포스트 코로나 시기 농촌 거주 노인의 호흡기감염 예방 프로그램 개발: 보건진료 전담공무원 대상 델파이 기법 (Development of a Respiratory Infection Prevention Program for the Rural Elderly in the Post COVID-19 Era: A Study Using Delphi Method of Community Health Practitioners)

  • Purpose: The purpose of this study was to develop a respiratory infection prevention program for the rural elderly in the post-coronavirus disease 2019 (COVID-19) era. Methods: The Delphi method was used to validate the contents of the program. Two rounds of Delphi surveys and one individual interview were conducted on four subjects and 16 categories with nine experts. Content validity was calculated using the content validity ratio (CVR) and coefficient of variation (CV). Results: This study verified the content validity of the existing program components, such as respiratory infection prevention characteristics, cough etiquette, correct hand washing, oral hygiene, correct tooth brushing, and exercise by walking. The study comprised 28 categories covering seven subjects, including the provision of knowledge and information about COVID-19, environmental management for respiratory infection prevention, and exercise training for immunity enhancement. Conclusion: This Delphi study examined the respiratory infection prevention program that was redesigned for the post-COVID-19 era and confirmed the validity of the educational contents. The findings of this study suggest that the program can be used practically for the prevention of respiratory infection among the rural elderly.

  • Purpose: The purpose of this study was to investigate the effects of emergency setting visitors' knowledge and behavior patterns in relation to prevention of respiratory tract infections. Methods: A descriptive survey was used. The participants were visitors to the emergency service in 'D' general hospital in 'D' city, and the data were collected from July 1 to September 1, 2016. The collected data were analyzed using frequency analysis, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and multiple regression analysis. Results: Factors influencing prevention of respiratory tract infections were visitors' education level, methods of dissemination of prevention information, and participants' knowledge with regard to preventive methods. The explanatory power was found to be 35% in the regression model. Conclusion: The findings indicate that visitors' education level, knowledge of infection prevention, and the dissemination of information regarding infection prevention by the hospital play an important role in the prevention of respiratory tract infections in emergency services in the hospital. These results highlight the need for a customized education program for prevention of respiratory tract infections in emergency settings. Programs should take into consideration the educational background of visitors, and provide them with appropriate information regarding infection prevention.

Tick-Borne Pathogens in Ixodid Ticks from Poyang Lake Region, Southeastern China

  • Ticks are the vectors of various pathogens, threatening human health and animal production across the globe. Here, for the first time we detected Ricketssia spp., Borrelia spp. and protozoan in ticks from Poyang Lake region in Jiangxi Province of eastern China. In 3 habitat categories and on 12 host species, 311 ticks from 11 species were collected. Haemaphysalis longicornis was the predominant species, accounting for 55.63%, followed by Rhipicephalus microplus, Haemaphysalis flava and Ixodes granulatus. Of the collected ticks, 7.07% were positive for tick-borne pathogens, and H. longicornis and H. flava were found to be co-infected with Ricketssia spp. and protozoan. H. flava was the most detected positive for tick-borne pathogens, whereas H. longicornis had the lowest infection rate, and the difference in infection rates between tick species was significant (${\chi}^2=61.24$, P<0.001). Furthermore, adult ticks demonstrated remarkably greater infection rate than immature ticks (${\chi}^2=10.12$, P=0.018), meanwhile ticks on Erinaceidae showed significantly higher positivity than ticks collected on other host species (${\chi}^2=108.44$, P<0.001). Genetic fragment sequencing and analyses showed at least 4 pathogen species presence in ticks, namely Borrelia yangtzensis, Rickettsia slovaca or Rickettsia raoultii related genospecies, Babesia vogeli and Hepatozoon canis or Hepatozoon felis related genospecies. The finding indicates that the abundant ticks can carry diverse pathogens in Poyang Lake region, and pathogen infection is highly related to species, vertebrate hosts and life stages of ticks.

새터민의 B형간염 인식과 지식에 관한 연구 (Awareness and Knowledge about Hepatitis B among North Korean Immigrants)

  • Purpose: The purpose of this study was to identify awareness and knowledge about hepatitis B virus (HBV) infection among North Korean immigrants who had resided in South Korea. Methods: This was a cross-sectional, community based study. Data were collected from a convenience sample of 198 North Korean immigrants aged 18 years old through 65 years old among those who had attended community centers, local churches, and gathering places in four cities. The questionnaire included 15 items about participants' knowledge on HBV infection and its consequences, mode of transmission, and preventive measures. Results: The mean score of HBV knowledge was $6.3{\pm}3.8$ (range 0~15). The items that over 60% of the respondents checked correct answer regarding knowledge about HBV were 'There is blood screening for hepatitis B infection', 'Hepatitis B can be transmitted through sharing needles with infected person'. Multivariate analysis showed that received education for the prevention of HBV infection and disease morbidity were significantly associated with the knowledge of HBV infection. Overall, approximately 11% of total variability in the knowledge of HBV infection could be explained by the 2 variables in this model ($R^2=.109$, p<.001). Conclusion: The results showed that North Korean immigrants need more education on HBV infection and prevention to improve their knowledge about HBV.

안치실 및 염습실 종사자의 감염예방 가이드라인 연구 (Prevention Guidelines for Infection-associated funeral directors in Korea)

  • 미국 등 선진국에서는 장례종사자들의 질병감염에 대한 위험성과 감염예방의 중요성이 심도있게 연구되어 이에 관련된 가이드라인이 제시되고 있으나, 국내에서는 이에 대한 연구가 미비하다. 국내에서 장례종사자의 건강관리는 장례업자나 종사자 개인의 노력에 전적으로 의존하기 때문에 매우 열악하며, 장례종사자의 감염에 대한 연구 및 역학 조사 등이 매우 필요하다고 판단된다. 본 연구에서는 선진국의 장례지도사 가이드라인을 참고하여 노출가능성이 높은 질병, 개인위생, 물질적 및 공간적 관리에 대한 한국형 장례종사자 감염관리 가이드라인 모형을 제시하여 장례식장내 효율적 감염관리 및 장례종사자의 감염예방을 이루고자 하였다.

  • Objectives : Dochim have been recorded in Korean medical classics as a diversified instrument for acupuncture to treat patients with various symptoms. Recently, there is a lot of Korean Medicine Doctors are increasingly using Dochim. However a study on using Dochim is a lack of infection control. Methods : We investigated about the existing infection about Dochim. Based on investigated information we presented methods for infection control during Dochim therapy. Results & Conclusions : Prevent infection during Dochim therapy performed at a primary consideration, and the prevention of infection shoould be treated.

수술실 간호사의 혈액매개감염 관련 지식, 위험지각과 감염예방행위 (Perception on and Behaviors for Blood-Borne Infection Prevention among Operating Room Nurses)

  • Purpose: The purpose of this study was to identify the factors related to blood-borne infection prevention behaviors based on the risk perception of the health belief model among operating room nurses. Risk perception factors included perceived susceptibility, barriers, benefits, and perceived severity. Methods: Data were collected from 121 operating room nurses working in four different hospitals in Daejeon and Seoul from June 30 to May 11, 2016. Results: The mean age was 31.2 years, and the average years of clinical experience in operating room was 7.9 years. The mean score of knowledge was 13.15. The mean score of perceived susceptibility, barrier, benefit, and perceived severity were 3.76, 3.70, 3.95, and 4.64, respectively. Blood-borne infection prevention behaviors had positive correlation with perceived benefits (p=.010), but negative correlation with sensitivity (p=.009) and barrier (p=.012). The hierarchical regression model on infection prevention behavior was statistically significant (F=4.85, p<.001). The sixteen percent of variance in behavior was explained by age (${\beta}=.18$, p=.038), perceived benefit (${\beta}=.20$, p=.030), perceived susceptibility (${\beta}=-.25$, p=.005), and perceived barrier (${\beta}=-.18$, p=.042). Conclusion: In order to increase infection prevention behaviors among operating room nurses, there is a need for developing specific education program focusing on appropriate management of equipment, instruments, and environment in operating room. In addition, support from the hospital organization level need to be provided as well.

Prevalence of Toxoplasma gondii Infection in Household Cats in Korea and Risk Factors

  • Several epidemiological surveys have reported the prevalence of Toxoplasma gondii infection in stray cats in Korea, but little information is available on T. gondii infection in household cats. The aim of the present study was to assess the prevalence and risk factors of T. gondii infection among household cats reared in Seoul, Korea. A total of 474 blood samples were collected from clinically healthy household cats. All samples were tested using ELISA and PCR. The risk factor analysis was based on a questionnaire filled out by the owners. The overall positive rate for ELISA and PCR assays was 2.2% (10/437) and 2.1% (10/474), respectively. With regard to the origin of cats, the positive rates among cats adopted from the animal shelter and veterinary clinic for stray cats were significantly different (P<0.05). Our study demonstrated that the positive rate of T. gondii infection in household cats was low and that this low prevalence was assumed to be associated with keeping the cats indoors and restriction of eating raw food and uncooked meat. Therefore, we suggest that the owners check the origin of the cats prior to adoption to prevent infection of other animals, including humans.

항암화학요법을 받는 유방암 환자의 극복력 구조모형 (The Structural Equation Model on Resilience of Breast Cancer Patients Receiving Chemotherapy)

  • Purpose: The purpose of this study was to construct and test a structural equation model on resilience of breast cancer patients receiving chemotherapy. Methods: Participants were 204 patients with breast cancer who received chemotherapy treatment. They participated in a structured interview, which included social support, depression, symptom experience, self-efficacy, hope, resilience, and infection prevention behaviors. Data were analyzed using SPSS/WIN 20.0 and AMOS 18.0. Results: Lower depression (${\gamma}=-.33$, p=.020) and symptom experience (${\gamma}=-.31$, p=.012) and higher self-efficacy (${\gamma}=.32$, p=.005) and hope (${\gamma}=.48$, p=.016) were influenced by higher social support. Greater resilience was influenced by lower symptom experience (${\beta}=-.18$, p=.016), higher self-efficacy (${\beta}=.49$, p=.023), and higher hope (${\beta}=.46$, p=.012), and these predictors explained 66.7% of variance in resilience. Greater resilience (${\beta}=.54$, p =.009) made an impact on greater infection prevention behaviors. Resilience mediated the relations of symptom experience (${\beta}=-.10$ p=.013), self-efficacy (${\beta}=.27$, p=.006) and hope (${\beta}=.25$, p=.009) with infection prevention behaviors. These predictors explained 24.9% of variance in infection prevention behaviors. Conclusion: The findings of the study suggest that breast cancer patients with greater resilience who are receiving chemotherapy participate in increased infection prevention behaviors. Further research should be conducted to seek intervention strategies that improve breast cancer patients' resilience.

항암화학요법을 받는 환자를 위한 웹기반 동영상 감염예방 자가관리 교육프로그램 효과: 예비연구 (Effect of Web-Based Video Self-Management Education Program for Infection Prevention in Patients with Cancer Receiving Chemotherapy: A Preliminary Study)

  • Purpose: The aim of this study was to examine the preliminary effects of applying a developed web-based video education program in patients with cancer receiving chemotherapy. Methods: A one-group pre-post test design was used. The web-based video education program to prevent infection consisted of 5 subjects. The program was confirmed to be valid by an expert group of doctors and nurses. Convenience sampling of 23 subjects was performed to evaluate the effectiveness of the web-based video education program. Data were analyzed based on nonparametric statistics using SPSS 25.0 software. Results: Per the results of the study, knowledge and self-management behavior of infection prevention significantly improved after application of the program; however, there was no significant difference in self-efficacy. Conclusion: It was confirmed that the web-based video education program is effective to improve the knowledge and self-management behavior of infection prevention. However, as a preliminary study, this study did not have external validity. Therefore, it is necessary to verify the effectiveness of the program through randomized controlled trials and confirm the true infection prevention effect that was not considered in this research design.
