집중치료실에서 지속적 뇌파검사의 뇌파 패턴 분석 (Analysis of the Continuous Monitored Electroencephalogram Patterns in Intensive Care Unit)

  • 이 연구의 목적은 중첩성 뇌전증을 발견하고, 처음 기록된 30분 뇌파 패턴을 통하여 경련 가능성을 알아보고자 한다. 국제표준 10~20법을 통하여 전극을 부착하였다. 2014년 1월부터 2015년 12월까지 중환자실에 입원한 경련 의심 환자를 대상으로 하였다. 뇌파의 패턴은 주기적 방전파, 전반적 주기적 방전파, 버스트 억제파, 초점 뇌전증파, 비대칭 배경파, 전반적 서파, 삼상성 형태의 일반화 된 주기적 방전파 등 7 가지 범주로 분류하였다. 원인별 분류는 5가지 범주로 구분하였다. 전체 128명 중 평균 나이는 $56.9{\pm}17.5$였고, 남:여 비율은 74:54명이였다. 평균 뇌파검사 기간은 $5.5{\pm}5.1$일 이었고 최장 33일 이였다. 주기적 방전파(N=7), 전반적 주기적 방전파(N=10), 버스트 억제파(N=6), 초점 뇌전증파(N=19), 비대칭 배경파(N=24), 전반적 서파(N=51), 3상 형태의 일반화 된 주기적 방전파(N=11)이었다. 중첩성 뇌전증 환자의 원인은 원발성 증상성(N=4), 급성원발성 증상성(N=9), 급성 증상성(N=6), 진행성 뇌병증(N=2), 열성경련(N=1)이었다. 지속적 뇌파모니터링 검사는 중첩성 뇌전증을 발견하는데 유용한 검사이고, 뇌파 패턴을 통하여 경련 발생 유무를 확인할 수 있었다.

Relationship Between Amyloid Positivity and Sleep Characteristics in the Elderly With Subjective Cognitive Decline

  • Background and Purpose: Alzheimer's disease (AD) is a neurodegenerative disease characterized by a progressive decline in cognition and performance of daily activities. Recent studies have attempted to establish the relationship between AD and sleep. It is believed that patients with AD pathology show altered sleep characteristics years before clinical symptoms appear. This study evaluated the differences in sleep characteristics between cognitively asymptomatic patients with and without some amyloid burden. Methods: Sleep characteristics of 76 subjects aged 60 years or older who were diagnosed with subjective cognitive decline (SCD) but not mild cognitive impairment (MCI) or AD were measured using Fitbit® Alta HR, a wristwatch-shaped wearable device. Amyloid deposition was evaluated using brain amyloid plaque load (BAPL) and global standardized uptake value ratio (SUVR) from fluorine-18 florbetaben positron emission tomography. Each component of measured sleep characteristics was analyzed for statistically significant differences between the amyloid-positive group and the amyloid-negative group. Results: Of the 76 subjects included in this study, 49 (64.5%) were female. The average age of the subjects was 70.72±6.09 years when the study started. 15 subjects were classified as amyloid-positive based on BAPL. The average global SUVR was 1.598±0.263 in the amyloid-positive group and 1.187±0.100 in the amyloid-negative group. Time spent in slow-wave sleep (SWS) was significantly lower in the amyloid-positive group (39.4±13.1 minutes) than in the amyloid-negative group (49.5±13.1 minutes) (p=0.009). Conclusions: This study showed that SWS is different between the elderly SCD population with and without amyloid positivity. How SWS affects AD pathology requires further research.

해양 실트질 모래의 비배수 동적 거동에 대한 평균 및 반복전단응력의 영향 (Effect of Average and Cyclic Shear Stress on Undrained Cyclic Behavior of Marine Silty Sand)

  • 해상풍력발전기의 기초는 바람, 조류, 그리고 파도 하중을 받기 때문에 해상풍력발전기 기초를 설계하는 데 있어 반복하중을 받는 기초지반의 전단거동 평가가 필요하다. 지반의 비배수 동적 전단거동은 반복하중 횟수, 수직 유효응력, 반복 전단변형률, 상대 밀도, 그리고 평균 및 반복전단응력의 조합에 영향을 받는다. 본 연구에서는 해양 실트질 모래의 비배수 동적 거동에 대한 평균 및 반복전단응력의 영향을 평가하기 위하여 반복단순전단시험(CDSS)을 수행하였으며 상대밀도 85%, 수직 유효응력 200kPa과 300kPa의 시험조건에서 15%의 이중진폭 동적전단변형률(${\gamma}_{cyc}$)과 영구전단변형률(${\gamma}_p$)를 파괴 기준으로 적용하였다. 시험결과는 설계 그래프와 등고선도로 나타내었다. 결과에 따르면 해양 실트질 모래의 비배수 동적 거동은 평균 및 반복전단응력과 두가지 전단응력의 조합에 의해 크게 변하는 경향을 보였다. 평균전단응력이 존재하는 경우에는 반복전단변형보다는 영구변형에 의해 파괴가 결정되는 것으로 나타났다.

초대형 해상구조물의 붕괴거동 및 최종강도 특성 (Characteristics for Progressive Collapse Behavior and Ultimate Strength of Very Large Marine Structure)

  • 초대형 해양 구조물은 매립 방법을 대신한 새로운 해양공간의 이용방법으로서 주목받고 있다. 따라서, 이와 같은 요구에 부합하기 위해서 초대형 해양 구조물이 제안되고 있다. 초대형 해양 구조물은, 매립 공법과 달리, 수심이나 해저의 지질에 관계없이 설치할 수 있고, 또 부체의 아래에 흐름이 존재하기 때문에 자연환경에 영향이 전혀 발생하지 않는다. 또한, 용이하게 조립 해체를 할 수 있기 때문에, 확장이나 철거를 쉽게 할수 있는 장점이 있다. 초대형 부유체 구조물 설계 기준안에 의하면, 구조안정성에 관한 항목 중, 부유체 구조물의 사용환경 및 설치환경에서 발생할 수 있는 최악의 해상조건에 있어 적절한 구조강도 여유를 갖는 것을 쥬정하고 있다. 따라서, 예상 가능한 하중 시나리오에 의해서, 적절한 구조 해석 및 실험을 수행하고, 안전성을 확인하도록 요구하고 있다. 전자에 관해서는 구조부재 레벨의 강도 평가를 수행하고, 후자에 관해서는 구조물의 파괴를 수반한 거동을 확인한다. 지금까지 탄성 응답 해석을 기초로 주요 구조부재의 강도 한계치를 기준으로 한 다양한 검도, 평가가 행해져 왔다. 그렇지만, 부재의 붕괴를 초과한 부하가 작용할 때의 구조 전체로서의 붕괴 거동 및 안전성에 관한 검토는 적다. 따라서, 본 연구에서는 이상 환경 조건하에서 발생 가능한 하중조건에 대해서 대형 해상구조물의 비선형 붕괴 거동을 파악한 것을 목적으로 하고 있다.

형태적 수렴 특성을 이용한 하구 분류 (Classification of Estuaries based on Morphological Convergence)

  • 하구의 분류는 크게 지형경관적 특성에 근거한 정성적인 분류와 수괴의 물리적 특성에 근거한 정량적 분류로 구분될 수 있으나, 전자는 하구의 특성을 정량화하기 어려우며 후자는 자료부족으로 인해 폭넓은 적용이 어렵다. 하구의 형태적 수렴 특성은 자료의 획득이 상대적으로 용이하면서도 하구의 조석전파 특성과 연관시킬 수 있어 하구분류 기준으로 사용하기 적합하다. 본 연구는 국내 19개 하구에 대해 하구의 형태적 수렴특성을 이용하여 하구별 특징을 파악하고 분류를 시도한 것이다. 하구의 수심과 너비자료를 이용하여 수심과 너비의 감소율(${\nu}$)과 무차원화된 하구의 길이($y_0$)를 구하고, 1차원 수심적분 운동방정식과 연속방정식으로부터 유도한 전이함수 ${\xi}({\nu},ky)$${\nu}$$y_0$을 대입하여 하구별 조위/조석 진폭비와 조위/조류 위상차를 도출하였다. 이 결과를 토대로 19개 하구의 특징을 파악하고 3개 그룹을 구분하였다. 국내 19개 하구는 크게 조위 변동이 조류 변동보다 우세한 제1그룹, 하구와 조류 변동이 조위 변동보다 우세한 제2그룹, 그리고 1그룹과 2그룹의 전이 형태를 갖는 제3그룹 하구로 분류된다. 민감도 분석 결과 하구분류 알고리듬은 하구 길이의 변화에 큰 영향을 받는 것으로 나타났다. 하구 길이가 과대 혹은 과소 추정되었을 때 본래의 조위/조류 진폭비는 과소 혹은 과대 추정되며, 본래의 조위/조류 위상차는 과대 혹은 과소 추정된다. 그러므로 형태적 수렴 특성을 이용하여 하구분류를 하고자 할 때는 하구 길이의 적절한 산정이 무엇보다 중요하다. 하구분류 알고리듬은 수심의 변화를 무시하여도 동일한 분류 결과를 주는 것으로 확인되었다. 이는 하구길이의 적절한 산정이 전제될 경우 하구분류 알고리듬이 자료의 제약 없이 대부분의 국내하구에 적용할 수 있음을 의미한다.

만성 폐쇄성 폐질환 환자에서 Tei 지수를 이용한 우심실기능 평가 (Assessment of Right Ventricular Function in Patients with Chronic Obstructive Pulmonary Disease Using Echocardiographic Tei Index)

  • 연구배경 : 만성 폐쇄성 폐질환 기능의 평가를 위해서 여러 가지 비침습적, 침습적 방법이 이용되나 그 정확도나 실용성에 있어서 한계가 있었다. 최근 도플러 심초음파상의 수축기, 이완기 및 박출시간을 조합한 기능지수인 Tei 지수가 우심실기능을 평가하는 유용한 방법으로 알려지고 있어 저자 등은 만성 폐쇄성 폐질환 환자의 Tei 지수를 구하여 우심실 기능을 평가하고 폐기능검사와의 상관관계를 알아보고자 하였다. 대상 및 방법 : 만성 폐쇄성 폐질환 환자 26예와 대조군 10예을 대상으로 간헐파 도플러를 이용하여 도플러 간격을 측정하였다. Tei 지수는 등용성 수축시간(ICT)과 등용성 이완시간(IRT)의 합을 박출시간(ET)으로 나누어 구하였고, PEP, ICT/ET, PEP/ET, IRT/ET를 측정하여 환자군과 대조군을 비교하였다. 폐기능과 Tei 지수의 상관관계를 구하였고, Tei 지수와 다른 도플러 간격의 상관관계를 알아보았다. 결 과 : Tei 지수는 환자군에서 대조군에 비해 유의하게 증가되어 있었고($0.45{\pm}0.17$ vs. $0.27{\pm}0.03$, p<0.01), ICT/ET ($0.32{\pm}0.08$ vs. $0.25{\pm}0.05$, p<0.05), PEP/ET ($0.46{\pm}0.10$ vs. $0.38{\pm}0.06$, p<0.05), IRT/ET ($0.29{\pm}0.16$ vs. $0.15{\pm}0.15$, p&0.05) 등은 대조군보다 증가되어 있었으며 박출시간은 단축되어있었다($255.2{\pm}32.6$ vs. $314.2{\pm}16.5$, p<0.05). 환자군에서 Tei 지수는 1초간 노력성 호기량과 역상관성을 보였고 (r=-0.469), 중증 폐기능 장애가 있는 3기 환자는 1기나 2기 환자에 비해 Tei 지수가 증가되어 있었다. 환자군에서 Tei 지수는 박출시간과 역상관성을 보였고 (r=-0.469), ICT/ET(r=0.453), PEP/ET(r=0.480), IRT(r=0.812), IRT/ET(r=0.896) 는 Tei 지수와 유의한 상관관계가 있었다. 결 론 : Tei 지수는 만성 폐쇄성 폐질환 환자에서 정상 대조군에 비해 유의하게 증가되어 있으며 폐기능 지표와도 유의한 연관성을 보여 만성 폐쇄성 폐질환 환자의 우심실 기능을 평가하는데 유용한 검사법으로 사료된다.

여성의 가사노동에 대한 태도 및 사회적 규범에 대한 여성의 인식이 가사노동시간의 성불평등에 영향을 미치는가?: 합리적 행위이론을 통한 매개효과 분석 (Do Women's Attitude to Domestic Works and Self-perception of Social Norms Enforce the Gender Division of Housework? - Analysis of Mediation Effects Using the Theory of Reasoned Action -)

  • 본 연구는 합리적 행위이론을 활용하여, 사회적 규범을 통해 형성된 여성 자신의 성역할에 대한 인지적 태도가 가사노동에 있어 성불평등적 행동으로 발현되는데 영향을 미치는지 실증적으로 분석하고자 한다. 이에 여성가족패널(Korean Longitudinal Survey of Women and Family) 5차 자료를 활용하여 기혼여성 중 18세 이상 59세 이하 4,435명을 분석대상으로 선정하여 연구하였다. 본 연구에서는 가사노동을 바라보는 여성자신의 태도와 사회적 요구에 의한 주관적규범을 독립변수로 하여 가사노동시간에 미치는 영향을 살펴보는 동시에 이들 변수들 간의 관계에서 합리적 행위이론에서 주장하는 가사노동 전담에 대한 행동의지가 어떻게 작용하는지에대해 알아보기 위해서 구조방정식 모형(Structural Equation Models: SEM)을 통하여 매개효과를 검증하였다. 연구결과 여성 스스로의 가사노동에 대한 태도는 가사노동시간에 직접적인 영향을 주는 동시에 가사노동 전담에 대한 행동의도를 통해 간접적으로 가사노동시간에 영향을미치는 것으로 드러났고, 주관적 규범은 가사노동시간에 직접적으로 영향을 주진 않으나 가사노동전담에 대한 행동의도를 통해 간접적으로 가사노동시간에 영향을 주는 완전매개효과가 나타나는 것으로 밝혀졌다. 본 연구를 통해 성별 간 평등한 가정생활을 보장해주기 위해 마련된일련의 제도를 인지하고 있음에도 가정 내의 성역할 분화가 여전히 나타나는 것은 단순히 제도 운용상의 문제만이 아닌 오랜 가부장적인 문화에 기반한 여성 스스로의 성역할 인식도 어느정도 영향을 미치고 있음을 알 수 있었다. 따라서 사회구성원들로부터 진정한 성평등 행동변화를 이끌어 내기 위해서는 남성이 상대의 성(gender)에 대한 가치를 존중해주는 것도 중요하지만 여성 스스로 자신의 성(gender)을 바라보는 고착화된 인식의 전환을 시도하는 등 다각적차원에서의 통합적 변화를 모색해 나가야 할 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.
