Biological assessment of a new ready-to-use hydraulic sealer

  • Objectives: This study compared the cytotoxicity, biocompatibility, and tenascin immunolabeling of a new ready-to-use hydraulic sealer (Bio-C Sealer) with MTA-Fillapex and white MTA-Angelus. Materials and Methods: L929 fibroblasts were cultivated and exposed to undiluted and diluted material extracts. Polyethylene tubes with or without (the control) the materials were implanted into the dorsa of rats. At 7 days and 30 days, the rats were euthanized, and the specimens were prepared for analysis; inflammation and immunolabeling were measured, and statistical analysis was performed (p < 0.05). Results: MTA-Fillapex exhibited greater cytotoxicity than the other materials at all time points (p < 0.05). The undiluted Bio-C Sealer exhibited greater cytocompatibility at 6 and 48 hours than white MTA-Angelus, with higher cell viability than in the control (p < 0.05). White MTA-Angelus displayed higher cell viability than the control at 24 hours, and the one-half dilution displayed similar results at both 6 and 48 hours (p < 0.05). At 7 days and 30 days, the groups exhibited moderate inflammation with thick fibrous capsules and mild inflammation with thin fibrous capsules, respectively (p > 0.05). At 7 days, moderate to strong immunolabeling was observed (p > 0.05). After 30 days, the control and MTA-Fillapex groups exhibited strong immunolabeling, the white MTA-Angelus group exhibited moderate immunolabeling (p > 0.05), and the Bio-C Sealer group exhibited low-to-moderate immunolabeling, differing significantly from the control (p < 0.05). Conclusions: Bio-C Sealer and white MTA-Angelus exhibited greater cytocompatibility than MTA-Fillapex; all materials displayed adequate biocompatibility and induced tenascin immunolabeling.

Study on development of DNA probe for identification of Prevotella intermedia G8-9-3 (Prevotella intermedia G8-9K-3을 동정할 수 있는 DNA 프로브의 개발에 관한 연구)

  • The purpose of this study is to develop species-specific DNA probe for detection and identification of Prevotella intermedia (P. intermedia) G8-9K-3. This study procedure includes (1) whole-genomic DNA extraction of P. intermedia G8-9K-3 (2) construction of the genomic DNA library, (3) screening of strain-specific DNA probe by reverse dot hybridization, (4) confirmation of strain-specific DNA probe by Southern blot hybridization, (5) determination of nucleotide sequences of strain-specific DNA probe. Twenty-eight recombinant plasmids containing Hind III-digested DNA fragments of P. intermedia G8-9K-3 were obtained. Reverse Dot Hybridization and Southern blot analysis data showed that one of them, Pig3, could be P. intermedia G8-9K-3-specific DNA probe. This datum indicates that this Pig3 DNA probe could be useful in detection and identification of the P. intermedia G8-9K-3 strain.

Current status of the types of dental filling and preventive materials among children in mixed dentition (혼합치열기 어린이의 치아우식 예방 및 충전 재료 현황에 관한 조사)

  • The purpose of this study was to examine current status of the types of dental filling materials and preventive materials among Korean children in mixed dentition. Seven elementary schools were randomly selected from four metropolitan regions in South Korea. The total of 711 children aged 9-10 years old were examined with their parents’ informed consents. Oral examination was conducted by one trained dentist. The mean number of dfs was $7.9{\pm}8.0$ ($ds=1.3{\pm}2.5,\;fs=6.6{\pm}7.6$) and DMFS was $1.4{\pm}2.2$ ($DS=0.2{\pm}0.8,\;FS=1.1{\pm}2.1$). The average of filled surfaces using amalgam, composites, sealants, and others were $2.8{\pm}3.9,\;1.6{\pm}2.8,\;2.9{\pm}3.2$, and $4.5{\pm}7.0$, respectively. The use of dental filling material according to region, the amalgam was the highest in Daegu and the resin was the highest in Pusan. In permanent teeth, about 70% of total filling surfaces had sealant and about 20% was resin fillings, and the proportion of amalgam filling surfaces was less than 10%.

Development and implementation of national competency standard: based on preventive dentistry and practice (일개대학 National Competency Standard (NCS) 교과목 운영에 관한 연구: 예방치학 및 실습 교과목 중심으로)

  • Objective: The purpose of the study is to investigate the curriculum development and operation based on national competency standard (NCS). Methods: The duty of the dental hygienist was analyzed based on DACUM by ten experts in January, 2011. The duty model of the dental hygienist was inspected after duty analysis. The subjects of choice were preventive dentistry and practice. The satisfaction with the subjects were carried out from March to June, 2015. Results: The duty analysis of dental hygienist by DACUM produced preventive dental treatment(11 tasks), oral health education(3 tasks), comprehensive dental hygiene treatment(6 tasks) and 12 categories(156 tasks). Preventive dental treatment was divided into preventive dentistry and practice, oral health education was changed into oral health education and practice, and comprehensive dental hygiene treatment was replace by comprehensive dental hygiene and practice. The contents of preventive dentistry and practice included outline, learning objective, related knowledge and self evaluation. Professional evaluation required mutual experience and evaluation of the students. The mutual evaluation of the students was $4.61{\pm}0.506$(dental plaque control) and $1.80{\pm}0.316$(tooth brushing). The professional evaluation was $1.73{\pm}0.274$(dental plaque control) and $1.60{\pm}0.322$(tooth brushing)(p<0.01). The satisfaction with preventive dentistry and practice was $4.61{\pm}0.506$(improvement in practical work ability), $4.58{\pm}0.511$(knowledge improvement) and $4.55{\pm}0.572$(NCS educational environment) in order. Conclusions: The operation of NCS curriculum is considered to improve practical work ability and to solve skill mismatch between dental industries and educational training institutions.

Application of the QLF technology to monitor recovery rates of enamel caries lesions with human saliva (법랑질 병소 회복율 평가를 위한 QLF 기술의 적용)

  • Purpose: The aim of this in vitro study was to assess changes in remineralization by stimulated human saliva over a short period of 48 hours with quantitative light-induced fluorescence (QLF) technology. Materials and Methods: Bovine incisor surfaces were demineralized for 10 days. Two types of stimulated saliva were collected from 7 healthy persons. 24 hours after tooth brushing (Stimulated saliva group) and immediately after tooth brushing with 1,000 ppm NaF dentifrice (Dentifrice saliva group). The specimens were immersed in saliva and fluorescence images were obtained by QLF-digital (QLF-D $biluminator^{TM}$,) at 2, 4, 6, 12, 24, and 48 hours fluorescence loss (${\Delta}F%$) of the lesions. A paired t-test was performed to assess fluorescence differences between before (${\Delta}F_{baseline}$) and after (${\Delta}F_{treatment\;time}$) the remineralization process. Results: Before the remineralization, the mean ${\Delta}F_{baseline}$ of the initial demineralized specimens was $-18.42{\pm}0.15$ (%). In both groups, the ${\Delta}F$ values obtained at baseline and after 2 hours were statistically significant (P < 0.001), indicating recovery of the lesions by approximately 40% after 2 hours. After 48 hours, remineralization rates were slightly higher (49%) for the stimulated saliva group than for the dentifrice saliva group (41%), but the difference was not statistically significant. Conclusions: With QLF minute degrees of remineralization by saliva can be measured in periods as short as 2 hours. Additionally no significantly higher effects of remineralization were observed in the dentifrice saliva group when compared to the stimulated saliva group.

  • Dental caries is multifactorial local disease which involves destruction of the hard tissues of the teeth by metabolities produces by microorganisms. Recently, there has been growing interest in the role of nitric oxide (NO) and glutathione (GSH) in caries incidence. The aim of the study was to survey the studies reported the association among salivary NO, GSH and dental careis. Three studies reported the association between NO and dental caries. However, the results were contradictory. Only one study showed negative association between GSH and dental caries. In Korea, NO showed negative association with Lactobacilli and GSH showed positive association with dental caries. These observations suggest the possibility that NO and GSH could be new biomarkers for dental caries. However, further study should be needed.

Effect of vibration during local anesthesia administration on pain, anxiety, and behavior of pediatric patients aged 6-11 years: A crossover split-mouth study

  • Background: Uncooperative behavior of children due to dental anxiety may interfere with the effective delivery of dental care and compromise the quality of treatment provided. Injection of local anesthesia is one of the most anxiety-inducing stimuli in pediatric dentistry. This study aimed to compare the efficacy of a child-friendly device, having a combined effect of vibration and distraction, with the conventional method of injection on pain, anxiety, and behavior of pediatric patients aged 6-11 years. Methods: This randomized, crossover, split-mouth study included 30 children requiring a bilateral inferior alveolar nerve block. The children were equally divided into two groups: group 1, aged 6-8 and group 2, aged 9-11 years. All children were injected with anesthesia using the conventional and device method in two separate sessions. They were assessed for anxiety by measuring the pulse rate before and during the administration of local anesthesia. Behavior was assessed using Faces, Legs, Activity, Cry, Consolability (FLACC) scale, and the child's experience while receiving anesthesia was assessed using the Wong Bakers Pain Rating Scale. Results: Results showed that the children who received local anesthesia using the device method had a lower mean pulse rate, FLACC scores, and pain rating scores than those who received local anesthesia using the conventional method. Conclusion: The device method was more effective than the conventional method in managing pain, anxiety, and behavior of patients aged 6-11 years. The device is a cost effective, simple, and child-friendly product for administrating local anesthesia in pediatric patients.

Evaluating anxiety levels and pain perception while administering local anesthesia using conventional, insulin, and deception syringes in 6-12-year-olds

  • Background: Injections are one of the most fear-provoking stimuli in dentistry. Painless administration of an injection is a vital step in alleviating anxiety, which in turn leads to good behavior in children. The aim is to evaluate and compare anxiety levels and pain perception using conventional, insulin, and deception syringes during the administration of local anesthesia in children. Methods: Forty-five children aged 6-12 were selected using a standard sample size formula and equally divided into three groups. Local anesthesia was administered using a conventional syringe to Group A participants and an insulin syringe for Group B participants. Group C participants were administered local anesthesia using a deception syringe by showing the patient disposable obturation tips, and eventually a conventional syringe was used for administration of local anesthesia after hiding them from the patient. Anxiety levels were assessed using Venham's Picture Scale and pulse rate at baseline and after administration of local anesthesia. The Wong-Baker Faces Pain Rating Scale was used to assess pain perception after the administration of local anesthesia. Results: Insulin and deception syringes showed better reduction in anxiety levels and pain perception than conventional syringes, demonstrating a high statistically significant difference. Conclusion: The use of insulin and deception syringes for administration of anesthesia was demonstrated to be effective in alleviating anxiety in children and is therefore recommended as an alternative to conventional syringes.
