The Effect of Prism Adaptation Following Traumatic Brain Injury: A case report

  • Background: The presence of visuospatial impairment can make patients slow functional recovery and impede the rehabilitation process in TBI patients. Objective: The aim of this study is to investigate effects of prism adaptation treatment for functional outcomes in patients following traumatic brain injury. Methods: The subject received prism adaptation treatment for 2 weeks additionally during traditional rehabilitation for 4 weeks. The Patient has prism adaptation treatment while wearing wedge prisms that shift the external environment about $12^{\circ}$ leftward. The patient received 10 sessions, 15-20min each session. Outcome measures were visuospatial deficit(line bisection, latter cancellation), Visual and spatial perception(LOTCA-visual perception and spatial perception), motor function of upper extremity(FMA U/E; Fugl-Meyer motor assessment upper extremity, ARAT; Action research arm test), balance(BBS; Berg Balance Scale), mobility(FAC; Functional ambulation classification) and functional level(FIM; Functional independent measure). All Assessments took place on study entry and post-treatment assessments were performed at discharge from the hospital. Results: After prism adaptation, the visuospatial impairment scores improved as indicated in the line bisection(-15.2 to -6.02), latter cancellation(2 to 0) and LOTCA- spatial perception scores(7 to 9). The upper motor function improved as indicated in the scores of affected FMA U/E(21 to 40) and ARAT(4 to 22). Ambulation and balance improved as indicated in the BBS scores(25 to 38) and FAC scores(0 to 4). ADL function improved as indicated in the FIM total scores 54 to 70(motor 34 to 61, cognition 20 to 29). Conclusion: Prism adaptation did improve functional level such as motor functions and ADL abilities in TBI patient. Further research is recommended.

  • We have analyzed the cause of black "mura" and measured it in prism light-guide plate (LGP). Properties of components used in a back light. unit (BLU) have changed by Simulation Tool. We get major factor of black "mura" for improvement in prism LGP. For the improvement of black "mura", removal of brightness "mura" at input light part must precede preferentially because of reflection characteristics of the prism LGP. Removal of brightness "mura" is improved by changing of input light part in LGP and dispersion treatment.

We get major factor of black "mura" for improvement in prism LGP.   • In this study, it was investigated about the variation of luminance properties in LED backlight with geometrical shape such as prism or lens shape on the light-entering face. Generally, LED light source has the loss in backlight system because it has horizontal direction light. PMMA(Poly-Methyl-Meta-Acrylate) is used LGP(Light-Guide-Plate) in conventional backlight system. PMMA has low refraction angle. so that. horizontal direction light was not used in backlight system. The addition of geometrical shape in light-entering face in LGP improves the luminance characteristics because it prevents horizontal direction light generation in light-entering-face. Prism or lens shape in same pitch disposition was almost same without these treatment such as the value of 1% rising. Besides. It is particular that the gap of these shape such as the value of 0.407~0.22mm prevents horizontal direction light, so then it contributes luminance rising at 5.6%.

A case of constitutional acupuncture treatment for symptoms after embedding therapy in Soyangin patient (소양인의 매선시술과 시술 후 증상에 대한 체질침 치험 1 례)

  • Objectives The purpose of this study is to compare the changes before and after constitutional acupuncture treatment in soyangin who received embedding therapy for wrinkle improvement. Methods A retrospective study reviewing medical records was conducted. The patient was classified as a Soyangin by a specialist based on K-PRISM, personality, appearance, voice and usual symptoms. Records on the degree of wrinkles, postoperative symptoms, and patient satisfaction were reviewed. Results The patient's wrinkles were improved after the embedding therapy. After therapy symptoms were fever, flushing, and burning sensation. After acupuncture, these symptoms were reduced. Patient satisfaction increased after constitutional acupuncture treatment. Conclusion As a result of embedding treatment in Soyangin patients, there was an effect of wrinkle improvement. As a result of constitutional acupuncture treatment, anxiety and stress were reduced before the procedure, and symptoms of heat, burning, and flushing were reduced after the procedure.

  • Unilateral neglect is a common behavioral syndrome in patients following stroke. Unilateral neglect is characterized by the failure to report or responds to people or object presented to the side opposite a brain lesion. The patients with unilateral neglect are severely disabled in all daily activities, have a poor rehabilitation outcome and therefore require professional assessment and treatment This article reviews recent efficacy of rehabilitation techniques includings; drug, cueing, scanning training, prism lense, eye patching, trunk rotation, neck vibration, TENS, limb activation, video feedback. The purpose of this reviews are: to clarify the different types of neglect, as a basis for understanding the tests and measures of unilateral neglect and give information about recent treatments.

Outcome of Continuous Renal Replacement Therapy in Children (소아에서 지속적 신대체요법의 치료 결과)

  • Purpose : There is growing use of continuous renal replacement therapy(CRRT) for pediatric patients, but reports about the use and outcome of CRRT in children is rare in Korea. We report our experiences of CRRT in critically ill pediatric patients. Methods : We reviewed the medical records of 23 pediatric patients who underwent CRRT at Asan Medical Center between May 2001 and May 2004. We evaluated underlying diseases, clinical features, treatment courses, CRRT modalities and outcomes. Results : Ages ranged from three days to 16 years with a median of five years. Patients weighed 2.4 to 63.9 kg(median 23.0 kg; 10 patients ${\leq}20kg$). The underlying diseases were malignancy(nine cases), multiple organ dysfunction syndrome(five cases), hyperammonemia(four cases), acute renal failure associated with liver failure(three cases), dilated cardiomyopathy(one case) and congenital nephrotic syndrome(one case). Pediatric Risk of Mortality(PRISM) III score was $17.6{\pm}7.6$ and the mean number of failing organs was $3.0{\pm}1.7$. Duration of CRRT was one to 27 days(median : nine days). Eleven patients(47.8%) survived. Chronic renal failure developed in two cases, intracranial hemorrhage in one case, and chylothorax in one case among the survivors. PRISM III score and the number of vasopressor before the start of CRRT was significantly lower in the survivors($12.7{\pm}4.2$ and $0.9{\pm}1.1$) compared with nonsurvivors($22.1{\pm}7.8$ and $2.4{\pm}1.4$)(P<0.05). Conclusion : CRRT driven in venovenous mode is an effective and safe method of renal support for critically-ill infants and children to control fluid balance and metabolic derangement. Survival is affected by PRISM III score and the number of vasopressors at the initiation of CRRT.

Trochlear Nerve Palsy Caused by Quadrigeminal Cistern Lipoma (사구수조지방종에 의한 도르래신경마비 1예)

  • Purpose: To report a case of trochlear nerve palsy caused by quadrigeminal cistern lipoma located in the dorsal midbrain. Case summary: A 65-year-old male visited our clinic for intermittent vertical diplopia over 2-year period. Symptoms of diplopia had worsened over the past two weeks. He had no previous medical history except having had diabetes for 1 month. The best-corrected visual acuity was 20/25 in the right eye and 20/20 in the left eye. Pupillary examination was not remarkable. Extraocular examination showed 4 prism diopters (PD) left hypertropia at distant gaze and 4 PD exotropia at near gaze, with adduction elevation of the left eye. The Bielschowsky head tilt test revealed 6 PD left hypertropia on the left gaze and orthotropia on the right tilt. Fundus examination showed excyclotorsion of the right eye and incyclotorsion of the left eye. Brain magnetic resonance imaging revealed quadrigeminal cistern lipoma. Prism glasses were prescribed to alleviate diplopia, and we followed up the lesions without further treatment. Conclusions: Trochlear nerve palsy can be caused by quadrigeminal cistern lipoma; however, it is uncommon for this condition to be caused by a compressive lesion. Prompt neuroimaging can be helpful to rule out the causes of this condition in patients with atypical symptoms.

Preparation and Characterization of Sol-Gel Derived $SiO_2-TiO_2$ -PDMS Composite Films

  • Thin films of the SiO2-TiO2-PDMS composite material have been prepared by the sol-gel dip coating method. Acid catalyzed solutions of tetraethoxy silane (TEOS) and polydimethyl siloxane (PDMS) mixed with titanium isopropoxide Ti(OiPr) were used as precursors. The optical and structural properties of the organically modified 70SiO2-30TiO2 composite films have been investigated with Fourier Transform Infrared Spectroscopy (FT-IR), UV-Visible Spectroscopy (UV-Vis), Differential Thermal Analysis (DTA) and prism coupling technique. The films coated on the soda-lime-silicate glass exhibit 450-750 nm thickness, 1.56-1.68 refractive index and 88-94% transmittance depending on the experimental parameters such as amount of PDMS, thermal treatment and heating rate. The optical loss of prepared composite film was measured to be about 0.34 dB/cm.

Tamoxifen Induces Apoptosis of Leishmania major Promastigotes in Vitro

  • Tamoxifen is an antagonist of the estrogen receptor and currently used for the treatment of breast cancer. The current treatment of cutaneous leishmaniasis with pentavalent antimony compounds is not satisfactory. Therefore, in this study, due to its antileishmanial activity, effects of tamoxifen on the growth of promastigotes and amastigotes of Leishmania major Iranian strain were evaluated in vitro. Promastigotes and amastigotes were treated with different concentrations (1, 5, 10, 20, and $50{\mu}g/ml$) and time periods (24, 48, and 72 hr) of tamoxifen. After tamoxifen treatment, MTT assay (3-[4,5-dimethylthiazol-2-yl]-2,5 biphenyl tetrazolium bromide assay) was used to determine the percentage of live parasites and Graph Pad Prism software to calculate $IC_{50}$. Flow cytometry was applied to investigate the induction of tamoxifen-induced apoptosis in promastigotes. The half maximal inhibitory concentration ($IC_{50}$) of tamoxifen on promastigotes was $2.6{\mu}g/ml$ after 24 hr treatment. Flow cytometry analysis showed that tamoxifen induced early and late apoptosis in Leishmania promastigotes. While after 48 hr in control group the apoptosis was 2.0%, the $50{\mu}g/L$ concentration of tamoxifen increased it to 59.7%. Based on the in vitro antileishmanial effect, tamoxifen might be used for leishmaniasis treatment; however, further researches on in vivo effects of tamoxifen in animal models are needed.
