Diagnostic Utilization of Laser Fluorescence for Resin Infiltration in Primary Teeth (유치의 레진침투법을 위한 레이저 형광법의 진단적 활용)

  • This study was performed to evaluate clinical use of laser fluorescence (LF) to identify early childhood caries lesions suitable for applying resin infiltration. 20 exfoliated primary molars with proximal caries were selected and cut buccolingually cross the central pit for regarding the mesial and distal surfaces respectively. 27 specimens corresponding to ICDAS code 1 and 2 were selected and the LF values were measured. When infiltrant resin was applied, double staining for microscopy detection has done simultaneously. Tooth samples were sliced with 0.7 mm thick. The maximum lesion depth, maximum penetration depth, and average penetration rate were measured from the confocal scanning laser microscope image. Pearson correlation analysis was performed. The intraclass correlation coefficient of LF values shows excellent agreement. LF values had positive correlation with penetration rate, but not lesion depth and penetration depth. Significant correlation between LF readings and penetration rate was verified in deep enamel caries and dentin caries except shallow enamel caries. Infiltrant resin could penetrate with a higher rate and LF values could be increased in more active caries lesions. In assessing radiologically similar caries lesion, laser fluorescence might be useful for identifying caries activity.

Detection of Proximal Caries Lesions with Deep Learning Algorithm (심층학습 알고리즘을 활용한 인접면 우식 탐지)

  • This study aimed to evaluate the effectiveness of deep convolutional neural networks (CNNs) for diagnosis of interproximal caries in pediatric intraoral radiographs. A total of 500 intraoral radiographic images of first and second primary molars were used for the study. A CNN model (Resnet 50) was applied for the detection of proximal caries. The diagnostic accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and area under ROC curve (AUC) were calculated on the test dataset. The diagnostic accuracy was 0.84, sensitivity was 0.74, and specificity was 0.94. The trained CNN algorithm achieved AUC of 0.86. The diagnostic CNN model for pediatric intraoral radiographs showed good performance with high accuracy. Deep learning can assist dentists in diagnosis of proximal caries lesions in pediatric intraoral radiographs.

INFLUENCE OF A SODIUM HYPOCHLORITE GEL ON MICROLEAKAGE OF COMPOSITE RESIN RESTORATIONS (차아염소산 나트륨의 사용이 복합레진 수복물의 미세누출에 미치는 영향)

  • This study evaluated the influence of chemomechanical caries removal agent $Carisolv^{TM}$(MediTeam, Sweden) for composite resin adhesion to sound human permanent and primary dentin. The buccal/labial surfaces of 80 permanent molars and 80 primary incisors were used. Four types of adhesives and one composite resin were used; AQ Bond(Sun Medical, Japan), Clearfil SE Bond(Kuraray, Japan), Single Bond(3M, USA), Scotchbond Multi-Purpose(3M, USA) and Z100(3M, USA). One drop of $Carisolv^{TM}$(MediTeam, Sweden) was pretreated on the dentin for 0 second(control) and 60 seconds. The specimens were thermocycled for 1,000 times in baths kept 5 degrees C and 55 degrees C with a 30 seconds dwell time. Shear bond strengths were tested and the data was statistically analyzed using one-way ANOVA with subsequent post hoc Scheffe test at p<0.05. $Carisolv^{TM}$ treatment significantly decreased the shear bond strength. Shear bond strength of permanent dentin was significantly higher than that of primary dentin. Clearfil SE Bond treatment groups showed the highest shear bond strength and AQ Bond treatment groups showed the lowest shear bond strength.

Effects of Artificial Saliva Contamination on the Bond Strength of Three Dentin Adhesives to Dentin of Primary Teeth (인공타액오염이 유치 상아질에서 3종 상아질 접착제의 결합 강도에 미치는 영향)

  • The aim of this study was to evaluate the effectiveness of artificial saliva contamination and decontamination procedures at different stages of a bonding procedure on the microtensile bond strength (MTBS) of two one-step self-etch adhesives and a two-step total-etch adhesive to dentin of primary teeth. Forty-two extracted sound primary molars were randomly divided into three groups depending on three adhesives, Scotchbond Universal Adhesive (SBU), All-Bond Universal (ABU) and Prime & Bond NT (PNT). For each adhesive, the teeth were allocated into seven groups. Except for control group, group 1, the groups were contaminated with artificial saliva at three different stages: the groups 2 and 3 - before adhesive application; the groups 4 and 5 - before adhesive polymerization; the groups 6 and 7 - after adhesive polymerization. Decontaminating procedures were rinsing, air-drying (group 2, 4, 6) and air-drying (group 3, 5, 7). The specimens were restored with composite resin (Filtek, Z350) and microtensile bond strength was measured. The data was analyzed with one-way ANOVA and Tukey HSD test (p < 0.05). In the control group, using PNT resulted in significantly higher bond strength than when ABU and SBU were used (p < 0.001). For three adhesives, the groups 2, 4 and 6 had greater bond strength than the groups 3, 5 and 7. Also, when the artificial saliva was contaminated before adhesive polymerization (group 4, 5), it showed a significantly lower bond strength. Generally the two-step total-etch adhesive generated a higher bond strength than the one-step self-etch adhesive. Artificial saliva contamination before adhesive polymerization led to a drastic decrease in bond strength, and rinsing with water followed by air-drying could not recover the bond strength.

Surgical Management of Dentigerous Cyst for Preserving Permanent Tooth Buds (영구치 보존을 고려한 함치성 낭종의 외과적 처치)

  • Dentigerous cyst is the most common developmental odontogenic cyst of the jaw which is associated with unerupted teeth. Treatment modalities range from enucleation to marsupialization. Enucleation is the process in which the cyst is completely removed, and this is usually indicated for smaller lesions. Larger cysts can be treated by marsupialization, which is a process that consists of making a surgical cavity on the wall of the cyst, emptying its content and maintaining the continuity between the cyst and the oral cavity. This procedure allows decompression of the cyst, regeneration of the bone-defected area, and also lets the wall of the cyst change into normal mucosa. This technique will protect adjacent structures and will promote spontaneous eruption of succedaneous teeth previously surrounded by the cyst. These are two case reports of a 5-year-old and an 11-year-old boy with dentigerous cysts found regarding supernumerary tooth and pulpotomized mandibular primary molars, respectively. Due to the large size of the cysts, both cases were treated with marsupialization. This case report shows that in cases regarding large-sized dentigerous cysts, marsupialization can be an effective surgical technique that promotes spontaneous eruption of cyst-involved succedaneous teeth and the preservation of adjacent teeth.

Alveolar ridge preservation with an open-healing approach using single-layer or double-layer coverage with collagen membranes

  • Purpose: The aim of this prospective pilot study was to compare alveolar ridge preservation (ARP) procedures with open-healing approach using a single-layer and a double-layer coverage with collagen membranes using radiographic and clinical analyses. Methods: Eleven molars from 9 healthy patients requiring extraction of the maxillary or mandibular posterior teeth were included and allocated into 2 groups. After tooth extraction, deproteinized bovine bone mineral mixed with 10% collagen was grafted into the socket and covered either with a double-layer of resorbable non-cross-linked collagen membranes (DL group, n=6) or with a single-layer (SL group, n=5). Primary closure was not obtained. Cone-beam computed tomography images were taken immediately after the ARP procedure and after a healing period of 4 months before implant placement. Radiographic measurements were made of the width and height changes of the alveolar ridge. Results: All sites healed without any complications, and dental implants were placed at all operated sites with acceptable initial stability. The measurements showed that the reductions in width at the level 1 mm apical from the alveolar crest (including the bone graft) were $-1.7{\pm}0.5mm$ in the SL group and $-1.8{\pm}0.4mm$ in the DL group, and the horizontal changes in the other areas were also similar in the DL and SL groups. The reductions in height were also comparable between groups. Conclusions: Within the limitations of this study, single-layer and double-layer coverage with collagen membranes after ARP failed to show substantial differences in the preservation of horizontal or vertical dimensions or in clinical healing. Thus, both approaches seem to be suitable for open-healing ridge preservation procedures.

Maxillary Incisor Replacement with the Ectopically Erupting Canine : Case Reports (이소맹출하는 견치의 상악 전치로의 대체사용 : 증례보고)

  • Impacted maxillary canines are the most frequently impacted teeth after the third molars. The exact etiology of impacted maxillary canines is unknown, but several complications may result from impacted maxillary canines. An early detection of ectopically erupting teeth can lead to performing interceptive treatment such as early extraction of primary canine and provide the best long-term results. In the absence of prevention, clinicians should consider orthodontic treatment followed by surgical exposure of the canine to bring it into occlusion. However, in cases when the finding ectopically erupting teeth and severe root resorption of adjacent teeth are found late, malposed canine can replace the injured teeth. In these presented cases, early diagnosis and treatment of ectopic eruption and root resorption were not performed. The maxillary incisor replacement with ectopically erupting canine can be the alternative treatment of choice with successful results. The reconstructed canine is planned to be checked periodically for the condition of composite resin restoration. Orthodontic treatment and dental implant are planned. This report shows that when early diagnosis was not done, maxillary incisor replacement with ectopically erupting canine could prevent uncertain prognosis of the adjacent teeth with root resorption and provide esthetic satisfactory with time saved and cost reduced.

  • The molecular mechanisms control the function of PDL(periodonta1 ligament) cells and/or fibroblasts remain unclear. PDLsl7, PDL-specific gene, had previousely identified the cDNA for a novel protein from cultured PDL fibroblasts using subtraction hybridization between gingival fibroblasts and PDL fibroblasts. The purpose of this study was to determine the regulation by growth factors and cytokines on PDLsl7 gene expression in cultured human periodontal ligament cells and observe the immunohistochemical localization of PDLsl7 protein in various tissues of mouse. Primary PDL fibroblasts isolated by scraping the root of the extracted human mandibular third molars. The cells were incubated with various concentration of human recombinant $IL-1{\beta}$, PDGF-BB and TGF\;${\beta}$ for 48h nd 2 weeks. At each time point total RNA was extracted and the levels of transcription ere assessed by reverse transcription-polymerase chain reaction (RT-PCR assay). polyclonal antiserum raised against PDLsl7 peptides, CLSVSYNRSYQINE and SEAVHETDLHDGC, were made, and stained the tooth, periodontium, developing bone, bone marrow and mid-palatal suture of the mouse. The results were as follows. 1. PDLsl7 mRNA levels were increased in response to PDGF (10ng/ml) and $TGF\;{\beta}$(20ng/ml) after treatment of the $IL-1{\beta}$, PDGF-BB and $TGF{\beta}$for 48 h. 2. PDLsl7 was up-regulated only by $TGF{\beta}$(20 ng/ml) after treatment of the $IL-1{\beta}$, PDGF-BB and $TGF\;{\beta}$ for 2 weeks and unchanged by the other stimulants. 3. PDLsl7 was a novel protein coding the 142 amino acid peptides in the ORF and the nucleotide sequences of the obtained cDNA from RT-PCR was exactly same as the nucleotides of the database. 4. Immunohistochemical analysis showed that PDLsl7 is preferentially expressed in the PDL, differentiating osteoblast-like cells and stromal cells of the bone marrow in the adult mouse. 5. The expression of PDLsl7 protein was barely detectable in gingival fibroblasts, hematopoetic cells of the bone marrow and mature osteocytes of the alveolar bone. These results suggest that PDLsl7 might upregulated by PDGF-BB or $TGF{\beta}$ and acts at the initial stage of differentiation when the undifferentiated mesenchymal cells in the bone marrow and PDL differentiate into multiple cell types. However, more research needs to be performed to gain a better understanding of the exact function of PDLsl7 during the differentiation of bone marrow mesenchymal and PDL cells.

  • The purpose of this in vitro study was to evaluate the efficacy of self-etching primer which was developed to simplify the bonding procedures by measuring the shear bond strength and observing the interfacial morphology. 90 flat dentinal surfaces were prepared by grinding the buccal and lingual areas of caries-free human deciduous molars. After bonding of composite resin to sample surfaces according to the manufacturer's direction and thermocycling, shear bond strengths were measured using Universal testing machine(Instron). Another groups of specimens were treated by hydrochloric acid to secure the resin only and those tags were evaluated under SEM for their length and forms and the morphology of the bonding sites were also observed. The result as follows. 1. Group III showed higher shear bond strength than group I and II but no statistically significant difference was founded between group I and II(p>.05). 2. Adhesive failure was predominant in group II whereas dentin detachment was the main failure pattern in group I and III. 3. Relating long resin tags of $100-200{\mu}m$ were observed in samples of all groups under SEM. In group I, homogeneously long resin tags were arranged rather tight whereas rather loosely arranged resin tags of various length were found in group II. Lateral branching of resin tags was the characteristic finding observed in group III.

Bite Force and Lip Closing Force Measurement in Preschool Children (학령 전 어린이의 교합력과 구순 폐쇄력)

  • The aim of this study was to determine the bite force and lip closing force in preschool children and to analyze the correlation between these forces by age, height and weight, respectively. Data were obtained from 98 children (56 males, 42 females) ranging from 3 to 6 years of age. The magnitude of the bite force was measured bilaterally corresponding with the 2nd primary molars using a bite force gauge, $GM10^{(R)}$ (Nagano Keiki) and the force of lip closure was measured using LIP DE $CUM^{(R)}$ (Cosmo Instruments). The averages of bite force for boys and girls were 217.69 N and 205.05 N, respectively. The relationship between bite force with age, height, and weight present significant positive correlation (p < 0.001, p < 0.001, and p < 0.001, respectively). The averages of lip closing force for boys and girls were 4.81 N and 4.07 N respectively. The relationship between lip closing force with age, height, and weight present significant positive correlation (p < 0.001, p < 0.001, and p < 0.001, respectively). No significant differences were observed between boys and girls (p > 0.05) in both forces. A significant correlation was observed between lip closing force and bite force (p = 0.002).
