Improving Accuracy and Completeness in the Collaborative Staging System for Stomach Cancer in South Korea

  • Background: Cancer staging enables planning for the best treatments, evaluation of prognosis, and predictions for survival. The Collaborative Stage (CS) system makes it possible to significantly reduce the proportion of patients labeled at an "unknown" stage as well as discrepancies among different staging systems. This study aims to analyze the factors that influence the accuracy and validity of CS data. Materials and Methods: Data were randomly selected (233 cases) from stomach cancer cases enrolled for CS survey at the Korea Central Cancer Registry. Two questionnaires were used to assess CS values for each case and to review the cancer registration environment for each hospital. Data were analyzed in terms of the relationships between the time spent for acquisition and registration of CS information, environments relating to cancer registration in the hospitals, and document sources of CS information for each item. Results: The time for extracting and registering data was found to be shorter when the hospitals had prior experience gained from participating in a CS pilot study and when they were equipped with full-time cancer registrars. Evaluation of the CS information according to medical record sources found that the percentage of items missing for Site Specific Factor (SSF) was 30% higher than for other CS variables. Errors in CS coding were found in variables such as "CS Extension," "CS Lymph Nodes," "CS Metastasis at Diagnosis," and "SSF25 Involvement of Cardia and Distance from Esophagogastric Junction (EGJ)." Conclusions: To build CS system data that are reliable for cancer registration and clinical research, the following components are required: 1) training programs for medical records administrators; 2) supporting materials to promote active participation; and 3) format development to improve registration validity.

Antibacterial Effects of Galla Rhois Extract against Streptococcus suis Infection in Mice

  • Streptococcus suis (S. suis) is a major swine pathogen and an emerging zoonotic agent and is an increasing public health problem across Asia. The present study was undertaken to estimate the antibacterial effect of GR extract and therapeutic effect of GR extract against S. suis infection in mice. At the concentration of GR extract 2.5 mg/ml, the antibacterial effect was not shown on S. suis. However, the antibacterial effect against S. suis was observed at the concentration of GR extract 5.0 mg/ml. Oral administration of GR extract at the dose of 10 mg/kg showed a therapeutic effect for S. suis infected BALB/c mice. The mortality of GR extract-treated mice at the concentration of 5, 10 and 20 mg/kg was 80%, 70%, and 50% at 12 days, respectively, while that of untreated mice was 100% at 8 days after a lethal dose of S. suis infection. The results of our study strongly indicate that GR extract has potential as an effective for S. suis infection in mice.

  • As 5G and AI continue to develop, there has been a significant surge in the healthcare industry. The COVID-19 pandemic has posed immense challenges to the global health system. This study proposes an FL-supported edge computing model based on federated learning (FL) for predicting clinical outcomes of COVID-19 patients during hospitalization. The model aims to address the challenges posed by the pandemic, such as the need for sophisticated predictive models, privacy concerns, and the non-IID nature of COVID-19 data. The model utilizes the FATE framework, known for its privacy-preserving technologies, to enhance predictive precision while ensuring data privacy and effectively managing data heterogeneity. The model's ability to generalize across diverse datasets and its adaptability in real-world clinical settings are highlighted by the use of SHAP values, which streamline the training process by identifying influential features, thus reducing computational overhead without compromising predictive precision. The study demonstrates that the proposed model achieves comparable precision to specific machine learning models when dataset sizes are identical and surpasses traditional models when larger training data volumes are employed. The model's performance is further improved when trained on datasets from diverse nodes, leading to superior generalization and overall performance, especially in scenarios with insufficient node features. The integration of FL with edge computing contributes significantly to the reliable prediction of COVID-19 patient outcomes with greater privacy. The research contributes to healthcare technology by providing a practical solution for early intervention and personalized treatment plans, leading to improved patient outcomes and efficient resource allocation during public health crises.

Measurements of Volatile Organics, Newly Designated Offensive Odorants in Korea in 2008 and 2010: a Case Study on Gutter System (2008년과 2010년 지정 VOC 계열 신규악취성분들에 대한 분포특성 조사: 도심 하수환경 중 빗물받이에 대한 연구)

  • In this research, a total of 11 newly designated offensive odorants in Korea in 2008 and 2010 (styrene (S), toluene (T), p-xylene (p-X), methyl ethyl ketone (MEK), methyl isobutyl ketone (MIBK), butyl acetate (BuAc), isobutyl alcohol (i-BuAl), propionic acid (PA), butyric acid (BA), isovaleric acid (IA), and valeric acid (VA)) were selected as target compounds and analyzed from two types of gutter system in the urban environment. Because of the environmental significance of these compounds as offensive odorants, the results are meaningful enough to explore their behavior and distribution in the urban environmental systems. In the course of this study, samples were collected three times a day from two different gutter systems representing the wet (W) and dry (D) conditions. A large fraction of volatile organic compounds (VOCs) data fell into method detection limit (MDL) range with exceptions of toluene, p-xylene, and methyl ethyl ketone. In contrast, the results of organic fatty acids were distinguished by the relative dominance of butyric acid and propionic acid over others. If the concentration data of all odorants were converted into odor intensity (OI), the results of aromatics, ketones, acetate, and alcohol groups approached zero level. However, odor intensity of organic fatty acids was noticeably higher with the value of 2.8 (on average) from both W and D system, suggesting their potent roles as odorants in gutter system.

  • This study was done to develope a computer program for the evaluation of quality of nurisng care. Because the professional nursing care should be evaluated consistantly, computer program for the measurement of quality of nursing care is necessary for the effective and efficient management of nursing quality. In this study, a computer program for gastrointestinal surgery patients was developed as a module. Visual basic 4.0 was used as a basic program for the development of this program, and Access 7.0 was used for the data base construction of the program. The systems of evaluation criteria were hierarchical. and the titles of the hierarchical criteria were evaluation area-evaluation item-indicator. Each evaluation area contained several items and each evaluation item contained several indicators. The numbers of the evaluation criteria for gastrointestinal surgery patients were 7 evaluation areas, 32 evaluation items. and 71 indicators. Content validity of the evaluation criteria(in this case, 32 evaluation items) was .95. Scoring could be possible with the evaluation items. For the scoring types of this program were two. norm-referenced type(option 1) and criterion-referenced type(option 2), the user can choose the type according to the purpose of the evaluation. With this computer program. accurate and consistant evaluation of the quality of nursing care could be expected. Also. by the rapid feedback to nursing care practice. quality of nursing practice could be improved rapidly. and the systems of the evaluation criteria developed in this study could be used for the development of other moduls for various kinds of patient groups. Because this computer program was developed only for the purpose of research, it is necessary to be refined commercially to be used in real nursing situation.

A report of 39 unrecorded bacterial species in Korea belonging to the classes Betaproteobacteria and Gammaproteobacteria isolated in 2018

  • In the project of a comprehensive investigation of indigenous prokaryotic species in Korea, a total of 39 bacterial strains phylogenetically belonging to the classes Betaproteobacteria and Gammaproteobacteria were isolated from various environmental sources such as soil, cultivated soil, sludge, seawater, marine sediment, algae, human, tree, moss, tidal flat, beach sand and lagoon. Phylogenetic analysis based on 16S rRNA gene sequences revealed that 39 strains showed the high sequence similarities (≥98.7%) to the closest type strains and formed robust phylogenetic clades with closely related species in the classes Betaproteobacteria and Gammaproteobacteria. In the present study, we report 14 species of 9 genera of four families of two orders in the class Betaproteobacteria and 25 species of 21 genera of 15 families of eight orders in the class Gammaproteobacteria, which have not been reported in Korea. Morphological, biochemical, and physiological characteristics, isolation sources, and NIBR deposit numbers are described in the species descriptions.

Risk Factor Evaluation of Musculoskeletal Symptoms for Guards

  • Objective: The objective of this study was to evaluate a work of guards, using an ergonomic method(work analysis and posture analysis). Background: Most studies about guards were conducted in the field of medical, problems of shift, and the physical problems of old workers and social problems. But, guards consist of vulnerability group so it needs an ergonomic research in musculoskeletal disorders. Method: A head of an ergonomic estimation was work analysis(determination of combined task, work tool, work time and frequency of combined task) and posture analysis(upper body and lower body) of workers based on the video. Results: The result showed that combined task of guards was classification of patrolling, security, cleaning and waiting. The security indicated the highest ratio in the work time of combined tasks. The results of posture analysis for guards indicated high value in neutral. But, lower arm indicated high value in bending(left: 59%, right: 50%). Conclusion: The results of ergonomic methods indicated that guards' physical work load was not high during work, but comfortable work environment would be required for old guards. Application: If an ergonomic rule can be integrated into existing work environments, the risk of occupational injuries and stress will be reduced.

  • Fundamental advances in the biotechnologies are exerting a profound influence on the health care, agricultural, industrial chemical, environmental, and other industrial fields. Korean government are now more and more realizing the importance of biotechnology as a main technology for the 21st century. But any technical progress is largely the result of a complex set of relationships among the firms, institutions and others involved in development. So understanding the complexity is very important to make promoting strategies and it is even critical in the field of biotechnology. The reason is that commercialization of research results in biotechnology is strongly related with the national science bases provided by academic and public institutes. And its applicable industrial sectors are very diverse. So it is very important to make a effective collaboration system among many R&D related agents. This article discusses and compares both USA and Japanese framework of national innovation systems in the field of biotechnology. The American Innovation system encourages basic research in the biological sciences, and fosters the creation of small venture firms that focus on the development of novel products. America's peculiar incentive structure, derived from its research and educational system, financial system, and regulatory environment has driven USA labs and firms to the forefront of many biotechnology fields. The Japanese institutional environment in contrast, supported the strategy of building production expertise. Firms were urged to use the new techniques as a way of leapfrogging into a second generation of bio-products, in that cost and production advantages count. But the strategy was not effective as expected and Japanese firms have remained competent but not prominent rivals. The differing situations in USA and Japan with regard to biotechnology have many suggestions for our bioindustry. In the conclusion of this article, we translate USA and Japan's experiences to some suggestions which guide for promoting Korea's biotechnology R&D and commercialization activities.

Roles of Cancer Registries in Enhancing Oncology Drug Access in the Asia-Pacific Region

  • Cancer registries help to establish and maintain cancer incidence reporting system, serve as a resource for investigation of cancer and its causes, and provide information for planning and evaluation of preventive and control programs. However, their wider role in directly enhancing oncology drug access has not been fully explored. We examined the value of cancer registries in oncology drug access in the Asia-Pacific region on three levels: (1) specific registry variable types; (2) macroscopic strategies on the national level; and (3) a regional cancer registry network. Using literature search and proceedings from an expert forum, this paper covers recent cancer registry developments in eight economies in the Asia-Pacific region - Australia, China, Hong Kong, Malaysia, Singapore, South Korea, Taiwan, and Thailand - and the ways they can contribute to oncology drug access. Specific registry variables relating to demographics, tumor characteristics, initial treatment plans, prognostic markers, risk factors, and mortality help to anticipate drug needs, identify high-priority research area and design access programs. On a national level, linking registry data with clinical, drug safety, financial, or drug utilization databases allows analyses of associations between utilization and outcomes. Concurrent efforts should also be channeled into developing and implementing data integrity and stewardship policies, and providing clear avenues to make data available. Less mature registry systems can employ modeling techniques and ad-hoc surveys while increasing coverage. Beyond local settings, a cancer registry network for the Asia-Pacific region would offer cross-learning and research opportunities that can exert leverage through the experiences and capabilities of a highly diverse region.

Development of a Linking Model between Community Resourses and Industrial Injury Insurance Organs for the Activation of Medical Rehabilitation Services for Industrial Injury Patients (산재환자의 의료재활서비스 활성화를 위한 산재보험시설과 민간시설간의 연계모형 개발)

  • Objectives: The purpose of this study is to develop a linking model between industrial injury insurance organs and local organizations providing social welfare health services, for the activation of medical rehabilitation services for industrial injury patients. Method: Research design for this study was a multi-step research through literature review, field research, and group interviews with persons in charge, to compare local society-oriented medical rehabilitation programs. The term of researches Sep.1 ~ Nov. 30, 2004. Results: 1. Home nurses from Workers Accident Medical Corporation have been taken to be adequate to case managers, who link industrial accident insurance institutions to local society services for the activation of medical rehabilitation services for industrial injury patients. They have been chosen for case managers because they have richer understanding and experiences of objects of industrial accidents than any other specialists, and because they have proved to be able to provide direct home services as a specialist. We have established the center for case management affairs within the workers accident general hospital, organized the committee for case management with doctors in charge, doctors in rehabilitation, rehabilitation consultants, social welfare workers, physical therapists, and nurses, determined objects of case managements from those of long-term recuperation, and constructed a course of case management containing from case management plans to evaluation. 2. We have made files of community resources, and organized the council of industrial accident administration to have it in charge of the adjustment and linking of services in case management affairs. 3. Because there are inequality of community resources between areas, differences in experiences in and bases for linking, and disparity of core organizations with active linking in the system of linking between public and civil sectors, we have taken a system of linking between parallel organs to be the core. In our linking model, workers accident general hospital, hospitals designated for industrial injury, and rehabilitation hospitals are linked in parallel, inadequate long-term recuperation managers are trusted to an workers accident medical corporations through examination by the examination committee in Korea Labor Welfare Corporation, and are dealt with by the committee for case management. Of the hospitals designated for industrial accidents, those running a home caring center provide home caring services for the handicapped at home from industrial injury. 4. Workers Accident Medical Corporation take part in medical rehabilitation, and Korea Labor Welfare Corporation in vocational/social rehabilitation. Furthermore, in the model, the latter should construct a system for job opportunities through Internet portals and provide cyber vocational consultation and introduction. Conclusion: Improvement of systems is needed to apply the linking model to practical affairs. Because this model is centered for practical affairs, it should be put under the analysis of effects, and evaluation of its adequacy to practical application, and its effects and efficiency through experimental running in the 8 workers accident general hospital in Korea.
