Enzyme-linked immunosorbent assay for detection of Trichinella spiralis antibodies and the surveillance of selected pig breeding farms in the Republic of Korea

  • Trichinellosis is a parasitic zoonosis of public health importance. It is caused by Trichinella spiralis which has a wide host range including humans. In the present communication, the ELISA technique was employed on a total of 803 blood samples from 7 selected pig breeding farms in 1996 for diagnosis and surveillance of trichinellosis. Out of the entire 803 samples, nine were found to be suspected while one was positive by ELISA. But western blot analyses employed for further confirmation have shown that all of 10 samples did not react to larval excretory-secretory product antigens. These results indicate that pig breeding farms included in the present study are free from trichinellosis . However, it does not mean Korea is free from trichinellosis since human trichinellosis has recently been reported. The necessity of continued surveillance for trichinellosis in both pigs and wild animals was discussed.

Surveillance of ASF-infected pig farms from September to October 2019 in South Korea

  • Fourteen African swine fever (ASF) outbreaks occurred in the pig farms in the northwestern region of South Korea, near the border with North Korea, from September 16, 2019 to October 9, 2019. Active and passive surveillance on the ASF-infected farms indicated that the infection was limited only to pigsties where the infected pigs were detected on the farm for the first time before further transmission to other pigsties and farms. This early detection could be one of the pivotal factors for the prompt eradication of ASF in domestic pig farms within 1 month in the northwestern region of South Korea.

  • Owing to global climate change, the global resurgence of vector-borne infectious diseases and their potential to inflict widespread casualties among human populations has emerged as a pivotal burden on public health systems. Tsutsugamushi disease (scrub typhus) in the Republic of Korea is steadily increasing and was designated as a legal communicable disease in 1994. The disease is a mite-borne acute febrile disease most commonly contracted from October to December. In this study, we tried to determine the prevalence of tsutsugamushi disease transmitted by chigger mites living on rodents and investigated their target vector diversity, abundance, and distribution to enable the mapping of hotspots for this disease in 2015. A total of 5 species belonging to 4 genera (109 mites): Leptotrombidium scutellare 60.6%, L. pallidum 28.4% Neotrombicula tamiyai 9.2%, Euschoengastia koreaensis/0.9%), and Neoschoengastia asakawa 0.9% were collected using chigger mite collecting traps mimicking human skin odor and sticky chigger traps from April to November 2015. Chigger mites causing tsutsugamushi disease in wild rodents were also collected in Hwaseong for the zoonotic surveillance of the vector. A total of 77 rodents belonging to 3 genera: Apodemus agrarius (93.5%), Crocidura lasiura (5.2%), and Micromys minutus (1.3%) were collected in April, October, and November 2015. The most common mite was L. pallidum (46.9%), followed by L. scutellare (18.6%), and L. orientale (18.0%). However, any of the chigger mite pools collected from rodent hosts was tested positive for Orientia tsutsugamushi, the pathogen of tsutsugamushi disease, in this survey.

Characteristics of Occupational Skin Disease Reported by Surveillance System (감시체계를 통하여 보고된 직업성 피부질환의 특성에 관한 연구 - 사업장, 특수건강진단기관, 피부과의사의 보고사례를 중심으로 기술 -)

  • Objectives: This study was carried out to estimate the magnitude of skin disease related to occupation and to find out the characteristics of it. Methods: We collected and analyzed the cases of occupational skin disease reported by surveillance system composed of doctors and nurses in 150 enterprises with dispensary or attacked hospital and physicians in 92 specific health examination institutes and 150 dermatologists from May to November, 1998. Results: Among members of surveillance system, 66 enterprises and 47 specific health examination institutes and 55 dermatologists reported 571 cases of occupational skin disease in 512 workers. Excepting 81 cases reported by dermatologists, We analyzed 490 cases reported by enterprises and specific health examination institutes. Among 490 cases, contact dermatitis was most common(368 cases, 75.1%) and the second was hyper or hypopigmentation(36 cases, 7.3%). When we analyzed the characteristics of workers with occupational contact dermatitis, male workers were 281 (79.2%) and female were 74(20.8%). 165 workers(64.5%) had chronic skin disease with repeated cure and relapse. 245 workers(72.5%) answered positively that their coworkers had similar skin disease. 27 workers(8.7%) experienced absence due to contact dermatitis related to occupation. To analyze the type of industries of workers with occupational contact dermatitis, automobile and trailer manufacturing industry was most common(105 cases, 29.6%) and the second was manufacturing industry for image, sound and communication equipment(55 cases, 15.5%). Organic solvent(183 cases, 46.7%) was the most common treating material of workers with contact dermatitis and the second was various kinds of chemicals(59cases, 15.1%). Conclusions: This is the first study using nationwide surveillance system to collect data of occupational skin disease. We found that many workers had skin disease related to occupation and characteristics of occupational skin disease were chronic and clustering. Therefore, we had to establish counterplan to manage occupational skin disease and to operate surveillance system to identify trends of occupational skin disease, continuously.

Immunoassay for Monitoring Pesticide Contamination in Agricultural Products

  • Much of the increase in agricultural productivity over the past half century has been due to the control of the pests with synthetic pesticides. The use of these pesticides has caused environmental problems and public health concern. The guidelines of maximum residue levels of pesticides in agricultural products has been well documented but more careful monitoring of their residues is required. Pyrethorid class pesticides are dominant in modern agricultural industry but public health concerns have been recently considered. The major route of pesticide exposure is the diet and with improved surveillance of pyrethorid residues in agricultural products their exposure should be controlled and minimized. In suitable products with reduced matrix effects such as agricultural products, aqueous samples, fruits and vegetables the use of immunoassays for pyrethorid residue monitoring could satisfy this requirement. Immunoassays have several advantages, namely they are highly sensitive, selective and cost-effective and enable large-scale sample handling and analysis in the laboratory.

  • There are many uses of control charts in health-care monitoring and in public-health surveillance. For example, control charts are used in monitoring and improvement of hospital performance, in monitoring chronic diseases and infectious diseases, and so on. We introduce the Variable Life-Adjusted Display (VLAD) chart and propose the method for choosing control limits of the VLAD chart to give specified in-control properties.

The Effects of Temperature on Heat-related Illness According to the Characteristics of Patients During the Summer of 2012 in the Republic of Korea

  • Objectives: This study was conducted to investigate the relationship between heat-related illnesses developed in the summer of 2012 and temperature. Methods: The study analyzed data generated by a heat wave surveillance system operated by the Korea Centers for Disease Control and Prevention during the summer of 2012. The daily maximum temperature, average temperature, and maximum heat index were compared to identify the most suitable index for this study. A piecewise linear model was used to identify the threshold temperature and the relative risk (RR) above the threshold temperature according to patient characteristics and region. Results: The total number of patients during the 3 months was 975. Of the three temperature indicators, the daily maximum temperature showed the best goodness of fit with the model. The RR of the total patient incidence was 1.691 (1.641 to 1.743) per $1^{\circ}C$ after $31.2^{\circ}C$. The RR above the threshold temperature of women (1.822, 1.716 to 1.934) was greater than that of men (1.643, 1.587 to 1.701). The threshold temperature was the lowest in the age group of 20 to 64 ($30.4^{\circ}C$), and the RR was the highest in the ${\geq}65$ age group (1.863, 1.755 to 1.978). The threshold temperature of the provinces ($30.5^{\circ}C$) was lower than that of the metropolitan cities ($32.2^{\circ}C$). Metropolitan cities at higher latitudes had a greater RR than other cities at lower latitudes. Conclusions: The influences of temperature on heat-related illnesses vary according to gender, age, and region. A surveillance system and public health program should reflect these factors in their implementation.

  • Injury in agriculture is a serious public health issue with a major impact on the lives of Korean farmers. It is one of the leading causes of death and is also a major cause of longand short-term disability. In 2001, the social cost of one accident in agricultural machinery was estimated as 97.7-97.8 million won that is 4 fold of farm household income in Korea. Effective prevention and control of injuries requires a system of surveillance that monitors the incidence of injuries, their causes, treatment and outcomes. This requires an integrated system of data collection, analysis and interpretation and communication. Creating effective injury surveillance system in Korea requires to establish a framework for a national agenda. Discussions regarding the development of the framework should address, but not be limited to issues related to Data Holdings and Linkages; Capacity and Skills; Communication; Interconnection; and Surveillance Products. Ideally, an injury surveillance system would meet the information requirements across all sectors, while allowing each to have the ongoing information it needs for its policy and programming needs. This study was carried out to develop a surveillance system of agricultural injuries in Korea. Study subjects were residents who lived in a typical agricultural area (Yangpyung area in Kyung-gi province). The main data sources were reports of village headmen, compared with data of 'National Emergency Management Agency', 'National Health Insurance Corporation', 'Insurance of National Agricultural Cooperative', and 'Emergency Medical Centers'. Each data were reviewed to validate the strengths and weaknesses.

Developing a Job Exposure Matrix of Work Organization Hazards in the United States: A Review on Methodological Issues and Research Protocol

  • Background: Most job exposure matrices (JEMs) have been developed for chemical and physical hazards in the United States (US). In addition, the overall validity of most JEMs of work organization hazards using self-reported data in the literature remains to be further tested due to several methodological weaknesses. Methods: This paper aims to review important methodological issues with regard to a JEM of work organization hazards using self-report data and to present a research protocol for developing a four-axis (job titles, hazards, sex, and time) JEM of major work organization hazards using the US General Social Survey-Quality of Work-Life (GSS-QWL) data (2002-2018; N = 7,100 workers). Results: Five methodological weaknesses in existing JEMs of work organization hazards using self-report data were identified: having only two axes (hazard and occupation), using psychometrically weak items and scales, including scales having little interoccupational variability, unresolved optimal minimum numbers of subjects per occupation, and low accessibility. The methodological weaknesses were successfully addressed in the proposed research protocol. Conclusion: The work organization JEM to be developed will significantly facilitate and strengthen occupational epidemiological studies on work organization hazards and major health outcomes, improve national and occupational surveillance of work organization hazards, and promote interventions for a healthy work environment in the US.

COVID-19 response survey study on health personnel in Jeju Special Self-governing Province (제주특별자치도 코로나19 대응 보건인력 대상 설문조사 연구)

  • The purpose of this study is to determine the issues of supplementation and improvement to prepare for the outbreak of new infectious diseases such as new variants of coronavirus disease 2019 (COVID-19) to guide work for a strategic new response to infectious disease. Public officials of Jeju Special Self-governing Province and health personnel responding to COVID-19, working at six public health centers in Jeju-do region were administered a survey about additional preparations to be made in the future, based on the period when COVID-19 was treated legally as a first-class infectious disease. Frequency analysis was conducted on the collected data. The Likert 5-point scale and Kruskal-Wallis test were used to compare the scores for effective response to emerging infectious diseases according to demographics. Among the important factors identified for effective response to new infectious diseases, 'facilitation of cooperation with public institutions' and 'facilitation of cooperation with private institutions' had the highest scores. In the future, when a patient presents with a new infectious disease, the step that needs to be supplemented in each phase of the public health center's response is 'immediate response team operation'. Further, public health centers responded that 'expansion of dedicated personnel related to infectious diseases' needs to be improved to respond to new infectious diseases. Along with the results of this study, considering the difficulties experienced by health personnel responding to new infectious diseases in preparation for future outbreaks of new infectious diseases, and to respond effectively, detailed and clear guidelines for responding to quarantine of patients of new infectious diseases will be needed.
