Psychosocial Response and Symptom Burden for Male Smokers with Lung Cancer

  • Purpose: Cigarette smoking causes many kinds of cancer, and it is more closely related with lung cancer, rather than other cancers. Smoking is the leading cause of lung cancer and ninety percent of the smokers are male in China, but there is little published data concerning the psychological responses in the male smokers with lung cancer and its influence on the symptom burden. The aim of the study was to verify the hypothesis that male smokers with lung cancer have more positive attitude and less symptom burden, comparing to male non-smokers. Methods: A total of 194 men with cancer in West China Hospital, Sichuan, China, were assessed by self-administered questionnaire. Psychological response was measured by the Chinese version of Mini-Mental Adjustment to Cancer scale (Mini-MAC), and symptom burden was measured by the physical symptom distress scale from the Rotterdam Symptom Checklist (RSCL). Results: We found that smokers with lung cancer got higher scores in positive attitude and a smaller symptom burden than non-smokers. Patients with education lower than high school got higher scores of positive attitude compared to college graduate patients (p=0.038). Smokers with lung cancer who knew the potential carcinogenicity of cigarette showed less negative emotions (p=0.011). The psychological response was not affected by age, clinical stage, cell type, smoking duration and amount. Conclusions: Male smokers with lung cancer have a more positive attitude and fewer symptoms, comparing to male non-smokers. Appropriate psychological intervention for non-smokers with lung cancer deserves more attention.

  • 도시철도 기관사들의 직무스트레스는 개인은 물론 사회전체에 치명적인 해악을 초래하고 엄청난 경쟁력 상실을 가져온다. 따라서 기관사들의 직무스트레스에 미치는 영향요인과 부정적인 결과들에 대하여 살펴보고 직무 스트레스를 감소시키는 방안을 모색하고자 한다. 본 연구에서는 직무특성(직무불안, 직무요구)과 조직특성(비합리적 직장문화)이 직무스트레스에 미치는 영향을 살펴보고 직무스트레스가 심리적 우울과 인적오류에 미치는 영향을 살펴보고자 한다. 또한 직무스트레스와 심리적 우울, 직무스트레스와 인적오류 간의 관계에서 조직지원의 조절효과를 살펴보고자 한다. 도시철도 기관사들을 대상으로 설문조사를 실시하였고 결과는 다음과 같다. 첫째, 직무불안, 직무요구 수준이 높을수록 직무스트레스에 정(+)의 유의한 영향을 미치며 비합리적인 직장문화는 직무스트레스에 정(+)의 유의한 영향을 미치는 것으로 나타났다. 둘째, 직무스트레스는 심리적 우울과 인적오류에 각각 정(+)의 유의한 영향을 미치며 셋째, 심리적 우울에 대해서는 직무스트레스와 조직지원지각의 상호작용효과는 유의한 것으로 나타났으나, 인적오류에 대해서는 직무스트레스와 조직지원지각의 상호작용효과는 유의하지 않은 것으로 나타났다.

주택파손 피해자의 심리적 충격 변화에 관한 추적조사 연구 (Longitudinale Study on the Psychological Trauma of Victims of Housing Damage)

  • 자연재해는 지역사회 전반에 극심한 스트레스를 발생시킨다. 다수의 경우 시간의 흐름에 따라 충격을 극복하지만, 일부는 PTSD와 같은 장기적인 정신적 피해를 경험하게 된다. 이에 재난과 심리적 충격에 관한 연구가 진행되고 있지만, 여전히 부족한 수준이다. 자연재해는 시설 붕괴, 인명피해 등 다양한 피해를 야기하기에 피해자가 경험하는 피해의 종류가 다르며, 경험하는 상실의 정도에도 차등이 있다. 특히 자연재해로 인한 가장 흔한 피해인 주택파손의 경우 피해자가 경험하는 충격은 매우 크다. 따라서 본 연구는 자연재해로 주택이 파손된 가구가 경험하는 심리적 충격을 확인하고, 추적연구를 통해 심리적 충격이 변화하는 특성을 확인하고자 하였다. 연구 결과, 주택이 파손된 경우 심리적 충격이 더 크게 나타나 주택의 파손으로 인한 심리적 충격이 다른 피해로 인한 충격보다 더 큰 것을 확인했다. 또, 재난으로 인해 발생하는 심리적 충격이라고 할지라도 불안, 우울, PTSD 그리고 전반적인 심리적 충격 등 그 성격에 따라 변화 양상에 영향을 미치는 요인이 다른 것으로 나타났다. 그 중, 보건의료 서비스는 모든 변수에서 유의하게 분석돼 재난 이후 안전과 관련된 기본서비스의 제공이 가장 필요로 된다고 해석된다. 또, 재난구호서비스 중 심리 안정 지원은 불안, 우울과 같은 단기적인 충격에는 긍정적인 영향을 미치나, 심리적 영향이 가중된 PTSD에서는 오히려 역효과를 보여 심리지원체계의 변화 및 발전이 요구된다고 할 수 있다.

도시철도 기관사의 직무스트레스 원인 특성에 관한 연구: 대전, 대구, 인천, 서울9호선 운영회사를 대상으로 (A Study on the Cause of Job Stress of Urban Railroad Drivers: Focused on Railroad Companies of Daejeon, Daegu, Incheon and Seoul Metro 9)

  • 본 논문은 도시철도 운영회사 중 대전, 대구, 서울9호선, 인천에 근무하는 기관사들의 직무스트레스 원인을 크게 임무(보직), 정신적, 신체적, 외부 환경적 등 네 가지 원인으로 나누어 알아보았다. 임무(보직)에 관련된 내용으로는 기관사의 직업에 대한 강한 책임의식의 발현으로 직업과 적성이 일치함을 알 수 있었으며, 대부분의 기관사가 바람직한 직업관과 긍지를 갖고 헌신적으로 일하는 분위기를 알 수 있었다. 하지만, 정신적인 원인과 신체적인 원인의 스트레스를 확인해 보면 수면 부족, 과제에 대한 부담감, 육체적인 피로, 무료한 일상 등에서 나타났으며 기관사의 불규칙한 근무여건과 부동적인 자세로 정시운행의 중압감, 규칙적이지 못한 식생활습관 등이 스트레스의 수치를 높게 만드는 근원이라 볼 수 있다. 또한, 외부 환경적 원인으로 살펴 보면, 사고장애로 인한 불안도가 심하여 약 4배에 가까운 반응을 보이고, 인사제도나 임금책정 관련 문제 역시 기관사의 직무스트레스 변화와 밀접한 관련이 있음을 알 수 있다.

Prader-Willi Syndrome 환자에서 여성형 유방의 치험례 (Treatment of Gynecomatia in a Patient with Prader-Willi Syndrome)

  • Purpose: Prader-Willi Syndrome(PWS) is a congenital chromosomal disorder characterized by compulsive and early development of obesity. Obesity is identified as the main cause of morbidity in PWS individuals. Also, body change for rapid weight gain, such as gynecomastia, can cause considerable functional and psychological trauma, We corrected successfully gynecomastia in PWS patient, so we reported our experience of surgical method and literature reviews. Methods: A 16-year-old male patient presented with gynecomastia. He was diagnosed as with PWS at pediatric department. We performed reduction mammaplasty using inferior pedicle and Wise pattern. Excision amount was 1350g in right breast and 1415g in left breast. Also, we managed upper and lateral fullness of breast with liposuction. Results: There were no specific complications, such as hematoma, infection, nipple-areola complex necrosis, and so on. Also, aesthetic and functional outcome was acceptable Conclusion: We experienced successful correction of gynecomastia in PWS patient, and found advantages of conventional reduction mammaplasty using inferior pedicle and Wise pattern at this specific situation.

식생활이 정서에 미치는 영향에 대한 고찰 (The observation on eating impact on psychological aspect)

  • Object : This paper is designed to inquire into the influence of eating habits on the emotion of modem people. Method : The methodology is used that searches for materials such as books and treatises on the relationship between eating habits and emotion. Result : Meat used for fast food comes from cows that are fed the feed containing growth hormone and antibiotic. Growth hormone extremely supplements fire or yang within the body while antibiotics undermines the process of yin, leading to an accumulation of damp-heat and inflammation of ministerial fire. Additives contained in instant foods also impinge on the process of yin, leading to poor digestion, damp-heat toxins and ministerial fire. Excessive consumption of meat results in incomplete combustion of fat and hinders the formation of essence. Subsequently, ministerial fire becomes hyperactive as a result of the lack of yin energy. Increased meat consumption in the human diet is coupled with unhealthy eating pattern in which people eat too much for dinner and too quickly. Eating too much disturbs digestion, making it slow and incomplete, leading to excess damp-heat conditions, insufficient amount of vital essence and eventually hyperactive ministerial fire. Milk is considered a catalyst of faster growth, making it suitable for calves. Milk intake rapidly increases the balance of yang and eventually cause hyperactivity of ministerial fire. Conclusion : It is estimated that the eating habits of modern people cause the process of yang, making them feel restless, impatient and aggressive.

불면증의 원인과 발생기전 (The Causes and Developmental Mechanism of Insomnia)

  • With the recent development of sleep medicine, insomnia has been perceived as a disease from a simple symptom. As there are various causes in a disease, proper treatment according to each cause is necessary for a more effective treatment In general, insomnia is classified into five categrories of physical, physiological, psychological, psychiatric and pharmacological aspects. However, such categorizations are often insufficient in treating insomnia. Furthermore understanding of the developmental mechanisms of insomnia is required. The function of sleep is developed and maintained through the balance of the reciprocal forces of sleep and arousal. These forces are contantly regulated by what is called a circadian rhythm. Sleep is induced by this rhythm which is affected by factors such as awakening time in the morning, amount of intellectual function, amount and time length of physical exercise and sunlight Insomnia could develop when this rhythm is delayed and leads to a "forbidden zone" which is a very difficult period for inducing sleep about two to four hours before the routine bedtime. Whereas sleep gradually develops in line with the circardian rhythm, arousal can occur very abruptly by any cause triggered by emotional discomfort or anxiety. Such characteristic and emotional factors as perfectionism, separation anxiety, secondary gain, insecurity, and negative cognition may provoke the inner anxiety and fear for insomnia, which can lead acute insomnia to a chronic one. As chronic insomnia is developed by multiple causes and factors, integrated approaches through analysis of above mentioned factors will be more effective in the treatment of insomnia than a simple administration of hypnotics.

치과기공사의 사기 요인 분석과 관리 방안에 대한 연구 (Study on Analyzing Main Cause of Dental Technicians' Moraleand Management to Promote their Morale)

  • The organization to which we belong is the place where we make social relationships, satisfy our desires of self-realization, and accomplish our common object. The emotional and psychological state of individual as well as the whole organization is able to be explained in the concept of morale. The researcher selected a total 238 dental technicians living in seoul, and conducted a research in a period ranging from April 1, 2004, to April1 10, 2004. The researcher analyzed the findings with the aid of SPSS (Statistical package for social sciences). The researcher came to reach the following conclusions on the basis of frequency, percentage, T-test, F-test, and multiple regression. The results of this study were as follows: First, there is a significant difference in recognizing morale due to their age, academic background, their work places, position, part of objective, and working hours(p<.05). Secondly, The researcher there is a significant difference in recognizing due to their social welfare, payment, fair and objective personnel management, enough opportunities of self development, and autonomy(p<.05). Lastly, this study is very limited in generalization because it deals with a small part of morale management plans and objectifies very limited organization. But, it is very meaningful in trying to approach morale management plans with an object of dental technicians. It is necessary that dental technicians should make efforts to control themselves.

Useful effect of a clinical shoe insole, Mubal®, as orthotics

  • Arthritis is a major cause of joint pain, stiffness, and subsequent disability which adversely affects quality of life. Seriously, it can lead to long term social and psychological effects including loss of independence, depression, and anxiety. Arthritis is usually treated with joint replacement surgery or medications. However, the artificial joint is temporary and pharmacological measures have side effects, such as addiction or hypersensitivity. Thus, orthotics has been developed to improve arthritis as a nonpharmacological measure. The increased regional load across compartments of articular cartilage is an important factor in the cause of the arthritis. Mubal$^{(R)}$, a clinical shoe insole, has a sliding function to help people to walk straight and realign the body balance. The slide of Mubal$^{(R)}$ reduces the knee joint loading in patients with arthritis. In addition, pumping function of Mubal$^{(R)}$ can mitigate arthritis by stretching the squashed nerves from lumbar to cervical vertebral and actively circulating blood of pelvic limb. In addition, Mubal$^{(R)}$ could help to stimulate the growth plate. Therefore, Mubal$^{(R)}$ can be used for the child with short stature as well as patients with arthritis.

The Health Effects of Whole Body Vibration

  • Objective: The purpose of this study is to review the hazards of whole body vibration(WBV) and prevent human injuries due to WBV. Background: Vibrations that shake the whole body in vehicles and vessels is WBV. Vibration at a specific frequency, and intensity in the human body can act as a hazard. But, it has not yet done enough research about effects of WBV. Method: We analyzed and reviewed occupational or ergonomic papers published past 30 years. Results: Exposure to whole body vibration can cause permanent physical injury or damage of the nervous system. In addition, it may cause problem in the cardiovascular system, digestive system, musculoskeletal system, endocrine system, reproductive system, and psychological system. The effect of vibration depends on its acceleration, duration, frequency, and direction. Conclusion: WBV-exposed human body experience a high risk of some illnesses such as lumbar spinal disorders. Some workers(bus drivers, truck drivers and heavy equipment operators) are the high risk population exposed to WBV. Application: The results of this study might help to judge and prevent occupational disease caused by WBV.
