from the 10th Korea Youth Risk Behavior Web-based Survey (KYRBWS) were analyzed using PASW Statistics 18.0 (SPSS Inc., Chicago, IL, USA). A total of 795 adolescents, whose mother or father's home country is not Korea, were selected as the final analysis subjects. Results: There was a correlation between the subject's stress level and oral health symptoms. Toothache was significantly increased 2.474 times (p<0.001) compared to lower stress levels. Gum pain and halitosis were significantly increased by 2.394 times and 1.820 times, respectively, compared with those who had low stress (0.001, 0.008). Conclusions: The results suggest that stress experienced by adolescents in multicultural families affects oral health symptoms. Therefore, it is necessary to identify the stress factors and levels in adolescents belonging to multicultural families whose physical and emotional changes are evident and develop a customized program considering their characteristics.

보건소 내원 환자들의 구강보건 인식도에 관한 조사 (A study on the patient's awareness of oral health in public health center)

  • There are two purposes in this study. The first one is to collect some oral health information through the investigations upon the oral health care of Korean adults. Referring to the result of the investigation. I wanted to furnish fundamental data with oral health education for each age and with a publicity booklet editing. That is the second purpose of this research. Among the people who visited Seo-gu Public Health Center in Kwang-ju, 207 people answered the questionnaires. There were 86 men and 121 women from the 20-aged to the 59- aged. The questionnaires covers knowledge aquisition path for dental caries prevention, the cognition degrees for the causes of dental caries and its prevention, the cognition degrees for the causes of periodontal disease and its prevention, the degrees for oral health methods, and the importance of oral health. The conclusions are as follows; 1. The cognition degree for dental caries prevention: The cognition for pit and fissure sealant was appeared most highly in thirties by 85.3%. but 62.1 % in fifties was answered, "Never heard". The cognition degree for fluoride application was appeared most highly in thirties by 73.5%. and the cognition degree for water fluoridation was most highly in forties by 54.2%. 2. The knowledge for pit and fissure sealant was acquired mostly through dental hospital in every age by 54.2%. 3. The knowledge for fluoride application was learned mostly through dental hospitals in twenties and thirties by 32.7%. and mostly through TV or radio health programs in forties and fiftieseach by 35.7% and 50.0%. 4. The knowledge for water fluoridation was acquired through TV or radio programs in all ages such as twenties. thirties. forties and fifties. Its rate was 57.8%. 5. The cognition degree of the cause of dental caries: 53.1 % of all ages think that dental caries can most frequently be caused by being lack of toothbrushing. 6. The cognition degree of the cause of periodontal disease: 58.5% people of all ages think that both dental plaque and calculus might be the main cause of periodontal disease. 7. The cognition degree of dental caries prevention: 72.8% people think that dental caries can be prevented by right tooth brushing method. and 8.7% people think that they can be prevented by scaling. However, 10.7% people of them were not interested in dental caries prevention. 8. The importance of oral health: 35.3% people think that teeth health is the most important, and 63.8% people think oral health is one of the most important health problems. Forties answered that tooth health was the most important thing, and fifties. thirties and twenties followed in the order. 9. The cognition for oral health maintenance: all ages(twenties. thirties. forties. and fifties) answered that right toothbrushing method was the best way to keep oral health by 69.1%.

Prisoners' Perception of Tobacco Use and Cessation in Chhatisgarh, India - The Truth from Behind the Bars

  • Background: Prisoners represent a population group that is disadvantaged, socially deprived and underprivileged, needing particular attention with regard to provision of necessary oral health care, health promotion and motivation and tobacco cessation. Considering the situation in prisons, smoking and tobacco chewing are burning issues related to health deterioration and economic loss that seem to be overlooked by the public health sectors. Aim: To assess prisoners' perception of tobacco use and cessation in Chhatisgarh, India. Materials and Methods: A pre-tested, close ended questionnaire was administered in the form of extensive face to face interviews, to assess perceptions regarding tobacco use and cessation in the central jail of Durg District of Chhattisgarh state, India. Results: Prevalence of tobacco usage amongst the prisoners was found to be 61%. Some 27% reported smoking, 44% used tobacco in the chewable form and 29% indulged in consuming tobacco in both forms i.e. smoked as well as chewed. Results suggest several recommendations for policy relevance such as provision of a prison dentist, a tobacco cessation counseling program and targeted eradication of oral cancer by educating the prisoners. Conclusions: Health is a fundamental human "right of everyone to the enjoyment of the highest attainable standard of physical and mental well-being". This applies to prisoners just as it does to every other human being. The alarming findings in the study suggest the need for dental treatment facilities and tobacco cessation counseling in prisons.
