A Comparison with Laser Needle, Conventional TENS, and Acupuncture-like TENS upon Pain and Blood Flow in Healthy People

  • Objective: Transcutaneous electrical nerve stimulation (TENS) is a treatment method for pain, and it can be divided into conventional TENS (C-TENS) and acupuncture-like TENS (A-TENS). More recently, high power lasers have increasingly been used to reduce pain caused by arthritis, residual neuralgia, and musculoskeletal disorders. The aim of this study was to compare the laser needle with C-TENS and A-TENS in terms of pain and blood flow in healthy people, as well as to confirm that the laser needle can replace TENS to treat pain. Design: A randomized controlled trial. Methods: The selected participants were divided using Minimize computer software into a laser group (n=13), a C-TENS group (n=13), and an A-TENS group (n=14); they underwent a pre-test for blood flow and pain in their forearm. The three groups received their respective interventions; they then underwent a second pain and blood flow test on the same spot. Results: No significant differences were observed in the A-TENS group between the pre- and post-tests, and a comparison among the three groups revealed no significant differences between the laser needle group and the C-TENS group in terms of pain. Regarding blood flow, no significant differences were found between the pre- and post-tests in the laser needle group; a comparison among the three groups only revealed a significant between the laser needle and A-TENS groups. Conclusions: This study confirmed that the laser needle can be used to treat pain when it is necessary to control blood flow.

A Study on Sakae Miki's Experience during Japanese Colonial Period with a Focus on His Activities in Colonial Korea (일제강점기 미키 사카에(三木榮)의 경력에 대한 고찰 - 조선에서의 활동을 중심으로 -)

  • Sakae Miki, a Japanese medical historian, was a leading figure in the study of medical history in the Korean Peninsula due to his three representative books, Bibliography of Korean medicine, ancient and mediaeval(朝鮮醫書誌), The History of Korean medicine and of diseases in Korea(朝鮮醫學史及疾病史), and A Chronological table of Korean medicine(朝鮮醫事年表). After graduating from Kyushu Imperial University's Faculty of Medicine in 1927, he moved to Seoul (then called Keijo) the following year and lived in colonial Korea until 1944. As a doctor and bureaucrat working for the Government-General of Chosen in colonial Korea, this study focused on his career activities. It was in 1928 when he entered Keijo Imperial University's Faculty of Medicine where he participated in experimental research under the supervision of Professor Shinosaki, who was the chief of the third Institute of Medicine. It was discovered that he received his doctorate in medicine from Kyushu Imperial University in August 1932. In 1933, he became an assistant professor of Keijo Imperial University and started working at the Keijo Prefectural Resident Hospital until 1935. In August 1935, he was appointed as the director of Suigen Provincial Hospital where he served until 1944. While actively practicing medicine in colonial Korea, he spent his spare time researching Korean medical history, which he used for the basis of his later publications.

The Synergism of Human Lactobacillaceae and Inulin Decrease Hyperglycemia via Regulating the Composition of Gut Microbiota and Metabolic Profiles in db/db Mice

  • This study aimed to evaluate the effects of Limosilactobacillus fermentum and Lactiplantibacillus plantarum isolated from human feces coordinating with inulin on the composition of gut microbiota and metabolic profiles in db/db mice. These supplements were administered to db/db mice for 12 weeks. The results showed that the Lactobacillaceae coordinating with inulin group (LI) exhibited lower fasting blood glucose levels than the model control group (MC). Additionally, LI was found to enhance colon tissue and increase the levels of short-chain fatty acids. 16S rRNA sequencing revealed that the abundance of Corynebacterium and Proteus, which were significantly increased in the MC group compared with NC group, were significantly decreased by the treatment of LI that also restored the key genera of the Lachnospiraceae_NK4A136_group, Lachnoclostridium, Ruminococcus_gnavus_group, Desulfovibrio, and Lachnospiraceae_UCG-006. Untargeted metabolomics analysis showed that lotaustralin, 5-hydroxyindoleacetic acid, and 13(S)-HpODE were increased while L-phenylalanine and L-tryptophan were decreased in the MC group compared with the NC group. However, the intervention of LI reversed the levels of these metabolites in the intestine. Correlation analysis revealed that Lachnoclostridium and Ruminococcus_gnavus_group were negatively correlated with 5-hydroxyindoleacetic acid and 13(S)-HpODE, but positively correlated with L-tryptophan. 13(S)-HpODE was involved in the "linoleic acid metabolism". L-tryptophan and 5-hydroxyindoleacetic acid were involved in "tryptophan metabolism" and "serotonergic synapse". These findings suggest that LI may alleviate type 2 diabetes symptoms by modulating the abundance of Ruminococcus_gnavus_group and Lachnoclostridium to regulate the pathways of "linoleic acid metabolism", "serotonergic synapse", and" tryptophan metabolism". Our results provide new insights into prevention and treatment of type 2 diabetes.

Education Need of the Visit ing Health Service Workers in Gwangju and Jeollanam-do Public Health Facilities (일부 공공보건기관 방문보건요원의 교육요구도 조사)

  • This study was aimed to provide the basic data for the development of effective educational program by reflecting the opinions of the visiting health service workers. The subjects were 144 visiting health service workers in Gwangju and Jeollanam-do area who responded the mail questionnaire. The data were collected from June to July, 2001 using questionnaire composed of the education need, knowledge by subjective appraisal, and experience of education. The major findings of this study were as follows: 1. The number of respondents who received at least one education within recent three years were 43(29.9%) at the central level, 57(39.6%) at the provincial level and 53(36.8%) at the district level. The satisfaction with education was higher at the central level than at the provincial and district level. 2. Knowledges by subjective appraisal on the 'chronic degenerative diseases management' and 'health promotion' was relatively high. while that of 'rehabilitation' was low. 3. The knowledge by subjective appraisal of visiting health service was related with experience of education and license status. The knowledge was higher in registered nurses than in nurse aids. The curricula related to increased level of knowledge of visting health service workers were 'elderly health care', 'rehabilitation' and 'psychiatric-mental health nursing' educations at the central level 'continuing education for the community health practioners' and 'psychiatric-mental health education' at the provincial level and 'elderly health care', 'rehabilitation' 'psychiatric-mental health' and 'acute diseases control' educations at the district level. 4. The respondents preferred elderly health management as the contents of education, officer group education as the method of education, province(30.4%) as the main body of education, exercise and practice as the form of education, 2-3 times per year as the frequency of education, and 3-5days as the period of education. The findings of this study could suggest that future education program should be planned to increae the knowledge level of visiting health service workers by reflecting their educational need.

Influential Factors on Turnover Intention of Nurses in Long-term Care Hospitals (노인요양병원 간호사의 이직의도에 영향을 미치는 요인)

  • Objectives : The purpose of this study was to identify levels of turnover intention of nurses in long-term care hospitals, and to explore influential factors on turnover intention. Methods : Data were collected with a structured questionnaires from 165 nurses. The data were analyzed with SPSS/WIN 21.0. Results : First, the average score for the practice environment cognition, job satisfaction, reward importance, and turnover intention were $3.14{\pm}0.21$, $3.18{\pm}0.32$, $4.02{\pm}0.53$, and $3.29{\pm}0.67$, respectively. Second, there were significant differences in the turnover intention according to the average monthly wage, total clinical career, present clinical career, work form, average monthly night shift and turnover experience. Third, the significant predictors of turnover intention were monthly salary, practice environment cognition, reward importance, monthly night shift and type of work explaining 67.0%. of the variance. Conclusions : It is necessary to conduct continuous and systematic research and to find ways that can prevent the resignation of nurses and improve cognition in the practice environment in long-term hospitals nurses.

A Study on Work Stress Perceived by Clinical Nurses (간호사의 업무 스트레스에 대한 연구 -정신과 간호사 중심-)

  • This study was designed to measure the degree of work stress perceived by clinical nurses working in psyciatric ward, and to identify the factors influencing the difference of work stress. The survey method was used and the subjects of this study consisted of 135 psyciatric nurses from 7 university hospitals and 4 provincial general hospitals. The data were collected from August 1 to 20, 1988. The instrument used for this study was PNOSS(Psychiatric Nurse Occupational Stress Scale) which was developed by Bai(1989). The confidence verification of this instrument was computed and the Cronbach $\alpha$ was 0.94050. Data were analysed by t-test and ANOVA on 5% significant level with SPSS program. The results of study were summerized as follow : 1) The degree of perceived stress among psychiatric nurses was considerably high(4.32 out of 6.00). 2) Among the stressors, inadequate staffing(5.04), hospital administrational problems (4.7) and the conflict of nurse-patient relationship were identified as the stress factors with high rank of degree of stress. 3) There was significant relationship between the degree of stress and the demographic variables of nurses such as the marital status, educational level, and the motivation of working in psychiatric ward. 4) There was significant relationship between the job-related variables such as the working duration, the motivation of working and nurse's attitude toward the psychiatric patients. 5) The relationship between several stress factors and some of the demographic and the job-related variables were significantly identified.

Simultaneous Determination of Glutamate, Glycine, and Alanine in Human Plasma Using Precolumn Derivatization with 6-Aminoquinolyl-N-hydroxysuccinimidyl Carbamate and High- Performance Liquid Chromatography

  • A simple, sensitive and reproducible high-performance liquid chromatography (HPLC) method has been validated for determining concentrations of glutamate, glycine, and alanine in human plasma. Proteins in plasma were precipitated with perchloric acid, followed by derivatization with 6-aminoquinolyl-N-hydroxysuccinimidyl carbamate (AQC). Simultaneous analysis of glutamate, glycine, and alanine is achieved using reversed-phase HPLC conditions and ultraviolet detection. Excellent linearity was observed for these three amino acids over their concentration ranges with correlation coefficients (r)>0.999. The intra- and inter-day precision were below 10%. This method utilizes quality control samples and demonstrates excellent plasma recovery and accuracy. The developed method has been successfully applied to measure plasma glutamate, glycine, and alanine in twenty volunteers.

A Study of the Perception Gap on Nursing Service between Consumers and Providers (간호서비스 질에 대한 소비자와 제공자의 지각차이에 관한 분석)

  • this study was to investigate the perception gap on nursing service between consumers and providers. Method: the questionnaire founded on the SERVQUAL was developed and distributed to 300 patients and 210 nurses at the three subjected general hospitals in three provincial city, Korea during February to March, 2001. For data analysis, Cronbachs' Alpha, frequency, t-test, and paired t-test were used. Result: 1. In the gap analysis on the 20 properties constituting nursing service, providers almost all perceived higher than consumer in quality. Among them, the number of properties being statistically significant was 7 in the expectation, 18 in the important and 7 in the performance. 2. In the comparison analysis of the perception gap on the expectation-performance and the important-performance, it turned out that the subjected hospitals had to improve their equipment and facilities immediately. It was suggested a good strategy to strengthen the responsiveness factor and the assurance factor of nursing service. Conclusion: it could be concluded that nurses have to recognize the blind spot of their perception and endeavor to take away the perception gap between consumers and providers.

Factors Influencing on Quality of Life in Gynecological Cancer Patients (부인암 환자의 삶의 질 예측요인)

  • Purpose: The purpose of this study was to measure the quality of life (QOL) and to identify the factors influencing QOL in gynecological cancer patients. Methods: The subjects of this study were 242 people who were receiving medical therapy or follow-up after surgery from one general hospital in Daegu. Data were collected from August 1, 2010 to January 31, 2011. A questionnaire including questions on QOL, distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were completed by the subjects. Results: The mean score of QOL was $70.68{\pm}13.40$. Religion, job, presence of spouse, level of education, household income, financial compensation, disease stage and recurrence were the significant factors related to QOL. Distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were also significant factors influencing QOL. Sixty eight percent of the variance in subjective overall QOL can be explained by body image, distress problem, distress score, anxiety, level of education and perceived health status (Cum $R^2$=0.689, F=76.316, $p$ <.001). Body image was the most important factor related to QOL. Conclusion: An integrative care program which includes general, disease-related and psychosocial characteristics of patients is essential to improve QOL in gynecological cancer patients.

Molecular Identification of Diphyllobothrium nihonkaiense from 3 Human Cases in Heilongjiang Province with a Brief Literature Review in China

  • Human diphyllobothriasis is a widespread fish-borne zoonosis caused by the infection with broad tapeworms belonging to the genus Diphyllobothrium. In mainland China, so far 20 human cases of Diphyllobothrium infections have been reported, and the etiologic species were identified as D. latum and D. nihonkaiense based on morphological characteristics or molecular analysis. In the present study, proglottids of diphyllobothriid tapeworms from 3 human cases that occurred in Heilongjiang Province, China were identified as D. nihonkaiense by sequencing mitochondrial cytochrome c oxidase subunit I (cox1) and NADH dehydrogenase subunit 5 (nad5) genes. Two different cox1 gene sequences were obtained. One sequence showed 100% homology with those from humans in Japan. The remaining cox1 gene sequence and 2 different nad5 gene sequences obtained were not described previously, and might reflect endemic genetic characterizations. D. nihonkaiense might also be a major causative species of human diphyllobothriasis in China. Meanwhile, the finding of the first pediatric case of D. nihonkaiense infection in China suggests that infants infected with D. nihonkaiense should not be ignored.
