어지럼을 호소하는 환자들 중 많은 환자에서 신경 이과적 검사에서는 이상이 없고 정신의학적으로 문제가 있는 경우가 있다. 이런 경우에 심인성 어지럼증이란 진단을 붙이는데, 이는 대개 정신의학적 문제로 인하여 어지럼이 발생한다. 어지럼증 환자의 2-4명 중 1명일 정도로 많은 편이다. 그러나 한국에서는 심인성 어지럼에 대한 진료, 연구 등이 부족한 편이다. 이에 저자는 한국 정신건강의학과 의사들이 심인성 어지럼에 대하여 많은 관심을 가지고 적극적인 동참을 하도록 하기위하여 이 논문을 발표하였다. 이 논문은 정신질환 특히 불안과 어지럼의 생물학적, 심리적 연관성을 포함한 심인성 어지럼의 원인 및 진단, 심인성 어지럼과 관련된 정신질환 및 각 정신질환에서의 어지럼의 특성, 그리고 심인성 어지럼의 치료에 대하여 고찰하였다. 또한 정신건강의학과 의사를 대상으로 하였기에 신경 이과와의 협진에 필수적인 중추성 및 말초성 어지럼에 대해서도 간략히 고찰하였다. 치료에서는 특정치료, 증상치료, 전정재활치료 뿐만이 아니라 심인성 어지럼의 치료에 필수적인 약물치료 및 인지행동치료에 대해서도 고찰하였다. 어지럼 환자에게 신경 이과적 검사 및 진료 시 기질적 문제가 없거나, 기질적 문제가 있을지라도 다른 정신과적 증상이 상당히 동반된 심인성 어지럼 환자라면 정신의학적 치료를 적용하여 치료성공률을 높이고 나은 귀결을 보일 것이다. 어지럼의 치료에는 다학제적 접근을 통한 환자-중심의 치료적 접근이 환자들의 고통 경감과 삶의 질의 향상을 도모할 수 있다. 이를 위해서 한국 정신건강의학과 의사들의 심인성 어지럼증에 대한 관심 증가 및 적극적인 참여가 필요하다고 제시한다.

Objectives: To propose Korean medicine treatments based on effects of Yanshimtang-Gamibang on three patients diagnosed with psychogenic dizziness. Methods: Korean medicine treatments based on Yanshimtang-Gamibang were applied to three patients diagnosed with psychogenic dizziness after experiencing a traffic accident. Effects of Korean medicine treatments based on Yanshimtang-Gamibang were evaluated using DHI (Dizziness Handicap Inventory), BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), and EQ-5D (EuroQol-Five Dimensions). Quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there were significant decreases in DHI and BAI scores as well as values of NRS, NDI, ODI, EQ-5D. The sleep quality of three patients also showed improvement. Conclusions: Korean medicine treatments based on Yanshimtang-Gamibang might be effective in people with psychogenic dizziness.

Objective : The purpose of this study is to investigate dizziness of Plain Questions $\ll$素問$\gg$ and Miraculous Pivot $\ll$靈樞$\gg$. Methods : We conducted a study on the original text paragraphs of Internal Classic $\ll$內經$\gg$ containing the dizziness and analysis of Yang, Ma, Zhang, Wang etc. We drew a parallel between dizziness from Internal Classic $\ll$內經$\gg$and matching diagnoses from western medicine. Results : The results were as follows. 1. Dizziness in Ok Ki Jin Jiang Ron <玉機眞藏論> and Pyo Bon Byeong Jeon Ron <標本病傳論> had relation to liver and was similar to dizziness caused by tension, hypertension, anemia and cerebrovascular accident etc. in western medicine. 2. Dizziness in Ja Yeol<刺熱>, O Sa<五邪> and Hai Ron<海論> had relation to kidney and was similar to dizziness caused by aging and peripheral vertigo concurrent with tinnitus and difficulty in hearing in western medicine. 3. Dizziness in O Sa<五邪> had relation to heart(pericardium) and was similar to dizziness caused by cardiac output loss and psychogenic dizziness in western medicine. 4. In Internal Classic $\ll$內經$\gg$ the main etiology of dizziness was infirmity(虛), which were Qi(氣) of the upper portion of the body being insufficient(上氣不足), blood depletion(血枯), deficiency of marrow-reservoir(髓海不足) etc. 5. In Dae Hok Ron<大惑論> etiology and pathogenesis of dizziness were mentioned and dizziness was similar to dizziness caused by eye disorder, psychogenic dizziness and central dizziness in western medicine. 6. In Internal Classic $\ll$內經$\gg$ the meridian of acupuncture points which was used much for dizziness was Bladder Meridian. Aqupunture points used in treatment of dizziness were Ch'onju(天柱), Kollyun(崑崙), Taejo, Chok-t'ongkok(足通谷) etc. Conclusion : We found out etiology, pathogenesis, treatments of dizziness in Internal Classic $\ll$內經$\gg$. Further we compared with western medicine to develop better understanding of dizziness.

A medical treatment of alternating current high-voltage electric field therapy is a method in which we get a healing effect, applying electric field to an organism through an artificial device. In order to estimate the clinical effects of alternating current high-voltage electric field load(HEALTHTRON) on the rehabilitation of stroke patients, improvement of a peripheral blood circulation, and psychogenic symptom, we used BEUNZEUNGHEYNG Instrument, NIHSS(the National Institutes of Health Stroke Scale), FIM(Functional Independence Measure)lnstrument to research BEUNZEUNGHEYNG and recovery of rehabilitative funation. and also we used Thermography, Pulse Meter to measure body temperature and pulse, blood elements, and neuro modulators. We have reached the following conclusions after researching the clinical effects of alternating current high-voltage electric field therapy on the recovery of rehabilitative function and peripheral blood circulation. 1. HEALTHTRON efficiently has increased the volume of peripheral blood circulation in stroke patients. 2. HEALTHTRON rapidly has treated the symptoms of sleeplessness, alertness, and dizziness of stroke patients. 3. HEALTHTRON rapidly has improved rehabilitative function of stroke patients and the adjustment to their activities.
