The purpose of this study is to identify job satisfaction and relative factors of job satisfaction in visiting nurses in public health centers. Data were collected by a self-administered questionnaire from May 1 to June 25, 1997. The subjects were 384 visiting nurses of public health centers in Korea. The results of this study were as follows; 1. Job satisfaction of Visiting Nurses was measured by a 5 point rating scale, the whole mean score was 3.08. The level of job prestige was highest among the six components of job satisfaction. The mean score of job perception was 4.05. 2. The factor affecting job satisfaction were job status, educational level and the number of visits home a month. 3. Factors affecting job perception were whether or not they had other certificates, whether they were educated about visiting nursing, and the status of these nurses. 4. Job perception and Job satisfaction had a significant correlation.
Objectives : Cancer is the leading cause of death in Korea. Therefore, a National Cancer Screening Program (NCSP) was launched in 1999. This study planned to evaluate the performance of the NCSP to identifying the influencing factors in relation to characteristic public health centers. Methods : To analyze the performance, the database of the NCSP records for 2002 was used. The performance index was measured by the goal achievement rate, which was defined by the real number of screenees against the expected number of screenees. Also, a survey was conducted by a self-administered questionnaire to identify the factors associated with the goal achievement rate. The questionnaire was divided into two sections. In the first section, the individual characteristics of the program coordinator in each public health center were measured, and second section was comprised of questions about the organizational characteristics associated with the NCSP. A total of 121 subjects from 241 public health centers completed the questionnaire. Results : Of the 121 public health centers (50.2% response rate), the average goal achievement rate was 72.8%. The results of the regression model showed that public health centers located in rural area (parameter estimates=38.2) and had great support from a head of center or province (parameter estimates=0.20) and tended to have higher goal achievement rates. However, the characteristics of the program coordinator, especially their knowledge of and attitude toward cancer screening, were not significantly related to the goal achievement rates. Conclusions : It appears that the most important associated factors to the goal achievement rate in the NSCP were the location of the public health center and the support for the NCSP from the head of the center or province.

From 1998, Oriental Medical Physicians(OMP) is distributed in Public Health Sector. but long term plan for Oriental Medicine in Public Health is not existed. So, this study is designed for the activation of OMP Subjects in this study were comprised 3 groups of oriental medicine related persons, the group of which are Students of 11 Oriental Medical Schools, Oriental Medical Physicians in Public Health Sector, Specialist Croup of Oriental medicine Policy(total 1,458 persons). Data were collected from July 1st to November 30, 1999. Direct interview with key persons, systematic interview by using of interview protocol, e-mail and facsimile have been conducted. The results of survey were coded by Excel 5.0, and analysed with SAS 6.12 statistical package. Inter-group difference determined by T-test, and descriptive statistics have been examined. Major findings can be epitomized as follows. 1) OMP disposition to multifarious organizations and institutes such as Public Health Centers, Public Health Sub-centers, Public Hospitals, Private Hospitals in Remote-Vulnerable Area, Community Social Welfare Centers, Institutes for Heath Policy Research, etc, will promise an effective use of Oriental Medical Physician. 2) Average number of patients treated by OMP was 22.8, average budget for oriental medical department, in which OMP were affiliated, was 39.6 million Won per year. Direct cost per every patient visit was 7,210.9 Won, which is considered expensive for public health service. Therefore, development and transformation for Oriental Medical Service in Public Health Sector is desirable in economic and political aspects. 3) It is recommended that ${\ulcorner}Advisory\;Committee{\lrcorner},{\ulcorner}Planning\;Commission\;for\; Public\;Health\;in\;Oriental\;Medicine{\lrcorner}$ should be established for the activation and for the support of Oriental Medical Physician in Public Health Sector, 4) Most effective programmes for oriental public health doctor are health service programmes for the elderly, Home health visit, chronic degenerative disease control services(p<0.001). 5) Standard guideline for facilities and equipments of Oriental Medicine Department in Public Health Center is needed for optimal supply of resources and activation of public health activity.
Purpose: Public health centers (PHCs) provide a variety of healthcare services according to the Regional Public Health Act (RPHA). We aim to analyze the changes in public healthcare services according to the legislation and the number of public health nurse (PHN) involved in the service, and propose an alternative to effectively distribute the public health nursing workforce for protecting the health rights of people. Methods: This is a review study that deduced the results from literature review. Results: During the revision of the PHC law to the RPHA, several services were added. Health related laws have forced the PHCs to provide services for these legislations. Consequently, the workload on the PHNs has been increasing. However, the PHNs have been consistently lacking. In 2017, there are 16.2 full-time nurses per center. Furthermore, About 50% of the PHCs are not meeting the minimum requirements of licensed or qualified health professional workforce outlined in 1997. In addition, 43% of the nurses in the PHCs are part-time nurses. Conclusion: We suggest that the minimum requirements of health professional workforce should be modified to reflect the increase in the workload of PHNs, and a legislation to enforce PHCs to fulfill these requirements is needed.
Purpose: This study attempted to estimate the need for home visiting nurse at public health centers. Methods: A model was generated to estimate a community's home visiting health service needs in 16 regions and a workload analysis was adopted to estimate the number of required Nurses. Data were collected from 16 public health centers using the South Korean government's open-information systems. Subjects were divided into three groups: vulnerable social group, bottom 10% income group, and bottom 20% income group. Results: The analysis revealed that 2,158 and 6,667 nurses were needed to provide home visiting health service for the bottom 10% and 20% income groups, respectively. It was estimated that for the vulnerable social group, 10,336 nurses were needed to provide home visiting health service, implying that the need-based demand for nurses is well over 5 times the number currently employed. Conclusion: The results indicate that the number of currently employed nurses is insufficient for the health management of vulnerable social groups. The government should consider active employment policies to encourage nurses to apply for home visiting health service.
Objectives: This study was conducted to assess dental health states of disabled people and analyze association between perception and awareness toward dental health and dental health status. Methods: The survey was performed from June 25 through October 30, 2004. A total of 548 disabled people participated in the study with details of 419 living in eight residential care centers located in Gyeongsangbuk-do and 129 children from a special school and two day-care centers. All subjects underwent oral examination and surveyed through a questionnaire. Parents of 129 children with disabilities were also surveyed through a separate questionnaire. Results: The dental caries experience rate was 82.1% of total 548 subjects. By age, those in their 20s experienced a high rate of dental caries with 87.5%. By educational level, those with a middle school education experienced a high dental caries rate with 91.8% (p<0.05). Of total subjects who experienced dental caries, 78.9% had experience in dental caries treatment. By age, those in their 10s showed a high rate of dental caries treatment with 87.4%(p<0.05). By educational level, those with a high school education showed a high rate of dental caries treatment with 87.7%(p<0.05). Those in residential care centers had a high rate of dental caries treatment with 82.1%, which is significantly higher than 68.8% of those who used day-care centers. A tooth extraction rate was 38.0% of total subjects. Those in their 40s had a higher rate of tooth extraction(p<0.01). Those in residental care centers had a significantly higher rate of extraction with 43.4%, compared with 20.2% of those in day-care centers. Of total subjects, 61.5% had plaque. A high rate of plaque formation was observed in those in their 40s(92.0%), those with a high school education(84.0%) and those with multiple disabilities(77.8%)(p<0.01). Among total subjects, 47.6% maintained healthy periodontal tissue. Those in their 40s and those with multiple disabilities had diseased periodontal tissue(p<0.01). Of 129 disabled children, 43.8% had plaque with parents who were not oral health-conscious while 18.6% had plaque with parents who were oral health-conscious, showing a significant difference(p<0.05) Conclusion: The results of the study suggest the need for educating parents with disabled children about oral health and strengthening programs for oral health for teachers working at special schools and day-care centers.
Purpose: We present improvements to the Korean home visiting healthcare service based on analysis of Korean home visiting healthcare services considering recent sociodemographic changes and demands for healthcare services. Methods: This is a review study in which the results are derived through a literature review and data analysis. We collected data through a search of electronic databases, Google Scholar, and governmental websites. Results: Changes in Korean home visiting healthcare services are classified into four stages: 'introduction (1990-2000)', 'pilot project (2003-2006)', 'nationwide expansion (2007-2012)', 'various types (2013-2018)'. Korean home visiting healthcare service based on public health centers has achieved outcomes such as improved health behavior and health management, increased health management ability, and establishment of comprehensive healthcare infrastructure. Conclusion: In the future, the demand for home visiting healthcare service will increase steadily because of deepening social polarization, rapid aging of the population, and increases in chronic diseases. To improve health management and health equity, we suggest that Korean home visiting healthcare service will expand to all the people as a core public health service. It is necessary to establish a management team for various types of home visiting healthcare service in the public health center.
Objectives: In most countries around the world, sex work is an illegal activity. Female sex workers (FSWs) in Iran hide their identities, and they are known to be a hard-to-reach population. Despite free access to HIV testing, fewer than half of FSWs receive HIV testing. The purpose of this study was to characterize the reasons for which FSWs do not seek testing at drop-in centers (DICs) and voluntary counseling and testing (VCT) centers in Iran. Methods: A qualitative study was conducted in 2016. The participants were 24 FSWs who received services at VCT centers and DICs for vulnerable females in the north of Iran and 9 males who were the clients of FSWs. In this study, we made use of purposive sampling and carried out a thematic analysis. Results: We found 4 major and 6 minor themes. The major themes were: fear of being infected (with HIV), stigma, indifference, and knowledge. Conclusions: Despite the significant efforts made by the government of Iran to establish and expand DICs for vulnerable females, the number of FSWs receiving services at these centers has not been very considerable. Consequently, by introducing and implementing training programs for peer groups, it may be possible to take steps toward establishing strategic programs for the control and prevention of HIV/AIDS.

Although most medical institutions in Korea use electronic medical records (EMR), there are many problems in the management and preservation of records when such medical institutions are closed. Records of closed medical institutions need to be systematically managed; however, the rate of closed medical institutions transferring records to public health centers is significantly low. Given that each medical institution has a different system and format, public health centers often cannot access records. In addition, there are no management standards that suit the reality of public health centers and the specificity of EMR. Recently, a strengthened Medical Law has been passed wherein records of closed medical institutions should be kept by health centers; therefore, this study focused on drawing up measures for efficient records management by public health centers. To this end, the relevant laws and management status were identified and an interview was conducted. After analyzing the problems, improvement plans in institutional, technical, and administrative aspects were proposed.
Purpose: To explore the barriers and solutions of immunization programs for children among vulnerable classes in the position of service providers in Community health centers. Method: Data were collected through focus group interviews with 3 groups (17 providers), and were analyzed according to the analytic method of Morgan (1998) and Krueger (1998). Results: The following five categories were identified: difficulties in obtaining a list of children among vulnerable classes, difficulties in first contact with them, difficulties in up-to-date immunization, various difficulties according to the types of vulnerable family and suggestions of solution. These results indicated that there are many difficulties in providing expanded immunization service for children among vulnerable classes in terms of manpower, cooperation of departments in public health centers and linkages between health and welfare. Finally, the characteristics of the types of vulnerable family need to be elucidated. Conclusion: To increase immunization rates among children of vulnerable classes, policies need to be prepared at the public health organization itself and the linkages between health and welfare and need for hiring additional personnel.
