Trimethoprim-sulfamethoxazole (TMP-SMX) has been used for the treatment of urinary tract infections, but increasing resistance to TMP-SMX has been reported. In this study, we analyzed TMP-SMX resistance genes and their relatedness with integrons and insertion sequence common regions (ISCRs) in uropathogenic gram-negative bacilli. Consecutive nonduplicate TMP-SMX nonsusceptible clinical isolates of E. coli, K. pneumoniae, Acinetobacter spp., and P. aeruginosa were collected from urine. The minimal inhibitory concentration was determined by Etest. TMP-SMX resistance genes (sul and dfr), integrons, and ISCRs were analyzed by PCR and sequencing. A total of 45 E. coli (37.8%), 15 K. pneumoniae (18.5%), 12 Acinetobacter spp. (70.6%), and 9 Pseudomonas aeruginosa (30.0%) isolates were found to be resistant to TMP-SMX. Their MICs were all over 640. In E. coli and K. pneumoniae, sul1 and dfr genes were highly prevalent in relation with integron1. The sul3 gene was detected in E. coli. However, in P. aeruginosa and Acinetobacter spp., only sul1 was prevalent in relation with class 1 integron; however, dfr was not detected and sul2 was less prevalent than in Enterobacteriaceae. ISCR1 and/or ISCR2 were detected in E. coli, K. pneumoniae, and Acinetobacter spp. but the relatedness with TMP-SMX resistance genes was not prominent. ISCR14 was detected in six isolates of E. coli. In conclusion, resistance mechanisms for TMP-SMX were different between Enterobacteriaceae and glucose non-fermenting gram-negative bacilli. Class 1 integron was widely disseminated in uropathogenic gram-negative baciili, so the adoption of prudent use of antimicrobial agents and the establishment of a surveillance system are needed.
From June 1979 to July 1988 for 9 years, total 440 cases of lung cancer[including biopsy and surgical specimen] of the Pusan Paik hospital were examined for the clinical and pathology study. The findings of the study are as follows; [1] The incidence of lung cancer started to increase from 1982, and it again remarkably increased since 1987. Such increase was solely brought by the increase of male lung cancer. Male and female ratio is 5.6: 1. [2] Histopathologically, the most prevalent type is squamous cell carcinoma[60.ado], and next are adenocarcinoma[15.6%] and small cell carcinoma[15.0%]. But in female alone, the most prevalent type is adenocarcinoma[40.3%], and next are squamous cell carcinoma[37.3%] and small cell carcinoma[11.9%]. [3] The absolute number of adenocarcinoma are approximately equally distributed among both sexes until 60 years of age. Above 61 years of age, mostly male was shown while female was not. Most probably, many female patient.- of that old age simply did not visit general hospitals for surgery in Korea. [4] Surgical treatment was performed in 8% of total cases of lung cancer. And most cases showed stage I progression of the cancer. Average size of the cancer was 5 cm in diameter in the operated 35 cases suggesting that the cancer could be detected more than 10 years ago before the time of surgery. [5] Lung cancer affected more in the right lung [right: left=1.6: 1], and each upper lobe of both lungs are affected about 1/4 of cases indicating that about 1/2 of all lung cancer develop from the upper lobes. [6] There are more nonsmokers[67.6%] among the lung cancer patients[male 64.6%, female 82.1%]. Probably, this will mean that there are other potent carcinogenic agents in our environment like automobile exhaust beside tobacco smoke. For the past history of lung disease other than cancer, tuberculosis is the most prevalent disease[16.1%, male 17.4%, female 9.0%]. Most of them is probably not related etiologically though this possibility is not completely denied.

Objectives : This study aimed to investigate the associations between physical disorders and prevalent/incident suicidal ideation in a community dwelling older population aged 65 years or over. Methods : 1204 people aged 65 years or over evaluated at baseline. Suicidal ideation was identified using the questions from the community version of the Geriatric Mental State Schedule ("GMS B3"). Reported physical disorders covering 11 common and generally chronic health problems were ascertained. Covariates included were depression, age, gender, years of education, accommodation status, past occupation, and current occupation. Of 1066 without suicidal ideation at baseline, 805 (76%) were followed 2 years later, and incident suicidal ideation was evaluated. Results : Prevalent suicidal ideation was significantly associated with 4 of 11 physical disorders: eyesight problems, persistent cough, heart disease and paralysis or weakness in one leg or arm. Incident suicidal ideation was associated with 3 physical disorders: asthma, high blood pressure and paralysis or weakness in one leg or arm. Both prevalent and incident suicidal ideation were significantly associated with increased number of physical disorders. Conclusions : Certain physical disorders were comorbid and precipitating factors of suicidal ideation in elders. And appropriate intervention and treatment of physical disorders might prevent suicidal ideation in elderly.
Background and Purpose: Stroke in young adults is relatively uncommon. Only 3.7-14.4% of all strokes occur in patients aged 15-45 years. Stroke in young adults has more various and diverse possible causes than stroke in the elderly. We studied to gain further insight into both pathogenic and etiologic determinants in young adults with stroke. Methods : We retrospectively reviewed the medical records of 230 young patients aged 15-45 years who were admitted to the 2nd internal medicine department of Kyung Hee Oriental Medical Center with a diagnosis of stroke between May 1995 and May 1999. We analysed clinical features and diagnostic tests, such as brain imaging, cerebral angiography, echocardiography, 24 hours holter monitoring and other laboratory tests. Results : 1. Of 230 young patients with stroke aged 15-45 years(176 males(76.5%) and 54 females(23.5%)), 140 patients(60.9%) showed ischemic stroke and 90 patients(39.1 %) showed hemorragic stroke. 2. The most prevalent age group was from 40 to 45 years with 142 patients(61.7%) 3. The most frequent site of 140 ischemic stroke was MCA territory in 93 cases(66.4%) and Multiple, VA territory, PCA territory, ACA territory in order of frequency. 4. The most frequent site of 90 hemorrhagic stroke was basal galglia hemorrhage 57 cases(63.3%) and subcortical 13 cases(14.5%), pons, thalmus, subarachnoid, cerebellum in order of frequency. 5. The causes of hemorrhagic stroke were hypertension 49 cases(54.5%), arteriovenous malformation 7 cases(7.8%), ruptured aneurysm 4 cases(4.5%), angioma 3 cases(3.3%). 6. The risk factors of ischemic stroke were smoking, alcohol drinking, hyperlipidemia, hypertension, obesity, heart disease, history of CVA, diabetes mellitus, in order of frequency. 7. The comparison of risk factors between ischemic and hemorrhagic stroke: hypertension was prevalent in hemorrhagic stroke, heart disease and history of CVA were prevalent in ischemic stroke. Conclusions: From the above results, we found that stroke in young adults had various possible causes. Young adults with stroke deserve an extensive but tailored evaluation which include angiography and echocardiography for diagnosis.
Objectives : This study was performed to investigate the efficacy of Korean Medical Therapy (KMT) for patients undergoing In Vitro Fertilization (IVF). Methods : We analysed clinical data of 61 patients who visited for KMT after IVF from January 2013 to December 2013. Results : 1. Above 2years (63.9%) was the most prevalent duration of infertility. 2. The mean age of patients was 36.0 years old. The most prevalent age was 31~35 (45.9%) and 36~49 (27.9%) was next. 3. Anjonicheon-tang was the most prevalent KMT prescription, followed by Sutae-hwan, Jogyeongjongok-tang . 4. Twenty seven patients (44.3%) succeeded in pregnancy. And twenty patients (32.8%) succeeded in delivery. 5. The average duration of KMT was 73.8 days. Pregnancy success group was treated for 78.1 days, and pregnancy fail group was treated for 57.0 days. As a result, pregnancy success group was more treated than pregnancy fail group by 21.1 days in KMT. Conclusions This study suggested that KMT affects improvement of the clinical pregnancy and delivery rate in IVF.
For estimating the prevalence of bovine infectious disease agents, the pathogens were classified as follows: the digestive disease agents, respiratory disease agents, reproductive disease agents, and tick-borne disease agents. This study covered 81 published papers regarding bovine infectious diseases in Korea that determined the presence of diverse pathogens or the antibodies elicited by the infectious agents in cattle from 2010 to 2019. In total, 59,504 cows were involved in the papers reporting the causative agents in their cases. The disease prevalence for the digestive, respiratory, reproductive, and tick-borne cases was 9.0%, 13.4%, 10.4%, and 7.8%, respectively. Bovine viral diarrhea virus, Escherichia coli, Mycobacterium avium subsp. paratuberculosis, and Eimeria spp were more significantly prevalent in the cows under one-year age than over one-year age. Bovine viral diarrhea virus, Escherichia coli, Mycobacterium avium subsp. paratuberculosis, and Anaplasma spp. were more significantly prevalent in Hanwoo than dairy cattle. Coxiella burnetii, Neospora caninum, and Theilieria spp. were more significantly prevalent in dairy cattle than Hanwoo. Tick-borne disease agents were more prevalent in cows grazing than the case in housing. Our analytic data obtained from this study emphasize the need for more studies on the occurrence of these pathogens according to the breed, age, and the region, to come up with bovine infectious disease control measures in Korea.
Purpose: To identify associated findings in glenohumeral joint in rotator cuff tear and evaluate its clinical significance, we examined minor and major changes during arthroscopic or mini open repair. Materials & Methods: We reviewed 66 patients of rotator cuff tear treated from March, 2001 to January, 2004. Of 38 cases of small to medium tear, average age was 53 years old and involved in dominant arm in 27 cases. Of 28 cases of large to massive tear, average age was 58 years old and involved in dominant arm in 26 cases. Minor and major associated changes of the glenohumeral joint were evaluated in the tendon of biceps long head, biceps pulley, cartilage of the glenoid and humeral head, labrum and synovium. Results: Minor changes in biceps tendon were in 35% of cases, biceps pulley in 18%, cartilage of humeral head in 27%, cartilage of glenoid in 18%, labrum in 38%, and synovium in 42%. Major changes in biceps tendon were in 6% of cases, biceps pulley in 35%, arthritis of humeral head in 3%, arthritis of glenoid in 2%, labrum in 6%, and synovium in 21 %. Major changes in biceps tendon were 5% in Group I and 7% in Group Ⅱ(p>0.05) and in biceps pulley, 18% and 57% in each (P<0.05). Minor changes of arthritis were prevalent in glenoid cartilage and major changes were more prevalent in humeral head. There were no differences in minor changes of labrum and synovium, but major changes were more prevalent in Group Ⅱ. Conclusion: The prevalence of intraarticular associated changes of rotator cuff tear were 63% in synovium, 54% in labrum, 53% in biceps pulley, 41% in biceps tendon, 30% in humeral head and 20% in glenoid cartilage in order. Major changes of biceps pulley, humeral head, labrum and synovium were more prevalent in Group Ⅱ.
The purpose of this study is to investigate the dietary patterns and behaviors of self-boarding college students in Wonju. This survey was carried out through questionnaries and the subjects were 118 man and 145 woman students of five colleges in Wonju. The results of this study were summarized as follows: 1. The students who live in Kangwon Province, reaching 49.0% of the subjects, are the most prevalent. In the frequency of homecoming, 'twice a month' is the greatest. One-room system is the most prevalent in the self-boarding patterns. 2. 90.8% of the students have the electric rice cooking pot. Portable gas range, gas range, electric stove, in this order, are more general as cooking appliances. Specially, more than half of students, either male or female, have refrigerators. 3. 80.4% of students cook their own meal. 'once a day' in cooking frequency is the most prevalent. 4. The students, either male or female, prepare breakfast and dinner for themselves at home, and have lunch at the school cafeteria. They choose boiled rice mostly at any mealtime. 5.In the frequency of drinking, 'three times a week' in man students and 'two or three times a month' in woman students are the most prevalent. The drink shop and their own self-boarding room are most preferred as the place for drinking in the both case of male and female. 6. In the quantity of drinking of male students, 'more than three bottles of Soju', 'two bottles of Soju', and 'one bottle of Soju', in this order, are recorded more frequently and in the case of female students, comes 'one bottle of Soju' and 'one bottle of beer' in that order. 7. Male students drink to have a dim consciousness and female students to feel good. In this case there are differences. 8. After drinking in the previous night, more female students have a breakfast than male. While more female students have any kind of food, most of male students have a broth to relieve hangover or ramyun. 9. The causes for no breafast after drinking were 'rising late', 'stomachache', 'habits' in this cider. 10. After students self-examine their own health, they express their health state as 'usual', 'very healthy' 'not healthy' in this order. Most of male and female students point out the stomach as a painful part.
Background : Oral leukoplakia(OL) and lichen planus(LP) are common soft tissue lesions characterized by white plaque or striae with erosion. The clinical characteristics of these diseases are similar but the cause and clinical course of them are very different. I compared OL with LP by analysizing clinical and histopathological characteristics and follow up study. Patients and methods : The clinical analysis of 200 patients with OL and LP was performed by review of dental and medical charts. And H/E slides were examined under the light microscope. we examined H/E slides by the light microscope. The follow up study of patients was performed. Statistical analysis was done using the SPSS/PC WINDOWS (version 13.0). Results : The age distribution of OL was in the range of 13-75 years old being most prevalent in the 5th decade and there was a tendency of male prevalent. The age distribution of LP was in the range of 20-79 years old being most prevalent in the 4th decade and there was a tendency of female prevalent. The most common site of involvement was the buccal mucosa in both diseases. The most common clinical features of OL and LP were white plaque type and white lesion with striae, respectively. In case of LP, the most common clinical sign was tenderness to palpation. Fifteen cases of OL and eight cases of LP showed epithelial dysplasia. Twelve cases of OL recurred after surgery of oral squamous cell carcinoma and 2 cases of LP were transformed into oral squamous carcinoma. Conclusion : There was statistically significant difference in age, sex, clinical signs of patients, frequency of epithelial dysplasia between OL and LP. The Pearson coefficient correlation efficient was 0.51(p < 0.05). The knowledge of the difference between OL and LP can help understand these diseases.
Purpose : Acute diarrhea in one of the most important diseases in children with high morbidity and mortality worldwide. Most of acute diarrhea is induced by viruses. Rotavirus and adenovirus are leading causes of severe gastroenteritis among infants and young children worldwide. Studies for adenovirus gastroenteritis in Korea are limited. We studied the prevalence of rotavirus and adenovirus gastroenteritis from March 1998 to June 1999 in Chungju area. Methods : Stool samples were collected from 143 children with acute diarrhea. Specimens were tested for group A rotavirus antigen and for adenovirus type 40 and 41 by using available commercial kits. Results : Among 143 samples, 37% were positive for rotavirus and 16% were positive for adenovirus. Rotavirus was most prevalent from January to March, 1999 and adenovirus was prevalent during September 1999. The greatest number of rotavirus infections occurred under 24 months of age, followed by 2~4 years of age. Adenovirus was most common in 2~24 months of age. Conclusion : Rotavirus was most prevalent in winter and early spring. In our study, rotavirus was prevalent in early spring and adenovirus was in autumn.
