Content Analysis of the Nursing Interventions and Telephone Calls to the Pediatric Nursing Unit (외래 및 퇴원환아 부모의 전화상담과 간호중재의 내용분석)

  • A descriptive study was conducted to describe the content of nursing interventions and incoming telephone calls from the parents whose child was discharged to home or who has planned immunization during the period from March 15, 1995 to November 30, 1995. Detailed notes on 145 telephone calls and 243 nursing interventions were recorded by head nurses on pediatiric nursing units were entered to data collection and content analysis. The results of the study are as follows : 1. Six analysis categories for the records on incoming telephone calls were identified judgement-dependence, dependent -coping, self-initiated coping, support, adjustment and intermediation need. 2. Five analytic categories for the records on telephone interventions were identified : guidance, mediation, facilitating self -care abilities, support, instrumental use. 3. Problems related to physical signs and symptoms, medication, immunization, and vital signs were most often cited as concerns by parents and caregivers. 4. Instruction, suggestion, provision knowledge and information, reassurance related to physical problems, medication, immunization and clinic visits were most often used as an nursing interventions by head nurses on pediatric nursing units. In spite of the fact that the telephone calls were initiated by the parents, dependency of parents during the telephone calls was remarkable. The dependency of parents on judgement and decision making of the pediatric nurses should be understood in terms of the psychosocial content as well as cultural characteristics. Therefore, it is suppested that telephone interventions focus on facilitating the self-care ability of the parents whose children have chronic conditions. The results of this study will be useful as an essential reference in providing effective for children and their families after discharged from the hospital. The results can also be used as reliable data for extended pediatric nursing service in the health care delivery system as well as for the development of telephone intervention service program in responding to the current health care environment.

Psychological Assessment of Patients with Iatrogenic Trigeminal Nerve Injury Using Symptom Checklist-90-Revised

  • Purpose: This study aims to assess the psychological characteristics of patients with iatrogenic damage of the trigeminal nerve using Symptom Checklist-90-Revised (SCL-90-R). Methods: A total of 22 patients who visited the clinic of Department of Oral Medicine, School of Dentistry, Chosun University were evaluated in the months from December 2012 to December 2014. Patients were included in the study if they presented with reported sensory changes due to iatrogenic trigeminal nerve injury. A total of 37 control group who has no symptoms of pain and sensory change were included in the study. All participants filled out the SCL-90-R instrument to assess psychosocial symptoms. Results: The T-scores of most of the SCL-90-R symptom dimensions were within normal range (less than 55) in the patients and the control group. The patients group showed higher T-scores of SCL-90-R than those of the control group, with statistically significance of somatization, anxiety, psychoticism, and positive symptoms distress. Female patients group had higher Tscores of SCL-90-R than those of male patients group, but there was no statistical difference, except for the positive symptom global index. The chronic patients group showed higher Tscores of SCL-90-R than those of the acute patients group, but there was no statistical difference, except for the Positive symptom global index. Conclusions: According to the psychological assessment of iatrogenic trigeminal nerve injury patients using SCL-90-R, the patients group, as compared to the control group shows a significantly higher score for somatization, anxiety, and psychoticism. Therefore, assessment of psychological state would be considered to treat patients with iatrogenic trigeminal nerve injury.

  • Precocious puberty is defined as the appearance of secondary sex characteristics before age 8 years in girls (or menarche before age 9 years) and before 9 years in boys. The age of onset of puberty is progressing to younger age. The pubertal activation of gonadotrophin releasing hormone(GnRH) release requires coordinated changes in excitatory or inhibitory amino acids, growth factors, and a group of transcriptional regulators. A number of factors affecting precocious puberty were explored, including the role of genetic factors, nutrition(body fat) and exposure to endocrine disrupting chemicals. In addition to the psychosocial disturbances associated with precocious puberty, the premature pubertal growth spurt and the accelerated bone maturation result in reduced adult height. Precocious puberty may be gonadotrophin-dependent premature activation of the GnRH pulse generator or gonadotrophin-independent. In rapidly progressing central precocious puberty, GnRH agonists appear to increase final height. However more data on the effect and long-term safety of GnRHa are needed.

Type of Gender Role Identity in Older Korean Men (남성노인의 성역할정체감 유형 연구)

  • Gender role identity is a key subject toward adaptation in later life. In this paper, we tried to understand the specific types and characteristics of gender role identity in view of older men. The research collected and examined 40 representative descriptions representing gender role identity, including 20 for masculinity and 20 for femininity from 40 older men. We obtained four types of gender role identity of older men from this research. "Powerful-expressive type" is energetic and reveals his brave and dominant tendency when they are working. "Powerful-taciturn type" is strongly independent and has a firm conviction, but they do not express those characters and remain silent. "Powerful-paternal type" handles his work with a driving force, and has a tender character. "Paternal-caring type" finds his own worth in life by caring for his family and children, and considers others more than himself. Future research is needed to verify how the types of gender role identity are linked to psychosocial adaptive aspects, health behaviors, mental and physical health.

Musculoskeletal Disorders Among Greek Physiotherapists: Traditional and Emerging Risk Factors

  • Background: Musculoskeletal disorders (MSDs) in the health industry are quite common, however, there have only been a few studies regarding physiotherapists, while in Greece, there is an apparent lack of research and data. The aim of this study is to investigate MSDs experienced by physiotherapists in Greece, their causes and specific measures, and good practices followed. Additional emerging risk factors will be examined. Methods: A questionnaire of MSDs followed by individual and workplace characteristics was completed by 252 physiotherapists. It covered the major workplace categories such as public hospitals, private rehabilitation centers, and private practices. Results: Analysis indicated that 89% of the respondents had experienced a work-related MSD; 32.2% of those injuries occurred within the first 5 years of working. The most lumbered physiotherapists were those working as private practitioners and almost half of the injured respondents chose to work while injured. The most common measure taken to tackle work related MSDs was found to be physical therapy sessions. Job satisfaction and psychosocial issues were also identified as side-effects of the economic slowdown. Conclusion: Physiotherapists in Greece were found to suffer from MSDs; workplace musculoskeletal injuries were quite common but under-reported. The body parts most affected were the lower back, the upper back, the shoulders, and the neck. There was a strong correlation between the workplace setting and the number of MSDs. A well-defined occupational safety and health management system and strict administration steering were found to reduce MSDs. The economic slowdown experienced in Greece during the execution of this study placed additional pressure on physiotherapists.

Association of Conflict at Home and School, and of Health-risk Behaviors with Career Stress among High School Students in Seoul (서울시 고등학생의 진로고민 스트레스와 가정 및 학교에서의 갈등, 그리고 건강위험행동과의 관계)

  • Purpose: The study aimed at identifying distributions of career stress and determining whether conflict at school or with family and health-risk behaviors could be associated with career stress. Methods: The subjects were 7,155 high school students in Seoul. Data were stratified random samples from Seoul student health examinations in 2010. Chi-square, trend test and multiple logistic regression were conducted. Results: Fifty six percent of subjects had career stress. Career stress, after adjusting for confound variables was associated with a significantly increased odds ratio (OR) for sociodemographic characteristics including females (OR=1.34), 12th graders (OR=1.56), 11th graders (OR=1.50), south area (OR=1.47), and northeast area (OR=1.40), for conflict at school or with family including violent threats made by family members or schoolmates (OR=2.00), thoughts of running away from home (OR=1.45), and needing of counseling for agony (OR=5.45), and for health-risk behaviors including sleep ${\leq}6$ hours/day (OR=1.23), nonuse of seat belts or protective euipment (OR=1.50), and frequently viewing pornography or chatting on adult Web sites (OR=1.23). Conclusion: Stress-coping skills and intervention strategies will be needed to enhance students' positive and to help them cope with psychosocial conflicts at school and with their families, and with health-risk behaviors, including sleep deprivation and nonuse of safety measures, including seat belts and protective.

Quality of Life of Antipsychotic-Induced Hyperprolactinemia in Patients with Schizophrenia (항정신병약물로 유발된 고프로락틴혈증을 가진 조현병 환자의 삶의 질)

  • Objectives : Antipsychotic-induced hyperprolactinemia causes physical symptoms, such as amenorrhea, galactorrhea, gynecomastia, sexual dysfunction, and bone density loss, as well as psychiatric symptoms, such as depression and cognitive impairments. This study aimed to clarify the associations among hyperprolactinemia caused by antipsychotics in patients with schizophrenia, psychiatric pathology, and psychosocial factors. Methods : Ninety-nine patients with schizophrenia in the psychiatry department of a university hospital were registered between 2015 and 2017. All participants were assessed using structured questionnaires to elucidate psychopathology, social function, quality of life, and hyperprolactinemia-related side effects. The standard levels for hyperprolactinemia were 24ng/mL for women and 20ng/mL for men. Results : The average prolactin levels were $73.45{\pm}49.37ng/mL$ in patients with hyperprolactinemia and $9.16{\pm}6.42ng/mL$ in those without hyperprolactinemia. The average prolactin level in women was significantly higher than that in men(p=0.04). Risperidone was most commonly administered in patients with hyperprolactinemia(58.1%, p<0.01), while aripiprazole was most commonly administered in those without hyperprolactinemia(44.7%, p<0.01). Patients with hyperprolactinemia had significantly higher Positive and Negative Syndrome Scale(p=0.03) and Patient Health Questionnaire-9(p=0.05) scores and had significantly lower Social and Occupational Functioning Assessment Scale(p=0.04) and Strauss-Carpenter Levels of Functioning Scale(p=0.03) scores than patients without hyperprolactinemia. There were no significant differences in side effects or quality of life between the two groups. Conclusion : These findings demonstrate that hyperprolactinemia confers negative effects on depression and social function, but does not directly affect the quality of life. These results suggest that patients with schizophrenia who take antipsychotics that increase prolactin or cause side effects of hyperprolactinemia need to be assessed and receive interventions for depression.

Types of Perception Toward Quarantine Measures among Patients Infected with COVID-19 (코로나19 (COVID-19)에 감염된 대상자의 격리입원에 대한 인식유형)

  • Purpose: This study was conducted to identify the types of perception toward quarantine measures at hospitals or community treatment centers among patients infected with COVID-19. Methods: This study applied Q-methodology. Two hundred and nineteen Q populations were constructed based on related literature and in-depth interviews with 5 adults infected with COVID-19 and who experienced quarantine and hospitalization. Interviews were performed from November 14 to 29, 2020. A total of 45 Q samples were extracted and Q sorting was performed using a 9-point scale for 30 adult subjects who experienced quarantine at hospitals and community treatment centers. The data were analyzed using the PC-QUANL program. Results: The perception of subjects toward quarantine measures was classified into the following six types: 'passive acceptance', 'social stigma perception', 'appreciation of daily life through awareness of the realities of illness', 'why me?', 'fearful perception', and 'positive meaning'. Conclusion: The perception of quarantine measure among patients with COVID-19 is identified as six types with positive and negative emotional characteristics. This result will contribute to the development of individualized strategies to address psychosocial health problems among patients with infectious diseases.

The Effect of Non-Pharmacological Intervention on Depressive Symptom in Elderly with Mild Cognitive Impairment : A Systematic Review of Randomized Controlled Trials (경도인지장애 노인의 우울증상을 위한 비약물적 중재 효과: 무작위 대조군 실험연구의 체계적 문헌고찰)

  • The purpose of this study was to systematic review about randomized controlled trials the characteristics and effect of non-pharmacological intervention on depressive symptom in elderly with mild cognitive impairment. We searched studies published from January 2011 to July 2021 in 3 databases. A total 1,455 studies were found and included 11 studies in final analysis. Methodological quality was assessment with the Cochrane's RoB(risk of bias) tool. Geriatric Depression Scale(GDS) was the most used as the assessment tool for identifying the depressive symptom. Intervention were yoga, psychosocial intervention, cognitive training, health education, multi-component intervention, game training, aerobic/pulmonary physiotherapy, art therapy, music reminiscence activity, memory specificity training, cognitive stimulation therapy and SWTW(sleep well, think well) program. Among the intervention programs, yoga, multi-component intervention and game training were effective in improving depressive symptom. This study provided a clinical evidence for planning and implementing intervention on depressive symptom in elderly with mild cognitive impairment.

A Study on Life Satisfaction of Elderly Women with Chronic Illness (만성질환 여성 노인의 삶의 만족도에 관한 연구)

  • The purpose of this study is to provide basic data for improving the life satisfaction of elderly women with chronic diseases by analyzing the factors affecting life satisfaction. Using the 6th Korean Retirement and Income Study, 1846 elderly women aged 65 years or older with chronic diseases were included in the study. In order to investigate life satisfaction, instrumental activities of daily living and interpersonal relationship according to general characteristics, descriptive statistics, t-test and ANOVA were used. We used hierarchical multiple regression analysis to find out the factors affecting life satisfaction. The results showed that elderly women with chronic illness were more satisfied with life when they were younger, spouses, higher education, proper sleep, regular exercise, and regular health checkups. This study has limitations in that the number of chronic diseases, types, and psychosocial aspects are not considered. However, it is significant that the elderly panel with the representative of the whole nation was used to grasp the life satisfaction of the elderly women with chronic diseases. Therefore, in order to improve the life satisfaction of elderly women with chronic illness, appropriate sleep, exercise, instrumental daily life ability, interpersonal intervention will be needed.
