  • Purpose: This study compared health behaviors between single-member and multiple-member households by gender in adults. Method: Study subjects were 21,757 adults 20-65-years-of-age from the Third Korea National Health and Nutrition Examination Survey in 2005. The relationship between health behaviors and residence characteristics was assessed using multiple logistic regression analysis while controlling for sociodemographic characteristics. Results: Younger women living alone were more likely to drink than older women living with others. Women who were in multiple-member households and had a low income were less likely to exercise regularly, to sleep well, and to have regular health examinations. Age, educational level, and income were significantly associated with smoking, exercise, and health examination rather than residence characteristics in men. Conclusion: Residence characteristics should be considered in developing a public health program to reduce or increase modifiable health behaviors as well as age, gender, education, and income.

  • Objectives : To investigate the current status of hospital-based health promotion programs in Korea and to elucidate the factors which affect to the process of implementation. Methods : We conducted a mail survey of all 875 hospitals in Korea from March to May 2001. In reference to 12 specific kinds of health promotion programs, hospital CEOs were asked whether their hospital have such programs, whether they are fully staffed and whether the program is paid for by the patients. Contextual factors(location, hospital type, number of beds, length of operation, public/private status, economic level of the community, the level of competition) and organizational factors (the extent of market, compatibility with vision, formalization), strategic types of the CEOs (defender/analyser/prospector) were also surveyed. The relationships between each variable and the implementation of health promotion services, activation of services, and the target groups(patient/community resident) were analyzed by univariate analysis and the independent effects of these variables were examined with multiple logistic regression. Results : 100 of 125 hospitals responding (84.8%) had mere than one health promotion program. However, they showed fluctuations in the adoption rate of each programs, meaning that comprehensive health promotion services were not provided. Many programs were not fully staffed and few hospitals had paid programs. In factors affecting health promotion service implementation, private hospitals showed a higher rate in implementation than public hospitals. In contrast, when the competition among nearby hospitals was intense, the level of implementation of service lowered. In the strategic type of the CEOs, the prospectors were shown to have instituted more health promotion programs in their hospitals and the analysers had a greater tendency to have programs for community residents than the defenders. Conclusion . Considering the above results, contextual factors may contribute greatly to the introduction of health promotion services in Korean hospital, although the CEO's personal preference and organizational factors play a larger role in the activation of services. Additionally, the CEO's personal preference may be the major influencing factor in the introduction of programs for community residents.

  • Objectives : Periodontal disease is a serious oral disease that frequently occurs among adults. The objective of this study is to provide necessary data for the development of an oral health care program that can effectively manage periodontal disease and subsequently maintain and enhance oral health. Methods : Data was collected from patients of a dental clinic in Daejeon, Korea from 1 July to 25 August 2009. A thesis submitted to the Committee of Graduate School of Public Health & Biotechnology Chungnam National University in partial fulfillment of the requirements for the degree of Master of Public Health conferred in February, 2010. Results : 68% of the patients were aware of periodontal disease, with older patients exhibiting better awareness. 48.0% replied that their periodontal health is good in general, with older patients inclined to say that their periodontal health was poor. 70.5% cited bad brushing habit or skipping the act of tooth brushing as the cause of periodontal disease, while 63.5% reported brushing their teeth an average of 3 times a day. 56.5% said that they brush their teeth correctly, and 63.5% told that they brush their teeth up and down, and left to right and 70.0% replied that they would attend an educational program for the prevention of periodontal disease, with older patients shown to more likely attend such a program. Conclusion : a program that can motivate people to become aware of the importance of oral health care should be developed and implemented. Such a program should include the dissemination of correct and accurate oral health care information and measures for educating people about the importance of prevention.

  • Background: South Korea has experienced problems with excessive pharmaceutical expenditures. In 2010, the South Korean government introduced an outpatient prescription incentive program to effectively manage pharmaceutical expenditures. Therefore, we examined the relationship between the outpatient prescription incentive program and pharmaceutical expenditures. Methods: We used data from the Korean National Health Insurance claims database, which included medical claims filed for 22,732 clinics from 2011-2014 to evaluate associated pharmaceutical expenditures. We performed multiple regression analysis and Poisson regression analysis using generalized estimating equation models to examine the associations between outpatient prescription incentives and the outcome variables. Results: The data used in this study consisted of 123,392 cases from 22,372 clinics (average 5.4 periods follow-up). Clinics that had received outpatient prescription incentives in the last period had better cost saving and Outpatient Prescribing Costliness Index (OPCI) (received: proportion of cost saving, ${\beta}=6.8179$; p-value < 0.0001; OPCI, ${\beta}=-0.0227$; p-value < 0.0001; reference = non-received). Moreover, these clinics had higher risk in the provision of outpatient prescription incentive (relative risk, 2.772; 95% confidence interval, 2.720 to 2.824). The associations were higher in clinics that had separate prescribing and dispensing programs, or had professional staff. Conclusion: The introduction of an outpatient prescription incentive program for clinics effectively managed problems with rapid increases of pharmaceutical expenditures in South Korea. However, the pharmaceutical expenditures still increased in spite of the positive impact of the outpatient prescription incentive program. Therefore, healthcare professionals and health policy makers should develop more effective alternatives (i.e., for clinics without separate prescribing and dispensing programs) based on our results.

  • The effort to measure and improve the quality of healthcare is a common health policy issue worldwide. Korean Value Incentive Programme is one of that effort, but some concerns exist. Compared to pay for performance program in other countries, it measures healthcare quality with relatively narrow performance domain using a small number of clinical indicators. It was designed without involving hospitals and other key stakeholder, and program participation was mandated. Highest and lowest performers get bonus and penalty using relative ranking. As a suggestion for development, the direction for quality management at the national level should be given first. Therefore the philosophy or strategy for quality improvement should be reflected to the program. And various domains and indicators of healthcare quality should be developed with active communication with healthcare providers. The evaluation method is necessary to be changed to provide achievable goal to the healthcare providers and attract quality improvement.
