The survey was carried out on 1,031 primary school children for about 1 month, from June 1, to June 30, 1974, for the purose of understanding indirectly scio-medical status of a remote rural area, reflecting health status of primary school children. The summarized results were as follows: 1) The average age of surveyed children according to school year by sex was older in girls than in boys. The entrance in school at suitable age was peaked as 80.9% in boys of 5th school year and 72.4% in girls of 3rd school year, and also sunk 68.8% in boys of 2nd school year and 58.7% in girls of 1st school year. 2) Prevalence rate according to W.H.O. diseases classification during last a month and a year. a) During last month: The diseases of respiratory system remarked the highest 101.1 in boys and 116.6 in girls. The next were diseases of digestive system (24.2 in boys and 32.1 in girls), The girls were higher than boys in prevalence rate. b) During last year: The diseases of diegestive system ranked the highest 133.0 in boys and next, the diseases of respiratory system (108.6 in boys). c) Present illness: Number of code III ranked the highest 129.2 and next XII(127.3) in boys but inverted in girls such as XII(144.9) and III(116.7) and also, Number of code XVII was prevalenced twice in boys than in girls. d) Chief complaints by sex: Abdominal pain ranked the highest 71.2 per 1,000 population and next headache (34.7) in boys but headache ranked the heighest 88.5 and next abdominal pain (63.3) in girls. e) Water drinking habit: Number of code IX was higher in habit of raw water drinker than in habit of boiled water drinker and healthy children were higher in boiled water habit group than in raw water habit group. The diseases of respiratory system were the highest 124.74 in girls and next, the diseases of digestive system (52.3 in girls). According to school year, healthy children were increased to higher grades. 3) Average sick day per total surveyed children and patient during last month and last year. a) Per surveyed 1,031 children was 0.28 days during last month and 0.98 days during last year. And also per patient was 1.78 days during last month and 3.22 days during last year. b) Average sick day was higher in girls than in boys during last month, but inverted during last year. 4) Average absented day per total surveyed children and patient during last month and last year. a) Per surveyed 1,031 children were 0.43 days during last month and 3.81 days during last year (omitted 1st school year). b) Per absented children were 2.0 days during last month and 7.10 days during last year. c) Per absented children were 8.16 days in boys and 3.17 days in girls. 5) Utilization of medical facilities: The drug-store was consulted 91.2% of the total utilized boys and girls. The strengthening of medical facilities and medical care activities in health subcenter is urgently required 6) Medical Expenses by period and sex: a) Average medical expenses per surveyed 1,031 children were 35.95 won, per patient were 298.04 won during last month. b) Average medical expenses per survey children (omitted 1st school year) were 80.56 won, per patient were 243.98 won and per treated patient were 318.87 won during last year. c) Medical expenses were higher in boys than in girls. 7) Rohrer index by sex, present illness and school year. a) Average Rohrer index was 129.8 in boys and 126.2 in girls. b) Average Rohrer index was increased for 1st school year to 2nd school year and thereafter falling down step by step by school year. c) Under 151-160 Rohrer index number of code III was the highest and above 151-160 Rohrer index of code XII was peaked.
Objectives: This study was performed to investigate hygienic behavior of food handlers and general population focusing on awareness of hand-washing and the microbial load of their hands. Methods: A questionnaire survey and microbiological analysis were carried out for sixty-four people each. Samples for microbiological analysis were collected through the glove-juice method from the hands, and were analyzed for the presence of aerobic plate counts, total coliform, fecal coliform, Escherichia coli, Staphylococcus aureus, and Salmonella spp. according to the Food Code of Korea. Results: In the survey, significant differences between the food handlers and general population (p < 0.01) were found in hand-washing frequency, duration, use of hand-washing agents, washing parts of hands, hand-drying method, and method of turning off water. In eight different situations among the ten particular situations in their daily life, more food handlers responded to wash their hands than general population (p < 0.05). Bacterial load on hands with general population was consistently higher than with food handlers (p < 0.05), however, percentages of positive hands of S. aureus and Salmonella spp. were not. Conclusions: Poor hand hygiene practices were indicated by the positive results for E. coli, S. aureus, and Salmonella spp. on the hands of some respondents in both groups. This study reveals that there is the need for programs or campaigns to increase hand-washing practices of both groups.
Objectives: There has been a great amount of controversy in the food industry over the effectiveness of using gloves. The objectives of this study were to examine the microbial contamination of food handlers' hands and determine if using gloves could ensure safe handling of foods. Methods: Samples were collected through the glove-juice method from the bare and gloved hands of food handlers at work and were analyzed for the presence of aerobic plate counts, total coliform, fecal coliform, Escherichiacoli, Staphylococcus aureus, and Salmonella spp. Microbiological testing was conducted according to the Food Code of Korea. Results: Microbial contamination was consistently higher with bare hands than with gloved hands, although the microbial loads on both hands increased over time. Under certain conditions, there were significant differences between the bacterial loads on bare and on gloved hands (p<0.05). E. coli, S. aureus, and Salmonella spp. were still alive on both bare and gloved hands at the time the food handlers finished their work. Conclusions: Wearing gloves was associated with a marked reduction of bacterial contamination of the hands. However, the practice of continuously wearing gloves during food handling increases the potential for cross-contamination of bacteria. The findings of this study emphasize the need for a rational use of gloves, and strict adherence to hand hygiene compliance among food handlers.
Objectives: This study was performed in order to investigate hand hygiene practices among food-service businesses employees based on the awareness of hand-washing and load of indicator bacteria on their hands. It focused on the comparison of full-time and part-time workers in food-service workplaces. Methods: A direct-interview questionnaire survey and microbiological analysis were carried out with sixty workers each. Samples for microbiological analysis were collected through a modified glove-juice method from the hands of the food-service workers and were analyzed for aerobic plate count, total coliform, fecal coliform, Escherichia coli, Staphylococcus aureus, and Salmonella spp. Microbiological analysis was done according to the Food Code of Korea. Results: Significant differences (p<0.01) were found in the survey between the full-time and part-time workers in hand-washing frequency, use of hand-washing agents, and hand-drying methods. More full-time workers responded to washing their hands after preparing food, after visiting outside, after handling raw materials, and before putting on gloves/when changing gloves than did part-time workers (p<0.05). No remarkable difference was found in bacterial load on the hands except in the aerobic plate count between the two groups. The detection of E. coli, S. aureus, and Salmonella spp. on the hands of some food-service workers in both groups revealed poor hand hygiene practices. Conclusions: The results of this study indicate that there is a need for training programs in order to improve hand hygiene practices and strict hand hygiene compliance by food-service workers.

Objective : The goal of the following statewide age and gender-coordinated cohort study in Korea is to find out if there is a link between acute myocardial infarction (AMI) and Parkinson's disease (PD). Methods : Utilizing the National Health Insurance Sharing Service cohort, patient data were collected. Six thousand four hundred seventy-five individuals with PD were distinguished by utilizing the International Classification of Diseases 10 code G20 and have enrolled in the PD group. The number of participants decreased to 5259 after excluding 1039 patients who were hospitalized less than one time or who visited an outpatient clinic less than twice. Then, 26295 individuals were selected as part of the control group after case control matching was conducted through 1 : 5 age- and gender-coordinated matching. The Cox proportional hazard regression analysis and Kaplan-Meier method were utilized to analyze the likelihood of AMI in PD. Results : After controlling for age and gender, the hazard ratio of AMI in the PD group was 3.603 (95% confidence interval [CI], 2.837-4.577). After that, the following hazard ratio of AMI in the PD group was modified against for co-morbid medical disorders, resulting in 3.551 (95% CI, 2.795-4.511). According to a subgroup analysis, in males and females aged <65 and aged ≥65 and in the non-diabetes and diabetes, hypertension and non-hypertension, dyslipidemia and non-dyslipidemia subgroups, the AMI incidence rates were dramatically higher in the PD group compared to that of the control. Conclusion : Individuals with PD have a greater chance of AMI, according to this cross-national study.
본 연구에서는 인천광역시 초·중학교 등에 식재료로 공급되고 있는 학교급식 농산물의 안전성을 확보하기 위하여 37품목 527건을 대상으로 잔류농약 실태조사를 실시하고 섭취량에 따른 위해평가를 실시하였다. 농산물 전처리 및 잔류농약 분석은 다종농약다성분 분석법으로 검사 가능한 373종의 잔류농약을 분석하였다. 학교급식 농산물 527건 중 12건(2.3%)에서 잔류농약이 검출되었으며 취나물 1건과 파슬리 1건은 잔류농약허용기준을 초과하여 부적합 판정(0.4%)되었다. 채소류 중 엽채류는 104건 중 7건(6.7%), 근채류는 55건 중 4건(7.3%), 박과 이외 과채류는 6건 중 1건(16.7%)에서 잔류농약이 검출되었으며 곡류, 두류, 서류 등에서는 잔류농약이 검출되지 않았다. 검출 농약에 대한 위해성 평가 결과 %ADI (Hazard index)는 0.0000-39.7425%로 학교급식 농산물 섭취로 인한 인체 위해성은 낮은 것으로 판단되었다. 또한, 농산물은 일반적으로 세척하여 섭취하므로 세척을 통해 농산물의 잔류농약을 크게 줄일 수 있고 조리 과정을 통해 잔류농약이 제거될 수 있으므로 실제 농약의 위해성은 더 낮아질 것으로 예상된다. 그러나 농산물은 소비자의 기호, 계절 등에 따라 섭취량이 달라져 위해성이 증가할 수 있을 것으로 보인다. 따라서 안전한 농산물의 생산과 공급, 소비를 위해서는 농민들의 올바른 농약 사용이 필수적이라고 할 수 있으며 지속적인 잔류농약 실태조사 및 위해평가를 실시하여 부적합 농산물의 유통차단 등 농산물의 안전성을 확보하기 위한 노력이 필요할 것으로 판단된다.
This study is to research cost accounting practice and to analyze propriety of patients' medical payment in artificiality kidney center. The researched cost datum of the year 2012 are as follows. - Hemodialysis medical treatment was reimbursed as much as 158,001 won in case of health insured patients, but payed-off as much as 135,810 won. - The average figure of the total hospitals and clinic center is 1,603,303 won, and one time cost of hemodialysis treatment is 154,487 won. Optimal treatment pay are suggested as follows. First, Regardless of the notified classification from MOHW(Ministry of Health and Welfare), 136,000 won of fixed price payment classification needs to be reclassified by patients, severity and tobe rearranged by fixed price payment system of hospitals. Second, Fixed payment code notified by the Ministry of Health and Welfare is recommended to be simplifies and to reflect according to contents of the medical treatment rendered to patients. Third, Establishment of artificial kidney center has to be risk managed because of its huge investment. Fourth, Cost analysis model has to be maintained as basis together with appropriate application of conversion index model mixed with SGR model.
Objectives : To identify the effects of supplemental private health insurance on health care utilization and expenditure under the mandatory National Health Insurance(NHI) system in Korea. Methods : The data were collected by the National Cancer Center in Korea. Cancer patients who were newly diagnosed with stomach (ICD code, C16), lung(C33-C34), liver (C22), colorectal cancer(C18-C20) or breast(C50) cancer were included as study subjects. Data were gathered using a structured questionnaire from face-to-face interviews, the hospital Order Communication System (OCS) and medical records. Clinical, socio-demographic and private health insurance related factors were also gathered. The differences of health care utilization and expenditure were compared between those who have private health insurance and those who do not using t-test and multivariable regression analysis. Results : Individuals with private health insurance spent larger inpatient costs than those without, but no differences were found in utilization in other service such as hospital admissions, hospital days and physician visits. Conclusions : We found that private health insurance exerts a significant effect on the health care expenditure in inpatient service. These study results can provide a rational basis to plan a national health policy regarding private health insurance. Further studies are needed to investigate the impacts of private health insurance on cancer patients' outcomes and survival rates.
인천광역시에서 유통 중인 농산물을 대상으로 2019년부터 2021년까지 최근 3년간 잔류농약 모니터링을 실시한 결과 총 7,069건 중 83건(1.2%)이 잔류허용기준을 초과하였다. 농약 잔류허용기준을 초과한 부적합 농산물의 비율은 1.0%, 1.4%, 1.1%으로 매년 유사한 수준으로 나타났으며 잔류농약 검출율은 8.2%, 8.4%, 7.4%를 보였다. 최근 3년간 diazinon, flubendiamide, procymidone, fluxametamide, fluquinconazole, hexaconazole 등의 순서로 부적합 검출빈도가 높았으며 농산물 분류별로는 전체 농산물 중 엽채류와 허브류에서 매년 83% 이상 발생하였다. 부적합 발생빈도가 높았던 농산물은 고수(잎), 참나물, 취나물, 파 등이었으며 고수(잎)은 3년 연속으로 부적합 건수가 가장 많았다. 농산물 생산지역별로는 경기, 인천, 충남, 전남 순으로 부적합 건수가 많았으나 지역별 출하 농산물의 검사건수 대비 부적합율은 1.5% 내외로 큰 차이는 없었다. 일부 농산물에서 농약 잔류허용기준의 최대 250배까지 잔류농약이 검출되는 사례도 발생하므로 부적합이 자주 발생하는 농산물, 부적합 검출빈도가 높은 농약 성분들의 철저한 관리가 요구된다. 특히, 농약 사용량 및 사용법 준수, 출하시기 조절 등 농민들의 교육 및 인식 개선이 필요할 것으로 보인다. 안전이 확보된 농산물의 생산과 유통, 소비자의 안전한 식생활을 보장하기 위해서는 농산물에 대한 잔류농약 모니터링을 지속적으로 실시하여 부적합 발생 농산물의 유통을 사전에 차단하는 것이 무엇보다 중요할 것으로 판단된다.
목 적 : 사회적 관점에서 로타바이러스의 사회적 비용을 추계하고자 하였다. 방 법 : 로타바이러스 연간 발생 환자수는 2002년 7월에서 2004년 6월까지 정읍지역에서 보고된 5세 미만 인구 1,000명당 입원 환자 11.6명, 외래 환자는 1,000명당 45.3명을 1년 단위로 환산하여 적용하였으며 의료비 자료는 건강보험심사평가원에 로타바이러스(A08.0)로 청구된 자료를 이용하였다. 2006년 12월부터 2007 년 5월까지 환자 보호자 조사를 실시하여 직접 비의료비 및 간접 비용을 산출하는 근거로 사용하였다. 결 과 : 2005년 5세 미만 아동 중 로타바이러스 감염으로 외래 이용한 환자수는 55,030명, 입원 환자수는 14,092명으로 추정하였다. 의료 비용은 110억원으로 전체의 82.8%를 차지했고, 교통비 및 기저귀 사용 등으로 인한 직접 비의료비는 16억원, 간호에 따른 작업 손실 비용은 6억 8000만원으로 로타바이러스 감염으로 인한 총 사회적 비용은 약 132억원 8천만원으로 집계되었다. 결 론 : 로타바이러스 감염은 의료비 뿐 아니라 비의료비와 간접 비용의 추가적인 지출을 가져오며 이는 사회적으로 큰 손실이 될 것이다. 그러나 본 연구에서는 보수적 추정을 하였으며 추후 연구에서 로타바이러스 감염이 보호자의 삶의 질과 건강수준의 저하에 미치는 영향을 비용 항목에서 충분히 고려할 필요가 있다.
