Trends of Breast Cancer and its Management in the Last Twenty Years in Aden and Adjacent Governorates, Yemen

  • Background: Breast cancer is the most common cancer of women and the principal cause of death in middle aged women. The objective of this study was to describe the trend of breast cancer and its management in Aden and adjacent south-eastern governorates of Yemen during the last 20 years. Patients and Methods: This is a retrospective analysis of previous studies on patients with breast cancer in Aden and adjacent south-eastern governorates, Yemen (January 1989 through December 2007). The studied variables were: sex, age, time and type of presentation, disease stage, pathological types and the performed surgical treatment. The sources of information were the treatment registry of Aden health office, archives of Al-Gamhouria teaching hospital; major referral and other public and private hospitals in Aden and Aden Cancer Registry. Results: The total number of patients was 476, 99% being females. The age range was 19-88 years. The most affected age was 30-50 years (60.5%), 95% presenting after one month of having breast symptoms. Forty-five percent presented with signs of advanced local disease, while 59.2% had palpable axillary lymph nodes on presentation. Early breast cancer (stages I-II) occurred in 47%, and late breast cancer (stages III-IV) in 51.5%. Invasive ductal carcinoma was the commonest pathology (89.3%). The main surgical treatment was mastectomy (modified radical mastectomy (50%). Conclusion: Breast cancer is predominantly a disease of young with late presentation and advanced disease. Improving health awareness and earlier diagnosis of the disease by health education, encouraging breast self-examination, and providing the mammography equipment and mammary clinics in hospitals are recommended. Establishment of oncology and radiotherapy centers in Aden is a necessity.

Clinical analysis of Pediatric Blow out Fracture (소아 안와 골절의 임상 분석)

  • Purpose: Because of traffic accidents and many criminal violences, the incidence of facial trauma has been increasing not only in adults but also in children. We planed this study to introduce our experience about pediatric blow out fracture and provide more information. Methods: We made retrospective study in 76 children with blow out fracture from January 2001 to September 2005 by retrospective chart review including detailed preoperative and postoperative evaluations, age, sex, cause, symptom and sign, and their post-operative complications. Results: Among our patients, 69 were male and 7 were female. The ages ranged from 7 to 18 years, which shows the greatest incidence of blow out fracture. Physical violence(46%) was the most common cause in this group and was followed by vehicle accident(28%), and fall down accident(17%). Left side(64%) showed slightly more incidence than right side(36%), but there were no statistical importance. Ecchymosis(88%) was the most common symptom and followed by periorbital swelling(68%) and diplopia(30%). 30 patients was diagnosed with another facial bone fracture and nasal bone(51%) was the most common associated facial bone fracture. Fourty four Patients(60%) got an orbital wall reconstruction in 7 days after trauma. After the operation, only 3 patients(4%) suffered from diplopia postoperative 3 month, and resolved in 4 years. Conclusion: The incidence of blow out fracture in children has been increasing every year, and violence has become more important etiology of pediatric blow out fracture and public and private education institutions were the most common place that blow out fracture originated. Accurate diagnosis and careful treatment plans are important in pediatric blow out fracture.

A USB DLP Scheme for Preventing Loss of Internal Confidential Files (내부 기밀파일 유출 방지를 위한 USB DLP 기법)

  • Recently industry is suffering severe losses of important internal information, and most of the leaks result from the insiders. Thus, various technological counter measures are being studied in order to protect private organization information. In this paper, among various internal information leakage path protection mechanisms, we propose a technique for preventing files which are defined as confidential from being copied or moved through USB. The implemented confidential file loss prevention system is composed of both signature insert manager and confidential file detect manager. The signature insert manager allows a system administrator to insert a special pattern specifying a classified confidential file, while the confidential file detect manager protects confidential file leakage when a user attempts to move or copy a confidential file to the USB memory. This results would be contributed to reducing the damage caused by leakage of confidential files outside of the company via USB.

Case Reports of Adipose-derived Stem Cell Therapy for Nasal Skin Necrosis after Filler Injection

  • With the gradual increase of cases using fillers, cases of patients treated by non-medical professionals or inexperienced physicians resulting in complications are also increasing. We herein report 2 patients who experienced acute complications after receiving filler injections and were successfully treated with adipose-derived stem cell (ADSCs) therapy. Case 1 was a 23-year-old female patient who received a filler (Restylane) injection in her forehead, glabella, and nose by a non-medical professional. The day after her injection, inflammation was observed with a $3{\times}3cm$ skin necrosis. Case 2 was a 30-year-old woman who received a filler injection of hyaluronic acid gel (Juvederm) on her nasal dorsum and tip at a private clinic. She developed erythema and swelling in the filler-injected area A solution containing ADSCs harvested from each patient's abdominal subcutaneous tissue was injected into the lesion at the subcutaneous and dermis levels. The wounds healed without additional treatment. With continuous follow-up, both patients experienced only fine linear scars 6 months postoperatively. By using adipose-derived stem cells, we successfully treated the acute complications of skin necrosis after the filler injection, resulting in much less scarring, and more satisfactory results were achieved not only in wound healing, but also in esthetics.

  • The international standards, ISO 15489 and Family Code, recommend using functional classification method both in public and private organizations. In this study made a comparative analysis of the details of classification systems through case studies on records classification systems of a total of seven comprehensive construction companies in Korea including three large corporations and four small and medium-size businesses. Findings of this study suggester the direction of developing construction records classification system and its methodology. By summarizing classification standards derived from these case studies, key construction records classification standards were presented.

  • The concept and discussion of copyright should reflect changes in using environments of digital properties and developments of technology. Further discussions are required about protection, transfer and usage of copyright for user created contents or digital activities. Digital archives should clarify the ranges of permitted usage or guides to quotation. One should not be biased toward regulating digital copying, but consider the value of diverse transformation of digital properties. This will trigger tension between private and public usage, or sharing of digital fabrications, which leads to the necessity of discussions on policy level. The interpretation of copyright is limited to protecting the copyright owner's right, but it should be suggested to widen the range to permitting fair use. The extended fair use of digital contents and by clarifying specific rules will activate creation and distribution of digital contents and contribute to more productive usage of innovation.

The Characteristics and Use of Moon Ik-Hwan's Prison Letters (문익환 옥중서신의 특성과 활용)

  • Moon Ik-hwan's prison letters are valuable historical materials managed by the Moon Ik-hwan House of Unification, reflecting an aspect of modern and contemporary South Korea. He wrote these prison letters during a period of approximately 11 years and three months. Today about 800 of them remain and are in an urgent need for measures to ensure their long-term preservation and Use. This study set out to introduce their current state that had been made to the public only in fragments, analyze their characteristics as objects of records management, and propose directions and strategies to provide them as part of online service. For these purposes, the investigator analyzed Moon's personal life and life in prison as the backgrounds of their creation and examined their unique characteristics as prison letters according to the scope and size, recipients, reproduction and circulation process, and the medium of lettercards. Based on these efforts, the study shed new light on the significance of use in private archives and provided a case of online utilization tasks and strategies based on the characteristics of letters as one of the representative types of personal records.

Evaluating Records and Their Descriptive Elements in the Records Management of Korea on the Basis of the Characteristics of a Record and Recordkeeping Metadata Standards (기록의 속성과 메타데이터 표준을 통해 본 한국의 기록·기록기술)

  • ISO 15489:2001 addresses the principles and requirements with which organizations, both public and private, should comply on the management of their records to ensure that adequate records are created, captured and managed. The standard defines the characteristics that a record should have through records management system as follows: authenticity, reliability, integrity, and usability. Authenticity means that records can be proven to be what it purports to be, to have been created or sent by the person purported to have created or sent it, and to have been created or sent at the time purported. Reliability means that the contents of the records can be trusted as a full and accurate representation of the transactions, activities or facts to which they attest and can be depended upon in the course of subsequent transactions or activities. Integrity refers to ensuring that a record is complete and unaltered. Usability means that records can be located, retrieved, presented and interpreted. In order to have these characteristics, a record should be persistently linked to the metadata necessary to document a transaction. Metadata is "data describing context, content and structure of records and their management through time." Metadata ensure the creation and maintenance of authentic, reliable and usable records and the protection of the integrity of those records. It could be implemented by creating and capturing records management metadata in systems that create and manage records. There have been some projects and standard initiatives to identify a core set of records management metadata. Included are the Australian Recordkeeping Metadata Standard and the British Metadata Standard which is part of the Requirements for Electronic Records Management System. Recently ISO/TS 23081-1 is published to implement metadata requirements within the framework of ISO 15489. Public records management system in Korea is ruled by the Act on the Management of Archives by Public Agencies and Administrative Records Management Regulation. This article evaluates records and their descriptive elements captured and maintained by the records management system in Korea on the basis of the international metadata standards.

Electromyographic and Clinical Investigation of the Effect of Platelet-Rich Plasma on Peripheral Nerve Regeneration in Patients with Diabetes after Surgery for Carpal Tunnel Syndrome

  • Background Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. Studies have shown that results of CTS surgery are poorer in patients with diabetes. In this study, the effect of platelet-rich plasma (PRP) on nerve regeneration was investigated through clinical and electromyographic findings in patients with diabetes who underwent CTS surgery. Methods A retrospective analysis of 20 patients with diabetes who had surgically decompressed CTS was conducted. Patients were divided into two groups. The study group received PRP treatment following surgery. The control group did not receive any treatment. Patients were assessed using electromyography and the Boston Carpal Tunnel Syndrome Questionnaire preoperatively as well as postoperatively at 3-month, 6-month, and 1-year follow-ups visits. Results There was a decrease in complaints and an improvement in sensory and motor examinations in both groups. The Boston Carpal Tunnel Syndrome Questionnaire scores did not show any statistically significant differences between the two groups. However, electromyographic findings showed that there were statistical differences between preoperative and postoperative (3 months, 6 months, and 1 year) results in both groups. When the two groups were compared using preoperative and postoperative (3 months, 6 months, and 1 year) electromyographic values, no statistically significant differences were seen. Conclusion Single injections of PRP did not have a significant impact on median nerve regeneration following CTS surgery in patients with diabetes. The effectiveness of multiple PRP injections can be investigated in patients with diabetes in future studies.

Impact of Nuclear Tests on Deforestation in North Korea using Google Earth-Based Spatial Images

  • The North Korean government conducted its first nuclear test in 2006 and more recently the sixth nuclear test on September 3, 2017. In order to identify how North Korea's nuclear tests have affected the environment, a scientific approach is required. Although North Korea's nuclear tests and their environmental destruction are not a severe threat to the environment of the Korean Peninsula at this time, identifying environmental damage and taking countermeasures in advance are essential to minimize their potential threats to the environments. The purpose of this study is to study the environmental impact of North Korea's nuclear tests using Google Earth image analysis. As a method of the study, we compare Google Earth images taken before and after each nuclear test was conducted in North Korea. To overcome limitations of the suggested comparison method, we cross-checked our results with those of previous scientific research. After the 1st-3rd nuclear tests, green spaces were found to be considerably reduced. In particular, when comparing the Google Earth images before and after the second nuclear test, some ground subsidences were observed. Such subsidences can cause tunnels on the mountainsides and cracks in rocks around the mountains, leading to the release of radioactive materials and contaminating groundwater. Besides, after the 4th-6th nuclear tests, decay and deforestation were observed not in the nuclear test sites, but in their surrounding areas. Especially after the 5th and 6th nuclear tests, the topography and the forests of the surrounding areas were severely damaged. In relation to North Korea's nuclear tests and their impact on the natural environment, we need to prepare various policy measures to reduce North Korea's environmental pollution and natural environment destruction. Those policy measures include the establishment of various cooperative governance between the Korean government, the private sector, the academia, NGOs, and international organizations.
