어린 알라스카 말라무트의 대퇴골에 발생한 골육종 (Juvenile osteosarcoma in the femur of an Alaskan malamute)

  • An 18-month-old male Alaskan malamute was euthanized due to slowly growing mass around the left femur after history of left posterior limb lameness. At necropsy, a firm confluent mass approximately 15cm in diameter was noted surrounding the femur and proximal portion of the tibia/ulna with traumatic ulceration of the overlying skin. On cross sections of the mass, many unencapsulated firm tan coalescing lobules were noted. The neoplasm consisted of closely packed spindle cells with homogenous eosinophilic material (osteoids) between the neoplastic cells. The pulmonary metastasis was confirmed. Based on the gross and histopathologic examinations, the case was diagnosed as juvenile osteosarcoma.

소아의 대퇴골 원위부 골단에 발생한 혈관종 (Juvenile Hemangioma Occurred in Distal Femoral Epiphysis)

  • 골성 골단에서 발생하는 혈관종은 극히 드물어서 그 보고를 찾아보기가 쉽지 않다. 우측 슬관절부 동통과 굴곡 구축을 주소로 내원한 5세 환아로 단순 방사선 및 자기공명영상 소견상, 대퇴골 원위부 골단 및 근위부 경골에서 병변을 보였으며, 생검한 결과 혈관종으로 진단되었다. 8년간 추시 관찰하였으며, 대퇴골 원위부 골단에 발생한 혈관종은 치유되었으며, 경골 근위부 골단에 발생한 병변도 치료없이 자연적으로 소실되었다. 또한, 하지 길이의 단축이나 슬관절 기능 장애는 발생하지 않았다.

  • In cementless total hip replacement(THR), an initial stability of the femoral component is important to long term fixation of femoral stem. The intial stability has close relationship with the relative displacement of prosthessis and sponge bone at the proximal of femur. After implantation of the proshesis, the surrounding bone is partially shielded from load carrying and starts to resorb. Stress shielding is the cause of the loss of proximal bone. Assessing stress distribution of femur is important to predict stress shielding. The initial stability and the stress shielding were investigated for two loading conditions approximating a single leg stance and a stair climbing. Three types of stems were studied by the finite element method to analyze the biomechanical effects of distal filling of cementless femoral stems. Three types of stems empolyed are a distal filling stem, a distal flexible stem, and a distal tapered stem.

  • Using FEM, we analyzed the stress changes on early osteonecrosis of femoral head as the necrotic area increases. A 3D FEM model of proximal femur was made from the CT scam data of fresh frozen adult proximal femur with the material properties from literature. The model consisted of 5994 elements and 5275 nodes. FE analyses were peformed using MSC/NASTRAN. At normal states, stress transmission is mainly along the primary compression trabeculae(PCT). Until 60% involvement of PCT with necrotic lesion, stress transmission is still along the remained PCT. When the PCT involvement is from 60% to 85%, stress transmission is along the PCT and margin of the necrotic lesion. When involvement is more than 85%, stress is mostly transferred to posterior part of femoral head.

고관절을 이루는 골격의 골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구 (Effect of Korean Medicine Treatments for Pain Reduction in Patients with Hip Fracture : A Retrospective Observational Study)

  • The aim of this study was to observe the effectiveness of Korean Medicine treatments on pain reduction of patients who were diagnosed hip fracture - Acetabulum and Proximal Femur. We reviewed the 17 patients' records who had admitted to Daejeon Korean Medical hospital and diagnosed fracture of acetabulum or proximal femur bones from January 1, 2011 to April 30, 2020 during the admission retrospectively. Statistical analysis was done using the IBM SPSS statistics 25 program. We measured Numeric Rating Scale to evaluate the treatment's efficacy. Korean Medicine treatments reduced median of Numeric Rating Scale reduced statistically significantly from 6.00 to 3.00(p<0.05). During the admission, patients didn't experience the side effects of Korean Medicine treatments. In conclusion, these results implied that Korean medicine treatments showed a positive effect on pain reduction of hip fracture patients. Further research is required to confirm the effectiveness of Korean Medicine treatments.

  • 대퇴골두 골단분리증은 청소년기에 발생하는 드문 근위대퇴골 질환이다. 그 병인은 완전히 밝혀지지는 않았으나 대퇴골두 골단의 역학적인 약화의 결과로 생각된다. 이는 역학적, 내분비학적, 면역학적 이상 때문에 발생하는 것으로 보인다. 내분비계 질환과 연관된 대퇴골두 골단분리증의 국내 문헌 보고는 거의 없으며, 특히 20세 이상에서 발생한 예는 찾아보기 힘들다. 저자들은 20세 이상의 성인에서 뇌하수체 기능 저하증과 관련된 대퇴골두 골단분리증의 몇 가지 증례를 경험하였기에 이에 대해 보고하는 바이다.

  • The indications for total hip replacement are increasing and not limited to osteoarthritis. Total hip replacement may also be done for trauma and pathological fractures in patients otherwise physiologically fit and active. This trend has led to an inevitable rise in complications such as periprosthetic femoral fracture. Periprosthetic femoral fracture can be challenging due to poor bone quality, osteoporosis, and stress fractures. We present a case of periprosthetic femoral fracture in a 71-year-old woman with some components of an atypical femoral fracture. The fracture was internally fixed but was subsequently complicated by infection, implant failure needing revision, and later stress fracture. She was on a bisphosphonate after her index total hip replacement surgery for an impending pathological left proximal femur fracture, and this may have caused the later stress fracture. Unfortunately, she then experienced implant breakage (nonunion), which was treated with a biplanar locking plate and bone grafting. The patient finally regained her premorbid mobility 13 months after the last surgery and progressed satisfactorily towards bony union.

  • Osteochondroma is the most common benign bone tumor which commonly occurs in the metaphysis of the long bones such as proximal humerus, tibia, and distal femur. It is rarely found in bones of the foot. Although they are benign lesions, when they occur in the foot, they are typically identified earlier than other regions because of the low proliferation of subcutaneous tissue in the region and may cause symptoms. We experienced a rare case of osteochondroma in a 60-year-old male which cause pain and swelling of the fourth toe.

  • The femoral fracture is the most serious problem of old ages haying osteoporotic fractures. First of all, prevention to reduce the incidence of hip fracture and to identify the risk factor is essential subject. The purpose of this study is to investigate which geometric parameters of proximal femur are related to the hip fracture risk in old ages. Author analyzed the bone density and bone content of over 60 years old women who had suffered hip fracture (n=60) and non fracture groups (n=60). Author concluded that geometric measurements of proximal femoral part made on dual energy x-ray absorptiometry can predict hip fracture independently of bone mineral density.
