Evaluation of prognosis related to compliance with supportive periodontal treatment in patients with chronic periodontitis: a clinical retrospective study

  • Purpose: The purpose of this study was to evaluate the prognostic effect of patient compliance with supportive periodontal treatment (PC-SPT). Chronic periodontitis patients were classified based on their compliance level, and factors affecting PC-SPT and the prognosis of PC-SPT were investigated. Methods: This study selected 206 patients who started SPT after receiving periodontal treatment between 2010 and 2012. Patients who continued SPT through February 2016 were included. The patients were classified according to whether they exhibited complete compliance (100% of visits), excellent compliance (${\geq}70%$ of visits), incomplete compliance (<70% of visits), or non-compliance (only 2 visits). Patient characteristics that could affect PC-SPT, such as age, sex, distance of the clinic from their residence, implantation, and periodontal treatment, were investigated. The number of newly decayed and extracted teeth, alveolar bone level changes around the teeth and implants, and implant removal were examined to evaluate the prognosis of PC-SPT. Results: Sex and the presence of an implant significantly affected PC-SPT. Additionally, the number of newly decayed and extracted teeth and changes in alveolar bone levels around the teeth and implants were significant prognostic factors related to PC-SPT. Conclusions: PC-SPT in chronic periodontitis patients will help maintain periodontal health and prevent further periodontal disease.

Effect of supportive periodontal therapy on the prevention of tooth loss in Korean adults

  • Purpose: The purpose of this retrospective study was to evaluate the effect of patient compliance with supportive periodontal therapy (SPT) on tooth loss in Korean adults. Methods: The periodontal records of 134 patients were reviewed for this study. They completed active periodontal treatment from 1999 to 2001 and were placed on a schedule of periodic follow-up visits for SPT. Patient compliance was classified into complete compliance (CC), erratic compliance (EC), and noncompliance (NC) groups. Re-examinations were carried out $11.0{{\pm}}0.8$ years after the active periodontal treatment. The prognosis for each tooth was determined as good, questionable, or hopeless according to the bone loss observed in pretreatment radiographs. Results: The rate of tooth loss of the CC group was significantly lower than that of the NC group. The tooth loss/patient and the tooth loss/patient/year were not significantly different between the three groups. The rates of tooth loss in the good, questionable, and hopeless prognosis groups were 6.7%, 9.5%, and 13.2%, respectively. For the teeth with a good prognosis, the rate of tooth loss of the CC group was significantly lower than that of the NC group (0.4% vs. 5.1%). For the teeth with a questionable prognosis, the CC group showed a significantly lower rate of tooth loss than did the EC group (4.1% vs. 30.7%) or the NC group (4.1% vs. 25.6%). For the teeth with a hopeless prognosis, the rates of tooth loss were not significantly different among the three groups. Conclusions: Within the limits of this study, the patients who showed a poor compliance with SPT were more likely to lose teeth than were the regularly compliant patients. However, the risk of tooth loss with a hopeless prognosis was high irrespective of the compliance.

완전탈구 후 재식한 영구 절치의 예후에 대한 후향적 연구 (Prognosis of Replanted Permanent Incisors after Avulsion Injury: A Retrospective Study)

  • 지금까지 완전탈구된 치아의 예후에 대해 많은 연구가 발표되었으나, 국내의 소아와 청소년을 대상으로 한 추적 조사 연구는 거의 없다. 이에 본 연구에서는 6-14세 환자 142명의 재식치 184개를 대상으로 전자의무기록지와 치근단 방사선 사진을 평가함으로써 재식된 영구 절치의 치유와 예후에 영향을 미치는 인자에 대해 분석하였다. 재식치의 치근 발육 단계가 낮을수록 치수의 재혈관화 가능성이 높았다. 구외 시간이 60분 이상인 경우 염증성 치근 흡수의 발생률은 미성숙 치아에서 높게 나타났으며, 대치성 치근 흡수의 발생률은 성숙 치아에서 높게 나타났다. 20일 이상 경과하여 발수한 경우 염증성 치근 흡수의 발생률이 증가하였으며, 대치성 치근 흡수에 비해 염증성 치근 흡수가 생존 기간의 감소에 더 큰 영향을 미치는 것으로 나타났다. 치근의 발육 단계와 재식치의 생존 기간 간에 상관관계는 없었으나, 대치성 치근 흡수가 발생한 경우 미성숙 치아의 생존 기간은 성숙 치아에 비해 짧았다. 본 연구는 장기간의 추적 조사를 통해 수행한 연구로서, 완전탈구된 치아의 치료 지침과 예후 평가에 대한 기초 자료로 활용될 수 있으리라 생각된다.

Does apical root resection in endodontic microsurgery jeopardize the prosthodontic prognosis?

  • Apical surgery cuts off the apical root and the crown-to-root ratio becomes unfavorable. Crown-to-root ratio has been applied to periodontally compromised teeth. Apical root resection is a different matter from periodontal bone loss. The purpose of this paper is to review the validity of crown-to-root ratio in the apically resected teeth. Most roots have conical shape and the root surface area of coronal part is wider than apical part of the same length. Therefore loss of alveolar bone support from apical resection is much less than its linear length.The maximum stress from mastication concentrates on the cervical area and the minimum stress was found on the apical 1/3 area. Therefore apical root resection is not so harmful as periodontal bone loss. Osteotomy for apical resection reduces longitudinal width of the buccal bone and increases the risk of endo-perio communication which leads to failure. Endodontic microsurgery is able to realize 0 degree or shallow bevel and precise length of root resection, and minimize the longitudinal width of osteotomy. The crown-to-root ratio is not valid in evaluating the prosthodontic prognosis of the apically resected teeth. Accurate execution of endodontic microsurgery to preserve the buccal bone is essential to avoid endo-perio communication.

Assessing the efficacy of apicoectomy without retrograde filling in treating periapical inflammatory cysts

  • Objectives: The necessity of retrograde filling after apicoectomy is controversial in cases of non-inflammatory cysts as opposed to bacteria-related periapical abscesses. This study aims to investigate whether the presence or absence of retrograde filling during apicoectomy has differential long-term prognostic implications between inflammatory and non-inflammatory cysts. Materials and Methods: This retrospective study included patients who underwent tooth apicoectomy during jaw cyst enucleation between 2013 and 2022, and who underwent follow-up cone-beam computed tomography for at least 6 months. The prognosis of the tooth was evaluated during the follow-up period according to the cyst type, the presence or absence of retrograde filling, mandible or maxilla, and location. Results: A total of 147 teeth was included in this study. All the operated teeth underwent preoperative root canal treatment by an endodontic specialist. Apicoectomy was performed for 119 inflammatory cysts and 28 non-inflammatory cysts. Retrograde filling was performed on 22 teeth with inflammatory cysts and 3 teeth with non-inflammatory cysts. All teeth survived the 3.5-year follow-up (range, 1.0-9.1 years). However, 1 tooth with an inflammatory cyst developed complications 1 year after surgery that required re-endodontic treatment. Conclusion: The prognosis of a tooth treated by apicoectomy without retrograde filling during cyst enucleation is favorable, regardless of the cyst type.

  • Maxillary anterior crown fractures are a common form of injury that mainly affects children and adolescents. The position of maxillary incisors and their eruptive pattern carries a significant risk for trauma. In the pre-adhesive era, fractured teeth needed to be restored either with pin-retained inlays or cast restorations that sacrificed healthy tooth structure and were a challenge for dentists to match with adjacent teeth. But. recently the development of adhesive dentistry has allowed dentists to use the patient's own fragment to restore the fractured tooth. The purpose of this paper is to present 2 cases of reattachment of fractured teeth which had different fracture mode, and to evaluate prognosis of cases.

상아질형성부전증 남매의 치료개입 시기에 따른 상이한 장기 예후 : 증례 보고 (Comparison of Long-term Prognosis in Siblings with Dentinogenesis Imperfecta depending on the Timing of the Treatment Intervention : Case Reports)

  • 상아질형성부전증은 상아질 형성 이상이 초래되는 유전성 질환으로 상염색체 우성 형질을 따른다. 상아질형성부전증은 유치와 영구치 모두 이환되며, 다양한 치아 변색, 상아질 및 법랑질 파괴, 심한 치아마모 등의 임상소견을 보인다. 방사선학적으로 치아는 가느다란 치근과 둥그런 치과의 형태를 보이고 치수강은 적거나 폐쇄되었다. 상아질형성부전증의 치료는 추가적인 마모를 방지하고 수직고경 회복을 목표로 한다. 본 증례는 치아의 마모와 변색을 주소로 내원한 상아질형성부전증을 지닌 남매의 상이한 15년간의 장기 예후를 보고하고자 한다. 두 환자에서 치료개입 시기의 차이에 따라 상이한 치료과정을 볼 수 있다. 첫 번째 환아는 대부분의 본인 치아를 살렸으나, 두 번째 환아는 모든 치아를 발치하였다. 이는 상아질형성부전증의 조기 진단 및 치료방법 결정에 도움이 될 수 있다.

전치부 매복치에 관한 교정학적 고찰 (ORTHODONTIC CONSIDERATION ON THE IMPACTED TEETH OF ANTERIOR SEGMENT)

  • A study on the impacted teeth of the anterior segment was carried out. The data for this study were compiled form 1739 outpatients of the Department of Orthodontics, College of Dentistry, Seoul National University. The following conclusions were obtained; 1. The frequency of the impacted teeth of the anterior segment was 2.8% and the frequency of male was 4.0%, that of female was 2.1%., respectively. 2. The order of occurrence of the impacted teeth in the anterior segment was maxillary canine mesiodens, maxillary central incisor, maxillary lateral incisor, mandibular canine. 3. The etiologic factors of the impaction were the space deficiency for eruption, cleft palate, the prolonged retention of deciduous teeth, dentigerous cyst. 4. The favorable results of treatment and prognosis were obtained from the impacted teeth of the normal shape after surgical exposure and adhered the plastic attachment and inducted them into the dental arch.

총의치를 위한 인공치배열과 교합 (The artificial teeth arrangement and occlusion for Complete denture)

  • In an edentulous situation, the oral rehabilitation with complete denture is substantially different from treatment of natural teeth. The artificial teeth arrangement and occlusion are important factor for the stability of the denture. Bilateral balanced occlusion is that stability of the denture is attained when bilateral contacts exist throughout all excursion and static states of the denture during function and, artificial teeth are arrangement in neutral zone where the forces between the tongue and cheeks or lips are equal. This is how dentist make a favorable prognosis. Therefore, dentists are have to be a little more involved in the selection and arrangement of artificial teeth.

The clinical prognosis of implants that are placed against super-erupted opposing dentition

  • Objectives: If teeth are missing, super-eruption of teeth in the opposing arch can occur in the area and can change the occlusal plane. When missing teeth are replaced with implants, the oral surgeon must determine whether or not the super-erupted teeth need to be treated in order to normalize the occlusal plane. In this study, we evaluated the clinical prognosis of dentition after implant placement and prosthetic treatment were completed in an occlusal plane altered by super-erupted teeth in the opposing arch without additional treatment of the super-erupted teeth. Materials and Methods: Twenty-two patients (9 males, 13 females) were treated with implants and prosthetics without addressing the super-erupted opposing dentition from April 2004 to August 2012 at Seoul National University Bundang Hospital. A total of 33 implants were placed. Values of crestal bone loss, survival rates, and surgical and prosthetic complications for an average of 29.6 months after prosthetic loading were recorded. Results: In one case, the cover screw was exposed after implant surgery. The mean crestal bone loss was $0.09{\pm}0.30mm$. Of the 33 implants, 31 survived, a survival rate of 93.94%. A prosthetic complication occurred in one case but functioned well after correction. Conclusion: Favorable clinical results from prosthetic complications, crestal bone loss, and implant survival rates were exhibited in implants next to a super-erupted opposing tooth.
