2000년 영덕군 홍역 유행시 유행 차이에 따른 2개 초등학교 일부 학생에 대한 역학 조사 (Comparative Epidemiologic Survey of Measles in Two Primary Schools)

  • Objectives : During March-May, 2000, a measles outbreak occurred at Youngduk, Korea. This county is divided into two areas with different historical and socioeconomic background. The outbreak occurred in one of these areas. We conducted a comparative epidemiologic study on the two areas in order to evaluate the factors related to the epidemic. Materials and Methods : We selected two groups, grades 3 and 5 in a primary schools in each area. We investigated outbreak-related factors using parent-questionnaires, the vaccination history from the student's health record and the records concerning the recent measles-outbreak from the local health center. Serologic test on measles-IgG and -IgM antibody was done. Results : The infection rate was 31.5% for the epidemic area and 3.7% for non-the epidemic area according to clinical or serological criteria (p<0.001). No difference was seen in the measles vaccination rate, residence at the time of vaccination or past measles infection history between the two areas. In the epidemic area, the attack rate for the 4-6 year-old MMR booster group(20.5%) was higher than the non-booster group(32.4%), but was not found significantly. Vaccine efficacy was 29.6% in the epidemic area and 87.0% in the non-epidemic area (p<0.001). The IgG level and positive rate were significantly different between the two areas (median 10727 IU/ml, 98.9% in epidemic area; median 346 IU/ml, 85.9% in the non-epidemic area, p<0.001). However, the IgG level and positive rate between the measles-cases and non-cases were not significantly different. Conclusions : This outbreak took place in mostly vaccinated children. These results suggest that a reduction of herd immunity for immunity failure after vaccination may be one of the feasible factors related to the outbreak pattern in the two areas. The results of the IgG level and positive rate suggest that re-establishment of a normal value for IgG level and of a qualitative method for IgG are needed.

The albumin, ${\beta}-globurin$ and ${\gamma}-globurin$ fractions of non-vaccinated bovine serum (Control) int declined, and a total protein and ${\alpha}-globurin$ level are advanced on the reversible rather than of vaccinated immumized bovine serum. (Table 5.) 2. Some few exists the to bring about changed in the individuals and sexual in a vaccinated bovine serum, however, Male serum globurin fractions are higher than Female globurin fractions percentage. (Table 1 and 3.) 3. Albumin fractions are Age-ablly variable, so that, younger's are rather lower than adult's such reported as in the another litratures. 4. In the monthly analysis of immunized bovine serum the first week to at dulation for third weeks were slowly advanced as variablly in serum-protein fractions of ater by the Black-leg No. 2 vaccination, then, albumin fractions were illustated as maximum ratio (42.73 3.49%) and increased much as 14.9% more than non-vaccinates, and ${\alpha}-globurin$ fractions indicated the minimum ratio(15.11 2.35%) at for 4th month after vaccination decrease much as about 7.71 % rather non-vaccinated normal bovine serum. (Table 4. and Fig 3.) Next, continuous advanced the ${\beta}-globurin$ fractions at first month as primary crisis in a diagram, and indicated the maximum ratio at 8th months as the second crieir on the its diagram of after vaccinati on, however, few changed in non-vaccinated bovine serum. (Figs 4.) Especially, ${\gamma}-globurin$ fractions are advanced the maximum ratio as 41. 45% 4.48% anp advanced to be widely range much as about 22.55% more than control serum at 5th months of after the vaccination. That is one of the most considerable evalution in Black-leg No. 2 vaccination to Korean calevs as great presence of the maximum immune antibodies at for 5th month after the vaccination. (Table 4. and Fig 4.) 5. In the relationship between vaccinated rabbit and Korean calves, serum protein fraractions were to be changed within the 3 weeks, so that albumin fractions of vaccinated immune bovine serum are increased as directly ratio, while vaccinated rabbit immunized serum showed the decrease as to reciprocal ratio. Although, conclude that ${\gamma}-globurin$ fractions are increased gradually by and large on the vaccinated immune bovine serum(B. P. S.) and vaccinated immune rabbit serum(R. P. S.) together. (Table 6).

닭에서 계두 예방 접종에 관한 연구 (Vaccination Studies against Fowl Pox in Chickens)

  • 파유미 종 닭에 계두 및 구두 백신을 접종하고 2종의 백신에 대한 방어 능력을 방글라대쉬의 야외의 닭 사육 조건에서 비교하였다. 'Take reaction'의 수율을 평가하여 이것을 더 나은 면역 반응과 관련지어 해석하였으며, 그룹 B와 그룹 C의 닭은 각각 93.33%와 100%의 면역 반응을 보여주었다. 1차 예방접종에 따른 평균 PHA 항체가는 B와 C 그룹에서 각각 $32{\pm}14.81$$33{\pm}13.66$으로 나타났다. 보강 접종 후 평균 PHA 항체가는 B 및 C 그룹의 전 처리군에서는 $46.93{\pm}16.52$$55.46{\pm}14.64$이었으며, 공격접종 2주 후 시점에서 향체가는 각각 $93.86{\pm}33.04$$110.93{\pm}29.29$로 나타났다. PHA 항체가는 백신 접종 후와 접종 후 처치군들에서 백신접종 전 항체가에 비해 현저하게 증가함을 보여주었다. 이렇게 현저한 PHA 항체가의 차이는 사용한 백신의 종류가 다양함으로 인해 발생되는 것이기도 하였으나, 백신 접종을 받은 모든 닭들은 감염에 대한 저항력을 보여주었다.

포도상구균 장내 C 형 변이독소 (SEC mutant)의 면역원성에 대한 연구 (Immunogenicity of staphylococcal enterotoxin C mutant antigen in mice and dairy cows)

  • Mastitis is one of the most significant cause of economic loss to the dairy industry. Especially, Staphylococcus aureus is a major contagious mastitis-causing pathogen in dairy cattle. Because of its high transmission rate and resistance to antibiotic therapy, staphylococcal mastitis presents a constant threat to the dairy industry. Staphylococcal enterotoxin C(SEC) produced by S aureus has been known as one of superantigens which are able to stimulate a large proportion of T lymphocytes independently of their antigenic specificity. In this experiment, we have conducted preliminary studies with mice and lactating cows to evaluate the immunogenicity and safety of the experimental vaccine consists of SEC mutant antigen on controlling the bovine mastitis associated with S aureus infections. The average value of somatic cell counts in quarter milk, isolation rate of S aureus were consistently decreased in SEC-SER vaccinated groups, whereas antibody titers were highly increased in SEC-SER vaccinated groups. Peripheral blood were also collected from the lactating cows to determine the proportion of leukocyte subpopulation associated with humoral immunity(HI) and cell mediated immunity(CMI). Proportion of leukocyte subpopulation expressing $BoCD2^+$(total T lymphocyte), $BoCD4^+$(T helper cell), $BoCD8^+$(T cytotoxic/suppressor cell) and NonT/NonB lymphocyte which are involved in CMI in SEC-SER vaccinated groups were decreased for the initial stage after first vaccination and then increased from ten weeks after first vaccination maintaining elevated level till 14 weeks after vaccination. In contrast, proportion of monocyte, MHC class II and B lymphocyte which are associated with the production of primary immune response in SEC-SER vaccinated groups were increased for the initial period and then decreased from ten weeks after first vaccination. We present evidence that vaccination of SEC-SER mutant antigen in lactating cows induced a significant proliferation of bovine T lymphocytes. These results suggest that SEC-SER mutant antigen used in this experiment might be one of potential immunogen in developing innovative vaccine against bovine IMI associated with S aureus. Additional challenge trials should be carried out to evaluate substantial protection against S aureus under the commercial farm conditions.

The Attenuation Mechanism and Live Vaccine Potential of a Low-Virulence Edwardsiella ictaluri Strain Obtained by Rifampicin Passaging Culture

  • The rifampicin-resistant strain E9-302 of Edwardsiella ictaluri strain 669 (WT) was generated by continuous passage on BHI agar plates containing increasing concentrations of rifampicin. E9-302 was attenuated significantly by 119 times to zebrafish Danio rerio compared to WT in terms of the 50% lethal dose (LD50). Zebrafish vaccinated with E9-302 via intraperitoneal (IP) injection at a dose of 1 × 103 CFU/fish had relative percentage survival (RPS) rates of 85.7% when challenged with wild-type E. ictaluri via IP 14 days post-vaccination (dpv). After 14 days of primary vaccination with E9-302 via immersion (IM) at a dose of 4 × 107 CFU/ml, a booster IM vaccination with E9-302 at a dose of 2 × 107 CFU/ml exhibited 65.2% RPS against challenge with wild-type E. ictaluri via IP 7 days later. These results indicated that the rifampicin-resistant attenuated strain E9-302 had potential as a live vaccine against E. ictaluri infection. A previously unreported amino acid site change at position 142 of the RNA polymerase (RNAP) β subunit encoded by the gene rpoB associated with rifampicin resistance was identified. Analysis of the whole-genome sequencing results revealed multiple missense mutations in the virulence-related genes esrB and sspH2 in E9-302 compared with WT, and a 189 bp mismatch in one gene, whose coding product was highly homologous to glycosyltransferase family 39 protein. This study preliminarily explored the molecular mechanism underlying the virulence attenuation of rifampicin-resistant strain E9-302 and provided a new target for the subsequent study of the pathogenic mechanism of E. ictaluri.

Humoral immune response to SARS-CoV-2 mRNA vaccines is associated with choice of vaccine and systemic adverse reactions

  • Purpose: Although the fast development of safe and effective messenger RNA (mRNA) vaccines against severe acute respiratory syndrome coronavirus 2 has been a success, waning humoral immunity has led to the recommendation of booster immunization. However, knowledge of the humoral immune response to different booster strategies and the association with adverse reactions is limited. Materials and Methods: We investigated adverse reactions and anti-spike protein immunoglobulin G (IgG) concentrations among health care workers who received primary immunization with mRNA-1273 and booster immunization with mRNA-1273 or BNT162b2. Results: Adverse reactions were reported by 85.1% after the first dose, 94.7% after the second dose, 87.5% after a third dose of BNT162b2, and 86.0% after a third dose of mRNA-1273. They lasted for a median of 1.8, 2.0, 2.5, and 1.8 days, respectively; 6.4%, 43.6%, and 21.0% of the participants were unable to work after the first, second, and third vaccination, respectively, which should be considered when scheduling vaccinations among essential workers. Booster immunization induced a 13.75-fold (interquartile range, 9.30-24.47) increase of anti-spike protein IgG concentrations with significantly higher concentrations after homologous compared to heterologous vaccination. We found an association between fever, chills, and arthralgia after the second vaccination and anti-spike protein IgG concentrations indicating a linkage between adverse reactions, inflammation, and humoral immune response. Conclusion: Further investigations should focus on the possible advantages of homologous and heterologous booster vaccinations and their capability of stimulating memory B-cells. Additionally, understanding inflammatory processes induced by mRNA vaccines might help to improve reactogenicity while maintaining immunogenicity and efficacy.

Knowledge Towards HPV infection and HPV Vaccines among Syrian Mothers

  • Cervical cancer is caused by HPV infection and can be prevented by early vaccination. Objective: To assess Syrian women's level of knowledge and determinants of good knowledge of cervical cancer, HPV infection and its vaccines. Methods: A cross sectional survey was undertaken among mothers with daughters in sixth grade classes enrolled in primary schools in Aleppo city, Syria. Samples were selected through cluster sampling and data collected using a self-administered questionnaire. Results: Less than a third of the mothers had heard of HPV infection and vaccines against cervical cancer and levels of knowledge were generally low. Good knowledge was associated with high education level, higher family monthly income, having few - less than four children, positive history of cervical cancer screening, and working or having relatives working in the medical field. The main source of information was television and few reported health care providers as a source of knowledge on HPV infection and vaccine. Conclusion: Since knowledge of HPV infection and its connection with cervical cancer and its vaccine are low, more efforts must be made to educate Syrians prior to introduction of any HPV vaccination programme. Public health efforts must focus on educating mothers, the public as well as health care providers.

건강한 한국 소아에서 HM175주 A형 간염 불활화 백신의 면역원성 및 이상반응에 관한 연구 (Immunogenicity and Reactogenicity of Inactivated HM175 Strain Hepatitis A Vaccine in Healthy Korean Children)

  • 목 적 : 환경적 변화에 잘 적응되는 특성을 지닌 A형 간염 바이러스는 환경과 위생이 개선된 현재에도 전 세계적으로 연간 140만명 정도가 발생되고 있는 중요한 전염성 감염 질환으로서 국내에서도 A형 간염 항체 보유율이 역학적 변화 상태에서 1996년 이후부터 10세에서 20세 사이의 연령에서 발생이 현저히 상승되는 양상을 보이고 있다. 국내에서 1996년부터 A형 간염 발생의 장기적 조절을 위하여 A형 간염 백신이 선별접종으로서 활용되고 있는 상황에서 저자들은 1세에서 15세 사이의 건강한 소아를 대상으로 A형 간염 백신에 대하여 면역원성 및 이상반응에 관한 기본적 임상연구를 실시하게 되었다. 대상 및 방법 : 본 연구는 1999년 2월부터 1999년 3월까지 가톨릭대학 성모자애병원, 인하대학병원, 순천향대학병원을 방문한 1세에서 15세 사이의 기저질환이 없으며 과거력상 A형 간염의 기왕력이나 A형 간염백신의 접종력이 없고 진찰상 건강한 소아들을 대상으로 국내에서 1997년부터 사용되고 있는 HM175주 A형 간염 백신(Havrix)을 1회 기초 접종하고 6개월 후에 추가 접종하여 접종때마다 단기간 내의 국소 및 전신적 이상반응을 확인하고 각 접종 1개월 후에 채혈 분리된 혈청에서 A형 간염 항체를 ELISA법으로 측정하여 면역원성을 평가하였다. 결 과 : 128명(남아 65명, 여아 63명; 평균 연령, 6.0세)이 1차 접종시부터 면역원성 및 이상반응에 대한 연구 대상아로 참여되었고 120명(남아 60명, 여아 60명; 평균 연령 6.0세)이 최종 연구기간까지 참여하였다. 면역원성 연구 결과 일차 접종 1개월 후 A형 간염 항체가 전체에서 양성으로 전환되었으며 평균 농도는 $389.2{\pm}392.7mIU/mL$, 기하평균치(GMT)는 266.4이었다. 추가 접종 1개월 후 면역혈청검사에서 역시 대상아 전체에서 양성을 보였으며 평균 농도가 $3,709{\pm}3,270mIU/mL$, 기하평균치가 2,502로 일차 접종 때보다 상승되었다. 한편 이차접종 1개월 후 면역검사에서 영아의 평균 A형 간염항체가는 남아의 평균 농도보다 유의하게(P=0.0017) 높았다. 이상반응 평가에서는 주사부위의 동통, 발적, 부종, 소양감, 발열감의 순으로 국소적 이상반응이 나타났고 권태감, 식욕부진, 두통, 발열, 오심의 순으로 이상반응이 일차 접종 후에 발현되었고 이차 접종 후에 발열은 전례에서 관찰되지 않았으며 권태감, 두통, 식욕부진, 오심, 구토, 발진이 관찰되었다. 이러한 이상반응들은 모두 3일 이내에 특별한 조처없이 소실되었다. 결 론 : 건강한 소아를 대상으로 국내에서 사용되고 있는 HM175주 A형 간염 백신에 대한 면역원성 및 이상반응에 관한 연구를 실시한 결과 1회 기초 접종 후 전례에서 방어항체가로 양전되고, 추가접종 후 방어 항체가가 매우 유의하게 상승되고 면역원성을 확인하였고, 국소 및 전신 이상반응은 모두 경미한 정도로 별다른 조치 없이 3일 내에 소실되어 안전한 백신임을 알 수 있었다.

Mercury Level in Hair of Primary School Children in Korea and China

  • Exposure to mercury was assessed in 125 Korean (Gwangju and Busan) and 373 Chinese primary school students (Xinguang village, Goumen town) using hair mercury analysis from November 2006 to September 2007. The geometric mean concentration of mercury was higher among Korean children with recording 0.73 ${\mu}g$/g, compared to Chinese children of 0.12 ${\mu}g$/g, which indicated statistically difference (P<0.01). The mean concentration of Korean children living near incineration facilities was higher by recording 0.76 ${\mu}g$/g while the average concentration of their counterpart in Korea reached 0.69 ${\mu}g$/g. In case of Chinese children, those who are living near power plants showed higher level with posting 0.16 ${\mu}g$/g while the others recorded 0.10 ${\mu}g$/g (P<0.01). Intake of fish was found to be related to hair mercury level. In case of Korean children, those with high fish intake recorded 0.79 ${\mu}g$/g in terms of the geometric mean concentration while the others with low fish intake posted 0.61 ${\mu}g$/g. Among Chinese children, those who often eat fish recorded 0.13 ${\mu}g$/g compared to the others with low fish intake of 0.11 ${\mu}g$/g. On the other hand, amalgam dental fillings have limited influence on mean hair mercury level. As for vaccination, within a month of vaccination, the geometric mean concentration of Korean children reached 0.76 ${\mu}g$/g, and in case of 15 days after injection, the level was 1.20 ${\mu}g$/g. In China, the level of children at one month after receiving injection stood at 0.15 ${\mu}g$/g while the level within 15 days was 0.13 ${\mu}g$/g. Multiple regression analysis showed that BMI, passive smoking, and fish consumption are closely related to hair mercury level among the Korean subjects. In China, hair mercury level was affected by age, location, passive smoking, fish consumption, and vaccination. Explanatory power was 21.6% with $R^2$=0.216.
