Although functional gastrointestinal disorders (FGIDs) are very common in pediatric patients, there is a scarcity of published epidemiologic data, characteristics, and management patterns from Saudi Arabia, which is the 2nd largest Arabic country in terms of area and the 6th largest Arabic country in terms of population, with 10% of its population aged <5 years. Functional constipation (FC) is an FGID that has shown a rising prevalence among Saudi infants and children in the last few years, which urges us to update our clinical practices. Nine pediatric consultants attended two advisory board meetings to discuss and address current challenges, provide solutions, and reach a Saudi national consensus for the management of pediatric constipation. The pediatric consultants agreed that pediatricians should pay attention to any alarming signs (red flags) found during history taking or physical examinations. They also agreed that the Rome IV criteria are the gold standard for the diagnosis of pediatric FC. Different therapeutic options are available for pediatric patients with FC. Dietary treatment is recommended for infants with constipation for up to six months of age. When non-pharmacological interventions fail to improve FC symptoms, pharmacological treatment with laxatives is indicated. First, the treatment is aimed at disimpaction to remove fecal masses. This is achieved by administering a high dose of oral polyethylene glycol (PEG) or lactulose for a few days. Subsequently, maintenance therapy with PEG should be initiated to prevent the re-accumulation of feces. In addition to PEG, several other options may be used, such as Mg-rich formulas or stimulant laxatives. However, rectal enemas and suppositories are usually reserved for cases that require acute pain relief. In contrast, infant formulas that contain prebiotics or probiotics have not been shown to be effective in infant constipation, while the use of partially hydrolyzed formula is inconclusive. These clinical practice recommendations are intended to be adopted by pediatricians and primary care physicians across Saudi Arabia.

Objectives: Tic disorders are highly heritable; however, growing evidence suggests that environmental factors play a significant role in their pathogenesis. Studies on these factors have been inconsistent, with conflicting results. Therefore, this study aimed to examine the associations of pre- and perinatal exposure to Tourette syndrome (TS) or chronic tic disorders (CTD) in Korean school-aged children. Methods: This case-control study used data from a large prospective cohort study. The primary outcome was TS/CTD diagnosis according to the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5) criteria and Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version-Korean Version. Demographic, pre-, and perinatal information was obtained from the maternal questionnaires. Data between the TS/CTD and control groups were compared using the chi-squared or Student's t-test, as appropriate. Two-step logistic regression analyses were used to test the association between TS/CTD and pre- and perinatal risk factors. Results: We included of 223 children (78 with TS/CTD and 145 controls). Significant differences in the demographic data between the two groups were observed. The male sex ratio, mean parental age, parental final education level, and family history of tics were included as confounders. In the final adjusted multivariable model, TS/CTD was significantly associated with antiemetic exposure during pregnancy (odds ratio [OR]=16.61, 95% confidence interval [CI] 1.49-185.22, p=0.02) and medically assisted reproduction (OR=7.89, 95% CI 2.28-27.28, p=0.01). Conclusion: Antiemetic exposure and medically assisted reproduction are significantly associated with the risk of TS/CTD. These results should be replicated in future prospective and gene-by-environment studies.
본 연구는 조산아 유치의 발육성 법랑질 결함을 정량화하는 평가 기준을 제시하고, 조산아의 재태기간, 출생 시 체중, 다양한 합병증 및 치료 이력에 따른 발육성 법랑질 결함의 심각도를 조사하고자 하였다. 조산아의 신생아집중치료실 입원 및 퇴원 기록을 후향적으로 평가하여 출생 정보, 합병증 진단 여부, 비경구 영양 및 기관내 삽관 기간을 조사하였다. 기존의 발육성 법랑질 결함 평가지표를 수정하여 Preterm Developmental Defects of Enamel (PDDE) index를 고안하였으며, 평가자는 기준에 맞추어 조산아 유치의 발육성 법랑질 결함을 법랑질 저광화와 저형성으로 구분하여 점수화하였다. 재태기간 28주 미만, 출산 시 체중 1000 g 미만인 군의 PDDE score는 유의하게 증가하였다. 기관지폐이형성증, 구루병, 뇌실내출혈, 괴사성 대장염 진단 이력이 있는 군, 기관내 삽관 일수가 50일 이상인군, 비경구영양 일수가 20일 이상인 군 역시 PDDE score를 유의하게 증가시켜 발육성 법랑질 결함의 위험인자임을 확인하였다. 본 연구는 조산아 발육성 법랑질 결함 위험인자를 파악하여 조산아의 구강 상태를 예측하고 설명하기 위한 기초 정보를 제공할 수 있을 것이다.
이 연구는 노화로 인해 나타날 수 있는 다양한 문제들 중 노인의 주거안전에 영향을 미치는 주거 요인이 무엇인지 알아보고자 실시되었다. 연구 진행은 첫 번째 단계로, 실제 65세 이상 노인 6명을 대상으로 포커스 그룹 인터뷰를 진행하였으며, 두 번째 단계로는, 국 내외 문헌고찰을 통해 확인된 노인 주거 안전에 영향을 미치는 요인과 포커스 그룹 인터뷰를 통해 확인된 요인을 바탕으로 설문을 구성하여 전문가에게 델파이 조사를 시행하였다. 델파이 조사에서는 전문가들의 의견을 수집하여 주거안전에 영향을 미치는 구성 요인에 대해 확인하는 과정을 반복하였으며, 구성된 요인에 대해 내용타당도 지수(Content Validity Index: CVI)를 통해 검증하도록 하였다. 연구 결과, 노인의 주거안전에 영향을 미치는 요인은 실외, 현관, 실내, 거실, 침실, 부엌, 욕실 및 화장실 총 7개 영역에 23개 하위 요인을 확인 및 추출할 수 있었다. 이 중 가장 많은 언급과 중요도를 보인 요인으로는 실내-외 영역의 계단 유무, 안전한 바닥, 실내 영역의 문턱 요인으로 나타났다. 따라서 노인이 기존의 주거지에서 안전하게 생활하기 위해서는 위의 요인들의 확인이 필수적이라고 할 수 있다.
Objectives: To evaluate the effectiveness of an integrated medical service model for a fibromyalgia patient and their caregiver, focusing on reducing pain and improving quality of life. Methods: A single-case study design was employed, involving a fibromyalgia patient and their primary caregiver treated at W University Hospital. The integrated medical service program, based on the Ministry of Health and Welfare's model, included medical consultations and complementary therapies such as psychological counseling, art therapy, music therapy, horticultural therapy, yoga, and meditation. The program was conducted weekly for 8 weeks, with each session lasting up to 100 minutes. Data collection involved both quantitative and qualitative assessments. Quantitative data included demographic surveys, psychological tests, health-related quality of life measures, pain indices, and sleep quality indices. Qualitative data were gathered through feedback evaluations and emotional assessments. Results: The patient showed improvements in mobility, self-care, daily activities, and anxiety/depression, with EQ-VAS scores increasing from 20 to 40 and pain perception decreasing from 67.41 to 42.58. The caregiver reported reduced anxiety/depression and an increase in EQ-VAS scores from 95 to 98. Both patient and caregiver exhibited emotional changes, with decreased depression and increased happiness. However, the patient showed an increase in fear and anger. Conclusions: The integrated medical service model positively impacted the emotional and psychological well-being of the fibromyalgia patient and their caregiver. Despite the limitations of a small sample size and a single-case study design, the findings suggest that an integrated approach can be beneficial. Larger-scale studies are needed to confirm and generalize these results.
목적 악성 종양의 완전 관해 후 발생한 흉강 내 림프절병증 환자에서 사르코이드증 유사 반응을 시사할 수 있는 영상의학적 특징을 알아보고자 한다. 대상과 방법 이 연구는 악성 종양의 완전 관해 상태에서 발생한 조직병리학적으로 확인된 사르코이드증 유사 반응을 보인 5명의 환자의 임상적 특징과 CT 및 18F-fluorodeoxyglucose(이하 FDG) PET/CT 소견을 분석하였다. 결과 기저 악성 종양으로는 유방암, 비인두암, 자궁내막암 및 림프종이 포함되며. 악성 종양의 완전 관해와 사르코이드증 유사 반응 진단 사이의 시간 간격은 6~78개월이었다. CT 소견으로는 양측 폐문 및 종격동 림프절병증(n = 5), 기관지혈관주위, 엽간열주위 또는 흉막하 폐결절(1~15 mm) 및 소엽내 간질비후가 포함되었다(n = 4). 18F-FDG PET/CT는 흉강 외 FDG 흡수 없이 양측 폐문 및 종격동 림프절의 대사항진을 나타냈다(n = 3). 모든 환자에서 코르티코스테로이드 치료 후 사르코이드증 유사 반응이 호전되었다. 결론 악성 종양의 완전 관해를 달성한 환자에서, 원발성 종양의 재발과 흉강 외 원격 전이 없는 상태에서 새로 발견된 양측 폐문 및 종격동 림프절병증은 림프관주위 폐결절의 유무에 관계없이 사르코이드증 유사 반응의 가능성을 시사할 수 있으며 불필요한 전신 화학요법을 예방하기 위해 림프절의 조직병리학적 확인이 필요하다.
본 연구는 발달장애아동을 대상으로 케어로봇을 치료에 활용하기 위해 발달장애 아동의 부모를 대상으로 소셜로봇이 갖추어야 할 기능과 요구 등을 분석하고자 시행한 질적연구이다. 연구대상자는 자녀가 발달장애 진단을 받았거나 1년 이상 특수교육을 시행하고 있는 만 18세 미만 자녀를 둔 주양육자 11명을 대상으로 진행하였다. 면담질문지는 반구조화된 질문지로 구성하여 소셜로봇에 대한 인식, 필요성, 기대, 염려 및 기타 의견 등으로 구성하였다. 면담은 COVID 19로 인해 비대면 온라인 화상 면담으로 이루어졌다. 연구결과, 4개의 주제가 도출되었다. 4개의 주제는 낯설지만 곧 마주할 현실, 능동적인 기능이 탑재된 소셜로봇, 소셜로봇의 외형과 재질에 대한 바람 및 소셜로봇에 대한 염려로 도출되었다. 본 연구를 통하여 양적연구에서는 파악할 수 없었던 주양육자의 소셜로봇에 대한 기대, 제안 등을 파악할 수 있었고 이를 향후 개발할 소셜로봇에 반영할 수 있다면 보다 수용성이 높은 긍정적인 결과를 도출 할 수 있을 것으로 기대한다.
연구목적: 연구자는 아동 치과 주치의 프로그램에서 필수 서비스와 중요도, 검진 주기, 치과위생사의 역할을 알아보고자 한다. 연구방법: 최종연구대상자는 치과위생사, 치과의사, 치위생과 학생으로 총 124명이고, 자기기입식 설문지를 작성했다. 수집된 자료는 독립 t-검정, ANOVA로 일반적인 특징에 따라 그 차이를 비교했고, 이들의 관계는 Pearson 상관관계분석으로 분석했다. 연구결과: 검진 대상자에게 구강 검사와 방사선사진 촬영 등은 꼭 필요한 검사항목이고, 칫솔질과 구강용품 사용, 정기 검진의 중요성을 교육해야 하며, 대구치 실런트와 치면세마와 같은 예방 처치를 제공해야 한다고 생각했다. 또한 치료 서비스에서는 광중합형 레진과 GI 충전이 필요하다고 응답했다. 검진 주기를 치료보다는 교육과 예방을 더 짧게 해야 한다고 생각했다. 치위생과 학생이 치과의사와 치과위생사보다 검진, 교육, 예방, 치료 서비스가 더 중요하다고 응답했다. 구강검진과 교육 및 치료 간의 상관관계가 높았고, 필수 서비스와 중요성도 정(+)의 상관관계를 가졌다. 결론: 필수적이고 중요하다고 생각하는 구강 검사, 교육, 예방, 치료 서비스를 치과주치의 프로그램으로 어린이에게 제공해야 한다. 구강보건 교육과 예방관리는 치료보다 더 자주 제공되어야 하고, 그 중요성을 치위생과 학생뿐만 아니라 서비스의 주체인 치과위생사와 치과의사에게도 강조할 필요가 있다.
Objective: The purpose of our study was to investigate the predictive abilities of clinical and computed tomography (CT) features for outcome prediction in patients with coronavirus disease (COVID-19). Materials and Methods: The clinical and CT data of 238 patients with laboratory-confirmed COVID-19 in our two hospitals were retrospectively analyzed. One hundred sixty-six patients (103 males; age 43.8 ± 12.3 years) were allocated in the training cohort and 72 patients (38 males; age 45.1 ± 15.8 years) from another independent hospital were assigned in the validation cohort. The primary composite endpoint was admission to an intensive care unit, use of mechanical ventilation, or death. Univariate and multivariate Cox proportional hazard analyses were performed to identify independent predictors. A nomogram was constructed based on the combination of clinical and CT features, and its prognostic performance was externally tested in the validation group. The predictive value of the combined model was compared with models built on the clinical and radiological attributes alone. Results: Overall, 35 infected patients (21.1%) in the training cohort and 10 patients (13.9%) in the validation cohort experienced adverse outcomes. Underlying comorbidity (hazard ratio [HR], 3.35; 95% confidence interval [CI], 1.67-6.71; p < 0.001), lymphocyte count (HR, 0.12; 95% CI, 0.04-0.38; p < 0.001) and crazy-paving sign (HR, 2.15; 95% CI, 1.03-4.48; p = 0.042) were the independent factors. The nomogram displayed a concordance index (C-index) of 0.82 (95% CI, 0.76-0.88), and its prognostic value was confirmed in the validation cohort with a C-index of 0.89 (95% CI, 0.82-0.96). The combined model provided the best performance over the clinical or radiological model (p < 0.050). Conclusion: Underlying comorbidity, lymphocyte count and crazy-paving sign were independent predictors of adverse outcomes. The prognostic nomogram based on the combination of clinical and CT features could be a useful tool for predicting adverse outcomes of patients with COVID-19.
Background/Aims: Radiofrequency treatment of the gastroesophageal junction using the Stretta procedure for treating gastroesophageal reflux disease (GERD) is observed to improve the symptoms and proton pump inhibitor (PPI) dependence and reduce the need for anti-reflux operations. As one of the largest studies in Europe, we evaluated the clinical outcomes of Stretta in patients with medically refractory GERD. Methods: A tertiary UK center evaluated all patients with refractory GERD who underwent Stretta between 2014 and 2022. Patients and primary care professionals were contacted to obtain information regarding the initiation of PPI and reintervention after Stretta. Results: Of the 195 patients (median age, 55 years; 116 women [59.5%]) who underwent Stretta, PPI-free period (PFP) data were available for 144 (73.8%) patients. Overall, 66 patients (45.8%) did not receive PPI after a median follow-up of 55 months. Six patients (3.1%) underwent further interventions. The median PFP after Stretta was 41 months. There was a significant negative correlation between PFP and age (p=0.007), with no differences between sexes (p=0.96). Patients younger than 55 years of age had a longer PFP than their older counterparts (p=0.005). Younger males had a significantly longer PFP than older males (p=0.021). However, this was not observed in the female cohort (p=0.09) or between the younger men and women (p=0.66). Conclusions: Our findings suggest that Stretta is a safe and feasible option for treating refractory GERD, especially in younger patients. It prevents further anti-reflux interventions in most patients and increases the lead-time to surgery in patients with refractory GERD.
