Development and validation of the Korean Nursing Delirium Scale (한국어판 간호 섬망 선별 도구 개발 및 검증)

  • Purpose: The aims of this study were to develop and test the validity of the Korean Nursing Delirium Scale (Nu-DESC) for older patients in hospital. Methods: The Korean Nu-DESC was developed based on the Nu-DESC (Gaudreau, 2005), and revised according to nursing records related to signs and symptoms of older patients with delirium (n=361) and the results of a pilot study (n=42) in one general hospital. To test the validity of the Korean Nu-DESC, 75 older patients whom nurses suspected of delirium from 731 older patients from 12 nursing units were assessed by bedside nurses using the Korean Nu-DESC. A Receiver Operating Characteristic Curve of the Korean Nu-DESC was constructed with an accompanying Area Under the Curve (AUC). Results: Specific examples such as irritable, kidding, sleeping tendency, which were observed by bedside nurses in Korea, were identified in the five features of signs and symptoms of delirium in the instrument. The Korean Nu-DESC was psycho-metrically valid and had a sensitivity and specificity of .81-.76 and .97-.73, respectively. The AUC were .89, .74. Conclusion: Results of this study indicate that the Korean Nu-DESC is well-suited for widespread clinical use in busy inpatients settings and shows promise as a research instrument.

Effectiveness of Incentive-Therapy to Alcohol Abuser Using Single System Design (단일체계설계를 활용한 알코올남용 인센티브치료 효과성 연구)

  • Incentive-Therapy approach on substance abuser has a solid theoretical base in Behavior Psychology and shows the effectiveness empirically in US. The purpose of this exploratory study was to evaluate the effectiveness of Incentive-Therapy on two alcohol abusers using ABA Single System Design in a community welfare agency, Korea. It proceeded for 12 week-baseline stage, 6-week intervention stage and 4-week postintervention stage through scanning whether alcohol abuser drinks or not by the alcohol analyzer as a target behavior. As a results, this study showed that two abusers' drinking were decreased in the middle of intervention stage in incentive-therapy but they showed differently in the postintervention stage : One sustained his abstinence and the other recurred to drink. Thus, we concluded Incentive-Therapy had better adaptability in community welfare agency in that alcohol abusers were driven to motivate for abstinence and then had an opportunity for psycho-social counseling and social welfare intervention consistently.

Management Factors Associated with Health and Safety Education in Korean Manufacturing Companies (산업장 안전보건교육 관리요인)

  • Objectives: Safety is a primary health promotion issue in worksite because injury induces multi-fold loss of the human and economic resources to profit organization. The purposes of this study were to describe worksite health and safety education and management status in Korean manufacturing companies. Methods: The original population size of Korean manufacturing industry in 2004 was 74,398 and 2,960 factories were selected by the multiple stratified sampling method for this study. The health and safety manager or representatives of the selected 2,960 companies successfully finished in the face-to-face interview survey about company's general characteristics, health and safety management style, health and safety education hours conducted by the Korean Occupational Safety and Health Agency. Results: The manufacturing companies in Seoul and Kyunggi areas, small size, and clothes and press industries were related to low health and safety management and education status. The companies which assigned at least one safety manager were 70.5% and which had a health and safety room within the company were only 9.3%. The companies which took the health and safety education for their regular blue-collar employees more than the legal education hours were under 56.1% and the percentage of the companies which took their health and safety education for newcomers less than the legal limits was lower than any other types of health and safety education in workplace. The significant strong workplace health and safety management variables in predicting employee health and safety education were psycho-social variables such as the company own health and safety regulation and the workplace health and safety management committee organization. rather than physical variables such as health manager employment, safety manager employment. Conclusions: Systematic and legal approaches are effective to encourage workplace health and safety education, specifically, through sustaining health and safety managers and building the company-wide health and safety management system. Furthermore, theses approaches should primarily focus on the small companies of which sizes were under 50.

The Impact of Social Support and Stress on Smoking among Students of Several Boy's High Schools in Daejeon, Korea (일부 남자 고등학생들의 스트레스, 사회적 지지, 환경적 특성과 흡연과의 관계)

  • Objectives: The purpose of this study is to evaluate the impact of psycho-social factors on smoking especially the impact of a social support and stress on smoking among boy's high school students. Methods: This study was conducted by administering a questionnaire to 442 boys' high school students living in Deajeon, Korea. This data were analyzed with the SPSS statistical package using t-test and logistic regression. Major findings of this study are as follows: Results: 1. Analysis of the relationship of smoking, stress and social support: The relationship between stress and smoking and between friend's support and smoking had a positive correlation. But the relationship between parents' support and smoking and between teacher's support and smoking had a negative correlation. 2. Multiple logistic analysis of the impact of stress, social support and general characteristics on smoking: The relationship between stress and smoking had a positive correlation as social support and general characteristics were controlled. In contrast, the relationship between social support -parents', friend's, teacher's support- and smoking had no significant correlations as stress and general characteristics were controlled. The risk factors of smoking of boy's high school students were smoking friends, drinking, a type of school, pocket money per month, and stress. And the most powerful predictor of smoking was smoking friends(OR=12.35). Conclusions: The results showed that the variables affecting mainly on smoking were the personal background -smoking friends, drinking, a type of school, pocket money per month- and stress. Therefore, these findings give useful information for constructing a program against smoking or educating about smoking.

An Exploratory Study on Decisions by foster Parents to care for Children with Special Needs : The Case of 'H' Foster Agency (특수욕구를 가진 아동에 대한 일반위탁부모의 위탁의사: 'H위탁보호기관'의 사례를 중심으로 한 탐색적 연구)

  • It is generally blown that foster children often have special needs such as disability and psycho-behavioral problems. It is also known that foster parents will provide better care with those children when they are coping well with foster children with special needs. The purpose of the study was to explore the perceptions foster parents have and what factors affect their decisions to care for children with special needs. Literature review reveals that successful foster parent care is related to the foster parents' motivation, role perception, and social support systems. foster parents' motivation is related to 'altruism toward foster child', 'the egocentrism of the foster parent', 'previous experience with adoption' and 'the past experiences of the foster parent'. Role perceptions of the foster parent consist of 'supporting the internal system of the foster child' and 'supporting the external system of the foster child'. The social support system is characterized as the limited notions of a family oriented concept. In addition, a foster parent's decision to look after special-needs children is related to their motivation while the decision to foster disabled children is related to the levels of available social support systems as well as motivation. The results of the study implies that the motivation level and that of social support system are closely related to different aspects of the special-needs foster child; this factor should also be considered when recruiting foster parents or placing special needs children.

A Study about Appraise on Acoustic Performance of small-scaled Multi-Purpose Hall, using Auralization (가청화를 이용한 소규모 다목적 홀의 음향 성능평가에 관한 연구)

  • In recent days, on account of the cultural development and the improvement of citizen's consciousness level, it is the real situation that the construction of small-scaled Multi-Purpose Hall where various cultural events could be performed, is on increasing. However, since the most of Multi-Purpose Hall had been designed and built up without any consideration on Acoustic Factor, many problems are on occurring thereat. Since those small-scaled Multi-Purpose Hall have been mostly used with the finishing material which contains a high degree of Acoustic Absorption indiscreetly, both diffusion and reflection of sound are not establishing properly, and because thereverberation of sound is very low, in case of musical performance by musical instrument, its sound hears too arid and stiff, there occurs some acoustic defect such as it becomes difficult for music appreciation with sufficient timbre, so that the capability improvement on the matter is urgently requiring situation. Therefore, this Study has tried to seize the satisfaction level about the small-scaled Multi-Purpose Hall after betterment of the acoustic performance by appraise the acoustic condition of the Hall, using Auralization Technique that can experience Virtual Acoustic Field regarding to the small-scaled Multi-Purpose Hall as its object. It is deemed that such research result could be practically used as the useful material which enables to bring a reduction effect of construction cost as well asenhancement of the acoustic performance through its presupposition?control on the acoustic problem when construction or renovation of other similar small-scaled Multi-Purpose Hall in the future.

Tonality Design for Sound Quality Evaluation in Printer (프린터 음질평가를 위한 순음도 설계)

  • The operating sound radiated from a laser printer includes tonal noise components caused by the rotating mechanical parts such as gear, shaft, motor, fan, etc. The negative effects of the tonal noise components need to be considered in the process of developing a sound quality index for the quantitative evaluation of the emotional satisfaction in terms of psycho-acoustics. However, in a previous paper, it was confirmed that the Aures tonality did not have enough correlation with the results of jury evaluation. The sound quality index based on loudness, articulation index, fluctuation strength has a little problem in considering the effects of rotating mechanical parts on the sound quality. In this paper, to solve the tonality evaluation problem, the calculation algorithm of Aures tonality was investigated in detail to find the cause of decreasing the correlation. The new tonality evaluation model was proposed by modifying and optimizing the masking effect, loudness ratio, and shape of weighting curve based on the basic algorithm of Aures tonality, and applied to two kinds of operating sound groups in order to verify the usefulness of proposed model. As a result, it is confirmed that the proposed tonality evaluation model has enough correlation and usefulness for expressing the tonalness in the operating sounds of laser printers. In the following paper, this results will be used to model the sound quality index as the input data by using the classification algorithm.

Characteristics of Women Who Have Had Cosmetic Breast Implants That Could Be Associated with Increased Suicide Risk: A Systematic Review, Proposing a Suicide Prevention Model

  • Literature indicates an increased risk of suicide among women who have had cosmetic breast implants. An explanatory model for this association has not been established. Some studies conclude that women with cosmetic breast implants demonstrate some characteristics that are associated with increased suicide risk while others support that the breast augmentation protects from suicide. A systematic review including data collection from January 1961 up to February 2014 was conducted. The results were incorporated to pre-existing suicide risk models of the general population. A modified suicide risk model was created for the female cosmetic augmentation mammaplasty candidate. A 2-3 times increased suicide risk among women that undergo cosmetic breast augmentation has been identified. Breast augmentation patients show some characteristics that are associated with increased suicide risk. The majority of women reported high postoperative satisfaction. Recent research indicates that the Autoimmune syndrome induced by adjuvants and fibromyalgia syndrome are associated with silicone implantation. A thorough surgical, medical and psycho-social (psychiatric, family, reproductive, and occupational) history should be included in the preoperative assessment of women seeking to undergo cosmetic breast augmentation. Breast augmentation surgery can stimulate a systematic stress response and increase the risk of suicide. Each risk factor of suicide has poor predictive value when considered independently and can result in prediction errors. A clinical management model has been proposed considering the overlapping risk factors of women that undergo cosmetic breast augmentation with suicide.

The Drinking Experience of Full-time Middle-aged House Wives (중년전업주부의 음주경험)

  • There are few research on the social drinking experience reported by women. Thus, it is necessary to understand the increasing tendency of womens drinking experience in the sociocultural context. This study was aimed to explore and delineate the drinking experience of full-time house wives base don Strauss and Corbin's grounded theory (1998). The subjects of this study were 10 full-time middle-aged house wives who were living in Seoul region. The data were collected through several in-depth interviews and observations until the theoretical saturation was achieved. The data were analyzed using Strauss and Corbin's grounded theory method. The results of this study generated a paradigm that the drinking experience of the full-time middle-aged house wives has a core category of 'free-oneself-from' and is also connected with mingling, anguish, accessibility, reaction of other people, change, personal resources, self-expression and building relationships by drinking. All processes from the occurrence of the central phenomenon to extinction are as follows; 1) Starting the process of the drinking experience for mingling and resolving anguish, and by having an easy accessibility; 2) Perceiving the process of reaction by other people, physical change of herself. and psycho-behavioral change; 3) Experiencing the process of free-oneself from thorough drinking; 4) Acknowledging the process of the resources for free-oneself-from; 5) Selecting the process of self-expression according to the resources; 6) Building the process of the relationship with drinking as a way of self-expression. Based on this paradigm, 4 hypothesis and 4 types are generated. Thus, this study suggests that it is useful to develop nursing interventions to improve the self-esteem of the full-time middle-aged house wives and to provide them various methods of problem solving strategies. This study also proposes that there should be a strategic program for the women so that they could make a route for positive self-expression by achieving a proper relationship with drinking.

Analysis of Nursing Intervention Studies on Patients with Breast Cancer in Korea (유방암환자 대상 국내 간호중재 연구 분석)

  • Purpose: This study was performed to systematically review the recently published nursing intervention studies. Methods: The literature was identified through the Korean Education and Research Information Service (KERIS), the Korean Information Service System (KISS), and National Assembly Library websites. Key words such as breast cancer, nursing, and intervention were used. The factors analyzed are as follows: 1) the characteristics of studies and study populations, 2) the classification of interventions, 3) outcome indicators and their effects, and 4) effective interventions. Results: Thirty two studies were included. Seventeen studies used a single intervention such as aerobic dance, TaiChi, foot massage, aromatherapy, or a stress-reduction method. Fifteen studies used combined interventions, including education, exercise, counseling, support, yoga or meditation. The data on 47 outcome indicators and their effects were segregated into psycho/spiritual outcomes, stress coping, physical outcomes, cardiorespiratory function, symptom management, arm and shoulder functions, fatigue, and quality of life. Some interventions had positive effects on stress, fatigue, and functions of shoulder. Conclusion: Various interventions are available for breast cancer patients, and some have had positive effects. However, more studies are required to develop evidence-based practice guidelines for nursing interventions.
