산업재해 근로자의 변화동기에 영향을 미치는 개인, 가족, 산업재해 요인 : 범이론 모형의 적용 (Individual, Family and Industrial Factors Related to Injured Workers' Motivation for Change : Application of the Transtheoretical Model)

  • 본 연구는 범이론을 적용하여 산업재해 근로자(이하 산재근로자)의 변화동기 수준과, 이에 영향을 미치는 요인을 개인, 가족, 산업재해 요인으로 구분하여 살펴보았다. 산업재해로 요양 치료를 받고 있는 305명의 근로자로부터 자기보고식 설문조사를 통해 수집한 자료를 분석한 결과는 다음과 같다. 첫째, 산재근로자의 변화동기 수준은 초이론 모형에서 대부분 전숙고, 숙고 단계에 머물러 있었다. 둘째, 산재근로자의 변화동기에 긍정적 영향을 미치는 개인 요인은 심리안정성, 장애수용도, 독립성이었고, 가족 요인은 가족의 지지도였으며, 산재 관련 요인은 주관적 재해상태, 재해 당시 직업, 직업 복귀 상태였다. 이러한 결과는 산재근로자의 재활을 위해 개인의 심리와 가족의 지지에 초점을 둔 개입이 필요함을 시사한다.

편마비 환자의 밀기증후군 유무에 따른 기능 회복의 특성 (Characteristics of Functional Recovery in Hemiplegia with and without Pusher Syndrome)

  • A 'pusher syndrome' encompassing postural imbalance and hemi-neglect is believed to aggravate the prognosis of stroke patients. The patients with pusher syndrome show a particular posture that pushing away from the unaffected side of the body. The objective of this study was to investigate associated proprioception, associated neuro-psychological symptoms and characteristics of functional outcome with and without pusher syndrome. The subjects of this study were 58 acute stroke patients who been rehabilitated at two university hospitals in Seoul and Buchun. Data were collected using proprioception test and line bisection test. The ability of ADL was assessed by the Modified Barthel Index, transfer by the Functional Independence Measure, and balance by the Modified Motor Assessment Scale. The results were that significant difference was found in the presence of proprioception, in the incidence of hemineglect and anosognosia, and in the score of ADL, transfer and balance between patients with and without pusher syndrome. Patients without pusher syndrome gained more motor score than patients with pusher syndrome. From improvement of view, patients with pusher syndrome gained the lowest score in ability of transfer. The finding suggest that the patients with pusher syndrome is a poorer functional outcome, be related to proprioception, hemineglect and anosognosia.

신장공여자와 신장수혜자를 가진 가족들의 사회심리적 적응 (Psychosocial Adjustment in Families with Kidney Donor or Recipient)

  • The purpose of this study was to generate a grounded theory of how families with kidney donor or recipient coped with kidney transplantations. Interview data from twelve families involved in kidney transplants was analyzed using the grounded theory method. Data analysis revealed that “protecting the family” was the main theme that represented family member experiences. In order to maintain family function and to protect the family from breaking up, family members had to adjust the family structure from the traditional style of a husband-centered family, to one that was patient health -centered. The process of this adjustment was a very long and difficult one, taking several years from the recognition of the kidney disease to the kidney transplants. Family members, especially spouses, employed nine different strategies to deal with various problems and conflicts which occurred during the process : 1) paying attention to the patient's illness and complications ; 2) accepting the patient's illness as the family's illness as well ; 3) managing the patient's illness and complications that occured ; 4) being thrifty ; 5)supporting the kidney donor ; 6) accepting and replacing the lost roles of the patient ; 7) keeping composure and encouraging the patient ; 8) sustaining the patient's independence ; 9) self-restraining sexual desires. These findings suggest that there is a developmental process where family members adjust to a kidney transplant over time. There is also a need for increased social and psychological health services for all family members over the course of kidney transplants.

공간분석을 이용한 지역별 비만율에 영향을 미치는 요인분석 (Analysing the Effects of Regional Factors on the Regional Variation of Obesity Rates Using the Geographically Weighted Regression)

  • Background: This study purposed to analyze the relationship between regional obesity rates and regional variables. Methods: Data was collected from the Korean Statistical Information Service (KOSIS) and Community Health Survey in 2012. The units of analysis were administrative districts such as city, county, and district. The dependent variable was the age-sex adjusted regional obesity rates. The independent variables were selected to represent four aspects of regions: health behaviour factor, psychological factor, socio-economic factor, and physical environment factor. Along with the traditional ordinary least square (OLS) regression analysis model, this study applied geographically weighted regression (GWR) analysis to calculate the regression coefficients for each region. Results: The OLS results showed that there were significant differences in regional obesity rates in high-risk drinking, walking, depression, and financial independence. The GWR results showed that the size of regression coefficients in independent variables was differed by regions. Conclusion: Our results can help in providing useful information for health policy makers. Regional characteristics should be considered when allocating health resources and developing health-related programs.

의복 동조 및 비동조의 관련 변인 연구 (Influential Variables on Clothing Conformiy and Nonconformity)

  • The purpose of this study was to identify the variables influential on Normative Clothing Conformity, Identificational Clothing Conformity, Clothing Anticonformity, and Clothing Independence. Four clothing-related variables (importance of clothing, confidence of clothing, recognition of clothing norm, and perceived risk of clothing), two personality variables (confer-mistic character and self-esteem) and six demographic variable (sex, age, years of education, job, income, and length of career) were included in the analysis. The responses of 714 fulltime employeed subjects from four different cities were analyzed. Business wear was used as the situational stimulus. As the results, career people conformed more identificationally when they felt clothing was important, had conformistic character, had low self-esteem, and felt psychological risk of clothing. They conformed more normatively when they felt social risk of clothing, recognized the clothing norms of the fim, had confoirnistic character, and recognized the clothing norms of the society. They anticonformed more when they felt clothing was important, were female, did not have conformistic character, had confidence of clothing, and felt less performance risk of clothing. And they acted more independently in clothing behavior when they had confidence of clothing, did not have conformistic character, felt peformance risk of clothing, and felt less social risk of clothing.

Useful effect of a clinical shoe insole, Mubal®, as orthotics

  • Arthritis is a major cause of joint pain, stiffness, and subsequent disability which adversely affects quality of life. Seriously, it can lead to long term social and psychological effects including loss of independence, depression, and anxiety. Arthritis is usually treated with joint replacement surgery or medications. However, the artificial joint is temporary and pharmacological measures have side effects, such as addiction or hypersensitivity. Thus, orthotics has been developed to improve arthritis as a nonpharmacological measure. The increased regional load across compartments of articular cartilage is an important factor in the cause of the arthritis. Mubal$^{(R)}$, a clinical shoe insole, has a sliding function to help people to walk straight and realign the body balance. The slide of Mubal$^{(R)}$ reduces the knee joint loading in patients with arthritis. In addition, pumping function of Mubal$^{(R)}$ can mitigate arthritis by stretching the squashed nerves from lumbar to cervical vertebral and actively circulating blood of pelvic limb. In addition, Mubal$^{(R)}$ could help to stimulate the growth plate. Therefore, Mubal$^{(R)}$ can be used for the child with short stature as well as patients with arthritis.

The Concept of Synchronization in the Process of Separation-Individuation Between a Parent and an Adolescent

  • Objectives: Humans experience the process of separating-individuating themselves from an object via the conflict between dependence and independence within the self. The separation-individuation theory focuses on the psychological process of individualizing oneself. Although adolescents' individuation from their parents is based on intrapsychic events, there is an increasing need for an intersubjective understanding of it. We applied intersubjectivity to adolescents and parents to interpret and find solutions for problems arising during their individuation process. Methods: This study retrospectively reviewed a case of a son and his father treated by the author. From the study subject, contents that represent adolescents and parents' interaction and separation in the individualization process were extracted and analyzed, and their experiences shared in this process were reconstructed from the therapist's perspective. Results: From the case involving an adolescent boy with conduct problems and his interactions with his father, the authors observed the phenomenon of intersubjectivity and proposed the concept of "synchronized individuation" between adolescents and parents. As adolescents rapidly grow and change, they experience various dynamic interactions with their parents. Through learning to tolerate the conflicts and ambivalent tension inherent in this individuation process, adolescents and their parents develop their new identity. Conclusion: "Synchronized individuation" should be understood as complementary to, rather than exclusive from, the existing concept of the separation and individuation. It offers a new paradigm with which to understand adolescent-parent conflicts in the process of separation-individuation.

대퇴경부 골절 환자의 입원 생활 (The Hospital Life of the Patient with Femoral Neck Fracture)

  • Nowerdays, the increase of traffic accidents and old age population make the Femoral Neck Fracture(FNF) patients increase. By the improvement of education and standard of living the patients demand better medical service than before. This study is designed to give practical help for the FNF patients by observing their hospital life and establish practical nursing strategies for the FNF patients. For these purposes the Ethnographic Participant Observation was adopted. By this study is focused on the hospital life patient's view. For this end, the field study adopted orthopedic ward in the C University Hospital with 400 beds in Seoul. The object patients of the study were twelve patients. The patients experienced five stages : Embarrassment, Conflict, Stability, Independent, and Extension Stage. The findings and prepared nursing strategies are stated as follows. First, in the Embarrassment Stage they suffered embarrassment, anxiety, pain, they could not do ordinary things. The patients who accidental fractures had anxiety from unfamiliar tests and from hospitalization itself. They lamented that they could not ordinary things, and do nothing but obeying the hospital, and endure the pain. They recognized the changed environment and resigned themselves to life in the ward. In this stage, full openness by the nurses is needed. Second, the attribute of the Conflict Stage were conflict, fear, curiosity, belief, reflection. When they sign the consentment form, they experience conflicts about the possibility of complication, fear of recovery from anesthesia, curiosity about the operation procedure, post - operation state, reflection on their past life, and promise to care for their family members after discharge and keep their religious life faithfully. And they accepted the operation depending on God, believing in modern medicine, and the surgeon. Asking for their changed informations, they expected positive results from the operation. In this stage, an empathic attitude by the nurses is needed. Third, the attribute of the Stability Stage were relief, gratitude, difficulty with excretion, and pain. When they awoke from anesthesia, they felt relief because of a the end of the operation, but they experienced extreme pain, difficulty of excretion in bed. They accepted the changed environment and expected recovery. In this stage, support by the nurses is needed. Fourth, the attributes of the Independence Stage were freedom, exercise, nurturing, anxiety, and discomfort. When they ambulated and exercised, they experienced freedom. They showed exhibited weakness of the digestive organs and discomfort hospital's space, structure, and facilities, the delay of medical certificate issue the lack of prompt response by the medical agents. They ate nurturious food and felt anxiety on the end of hospital life and returning to their ordinary life. They showed the independence of overcoming their environment by increasing exercise and expected their discharges. In this stage, respect by the nurses is needed for the patients to, overcome their environment and prepare for their independence. Fifth, the attributes of the Extension Stage were pessimism, isolation, dissatisfaction, and pain. Accompanied injury and old age made their ward life extend to over seven weeks. They exhibited weariness, melancholy, skeptisis, general pessimistic feeling, and desperation caused by their isolated life. They experienced the digestive discomfort caused by the prolonged medication and psycological pain caused by long-time hospitalization. As a, result, their dissatisfaction on the human, physical, and systematic environments had been increased. They acquired critical power and sought for something to do spending their time. They expected vaguely about the returning of their ordinary life. In this stage, counseling is needed by the nurse to overcome positively their psychological, social, and physical problems. The process of the FNF patient's ward life starts from the dependent state, when they are hospitalized, and gradually progresses to self-fulfillment in order to keep independent life. As a result, the FNF patients showed "Response in Challenge" or "Adaptation in Conflict" through their experiences of social, physical, and psychological difficulties.

남성 청년의 노숙진입 시기에 따른 노숙 경로 (Pathways to homelessness: The case of young adults in Korea)

  • 본 연구는 최초 노숙진입 시기 차이에 따른 남성 청년 노숙인 두 그룹의 노숙경로를 밝혀 구체적인 노숙의 원인을 추적하는 것을 목적으로 한다. 기존의 노숙 원인에 대한 논의는 구조냐 개인이냐의 이분법적 원인론에 머물러 있거나 노숙에 영향을 미친 복잡한 원인을 다루지만 결국 이를 단순화시켜 설명하는 경향이 있었다. 본 연구는 각 청년노숙인 사례의 노숙 위험요인을 도출하고, 사례의 생애 경험의 맥락 속에서 노숙에 영향을 미친 핵심위험요인의 연대기적 배열을 통해 좀 더 세밀하게 위험요인간의 인과성을 추적하여 노숙진입의 경로를 살펴보았다. 그 결과 청년기에 노숙에 진입한 그룹(A1)과 10대 때부터 노숙을 경험한 바 있는 그룹(A2)은 노숙의 진입 경로가 상이함을 확인하였다. 양쪽 모두 청년기에 극한의 상태인 노숙을 경험하고 있다는 공통점이 있지만, 전자의 경우는 가족, 사회로부터 입은 외상으로 인해 정신적인 고통에 시달리고 이로부터 이탈해 고립을 경험하다 노숙에 이르게 된 경우로 대표된다면 후자의 그룹은 이미 청소년기부터 위기청소년의 상황이 시작되었고, 학교, 가정에서의 불안정성이 매우 높았던 것을 확인할 수 있었다. 청소년기의 노숙 경험이 청년기로 곧장 이어지는 경우는 많지 않았지만 비노숙 기간이 잠시였을 뿐 청년기에 노숙으로 재진입하고 있었다. 이러한 결론을 통해 성인기의 삶의 토대를 다져야 하는 청년기에 극단의 상황에 처한 청년들의 위험요인과 진입경로에 따른 차별화된 개입, 사례관리, 상담에 대한 정책적 접근이 필요하다는 것을 제언하였다.

Strengthening Families: The Re-structuring of Family Services in Hong Kong

  • The problems and needs of families in Hong Kong are becoming more complex. Traditional family structures and functions are rapidly eroding. Moreover, family problems are further exacerbated by a growing number of socio-economic issues including rising divorce rates and extra-marital affairs. continuous family reunion migration from Mainland China, emotional and financial cost of caring for the older people and the changing economy which has pushed more families into hardship. In effect, more and more families are becoming vulnerable to risk. Traditional family services in Hong Kong are varied in mode, operation and provision, often overlapping in service boundaries with other family-oriented programs. In many instances, family service centers are stretched far beyond their capacity. As a consequence, they become too reactive, remedial and casework dominated. The University of Hong Kong Consultant Team recommended that family service programs have to protect vulnerable families and strengthen family capacities to promote maximum independence. The direction of family services is summarized as: strengthening families; child-centered, family-focused and community-based. New integrative family service centers aim at attaining the principles of promoting accessibility to users with minimum physical, psychological and administrative barriers; early identification of needs and intervention before the further deterioration of problems; integration of services cutting across program boundaries, and partnership between service providers to achieve efficient and effective use of scarce resources. Under the new model, multi-skilled teams can respond more proactively to a wide range of social needs, rather than addressing needs in isolation. To demonstrate the effectiveness of the new model, the consultant team has been commissioned by the government to evaluate the performance of these two-year pilot projects. More importantly, a coherent and family-friendly social policy should be formulated to strengthen family capacity against family-related problems.
