Essential Occupational Safety and Health Interventions for Low- and Middle-income Countries: An Overview of the Evidence

  • There is still a considerable burden of occupational diseases and injuries in the world. It is not well known which interventions can effectively reduce the exposures at work that cause this burden. The objective of this article is to summarize evidence from systematic reviews of interventions to prevent occupational diseases and injuries. We included systematic reviews of interventions to reduce the incidence of work-related cancer, dust-related diseases, occupational asthma, chronic obstructive pulmonary disease, noise induced hearing loss, back pain, and occupational injuries. We searched Medline and Embase with predefined search strategies to locate systematic reviews of these interventions. We found 23 systematic reviews of which the results are also applicable to low- and middle income countries. Effective measures to reduce exposure leading to work-related cancer, dust-related diseases, asthma, chronic obstructive pulmonary disease, noise, and injuries are available. However, better implementation of these measures is needed. Regulation, enforcement of regulation, and incentives for employers are effective interventions to achieve this goal. There is evidence that feedback and rewards for workers help in reducing occupational injuries. There is no evidence in many studies that back pain can be prevented. Personal protective equipment technically has the potential to reduce exposure but this is difficult to put into effect. There is no evidence in the studies regarding the effectiveness of education and training, preventive drugs, or health examinations. There is evidence that the implementation of technical measures enforced by regulation can prevent occupational diseases and injuries. For other interventions such as education or health examinations, there is no evidence that supports their effectiveness. More systematic reviews are needed in the area of injury prevention.

우리나라 전염성 질환의 변화 양상 (Changing Patterns of Communicable Diseases in Korea)

  • Before twentieth centuries and during early twentieth centuries, communicable diseases were the major cause of morbidity and mortality in Korea. But reliable data are not available. After 1975, the overall morbidity and mortality from communicable diseases, rapidly declined. Recently many new pathogenic microbes were recognized: L. monocytogenes, Hantaan virus, Y. pseudotuberculosis, P. multocida, L. pneumophilia, Human immunodeficiency virus (HIV), G. seoi, H. capsulatum, C. burnetii, V. cholerae O139, C. parvum, F. tularensis, E. coli O157:H7, B. burgdorferi, S. Typhimurium DT104, Rotavirus, hepatitis C virus and so on. Since the first HIV infection recognized in 1985, the reported cases of infection and deaths from HIV/AIDS have been steady increased each year. Legionnaire's disease, E. coli O157:H7 colitis, listeriosis and crytosporidiasis have been occurring just sporadically among immunocompromized cases. Many re-emerging communicable diseases were occurred in Korea: leptospirosis, malaria, endemic typhus, cholera, tsutsugamushi disease, salmonellosis, hepatitis A, shigellosis, mumps, measles, acute hemorrhagic conjunctivitis, brucellosis and so on. Leptospirosis and tsutsugamushi diseases have been noticed as major public health problems since 1980s. The malaria that had been virtually disappeared for a decade has reappeared from 1993 with striking increase of patients in recent 3-4 years. The distributions of salmonella and shigella serotypes have been changed a lot in recent few decades. Furthermore rapid emergence of antibiotic-resistant bacterial strains induces more difficult and complex problems in control of communicable diseases. We must recognize on the importance of environment and ecosystem conservation and careful prescription of anti-microbial agent in order to prevent communicable diseases.

딥러닝 기술을 이용한 넙치의 질병 예측 연구 (A Study on Disease Prediction of Paralichthys Olivaceus using Deep Learning Technique)

  • 수산 양식장 질병 감염의 확산을 사전에 차단을 위해서는 양식장의 수질 환경 및 생육 어류의 상태를 실시간 모니터링하면서 어류의 질병을 예측하는 시스템이 필요하다. 어류 질병 예측의 기존 연구는 이미지 처리 기법이 대부분이었으나 최근에는 딥러닝 기법을 통한 질병 예측방법의 연구가 활발히 진행되고 있다. 본 논문에서는 수산 양식장에서 발생할 수 있는 넙치의 질병을 딥러닝 기술로 예측하는 방법에 대한 연구결과를 소개하고자 한다. 이 방법은 양식장에서 수집된 카메라 영상에 데이터 증강과 전처리 포함하여 질병 인식률의 성능을 높인다. 이것을 통해 질병 어류를 조기 발견으로 양식 어업에서 어류 집단 폐사 등 어업 재해를 예방하고 지역 수산 양식장으로 어류의 질병 확산 피해를 줄여 매출액 감소 차단될 것으로 기대한다.

유니폼의 감염인식, 세탁 규정 인식, 감염병 예방 의도에 관한 융합연구: 개인주의, 집단주의, 자아존중감 중심으로 (Convergence Research on Infection Awareness of Uniforms, Recognition of Laundry Rules, and Intention to Prevent Infectious Diseases: Focusing on Individualism, Collectivism, and Self-esteem)

  • 본 연구는 강원도 일개 대학 4년제 치위생학과 276명을 대상으로 2021년 11월 24일부터 11월 26일까지 구글 온라인설문을 통하여 연구하였다. 본 연구의 목적은 유니폼의 감염인식과 세탁 규정 인식, 감염병 예방 의도를 개인주의 및 집단주의 자아존중감을 통해 알아보는 것으로, 향후 감염병 시기에 학생의 감염 교육 방법 모색을 위해 시행되었다. 통계는 SPSS Statistics 24.0, AMOS 21.0을 사용하여 빈도 분석, 탐색적 요인분석, 확인적 요인분석, 신뢰도 분석, 구조방정식, ANOVA 분석을 시행하였다. 연구결과 유니폼 감염인식, 유니폼 세탁 규정 인식, 자아존중감, 개인주의, 집단주의 감염병 예방 의도의 모형이 적합함을 확인하였고(p<.001), 구조방정식 결과 자아존중감은 개인주의, 집단주의 감염병 예방 의도에 영향을 미치는 것으로 나타났다(p<.001). 집단주의는 유니폼 감염인식, 유니폼 세탁 규정 인식, 감염병 예방 의도에 영향을 미치는 것으로 나타나 감염예방을 위한 인식 변화에 자아존중감과 집단주의가 영향을 미치는 것을 확인하였다. 향후 치위생학과의 감염관리 교육시 집단주의를 고려한 유니폼 세탁 방법에서 활용이 가능할 것이다.

The Effect of Acute Respiratory Disease Infectious Diseases on the Life Change of People with Developmental Disabilities and Their Families

  • Patients with acute respiratory diseases, such as Middle East Respiratory Syndrome (MERS) due to COVID-19, must wear masks, protective clothing, face shields, and gloves to prevent infection during treatment and performance. Even if it is applied to disabled people, families who protect them are severely mentally tired from severe physical fatigue and stress from exposure to high-risk infectious diseases. As such, the spread of infectious diseases such as respiratory diseases has not only caused difficulties in using existing welfare and medical services but also caused various problems throughout the daily life of disabled people due to the prolonged infectious disease, and its scope is gradually expanding. Therefore, it should not be overlooked that disabled people may experience various difficulties, from the spread of infectious diseases such as respiratory diseases to isolation, diagnosis, and treatment, and it is time to actively assess the life changes felt by families caring for disabled people and consider and research to provide adequate services. According to the survey of disabled people is being conducted in the context of the spread of infectious diseases such as respiratory diseases, while research on the spread of infectious diseases such as respiratory diseases is rare for parents with disabilities. There is a need for additional investigation into the characteristics in other areas of everyday life, including the health field, which is deteriorating through prior research. Therefore, through this survey, the purpose of this study is to investigate the life changes of parents with disabilities in the context of the spread of infectious diseases such as respiratory diseases and to compare and analyze them to find out how parents were affected by each type of disability. It will be used as evidence to identify more necessary needs and problems for parents with disabilities in the spread of infectious diseases such as respiratory diseases and to provide more appropriate health care and welfare services in the future.

  • Digital therapeutics (DTx) are emerging as a novel solution to improve lifestyle and prevent non-communicable diseases. Obesity is a complex, multi-factorial, chronic condition that requires patient-centered lifestyle modification. DTx, such as mobile applications and wearables, may offer easily accessible, efficient, and personalized care in the field of obesity and metabolic diseases. Yet, there is controversy over its clinical usefulness. This review will provide a comprehensive overview of DTx, including its potential role and current limitation in obesity care, based on recent literature.

Newborn Screening for Lysosomal Storage Diseases in Taiwan

  • Lysosomal storage diseases (LSDs) are a group of rare inherited metabolic disorders caused by the deficiency of specific lysosomal enzymes and subsequent accumulation of substrates. Enzyme deficiency leads to progressive intra-lysosomal accumulation of the incompletely degraded substances, which cause dysfunction and destruction of the cell and eventually multiple organ damage. Patients have a broad spectrum of clinical phenotypes which are generally not specific for some LSDs, leading to missed or delayed diagnosis. Due to the availability of treatment including enzyme replacement therapy (ERT) and hematopoietic stem cell transplantation for some LSDs, early diagnosis is important. ERT products have been approved with optimal outcomes for some LSDs in the recent decades, including Gaucher, Fabry, mucopolysaccharidosis (MPS) I, Pompe, MPS VI, MPS II, and MPS IVA diseases. ERT can stabilize the clinical condition, prevent disease progression, and improve the long-term outcome of these diseases, especially if started prior to irreversible organ damage. Based on the availability of therapy and suitable screening methods in the recent years, some LSDs, including Pompe, Fabry, Gaucher, MPS I, MPS II, and MPS VI diseases have been incorporated into nationwide newborn screening panels in Taiwan.

키위 잎 주요 병 진단 및 방제 (Diagnosis and Control of Major Leaf Diseases on Kiwifruit in Korea)

  • 우리나라에서 재배되고 있는 키위나무 잎에는 궤양병과 세균성점무늬병처럼 세균에 의한 병과 잿빛곰팡이병, 흰가루병, 과실곰보병을 비롯하여 여러 가지 형태 점무늬병 등 곰팡이에 의한 병이 발생하여 피해를 주고 있다. 이 총설에서는 키위나무잎에 발생하는 피해를 주는 주요 병에 대하여 병징과 발생특성을 요약하고 지난 30년간 수행한 연구 업적과 현장 경험 그리고 세계적인 주요 연구 산물들을 기초로 하여 키위나무 잎에 발생하는 주요 병에 의한 피해를 경감할 수 있도록 농가에서 실용적으로 사용할 수 있는 방제방법을 제시하고자 한다.

한국인의 평생건강관리 프로그램 (Lifetime Health Maintenance Program for Korean)

  • Lifetime Health Maintenance Program(LHMP) for Koreans is the Korean guide to clinical preventive services which include screening for chronic diseases, counseling and immunization. The Korean Task Force on the LHMP vigorously reviews evidence for interventions to prevent over 50 different illnesses and conditions which are important in Korea. The problems addressed in this program are common ones seen every day by primary care physicians in Korea: cardiovascular, infectious diseases, gastrointestinal and endocrine diseases, cancers, alcohol and smoking, and many others. Primary care clinicians have a key role in screening for many of these problems and immunizing against others. Of equal importance, however, is the clinician's role in counseling patients to change unhealthful behaviors related to diet, smoking, exercise, injuries, and sexually transmitted diseases. The recommendations are grouped by age, sex, and other risk factors.

사이질 폐병의 최신지견: 특발사이질 폐렴을 중심으로 (Clinical Year in Review of Interstitial Lung Diseases: Focused on Idiopathic Interstitial Pneumonia)

  • Interstitial lung disease (ILD) is a group of diseases characterized by pulmonary interstitial inflammation. Finally the inflammation results in pulmonary fibrosis and impairment of oxygen transportation. The causes of idiopathic interstitial pneumonia (IIP) are unknown. Diagnosis of IIP is not easy, especially distinguising between nonspecific interstitial pneumonia and usual interstitial pneumonia (UIP). First line treatments of IIP include corticosteroids and immune modulators, which have limited effect. Currently, several drugs are being researched to prevent and treat fibrosis. Newer drugs that may useful to treat pulmonary fibrosis include endothelin receptor antagonist, recombinant soluble TNF receptor antagonist, and cotrimoxazole. The causes of IIP are largely unknown, treatment is not specific, and prognosis is poor. Recent studies are underway to investigate the pathogenesis and treatment of IIP and pulmonary fibrosis. As the pathogenesis of IIP is elucidated, better treatments will emerge.
