일부 농촌지역주민의 보건의식과 일차의료 이용 실태에 관한 조사 (A Study on the Awareness of Health and the Utilization of Primary Health Care in Rural Areas)

  • This study was to examine the awareness of health and the utilization of primary health care in rural areas. The data were obtained from self-administered questionnaire conducted 450 parents of Mi Gum md Su Dong middle school students in Nam yang Ju city, Kyung Gi-Do, Korea, from December 15 to 20 in 1993. The results were as follows : 1) Among the causes of disease, 'insufficient health care' was the highest(39.1%), and 'bad environment'(28.9%), 'complexity of life style'(17.8%) in next order. 2) In the priority between 'daily farm work' and 'primary health care', only 45.6% of respondents answered that primary health care is more important than the daily farm work. The 29.8% of respondents answered 'daily farm work', and the 23.1% answered 'the equal of the both'. 3) The 63.6% of the respondents recognized correctly, the meaning of primary health care. And the rate of information source in primary health care were 'TV and Radio'(42.2%), 'medical facilities'(23.3%), and 'newspaper and magazine'(11.3%) in order. 4) In the choice rate of medical facilities for primary health care, 'drug store' was the highest(34.9%), and 'local private clinic'(34.7%), 'health (sub)center'(15.8%), 'hospital'(10.2%) in next order. 5) The 53.5% of the respondents had experienced to visit the health (sub)center more than once. And the disfavorite reasons of health (sub)center were 'insufficient equipment'(36.7%), 'inavailable time to visit'(26.9%), and 'poor treatment'(9.1%). 6) Among the preference of the physicians for primary health care, 'specialist' was the highest rate of the respondents(54.2%), and 'general practitioner'(32.4%), and 'family doctor'(9.8%) in next order. The major obstacles in utilizing the medical facilities for primary health care were 'daily farm work'(41.6%), 'distance'(21.1%) and 'medical cost'(10.4%) in order. 7) The weakened reasons in health (sub)center function were 'insufficient medical equipment'(44.4%), 'the lack of resident's understanding for health (sub)center'(21.8%), and 'short thought of duty in health (sub)center personnel'(16.9%) in order.

일 보건진료소장의 수필에 나타난 지역사회간호사의 역할 (The Role of Community Health Nurse in Assay Written by a Nurse Practitioner of Primary Health Care Post)

  • Purpose: The purpose of this study was to understand the role of community health nurse through a nurse practitioner of primary health care post. Methods: An assay, 'Even if we were allowed to look at', written by a nurse practitioner of primary health care post was analyzed with the contents analysis method. Results: In the assay, we checked the following roles: client-oriented, delivery-oriented, and population-oriented roles described and classified by Clark. In particular, direct care such as in-patient care, home visiting nursing care, and drug prescription was frequently performed. Moreover, community health nurse has been listening, counselling, expressing sympathy, and advocating vulnerable elderly people economically and psychologically. Conclusion: The assay gave us a better understanding of the role of community health nurse, and we need more assays delineating the role of community health nurse in others setting as well primary health care post.

치매노인을 돌보는 가족원의 부양부담감과 가정간호요구도 (The Home Care Need and the Burden of a Primary Family Care Giver with Senile Dementia Patients)

  • This research was conducted to identify the following: the home care needs of patients with dementia and the burden on the primary family care giver: to provide basic data required to develop nursing intervention for the care giver: and to suggest recommendations for medical institutions and social services that could reduce the burden on the families of people suffering from dementia. subjects of this research were 53 patients of the two Public Health Centers of Cheju Province who are suffering from dementia and their families. The instrument used in the research was Kuen. Jung Don (1994)' s assessment tool of burden in the primary family care giver who has parents with senile dementia and Yoo. Young Mi(1998)'s assessment tool of home care need. modified by the researcher in the questionnaire by a Likert rating scale. The period of data collection was from February 8. 2000 to March 10. 2000. Collected data was analyzed by SPSS, using mean, standard deviation. ANOVA, t-test and Pearson correlation coefficient. The result of this research was that there was not a significant correlation between the burden on the care giver and the level of dementia, its duration, the patient's ability to perform daily tasks, the period of care giving. and the use of social services, although the lower the patient's ability to perform daily tasks. and the worse the care giver's own health situation, the higher the burden on the primary family care giver. The following suggestions are made based on the results of this research. 1. More than half of the subjects don't use social facilities and services. More publicity and referral efforts are needed about medical institutions. nursing institutions and other facilities that specialize in services for dementia sufferers and their families. 2. Nursing services should include intensive education for the primary care giver in the most important aspects of home care. 3. Further research should be done, and should include data from all parts of Cheju Province.

Erlotinib-Related Spontaneous Pneumothorax in Patient with Primary Lung Cancer

  • Spontaneous pneumothorax (SPTx) associated with primary lung cancer is quite rare, but has been reported as the initial presentation or a complication of disease progression. Moreover, chemotherapy-related SPTx in primary lung cancer occurs at a very low frequency, accounting for less than 0.05% of all cases. Here, we report the first case of erlotinib-related SPTx in a patient with advanced lung adenocarcinoma in Korea. After 3 cycles of cisplatin-based chemotherapy as first-line therapy, erlotinib was administered as second-line treatment. Asymptomatic SPTx accompanied by a significant decrease in tumor size was observed in the left lung 7 weeks later. The patient received continuous administration of erlotinib, without additional treatment. This case showed that SPTx can occur in patients with primary lung cancer receiving erlotinib, and asymptomatic chemotherapy-related SPTx in primary lung cancer may not require therapeutic intervention.

보건진료소 처방의약품 사용실태 및 개선안 (Prescription Drug Use in Primary Health Care Posts and Its Improvement Scheme)

  • Purpose: The prescription drug list for primary treatment by community health practitioners has been maintained for 30 years without any modification. Thus, this study will suggest an improvement scheme of prescription drug list for primary health care posts through an analysis of drug use in those posts. Methods: A questionnaire survey was implemented with community health practitioners from April to June in 2012. A total of 1,249 copies were analyzed. As for the databases of drug use in the integrated information, a total of 154,229 diagnoses selected in the method of stratified cluster sampling from 39 primary health care posts' data were analyzed. We consulted some experts about the prescription medication list, and referred to the medication information on-line home page for up-to-date drug information. Results: This study ultimately suggests 77 prescription drug items for primary health care posts by eliminating 35 items and replacing 1 item from the original list, and adding 4 items to it. Conclusion: This study will provide basic data for revising the prescription drug list in primary health care posts by periodically reflecting adverse effects in the existing drugs, demographic and environmental changes, and development of new drugs.

일차보건의료 역량 개발을 위한 지역사회 기반 참여형 국제보건사업 모델 개발: 에콰도르 일개 지역을 중심으로 한 사례연구 (Development of a Community-based Participatory Global Health Project Model for Primary Health Care Capacity Development: A Case Study from a Rural Community in Ecuador)

  • Purpose: The aims of this study were to identify successful strategies and propose a community-based participatory global health project model for primary health care capacity development. Methods: The study used case study methodology. A The unit of analysis was an international cooperation health project entitled "Community-based Primary Health Care Improvement in San Lorenzo, Ecuador" using community-based participatory research conducted in 2007~2008. Data were collected through windshield surveys, focus group discussion, key informant interviews, and provider surveys. Results: Identified successful strategies for the international cooperation health project were reciprocal partnership between researchers and community, partners' capacity building, south-to-south cooperation, and continuous monitoring and feedback. Community participation was found to be an essential tenet to guarantee the improvement of primary health care in the underserved rural community. Evidence from the activities of community health practitioners in Korea was applicable to the development of training programs for primary health care providers in Ecuador. Conclusion: Strategies for strengthening primary health capacity may be tailored depending on socio-cultural, political, and economical situations of each country. The model, however, would be applicable to the entire process of community-based global health projects in underserved rural communities of other countries.

Cancer Care Burden among Primary Family Caregivers of Iranian Hematologic Cancer Patients

  • Background: Providing care for hematologic cancer patients may lead to many negative complications in different aspects of life in their family caregivers. Based on a wide review of relevant literature, there are limited data about the burden of giving care for hematologic cancer patients on their primary family caregivers in Iran or other Middle Eastern countries. Therefore, the aim of this study was to investigate the cancer care burden on primary family caregivers of hematologic cancer patients, in terms of physical, psychological, social, spiritual, and financial aspects. Materials and Methods: In this descriptive study, 151 primary family caregivers of hematologic cancer patients referred to two cancer care centers in East Azerbaijan Province in northwest of Iran participated. The Financial Distress/Financial Well-being Scale, Hospital Anxiety and Depression Scale, Vaux Social Support Questionnaire, Spiritual Well-being Scale, and SF-36 were used for data collection. Data analysis was performed with SPSS software. Results: The findings of this study indicated that the primary family caregivers experience a high level of financial distress and a significant percentage of them suffered from anxiety and depression. In addition, the physical quality of life in these caregivers was moderate. On the other hand, spiritual health and social support of participants was at an acceptable level. Conclusions: Iranian primary family caregivers of hematologic cancer patients experience many problems in physical, psychological, and financial aspects of their life. Therefore, developing care plans for reducing these problems appears necessary.

조부모의 학령 초기 손자녀 대리양육 경험에 관한 질적 연구 (Listening to the Voices of Grandparents Raising Primary-Grade Grandchildren Using a Qualitative Study)

  • Objective: The present study examined the perspectives of grandparents raising their grandchildren in an attempt to better understand grandparents' child-rearing experience while providing kinship foster care to their primary-grade grandchildren. Methods: Data were collected through individual in-depth interviews with eight grandparents who have raised one or two primary-grade (ages 8-10) grandchildren using a qualitative approach. Results: First, the participants viewed the reason for their kinship foster care as a failure for caring for their own children and accepted the present grandparent-care provision as their responsibility. Second, the participants communicated constant struggles with their own health and grandchild-care as well as positive/negative emotions associated with the care provision. Third, most of the participants did not fully understand the developmental needs of their primary-grade grandchildren. Fourth, the participants articulated concerns for their primary-grade grandchildren's learning, peer interactions, school adjustment, and extra-curricular activities. Lastly, the participants all agreed on hoping to raise grandchildren with good personality traits as members of a society and to have them fill the gap from the loss of their parents. Conclusion: Although most participants accepted the current circumstances as their obligation, they still noted difficulties in child-care provision. Given the developmental needs of grandchildren and the resource needs of grandparents, proper and continuous intervention approaches should be developed/provided.

낙동강 유역 지역민의 간흡충 관련 지식, 태도 및 감염실태 (Knowledge, Attitude and Prevalence Status of Clonorchiasis for the Inhabitants in the Nakdong Riverside Area)

  • Purpose: This study was done to investigate the knowledge, attitude and prevalence status of Clonorchiasis Sinensis (C.S.) for inhabitants in the Nakdong riverside area. Method: Data were collected from December 16, 2008 to January 10, 2009. Participants were 552 people who responded to a questionnaire and had a stool examination. Results: The overall prevalence of C.S. was 13.8%. As for knowledge of C.S., the Clonorchiasis group had a mean score of 10.78 $({\pm}3.54)$ on a 17-point scale compared to a score of 10.58 $({\pm}4.15)$ for the negative group. The intended behavioral change related to risk of infection, according to presence of infection or not, was not significant but the Clonorchiasis group had a greater desire for behavioral change intention than the negative group. Conclusion: These results suggest that prevalence investigation of C.S. in all areas in the Nakdong riverside is needed and indicates that it is necessary to set up persistent and systematic management programs for patient care and prevention of C.S..

Primary Care Physicians' Cancer Screening Recommendation Practices and Perceptions of Cancer Risk of Asian Americans

  • Asian Americans experience disproportionate incidence and mortality rates of certain cancers, compared to other racial/ethnic groups. Primary care physicians are a critical source for cancer screening recommendations and play a significant role in increasing cancer screening of their patients. This study assessed primary care physicians' perceptions of cancer risk in Asians and screening recommendation practices. Primary care physicians practicing in New Jersey and New York City (n=100) completed a 30-question survey on medical practice characteristics, Asian patient communication, cancer screening guidelines, and Asian cancer risk. Liver cancer and stomach cancer were perceived as higher cancer risks among Asian Americans than among the general population, and breast and prostate cancer were perceived as lower risks. Physicians are integral public health liaisons who can be both influential and resourceful toward educating Asian Americans about specific cancer awareness and screening information.
