보호행동전략이 성인의 음주폐해에 끼치는 영향에 관한 연구 (A Study on the Effects of Protective Behavioral Strategies on Alcohol-Related Harms in Adults)

  • 우리나라는 신체적 위해 뿐만 아니라, 사회경제적으로도 규모와 심각도를 더해가고 있는 음주폐해의 감소를 위해 다양한 방안들을 모색하고 있다. 그 중에서도 개인들의 음주행태 개선은 음주폐해를 줄이는 가장 직접적인 방법이다. 외국에서는 '보호행동전략'의 사용이 음주폐해를 줄이는 효과적인 방안으로 알려져 있는데, 우리나라에서는 성인을 대상으로 이를 조사하여 검증한 연구들이 매우 부족하다. 따라서 본 연구는 보호행동전략의 사용이 음주폐해 감소에 유의한 영향을 끼치는지를 확인하고자 하였다. 전국 성인 남녀 2,000명을 대상으로 한 연구 결과, 보호행동전략의 사용은 음주폐해를 줄이는 것으로 나타났으며, 음주폐해에 가장 큰 영향을 끼친 것은 심각한 폐해감소 전략 영역이었고, 다음으로 음주방법 제한 영역이었다. 음주량 감소 영역의 영향은 유의하지 못한 것으로 나타났다. 이 외에도 음주빈도, 폭음, 음주 동기는 음주폐해에 정적인 영향을 끼쳤으며, 사교 동기의 경우만 부적인 영향을 끼쳤다. 본 연구에서는 향후 종단연구를 통한 면밀한 요인들 간의 관계 파악, 대상층을 세분화한 보호행동전략 사용 강화 방안에 관한 후속 연구들을 제안하였다.

Interventions to Reduce the Problems of Abused Children and Adolescents in Residential Facilities in South Korea: An Integrative Review

  • Purpose: The purpose of this study was to review the literature on intervention studies for abused children and adolescent in residential facilities in South Korea (ACARF-K). The goal was to understand the problems they experience, to evaluate the content and effectiveness of interventions applied to them, and to develop evidence-based nursing intervention programs. Methods: We used four electronic databases to search for relevant articles. 18 studies according to Whittemore and Knafl's integrative review method to synthesize the literature. Results: The ACARF-K experienced problems in biophysical, psychological, and sociocultural domains related to attachment impairment. Effective intervention strategies were building trust through empathy and fulfillment of needs, encouraging ACARF-K to express themselves and helping them to clarify emotions in an unthreatening environment, and improving their self-concept through activities in which they experienced achievement. Conclusion: Interventions are needed to help restore attachment damage among ACARF-K. The interventions in this study utilized emotional, cognitive, relational, and behavioral therapeutic tools to improve their psychological and social capacities. Future intervention programs for ACARF-K should include these key elements.

정신과의 분자생물학 적용 (Molecular Application in Psychiatry)

  • The development of molecular biology has brought many changes in psychiatry. Molecular biology makes us possible to know the cause of mental disorders that provide the way to prevent the disorders, and to develop various accurate diagnostic and treatment methods for mental disorders. The author discusses the concept, cause, and treatment of mental disorders in the aspect of molecular biology. Importing the methods of molecular biology into psychiatry, we can anticipate to get a number of the goals of psychiatric genetics, including identification of specific susceptibility genes, clarification of the pathophysiological processes whereby these genes lead to symptoms, establishment of epigenetic factors that interact with these genes to produce disease, validation of nosological boundaries that more closely reflect the actions of these genes, and development of effective preventive and therapeutic interventions based on genetic counseling, gene therapy, and modification of permissive or protective environmental influences. In addition to their capacity to accelerate the discovery of new molecules participating in the nervous system's response to disease or to self-administered drugs, molecular biological strategies can also be used to determine how critical a particular gene product may be in mediating a cellular event with behavioral importance. Molecular biology probably enables us discover the environmental factors of mental disorders and allow rational drug design and gene therapies for mental disorders, by isolation of gene products that facilitate a basic understanding of the pathogenesis of these disorders. A specific genetic linkage may suggest a novel class of drugs that has not yet been tried. With respect to gene therapy, the hypothetical method would use a gene delivery system, most likely a modified virus, to insert a functional copy of a mutant gene into those brain cells that require the gene for normal function.
