Objectives: The study was to propose strategies and directions how to manage the hypertension and diabetes in communities. Methods: The survey data from 606 patients with hypertension or diabetes based on Community Health Survey, 2013 were analyzed and the hypertension and diabetes projects in communities for last 10 years were reviewed. Results: The patients visiting the primary clinics had statistically significant lower rates than those of teaching hospitals in physician's recommendation experience, perception level of attention from doctors, self-efficacy and health habit practice level. Since the Hypertension and diabetes registration and management system in 2007, there have been several trials for management of hypertension and diabetes such as Chronic diseases management system on the primary clinics, Community based primary medical care pilot projects, Post-national health screening management, and Pilot project on reimbursement for chronic diseases care services. Conclusions: The upmost urgent task might be to have a support system for patients' self care affiliated with primary clinics. To achieve it, it is necessary to expand the current Hypertension and diabetes registration and management system into nation and to find a way to attract the active participation from primary clinics.
The prevalence of helminthic infections was surveyed on a total of 6,178 residents (males 2,549 and females 3,629) in 102 villages of 9 provinces in Lao PDR between 2007 and 2011 under the project of Korea-Laos Collaborative Project for Control of Foodborne Trematode Infections in Lao PDR. Fecal specimens were collected and examined by the Kato-Katz thick smear and Stoll's egg counting techniques. The overall liver/intestinal helminth egg positive rate was 71.9% with a single or mixed infections with Opisthorchis viverrini and minute intestinal flukes (Ov/MIF), Ascaris lumbricoides, hookworms, Trichuris trichiura, Trichostrongylus sp., echinostomes, Taenia spp., and others. Ov/MIF revealed the highest prevalence (55.6%) followed by hookworms (27.8%) and T. trichiura (6.5%). The endemic regions with the highest prevalence of Ov/MIF were Savannakhet, Khammouane, Vientiane (Nam Ngum), Champasak (Khong Island), and Saravane Province. High prevalences of A. lumbricoides (33.8%), hookworms (47.8%), and T. trichiura (32.6%) were observed in Phongsaly, Luang Prabang, and Vientiane (Nam Ngum) areas, respectively. The results of this study highlight helminth parasites of current public health significance in different areas of Lao PDR.

This study aims to research the characteristics of space composition and location for the rural community center. The phenomenon of urban areas from rural areas by the dismantlement of the community is being increasingly deepened. The current village welfare center in the rural villages of the dismantlement phenomenon of the community in rural areas can not be solved alone. The location of the Community Center in rural villages for the interaction of existing local inhabitants and should be located where possible near existing villages. This thesis studies and analyzes the characteristics of rural villages in the rural new town project districts. The basic function of the community center space analysis model is the Public service space, the Communication space, the Health space, the Amenity space, the Education space(including the childcare). This study analyze the local characterristics, the location type and the floor plan characteristics about the Community Center in rural villages by the rural new town example project.
The Dr. LEE Jong-wook-Seoul Project (DLSP) in the Lao People's Democratic Republic (PDR) is a long-term development cooperation initiative aimed at upgrading the medical education capacity at the University of Health Sciences (UHS) in the Lao PDR. Under the project, eight faculty members from UHS were trained at the Seoul National University College of Medicine for a 12-month period during 2010-2011. Using a training-of-trainers model, we estimated the effect of DLSP on the learning outcomes of students, which was measured using the standard test scores of the students. The questionnaire developed by the Medical Education Assessment Consortium in Korea was given to the students and both the percent-correct and standardized scores were examined. The evaluation strategy was to compare the learning outcomes of the students taught by the trainees with those that were not and to compare the medical topics taught by the trainees with other topics. It was found that the first group of trainees in the DLSP was associated with an increase of 1.5 in the percent-correct test scores for basic medicine. While the interpretation requires caution, it is consistent with the view that a training program for faculty members could be effective in improving the teaching capacity of medical schools in developing countries.

Objectives: The Regional CardioCerebroVascular Center (RCCVC) project was initiated to improve clinical outcomes for patients with acute myocardial infarction or stroke in non-capital areas of Korea. The purpose of this study was to evaluate the outcomes and issues identified by the Busan RCCVC project in the treatment of ST-segment elevation myocardial infarction (STEMI). Methods: Among the patients who were registered in the Korean Registry of Acute Myocardial Infarction for the RCCVC project between 2007 and 2019, those who underwent percutaneous coronary intervention (PCI) for STEMI at the Busan RCCVC were selected, and their medical data were compared with a historical cohort. Results: In total, 1161 patients were selected for the analysis. Ten years after the implementation of the Busan RCCVC project, the median door-to-balloon time was reduced from 86 (interquartile range [IQR], 64-116) to 54 (IQR, 44-61) minutes, and the median symptom-to-balloon time was reduced from 256 (IQR, 180-407) to 189 (IQR, 118-305) minutes (p<0.001). Inversely, the false-positive PCI team activation rate increased from 0.6% to 21.4% (p<0.001). However, the 1-year cardiovascular death and major adverse cardiac event rates did not change. Even after 10 years, approximately 75% of the patients had a symptom-to-balloon time over 120 minutes, and approximately 50% of the patients underwent inter-hospital transfer for primary PCI. Conclusions: A decade after the implementation of the Busan RCCVC project, although time parameters for early reperfusion therapy for STEMI improved, at the cost of an increased false-positive PCI team activation rate, survival outcomes were unchanged.
Environmental monitoring system has been adopted and supplemented as inspection measures for the quantitative and qualitative changes of environmental impact assessment (EIA). This study compares the results of environmental impact assessment with the results of post-environmental investigation using a correction and principal component analysis (PCA) in the housing development project. Correlation analysis showed that most of air quality variables including TSP, $PM_{10},\;NO_2$, CO were linearly correlated with each other in the environmental impact assessment and the post-environmental investigation. In the water quality, pH and BOD were well correlated with the DO and SS, respectively. As a result of correlation analysis in the noise and vibration, noise in day and night and vibration in day and night were related to each other between EIA and the post-environmental investigation. From the results of analysis of soil, Cu with Cd, Cu with Pb, and Cd with Pb were related to each other in EIA. Principal component analysis (PCA) showed a powerful pattern recognition that had attempted to explain the variance of a large dataset of inter-correlated variable with a smaller set of independent variables (principal components). Principal component (PC1) and principal component (PC2) were obtained with eigenvalues> 1 summing almost $90\%$ of the total variance in the all of the items(air, water, noise, vibration and soil) in EIA and post-environmental investigation.
The Korean government required the mandatory ratio of public housing including the housing unit for elderly in February 2012 and the number of them is expected to increase accordingly. To prepare for an aged society, these enabling ubiquitous technology will be able to support efficient health care services, which was mentioned in various studies. Therefore, the aim of this study is to draw the design elements for an apartment for providing health care services according to the degree of independence of elderly people and to grasp the current situation and work out tactics by the evaluation of four actual cases. In housing unit in the elderly housing project, design elements supporting amenity improvement was rated to the highest importance, however, in reality, was reflected at lower rate while the risk management plan for public space was applied at the highest rate. Design elements required by new technology such as health monitoring and community management with outside world are still not reflected, however, in order to realize the concept of 'aging in place', health care services in various aspects should be considered in housing planning.
The improvement of civil registration reguires continuous study rather than periodic efforts. More and better statistics, however, are urgently required to formulate development programs and planning. Data obtainable from the civil registration are usually marred by errors of omission which are difficult to correct. This study aimed at finding out the problems occuring when a set of crude birth registration data in a rural area is used. Data Sources of this study are: 1) For birth registration: government birth registration records obtained from myun office and other government offices. 2) For the actual number of births: birth and child records from the Kang Wha Community Health Project The study area is Sunwon Myun and Naega Myun in Kang Wha Gun, Gyunggido. The referrance period for the accumulated data is one full year: Jan. 1st 1975-Dec. 31st 1975 Major findings are as follows: If the number of registered births is compared with the actual number of births which occured in the target area, the former is far greater than the latter. The general assumption usually is, that the actual number of births exceeds the registered number of birth in Korea. The observation from this specific study in this specific target area, shows the opporsite trend. The number of births which were registered is 550. This is more than two times as much as the number of births which actually occured during the year of 1975 in the study area namely 256. The difference comes mainly from the fact that many cases of births from other areas were registered in the target area. In other words birth is not registered where it occured but where the permanent residence adress is. Among 550 births registered in the target area 66% did not occur in the target area. Only one third of all registered births were registered within the legal period for birth registration which is 2 weeks. 34% of the registered births actually occured in 1974, but were registered in 1975. In 55% of the cases a difference was observed between the actual date of birth and the registered date of birth. From the 256 births which occured in the target area, only 153 births (59%) were registered at the myun office and the remaining 130 births (41%) were not resistered there in the year of study. 6% of the 550 cases listed as registered have no seperate registration sheets. Nevertheless, they definitly have been registered in the birth list at the myun office. 3% of the 550 cases are not recorded in this list but have a separate registration sheet at the myun office. In conclusion, birth registration data have many errors and problems. Their usefulness as. a source for vital and other statistics should be reconsidered. A series of sound methological studies will be necessary to establish their actual usefulness. A continuous and permanent compulsory system of birth recording is needed.
In this study, we attempted to enrich cyclo-His-Pro (CHP) using enzymatic hydrolysis of yeast and to evaluate the functionality of yeast hydrolysate (YH)-enriched CHP. Flavourzyme offered a better performance in enhancing CHP content than other proteases. The CHP enrichment conditions were optimized as follows: addition of 1% Flavourzyme, 48-h incubation at 60oC, and pH 6.0. The CHP content significantly increased by 20-fold after ultra-filtration (UF). Maximal CHP translation was obtained after heating for 8 h at 50oC and pH 7.0. YH showed poor foaming capacity between pH 3.0 to 9.0. The emulsifying activities of YHs were slightly higher at near acidic pH. Increase in heating temperature and time resulted in decreased CHP content. The results indicate that YH is more heat stable after UF. Therefore, the CHP in YH after UF can be used as a food additive with physiological CHP activity and high heat stability.
The purpose of this study is to explore the experiences and challenges of public guardianship services project for the mental disabled as a system of supporting the decision-making and the rights advocacy. For this purpose, interviews with 11 social workers who act as public guardians were conducted and a model of practice was suggested through analysis using the grounded theory of Strauss and Corbin(1998). As a results, the possibility of rights advocacy through public guardian services for the mentally disabled, the positive change of attitude of the facilities and the establishment of direction for the restoration of rights for the mentally disabled were derived. Public guardians experienced excessive empowerment of their guardians or difficulties in the process of their duties, but were also able to defend the rights of the mental disabled and support self-determination right. Through this study, expansion of budgets and expansion of beneficiaries for public guardianship services, establishment of infrastructure for integration in the community, set-up a support organization for public guardian services and Raising awareness of the mental disabled and public guardian services were suggested.
