2차원 Side Plate FEM을 이용한 인공고관절 골흡수 연구 (A Study on the Bone Resorption of Artificial Hip Replacement by Two-Dimensional FEM)

  • Clinically, proximal bone resorption in the femur is frequently seen postoperatively on the follow up X-rays after total hip replacement(THR). We developed the finite element model of cementless THR. The model is two dimensional side plate model, whereby the three dimensional structural integrity of the bone can be accounted for by a separate two dimensional mesh, a side plate. The subject of this article is the development and application of this two dimensional side plate FEM to study the reverse effect of the various degree of bone resorption of femur after THR. The results of this study indicates that two dimensional side plate model is good and simple alternative to complex three dimensional model and the severity of the proximal bone resorption has the effect of more increasing stress on the cortex at the level of femoral stem tip.

  • Ipsilateral fractures of proximal femur with shaft and condylar region are very rare. Current concept of management is based on fixation of each fracture as independent entity using separate fixation modalities for proximal and distal parts of femur. However, we considered that antegrade femoral nailing with cephalomedullary screw fixation is a good option for ipsilateral multi-level femoral fractures. Here, we present an experience of satisfactory treatment for ipsilateral femoral neck fracture, subtrochanteric fracture, comminuted shaft fracture with supracondylar fracture following road traffic accident.

이중에너지 X-ray 흡수법을 적용한 근위 대퇴골 및 요추부 골밀도 검사의 상관관계 분석 (The Correlation Analysis of BMD in Proximal Femur and Spine with Dual Energy X-ray Absorptiometry)

  • 본 논문에서는 외래환자의 골밀도 검사를 통해 대퇴골 및 요추부에서 정확한 검사를 얻을 수 없을 경우 두 부위간의 골밀도 검사 수치간의 상관관계를 밝혀 검사 결과를 유추하는 데 목적이 있다. C대학교 부속병원을 내원한 환자 62명을 그룹1(40세 미만, 21명), 그룹2(40세 이상~60세 미만, 21명), 그룹3(60세 이상, 20명)으로 세 개 그룹으로 분류하였다. 이들의 T-score와 Z-score를 조사하여 평균값과 상관관계를 평가하였다. 두 부위의 검사 수치를 이용하여 대응표본 T-검정을 통해 상관관계를 분석하였다. 통계 분석 통해 다음과 같은 결과를 얻었다. 첫째, 연령이 높아질수록 T-score가 낮아지고 있고 60세이상 고령자의 경우 골 감소증이 발생하였다. 둘째, 세 그룹으로 비교를 통해 골밀도 검사에서 대퇴골과 요추부 두 부분에 대한 Z-score와 T-score의 상관관계가 높았으며 골밀도값의 결과는 매우 높은 상관관계를 가지고 있었다. 본 연구는 상관관계를 통해 어느 한 부위의 결과로서 다른 부위의 결과를 유추할 수 있는 임상적 유용성이 있을 것으로 생각되며, 이는 골다공증 환자의 처치 및 예후를 결정할 수 있는 보조적인 인자로 활용될 것으로 기대된다.

연령과 골소주 특성의 골량에 대한 연관관계 (Relationship of bony trabecular characteristics and age to bone mass)

  • Purpose : Bony strength is dependent on bone mass and bony structure. So this study was designed to investigate the relationship between the bone mass and bony trabecular characteristics. Subjects and Methods : Study subjects were 51 females (average age 68.6 years) and 20 males (average age 66.4 years). Bone mineral density (BMD, $grams/cm^2$) of proximal femur was measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized radiographs of proximal femur. A customized computer program processed morphologic operations (MO) of ROIs. 44 skeletal variables of MO were calculated from ROIs on the Ward's triangle and greater trochanter of femur. WHO BMD classes were predicted by MO variables of the same ROI. Classification and Regression Tree analysis was used for calculating weighted kappa values, sensitivity and specificity of MO. Results : The discriminating factors of morphologic operation were branch point, branch point [per cm sq]. Age also played important role in distinguishing osteoporotic classes. The sensitivity of MO at Ward's triangle and Greater Trochanter was 91.8%, 65.6%, respectively. The specificity of MO was 100% at Ward's triangle and Greater Trochanter. Conclusion : Bony trabecular characteristics obtained using radiological bone morphometric analysis seem to be related to bone mass.

한우 대퇴골의 연골내골화에 관한 조직화학적 연구 (Histochemical study on the endochondral ossification of the native Korean cattle femur)

  • The present study was undertaken to provide basic data on fetal endochondral ossification for the native Korean cattle femur. This study was determined to the both earliest stages of chondrification and ossification by histochemical methods. The forty-five pairs of femur, from a series of embryos and fetuses ranging from 11 to 100mm in crown-rump(C-R) length, were used. These samples were divided into 9 groups. The groupings were based on C-R length, as the first grouping being 11~20mm, the second grouping being 21~30mm and so on. The results were as follows: 1. Alcianophility and PAS reaction were markedly increased in the perichondrium and interterritorial matrix in the 3rd group(C-R length 31~40mm). These reactions were decreased in the territorial matrix and in the adjacent area to the cartilage canal in the 5th group(C-R length 51~60mm). 2. Calcium deposits and collagen substances were observed initially in the 3rd group(C-R length 31~40mm). The calcium deposits and calcified cores were observed in the trabeculae of the proximal epiphyseal plate of the femur in the 8th group(C-R length 81~90mm).

혈관부착 생비골 중첩 이식술 (Free Vascularized Fibular Transfer with Double Barrel Fashion)

  • Free vascularized fibular is the most usuful bony donor of the long bone reconstruction in reconstructive microsurgical field. It has many benifits such as very strong strut tubular bone, very reliable vascular anatomy with large vascular diameter with long pedicle, minimal donor site morbity too. In that situations of the huge long bone defects in distal femur or proximal tibia, the defective bony shape and strength of the transplanted fibular bone is not enough if only one strut of the fibula is transfered. The bony circulation of the fibula has two ways, one from nutrient artery via peroneal artery through nutrient foramen which makes endosteal arterial network inside of the fibula, another way is periosteal network through outside encircling vascular network of the bone which distributed in muscle sleeves of the fibular diaphysis. Authors modified free vascularized fibular bone graft with transverse osteotomy is made from the anterolateral aspect of the fibular shaft just distal to entry of the nutrient artery. This produces two vascularized bone struts that may be folded pararell to each other but that remain connected by the periosteum and muscle cuff surrounding the peroneal artery and veins. The proximal strut is vascularized by both a periosteal and endosteal blood supply, whereas the distal strut is vascularized by a periosteal blood supply alone. This procedure can call "doule barrel" free vascularized fibular graft. We performed 7 cases of doule barrel fashined fibular transplantation on distal femur and proximal tibial large defects. Average bone union time takes 7 months from that procedure. There were no significant bone union time differences between both proximal and distal struts. After solid union of the transfered double barrel fibular graft, there were no stress fracture in our series. We can propose double barrel free vascualized fibular graft is usuful method in that cases with very large bone defect on large long bones especially metaphyseal defects.

노년기 남성의 칼슘 영양 상태, 육체적 활동량과 골격 대사에 관한 연구 (Dietary calcium intake, physical activity, and bone mineral density in elderly men)

  • The relationships between nutrients intake, physical activity and bone mineral density were investigated in 19 elderly men aged 71-80 years. A trained nutritionist interviewed usual dietary intake and daily activity with a questionnaire, and bone mineral density was measured at the lumbar spine and three regions of the proximal femur (femur neck, Ward's triangle and trochanter) with a Luna DP3 dual photon absorptiometry. The correlations between dietary calcium intake and bone mineral density at the lumbar spine and trochanteric region were significant at P<0.05 and P<0.01 level respectively. the significant correlations were also found between vitamin A(P<0.005), riboflavin(P<0.01), and ascorbic acid(P<0.05) intake and bone mineral density at these sites. Higher physical activity was associated with greater bone mineral density of four sites, but this was not significant. But there were significant relationships between total energy expenditure and bone mineral density of the lumbar spine(P<0.01), femur neck (P<0.05) and Ward's triangle(P<0.05). In this study the results revealed that bone mineral density of the lumbar spine and trochanteric region were associated with dietary calcium intake. And bone mineral density of the femur neck and Ward's triangle were related to physical activity but not to nutrients intake. In conclusion, dietary calcium intake seems to be a important factor for greater bone mineral density. Further evidence will be needed that physical activity protects against bone fracture and osteoporosis in the edlerly.

어린 알라스카 말라무트의 대퇴골에 발생한 골육종 (Juvenile osteosarcoma in the femur of an Alaskan malamute)

  • An 18-month-old male Alaskan malamute was euthanized due to slowly growing mass around the left femur after history of left posterior limb lameness. At necropsy, a firm confluent mass approximately 15cm in diameter was noted surrounding the femur and proximal portion of the tibia/ulna with traumatic ulceration of the overlying skin. On cross sections of the mass, many unencapsulated firm tan coalescing lobules were noted. The neoplasm consisted of closely packed spindle cells with homogenous eosinophilic material (osteoids) between the neoplastic cells. The pulmonary metastasis was confirmed. Based on the gross and histopathologic examinations, the case was diagnosed as juvenile osteosarcoma.
