Materials for posterior teeth includes amalgam, gold inlay and composite resin inlay. Amalgam and gold inlay have unsatisfyine esthetics. And because they simply obturate the cavity preparation, they do not strengthen the remaining tooth structure. Posterior composite resin has become established in recent years. However, its polymerization shrinkage and insufficient wear resistance were the most undesirable characteristic. The physical and mechanical properties of the composite resin inlay are further improved through heat treatment in an oven. The major part of polymerization contraction of the resin inlay takes place be fore cementation, and possible gap formation is only due to shrinkage of the thin layer of resin cement. With the semidirect technique, the inlay material is placed directly in the prepared tooth, and the primary polymerization is made by light activation with a handhold curing unit. Additional curing may take place extraorally with use of different curing ovens. It provides the patient with the benefits of luted restorations without the procedure of indirect lab-made inlay. I report three successfully treated cases by semidirect resin inlay technique. Entire clinical steps are described in detail with some discussions on the outcome.

Pediatric dentistry is differentiated from other fields of dentistry in that it provides comprehensive dental care for children and adolescents. In early days, pediatric dentistry used to be confined to the caries treatment and extraction of primary teeth However, the practice spectrum of pediatric dentistry has broadened to the orthodontic, surgical, esthetic and preventive treatments A survey that contained questions about practice patterns were mailed to 50 pediatric dentists, and 21 surveys were returned. Results were as follows: 1. The average number of patients per week was 82.4, and the average number of treated teeth per patient was 2.35. 2. Preventive treatments comprised 15.7%, restorative treatments 55.7%, endodontic treatments 15.6%, surgical treatments 10.5%, and orthodontic treatments 2.4%. 3. In restorative treatments, amalgam restoration comprised 3.8%, glass ionomer 5.5%, composite resin 63.0%, and stainless steel crown 27.7%.

Molar Incisor Hypomineralization(MIH) have recently gained intensive research interests and various clinical trials. Most prevalence, etiology, treatment studies in MIH were carried out in the European countries, and data from the Korean were seldom. This study aimed to investigate the prevalence of MIH and the status of dental caries and treatment on the first permanent molars in primary school children. For this study, 1,344 primary school students in Busan and Ulsan cities were examined directly the permanent incisors and first molars. The results of the survey were as follows: 81 MIH cases were identified among 1,344 cases. The prevalence of MIH in this group of children was 6.0%. Children with MIH showed a significantly higher DMFT value for permanent teeth than children without MIH. The mean number of decayed, missed, and filled in 1st permanent molars(DMFT index) was 1.17. The rate of children with restoration on 1st permanent molar was 13.6% and the sequence of restoration materials was as follows : composite resin, amalgam, gold inlay.

Background: Oronasal/antral communication, loss of teeth and/or tooth-supporting bone, and facial contour deformity may occur as a consequence of maxillectomy for cancer. As a result, speaking, chewing, swallowing, and appearance are variably affected. The restoration is focused on rebuilding the oronasal wall, using either flaps (local or free) for primary closure, either prosthetic obturator. Postoperative radiotherapy surely postpones every dental procedure aimed to set fixed devices, often makes it difficult and risky, even unfeasible. Regular prosthesis, tooth-bearing obturator, and endosseous implants (in native and/or transplanted bone) are used in order to complete dental rehabilitation. Zygomatic implantology (ZI) is a valid, usually delayed, multi-staged procedure, either after having primarily closed the oronasal/antral communication or after left it untreated or amended with obturator. The present paper is an early report of a relatively new, one-stage approach for rehabilitation of patients after tumour resection, with palatal repair with loco-regional flaps and zygomatic implant insertion: supposed advantages are concentration of surgical procedures, reduced time of rehabilitation, and lowered patient discomfort. Cases presentation: We report three patients who underwent alveolo-maxillary resection for cancer and had the resulting oroantral communication directly closed with loco-regional flaps. Simultaneous zygomatic implant insertion was added, in view of granting the optimal dental rehabilitation. Conclusions: All surgical procedures were successful in terms of oroantral separation and implant survival. One patient had the fixed dental restoration just after 3 months, and the others had to receive postoperative radiotherapy; thus, rehabilitation timing was longer, as expected. We think this approach could improve the outcome in selected patients.

Axenfeld-Rieger syndrome is a rare autosomal dominant disorder characterized by various ocular and extraocular malformations. The incidence of Axenfeld-Rieger syndrome has been estimated to be 1 per 200,000. The syndrome is characterized by short body stature, delayed bone age, and deficient or arrested development of neural crest cells involving the anterior chamber of the eyes, facial bones, teeth, periumbilical skin, and cardiovascular system. This is a case of a 6 year-old girl, who came to the department of the pediatric dentistry, Yonsei University Dental Hospital, for evaluation and treatment of multiple congenital missing permanent teeth. The patient presented typical dental, craniofacial, and systemic features of Axenfeld-Rieger syndrome, such as glaucoma, oval pupil, heterochromatic iris, umbilical hernia, and delayed bone age. On the panoramic view, 3 primary teeth were missing and 13 permanent tooth germs were absent. On the lateral cephalograph, underdevelopment of the maxilla and normal growth pattern of the mandible were confirmed. Periodic dental follow-up is planned for evaluation and interceptive treatment of her dental and craniofacial problems. Denture or removable space maintainer with a pontic is considered for esthetic and functional restoration. In addition, orthognathic surgery is also planned in future to prevent further midfacial skeletal deformation. Early diagnosis of Axenfeld-Rieger syndrome is very important to prevent exacerbation of complications, such as glaucoma and skeletal deformities.
Statement of problem: when using resin for class II restoration, micoleakage by instrumentation can be regarded as the primary negative characteristic. A review of the available literature suggests that using flowable resin as liner to decreased microleakage. Purpose: The aim of this study was to determine the influence of the nanofilled flowable resin lining on marginal microleakage after load cycling in class II composite restoration fillings using nanofiller resin. Material and methods: 24 extracted premolars were prepared with class II cavity. F group was restored the nanofilled resin with the nanofilled flowable resin as liner. NF group was restored the nanofilled resin only. After restoration, an experiment was performed on 2 groups using a 300N load at 104, 105 and 106 cycles. Prior to and before each load cycling, it was gauged length on total marginal microleakage, axial marginal microleakage and buccal, gingival, lingual marginal microleakage. Data were analyzed with the Mann-Whitney test & Kruskal-Wallis test. Results: There were statistically significant differences between 2 groups and between individual groups. (P <.05) The result showed less microleakage in teeth restored by the nanofilled resin, which was lined by the nanofilled flowable resin. Conclusion: There was significant reduction in microleakage when the nanofilled flowable resin lining was placed underneath the nanofilled resin in class II composite restoration fillings.

Early childhood caries which mainly affect maxillary anterior area, defined as 'the presence of 1 or more decayed, missing, or filled tooth surfaces in any primary tooth in a child 71 months of age or younger'. Extraction of teeth when early childhood caries affected in maxillary primary incisors often develops progressively, result in severe destruction of crowns, acute or chronic pulpitis, and periapical abscess formation. Maxillary primary incisors are need to preserve as possible, because the early loss of maxillary primary incisors may lead to various functional, esthetical, and psychological problems. It is necessary to the availability of an easy to perform technique capable of providing efficient, durable, functional, and esthetic restorative methods. Polyethylene fiber-reinforced post can be used in strengthen of composite resins that is esthetic and good physical and mechanical properties. $Ribbond^{(R)}$ is made from an polyethylene fiber, has numerous usages, its surface is treated to enhance adhesion to resins, ease of manipulation, relatively cost effective. We report this case, had restored of maxillary primary incisors with severe coronal destruction due to affecting severe early childhood caries, using polyethylene fiber-reinforced posts, composite resin cores, and celluloid crowns. We could obtain good result of treatment.

The purpose of this study was to compare the marginal leakage of resin liner which was used as a liner to amalgam restoration. Control group composed of no liner group, copalite group and experimental group composed of All-bond 2, Vitrebond, Superbond D-liner applied groups were evaluated. Cl.V preparation with a size of $3{\times}2{\times}1.5mm$ on 70 extracted primary molars were made and applied appropriate liners for each groups. After amalgam filling and polishing, polished specimens underwent temperature changed from $5^{\circ}C\;to\;55^{\circ}C$ a thousand times. After thermocycling, specimens were placed in 2% methylene blue dye solution for 72 hours in an incubator set at $37^{\circ}C$. The teeth were sectioned buccolingually and the degree of dye penetration was observed with a spectroscopy. (${\times}30$) The following results were obtained. 1. Vitrebond, All-bond 2, Superbond D-liner group exhibited a statistically significant lower degree of dye penetration than no liner and copalite group. (Kruskal-Wallis analysis, P<0.05) 2. No liner group and copalite group exhibited a similar degree of dye penetration. (Mann-Whitney analysis, P>0.05) 3. All-bond 2 group exhibited a statistically significant lower degree of dye penetration than Vitrebond group, (Mann-Whitney analysis, P<0.05) Superbond D-liner group also exhibited a lower degree of dye penetration than Vitrebond group but was statistically insignificant. This study was conducted for the purpose of evaluating the stainless steel crowns on extracted primary molars and thus identifying frequent errors and defects. Visual assessment and micro-computed tomography (micro-CT) image analysis were performed on 97 primary molars for evaluation of the state of marginal adaptation, cement loss, secondary caries, ledge formation, attritive perforation and marginal polishing defect. The results were as follows: In the examination of object teeth by evaluation criteria, cement loss was found most frequently (98%), followed by secondary caries (42.3%), marginal polishing defect (41.2%), ledge formation (29.9%) and attritive perforation (17.5%), in this order. The cement loss at the margins showed a significant relationship with marginal gap and secondary caries: the larger the marginal gap is, the more frequent is the cement loss (p < 0.05). The average marginal gap was $0.31{\pm}0.26mm$ and showed the highest value in the maxillary 2nd primary molars. The location of the crown margin above the cementoenamel junction was found most frequently and it was found that the higher the crown margin is located, the less the marginal gap becomes (p < 0.05). In conclusion, it is thought very desirable to pay closer attention to crown margins and shapes for stainless steel crown restoration in order to minimize the marginal gaps and consequent cement loss.

The purpose of this study was to provide clinical recommendations for restoration with selection of the most similar zirconia crown by 3-dimensional analysis of the shape of the maxillary primary central and lateral incisors in Korean individuals and prefabricated zirconia crowns. The average shape of the sound maxillary primary central and lateral incisors in 300 children was reproduced by 3-dimensional scanning. Zirconia crowns of 4 manufacturers (NuSmile $ZR^{(R)}$ Crown, Cheng $Crowns^{(R)}$, Kinder $Krowns^{(R)}$, and EZ $Pedo^{(R)}$ Crown) were scanned 3-dimensionally, and coordinates for comparison of the shape were measured to evaluate the similarity between the teeth and crowns. The most similar crowns were selected by comparing the mesiodistal length, crown height, crown shape ratio, distance between the same coordinates of a tooth and crown, the radius of curvature of the labial surface, and the volume. As a result of analysis, Cheng $Crowns^{(R)}$ size 3 and NuSmile $ZR^{(R)}$ Crown size 2 were the most similar crowns in the maxillary primary central and lateral incisors, respectively. Scanning the inner surface of the crowns and evaluating the amount of tooth reduction required suggested that an overall lesser amount of tooth reduction compared to that presented by the manufacturer's guidelines should be performed.
